Inspect/test AFFF system communications. a. Test emergency lighting available and operating at each station. a. Relay operated lantern. - Emergency lighting was inop. - Rubber boot was removed. - Emergency lighting was not installed. - Emergency lighting was not adequate. b. Emergency fluorescent lighting (NEALS). - Emergency lighting was inop. - Rubber boot was removed. - Emergency lighting was not installed. - Emergency lighting was not adequate. 3. Were all system valve handwheels painted the correct color code, label plate and classification plates installed? - AFFF concentrate valve handwheels were not painted striped light blue/red. - AFFF solution (concentrate/salt water mix) valve handwheels were not painted striped red/dark green. - Firemain COV handwheels were not painted red. - COV ( ) was missing its classification plate or incorrect classification IAW the ships DC Book. - COV ( ) was missing its label plate or not labeled according to function. 6641/004, M /004, Q-2, Q-3 Refs (c), (e) 1
2 INSPECT AFFF STATIONS/SYSTEM CHECK/INSPECT COMPONENT/SUB-COMPONENT IAW 4. Inspect all system valves. (Alignment, locking devices, deterioration, fasteners and function) - Valve was missing a locking device. - Valve was missing a tamper seal. - Valve was leaking. - Valve leaked by the seat. - Hytrol valve was painted. - Hycheck valve was painted. - Powertrol/Powercheck valve was painted. - Valve had ferrous fasteners. - Valve was not properly aligned. - Inspect all flanges, unions, couplings, paking glands for leakage 5. Inspect all actuation points. (Numbered and classified) - Push buttons were not labeled. - Push buttons were not classified. - Pushbuttons were not painted striped red/green. 6. Inspect photoluminescent markings applied at all - Photoluminescent marking was missing. - Photoluminescent marking was damaged. 7. Inspect warning signs. - Hearing warning sign was not posted in AFFF pump area volt warning sign was missing from SOPV. 8. Inspect deck drains. - Deck drains were not labeled. - Deck drain was not operational. - Deck drain was not classified IAW ships DC Book. - Missing deck drain screws. - Missing deck drain tee wrench. - If installed, deck drain remote operating gear (ROG) was inop. 9. Inspect tank level & indicator. - Tank was less than 95 percent full. - Guard was missing, if required. - Tank level indicator was inop or inaccurate. - If installed the remote tank level indicator was inop or inaccurate. - Drain valve leaked/leaked by. - Drain valve was missing locking device. - Valve handwheels were missing from tank level indicator COV s, if installed. 10. Inspect gages. - Gage(s) was/were inop. - Gage(s) was/were not calibrated. - Gage(s) was/were out of calibration. - Gage(s) was/were not mounted. - Gage(s) was/were leaking. - Gage(s) COV (Hoke) was/were inop or corroded. 11. Visually inspect all SOPV s - SOPV telltale indicated leakage. - SOPV was not labeled according to function. - SOPV had excess wiring. - SOPV was not classified. - SOPV was missing non-ferrous plug in the bottom. - SOPV was missing screws in the base plate. - SOPV was leaking. - SOPV was missing 440 volts label. - SOPV box was corroded. - SOPV box had dirt and debris inside the electrical box. 12. Inspect / Test Stations ABT. a. Inspect ABT. - ABT was not properly labeled. - ABT was damaged. - Power available light was out or inop. actuation points? 3 INSPECT AFFF STATIONS/SYSTEM CHECK/INSPECT COMPONENT/SUB-COMPONENT IAW - ABT control switch was broken off. b. Test ABT with system running. - ABT failed to operate. - ABT would not switch from primary power to alternate power in manual mode. - Pump did not continue to run when shifted to alternate power. - ABT would not switch from alternate power back to primary power in auto mode. - Power available light was inop. - ABT test switch was broken. 13. Inspect and test AFFF pump. (Place pump in recirc, concentrate discharge valve closed, and operate pump locally) - Manually cycle the master SOPV T-Handle and verify the concentrate pump starts IAW the NAVSEA Standard installation DWG. (If pump does not start, manually start the pump at the controller.) - Pump vibrated heavily. - Pump foundation required preservation. - Pump leaked AFFF from mechanical seal. - Pump coupling was loose. - Pump and/or piping/valves leaked. - Pump drive belt was loose/worn/damaged. - Pump was inop. - Pump discharge pressure was PSI. - Check pump rotation to ensure proper direction 14. Inspect and test pump relief valve. (220 to 230 BPPS with POWERTROL service valves). ( with HYTROL service valves.) (Do not exceed 300 psi pump discharge pressure when testing) - Relief valve was not set IAW PMS (to high/to low). - Relief valve was inop. - Relief valve could not be adjusted. - Relief valve leaked. - Relief valve was not tagged with relief valve setting and date. 15. Verify proper AFFF concentrate. AFFF Concentrate from BUCKEYE corporation is not permitted. Permitted list includes 3M, ANSUL, CHEMGUARD and KIDDE products. 16. Inspect last concentration analysis within PMS standards. - Last concentration analysis was not IAW PMS. - Last concentration was out of specs. - Last concentration was out of periodicity. - If unsat or past periodicity the ship will have to conduct a concentration analysis test prior to getting underway. 17. Perform concentration analysis test. (High and low demand off of each station) Ref (a) M-1 Ref (a) Q-2 Ref (f) a. Perform low demand test. (From a hose nozzle. Note: Ensure the correct size nozzle is used) - Ship did not have inline pressure gage. - Inline pressure gage was out of calibration. - Failed to allow foam to discharge from the nozzle for 1 minute. - Line gage pressure was less than 90 psi for a 95 GPM nozzle or 52 psi for a 125 GPM nozzle. - AFFF concentration was out of PMS specs. - Quantabs were expired. b. Perform high demand test. (From flight deck or bilge sprinkling system) Note: The sample can be obtained from a hose reel in the vicinity of the sprinkler system, fed from the same station, while the sprinkler system is running. - Failed to allow foam to discharge from the nozzle for 1 minute. - AFFF concentration was out of PMS specs. 3
4 TEST AFFF BILGE SPRINKLING CHECK/INSPECT COMPONENT/SUB-COMPONENT IAW 1. Test all bilge sprinkling systems with AFFF. - AFFF bilge sprinkling system was not installed. a. From all push buttons / activation locations. (Do not keep on/off pushbutton depressed for more than 3 seconds) - SOPV failed to rotate. - Push button was inop. - SOPV was wired backward. - Push button control was not located along side Halon actuation controls. b. Verify foam sprays out the nozzles. - Nozzles were clogged. - Nozzle was obstructed. - Piping was ruptured. c. Inspect and Test SOPV s. (Do not cycle more than 5 times in 5 minutes) - SOPV telltale hole indicated leakage. d. Inspect Hy-check/Hytrol and Powercheck/Powertrol valves. - Hy-check/Hytrol valves failed to open/close. - Powercheck/Powertrol valves failed to open/close. - Valve stems failed to rise. - Valve stem indicating pins were missing. - Telltale leaked during testing. 2. Test sprinkling coverage with foam. (Verify AFFF coverage, normally completed underway) a. Inspect AFFF coverage. - area had no coverage. - nozzles were clogged or obstructed causing bilge coverage deficiencies. Ref (a) R-5M Ref (a) Q-2, R-5M, R-6A Ref (a) R-6A Ref (a) R-5M, R-7M Ref (a) Q-2, R-5M, A-5, R- 6A, R-7M TEST AFFF OVERHEAD SPACE SPRINKLING CHECK/INSPECT COMPONENT/SUB-COMPONENT IAW 1. Test all overhead sprinkling systems with AFFF. - AFFF sprinkling system was not installed. - a. From all push buttons/ activation locations. (Do not keep on/off pushbutton depressed for more than 3 seconds) - SOPV failed to rotate. - Push button was inop. - SOPV was wired backward. - Push button control was not located along side Halon actuation controls. b. Verify foam sprays out the nozzles. Note: Plastic tubes or socks can be installed on the overhead nozzles. - Nozzles were clogged. - Nozzle was obstructed. - Piping was ruptured. c. Inspect & Test SOPV s. (Do not cycle more than 5 times in 5 minutes) - SOPV telltale hole indicated leakage. d. Inspect Hy-check/Hytrol and Powercheck/Powertrol valves. - Hy-check/Hytrol valves failed to open/close. - Powercheck/Powertrol valves failed to open/close. - Valve stems failed to rise. - Valve stem indicating pins were missing. - Telltale leaked during testing. 5 TEST AFFF HOSE REELS CHECK/INSPECT COMPONENT/SUB-COMPONENT IAW 1. Test hose reel for free operation. - Hose reel would not turn freely. - Hose reel crank was missing. - Hose reel crank would not rotate without interference. 2. Was hose reel COV? - If installed, the manual ball valves were open/closed and sealed in the main engineering spaces. - If installed, the manual ball valves were closed and sealed outside the main engineering spaces. - If installed, the manual ball valve latching device pins were removed IAW NAVSEA advisory directing the removal of the latching device from AFFF hose reels equipped with ball valve. - If installed, the manual gate/globe valves were open and sealed. 3. Inspect nozzles. a. Inspect 1.5 inch nozzles. - Nozzle was a 125gpm vice a 95gpm nozzle. (interior nozzles) - Nozzle was 95gpm vice 125gmp. (top side nozzles) - Nozzle was inop. - Nozzle was clogged. - Nozzle would not go into the flush position. - Nozzle would not cycle through all positions. 4. Inspect anti-crimp strap. - Anti-crimp strap was not installed. - Anti-crimp strap was loose. - Anti-crimp was of the wrong material. 5. Inspect hose. - Hose was out of hydrostatic test periodicity. - Hose was damaged. - Ensure hose shape is acceptable 6. Test hose reel. a. Test hose reel push button. (Do not keep on/off pushbutton depressed for more than 3 seconds) - Hose reel push button was inop. - -AFFF hose reel SOPV was inop, if installed. b. Test hose reel. - Hose reel COV was inop. - Hose reel COV leaked by. - Swivel joint leaked. - Hose leaked at swivel joint. - Hose was too short to reach the next hose reel or the re-entry hose reel would not reach the furthest corner of the space. c. Test nozzle. - Nozzle was clogged. - Nozzle was inop. - Nozzle would not go into the flush position. - Nozzle would not cycle freely.
