Failure to provide both current certificates may cause a penalty to be due and non-issuance of a license. **NOTICE** All liquor licenses expire on December 31st in the year the license is issued and should be renewed by December 15th of each year to avoid being charged a delinquent penalty of 25%. Failure to renew by January 1 of the following year may result in the application being denied and the collector may, without notice or hearing, suspend the right to do business. In addition to the suspension, anyone serving alcoholic beverages shall be guilty of committing a misdemeanor and shall be charged accordingly. The Inventory/Property Tax Verification Certficate must be returned completed and notarized for council approval of your Renewal. Failure to return a completed, notarized form will delay your renewal and may result in the application being denied and the collector may, without notice or hearing, suspend the right to do business. A certificate of general liability insurance with a minimum coverage amount of $100,000 with the City of Sulphur as a certificate holder. 3. Copy of your Louisiana state license, if required, according to the type of trade or the cost of each job. If doing mechanical work, you may submit your state or City of Lake Charles license. Specialty Contractors Must Comply with the additional following requirements: Electricians must obtain in addition to the above requirements a Master Electrician's license and must purchase the Journeyman's license for each journeymen on the job with the City of Sulphur before obtaining any permits to do work in the city. See Schedules Master Electrician and Journeyman Electrician. The names of the Journeymen must be provided with the application. Mechanical contractors must obtain in addition to the above requirements a Mechanical Contractors license (See Schedule ) with the City of Sulphur before obtaining any permits to do work in the city. Plumbers must provide a copy of their Louisiana master plumber s license and all other requirements. All debris must be removed by contractor and not put curbside for city pick up.

If doing construction work, this license will be honored and will exempt a Contractor's Occupational license fee to the City of Sulphur. 2. A $5000 surety/license & permit bond made favorable to the City of Sulphur stating the trade classification in which you are operating. 3. A certificate of general liability insurance with a minimum coverage amount of $100,000 with the City of Sulphur as a certificate holder. 4. Copy of your Louisiana state license, if required, according to the type of trade or the cost of each job. If doing mechanical work, you may submit your state or City of Lake Charles license.
