1 Purdue University Agricultural Safety and Health Program 994 Indiana Farm Fatality Summary Compiled by the Purdue University Agricultural Safety and Health Program Agricultural and Biological Engineering Department Purdue University The Purdue Agricultural Safety and Health Program idenified 36 farm work-related fatalities in 994. Farm fatlaity reports are compiled through a variety of sources, including news reports, and personal interviews. Several cases were identified with the assistance of the Indiana Fatality Assessment and Control Evaluation (FACE) Program at the Indiana State Department of Health, which documents occupational fatalities identified through coroners reports, as well as news articles and on-site investigations. The average age of the 994 farm fatality victims was 5 (Table ). A large proportion (4%) of the recorded fatalities resulted from incidents involving tractors (Figure ). Motor vehicle incidents not clearly involving agricultural equipment are excluded.

Daviess F Run over by tractor driven by father in the farm yard. She approached without being noticed., internal Dearborn F Run over by hay wagon. Internal M Overturned tractor and crushed while towing logs up a hill. Delaware M Struck from rear by car (driver: 8) while towing hog trailer with tractor on roadway. Thrown from tractor onto road as the tractor flipped up (0:45 pm). Dubois F Crushed by old axle which fell on her while she was playing in a barn M Fell into silage chopper and entangled leg while apparently attempting to unclog the machine (4:5pm). Gibson M Struck head-on by semi-truck while towing a mower with tractor on roadway. Thrown from tractor and pinned beneath truck. Greene M Entangled in grain auger while helping unload corn. Hancock M Fell from barn loft onto concrete floor while moving hay bales. Apparently grasped string on bale near edge which broke., head Chest, amputation

Martin M Overturned tractor apprently after driving the tractor into a creek while clearing brush with a rotary mower. Pike M Overturned tractor ( Biventricular dilation listed by coroner as primary cause of death). Monroe Unk M Overturned tractor (rearward) and crushed while towing disc harrow up steep incline. Pulaski * 75 M Overturned tractor in ditch and drowned while towing a grain wagon M Entrapped and suffocated in grain bin while attempting to unclog the auger., neck, chest Internal Heart, multiple Suffocation Randolph M Struck by falling tree M Overturned tractor on side of county road embankment (7:00 pm). Spencer M Struck on head by branch when cutting wood.,chest

The entire truck/trailer slid down a hill and struck an embankment M Kicked in chest by a cow (8:00 am). Chest Warrick M Fell into flowing grain and engulfed while attempting to kick caked grain loose from top of bin. Auger was running and two others were present at time of incident. Wayne Died: Updated: 8/3/ M Overturned tractor and crushed while brushhogging on hill (7:00 pm). 55 M Entangled in PTO while oiling the chains on a running feed grinder (:30 pm) M Crushed under combine head while victim was working on the machine with two others.
