4 はじめに 現在 人道的援助を必要とする人々は 1 億人以上います これは第 2 次世界大戦以降の最大の数値です 紛争により避難を余儀なくされたり 災害のために住処を追われた人々のなかには 女性や思春期の少女が何百万人もいます 本書では彼女たちのニーズを充たし 権利を保障するための行動を呼びかけています 人道的な環境に置かれた女性と思春期の少女非の健康と権利を保護することに関しては過去 10 年間に顕著な進展があったものの ニーズの増大のほうが資金やサービスの増加をはるかに上回りました それでも 健康と権利を保護するサービスは たとえ安定した時期であっても 自分たちが直面する多くの問題に立ち向かう力のない もっとも脆弱な思春期前半の少女にとっては非常に大切なものです 開発途上国のなかには 通常であっても 10 歳の少女が 意思に反して結婚させられたり 人身売買されたり 家族からもあらゆる社会的支援からも引き離されることがあり 教育 保健 よりよい生活に向けた機会を手にすることが非常に限られているところがあります 何らかの危機が発生すると これらの危険の度合いは増し 少女の脆弱性も合わせて増大します この少女の将来展望はさらに暗くなってしまいます 性的暴力の標的となり HIV に感染し 思春期を迎えると同時に妊娠させられることもあります こうして将来が狂わされてしまうのです 世界中のどこにいようと 10 歳の少女すべてに健康 尊厳 安全を求める権利があります 1994 年に開かれた国際人口開発会議において 179 カ国 の政府は いかなる状況においても その権利を支持することを誓約しました しかし あまりにも長きにわたって 人道的支援はあっても 10 歳の少女は置き去りにされ 安全でない出産や悲惨な結果につながる暴力にあう状況に追いやられてきました どの少女にとっても思春期から成人期まで安全に成長するうえでセクシュアル / リプロダクティブ ヘルス ( 性と生殖に関する健康 ) そして情報を手にすることは不可欠です 10 歳の子どもが 15 年後に権利も健康も守られていることを想像してみてください そして逆に その子が武力紛争 荒廃 混乱により すべての権利 機会 尊厳が否定された世界のなかにいることを想像してみてください この少女やその地域社会や国を見捨てることは選択肢ではありません 私たちはつまるところ一つの世界であり 政府には人権を保護し 国際法を順守する責任があります 世界市民として 私たちには支援と連帯感を提供する義務があります 今日 妊産婦死亡の 5 分の 3 は 非人道的で不安定な状況のなかで起こっています 毎日 この状況下にある女性と思春期の少女 507 人が妊娠合併症や緊急事態や不安定な状況での出産が原因で亡くなっています さらにジェンダーに基づく暴力による残酷な犠牲が依然として続き 命を奪い 平穏と 2 はじめに

6 概 要 現在生きている人々のなかには 危機に遭遇して人生を一変させられた人が 10 億人以上も いる 戦争 政情不安 伝染病 災害などの後の混乱や破壊は長く尾を引く 今 その数は 5950 万人と推定される これは第 少女のセクシュアル / リプロダクティブ ヘルス 2 次世界大戦の破壊的状況以降のいかなる時よりも ライツを支持した その約束は どんな状況でも 大きい この他に自然災害による被災者は年間 2 どんな時でも その権利を支持し 実現させること 億人にも上る を含む 人道的な危機にあってもその責任が軽減さ 危機によって家を追われた人が増えており 人によっては 挫折は一時的なものであるが 他 れることはない この約束を守ることは 危機以前 の人々にとっては それが生涯にわたることもあり 危機下 事後をとおして女性と少女が包括的なサー うる 現在 難民は平均 20 年間も故郷を離れて暮 ビスを利用できるよう保証することを意味する 多 らしている くの危機に見舞われた国々に必要なことは 政府が 紛争や危機下にある女性と少女にとって妊娠 出 産はさらなる脆弱性となる 予防できる妊産婦死亡 の 60 は 紛争や自然災害 避難生活を生き抜こ うとしている女性たちに起こっている リスク Photo UNFPA/Sawiche Wamunza 4 一連の国際的な合意文書のなかで 世界は女性と 概 要 責任意識を高めてそのための活動を拡大することで ある

7 不安定な世界のなかで女性と少女は不当な代価を払っているさまざまな尺度からみて 5 6 年前よりもさらに多くの国が不安定な状況にあり 紛争や災害の影響によって脆弱になっている 多くの要因から人も国もともに脆弱な状況にある その要因のひとつが貧困で 10 億人以上がいまだに極度の貧困状態で暮らしている この状況で個人も国全体も開発の梯子のはるか下の段に押し止められ 上段によじ昇ることは見果てぬ夢である つまり まともな仕事はなく 社会サービスの質などは望むべくもない 地理的条件も要因のひとつである 国によっては 気候変動のために激甚の度合いを増している自然災害の通り道にある 都市化の歴史的な進展で都市住民への危険が増しているが 土砂崩れの起きやすい丘の中腹などの不安定な地盤に粗末な家を建て 不法居住地で暮らしていることの多い貧困層には危険はさらに大きい 女性と少女にとって これらを含む多くの要因が 重なると差別とジェンダーの不平等につながる まず 所得 土地その他の資産 医療を受ける手段 教育 社会的つながり 政治的発言権 法の下の平等の保護 基本的人権の実現 これらのほとんどすべてについて 女性と少女が手にしているのは男性よりも少ない ひとたび危機が発生すると 女性と少女は不当なまでに不利な立場におかれ 生き延び 立ち直るための備えも能力も低い どのような危機であれ そのさなかと事後には ジェンダーに基づく暴力が戦争の武器として使用されることもあり急増する 災害や紛争を原因とする経済的困窮から女性が性の取り引きをさせられたり 人身売買される羽目になりかねない 基本的なセクシュアル / リプロダクひとたび危機が発生すると 女性と少女は不当なほど不利な立場におかれ 生き延び 立ち直るための備えも能力も低い 女性と少女は不当なほど不利な立場におかれる 1 億人以上が 人道支援を必要としているが その 4 分の 1 は 15 歳から 49 歳の女性と少女 である 高まるリスクとは HIVを含む性感染症 意図しない 望まない妊娠 妊産婦死亡と疾患 性とジェンダーに基づく暴力 世界人口白書

8 ティブ ヘルスサービスすらないため 危機的状況での出産は命取りにつながることがある 思春期の少女にとってその恐れはさらに大きい 以上のようなリスクの原因は共通している それは性別 年齢別その他の特性にかかわらず誰もが共通してもっている人権の尊重が欠如していることである その権利のなかにはリプロダクティブ ライツもある これは 1994 年の国際人口開発会議で画期的な行動計画にまとめられ 179 カ国が合意したもので 国連人口基金が活動の指針としているものである 危機の前 中 後にわたって 女性と少女はセクシュアル / リプロダクティブ ヘルスの障害に直面する 2015 年の時点で人道的援助を必要とする人が 1 億人以上いるが その 4 分の 1 は出産可能年齢の女性と思春期少女であると推定される ジェンダー別 年齢別のニーズを含め これらの人のニーズをすべて充足できないような支援は 効果的であると は到底みなされない 今日まで 女性と少女のセクシュアル / リプロダクティブ ヘルスのニーズを充足することを目指した支援は 需要に追いついていない 確かに過去 10 年間で女性と少女向けの人道的サービスには顕著な進歩があった それでも活動面でも資金面でも需要と供給には大きな差がある その理由のひとつはジェンダーの不平等と性別 年齢別 その他の要因による差別であり それは善意の人道的対応のなかにすら見られる 災害や紛争が及ぼす影響は人々の集団によってそれぞれ異なることを考慮しない介入は 結果として不平等を常態化することもありうる 危機下でも妊娠 出産 避妊に関するサービスは提供せず 単に通常の保健ケアだけを提供したとしたら すでに不利な立場にある女性と少女をますます危険な状況に追いやることになる 危機の混乱初期には 食料 避難所 負傷の手当てなどが差し迫ったニーズに思われ ジェンダーや 対応 Photo Panos Pictures/Chris de Bode 6 概 要

9 その他の差別は状況が落ち着くまではと放置される しかし このように考えるのは 現実に目をつぶることにつながり 女性と少女を支援から締め出すか または暴力を受けやすい状況のまま放置することになる 性別その他の要因別のデータが限られていること 現地で最初に対応する多くの人たちにジェンダーの専門的知識が不足していることが このような認識を共有するのを妨げている 現実の世界にある危機の規模を考え 最大の犠牲者は誰かを考えると これまでの取組み方法を 危機被害者の多様性を考慮に入れた方法 またリスクを減らし 回復を早め 立ち直る力を強くするために多様性をうまく活用する方法に変容させていく時ではないか 人道的活動の中心にセクシュアル / リプロダクティブ ヘルスをわずか 20 年前まで 人道的対応のなかでは水 女性と思春期の少女に人道的なサービスを提供することに関しては過去 10 年間に顕著な進展があったものの 活動面でも資金面でも大きな格差がある 食料 避難施設が優先され セクシュアル / リプロダクティブ ヘルスは後回しにされた しかし 1990 年代初頭から多くの研究結果と証拠が蓄積され 危機における女性と少女の健康問題が明らかになってきた 多くの人道的介入は 今では妊娠と出産にかかわるニーズを充たすようになっており 性的暴力 ジェンダーに基づく暴力や HIV を含む性感染症に対する彼女たちの脆弱性を防いだり それらに取り組んでいる これらのニーズを充足することが人道的な責任であり 人権の擁護 尊重につながることは広く受け 危機の発生当初からの基本的活動とサービス 優先サービス 緊急産科 新生児ケア 産科緊急時の専門医照会体制 目標 妊産婦と乳児の死亡を予防する 清潔で安全な出産用の物資 避妊 コンドーム 抗レトロウイルス剤 レイプ被害者向け医療 HIV 感染を減らす 性暴力を予防し 性暴力の結果をケアする 世界人口白書

10 入れられているが それだけでなく セクシュアル / リプロダクティブ ヘルスケアを受けられるようにすることが回復とリスク低減 さらに立ち直りへの道であることは明らかである その効果は女性と少女だけでなく その他の人々にも拡大していく さまざまな面での不平等をなくそうとする人道的プログラムと合わせて セクシュアル / リプロダクティブ ヘルスケアを受けられるようになれば 人道的活動のすべての面に積極的な波及効果をもたらす 基本的な変革が必要である 時には何十年にわたって長引く災害や紛争に対して それが発生してから対応していくことから 個人と地域社会が災害や紛争を耐え抜き 復興回復できるように 予防 備え 能力づくりに力点を移していかなければならない 持続不可能な資金調達が人道的活動の変革を促す人道的活動の最大の特徴は 国連を含めた主要な国際組織が調整し 管理していることである その経費は伝統的に経済協力開発機構 (OECD) の構成国の出資金でまかなわれてきたが その他の国々と民間基金も重要な役割を果たし始めている 2011 年以降 人道支援の需要は毎年高まっているものの 拠出金のほうは需要と歩調を合わせているわけではなく その差は例のないほど開くばかりで 支援を 必要とする何百万人もの人々に向けた対応は不適切で不十分である 拡がるばかりの需要と供給の差は 現在の資金調達の仕組みが持続不可能であることを示唆している それはまた変わりばえのしない人道支援活動についても言えることかもしれない 危機は今後も続き 緊急ニーズは常に充足を必要とする それでも基本的な変革が必要である 時には何十年にわたって長引く災害や紛争に対して そ 回復力 Photo UNFPA/Ben Manser 8 概 要

11 れが発生してから対応していくことから 個人と地域社会が災害や紛争を耐え抜き 復興回復できるように 予防 備え 能力づくりに力点を移していかなければならない 反応と対応から 備え 予防 立ち直りに向けて重点を傾ける私たちはいっそう回複力のある 脆弱でない世界を構築することを目指さなければならない そのような世界では 国内であれ国際間であれ 開発は包摂的で公正であり すべての人のあらゆる権利が支持される 女性と少女がさまざまな面で不利な扱いを受けずに それぞれの可能性を十分に発揮できるよう平等に資格が付与され 地域社会や国の発展と安定に寄与できるような世界である その世界では どの国も自国の経済と政治体制を管理し 国民すべてにまともな仕事 セクシュアル / リプロダクティブ ヘルスケアも含めた質の高い基本的サービスを保証する 公共政策の方針を打ち 立てた国々のなかでは 公正で包摂的な開発に投資することが最良であり もっとも公平で人間的な投資であるという健全な理解が生まれるであろう その広範な成果として 危機のリスクや影響を軽減することも含まれる 現在よりも回復力があり 脆弱でない世界に向けて変容するには 危機管理能力 ならびに危機発生のずっと以前から十分な能力を備えた組織の存在が必須である まずリスクについて全体的に理解すべきである それがあってこそ リスク軽減の方策に効果的な投資ができる リスクが避けられない国では 最悪の事態を限定的にするために 先を見越した備えが重要である どの国にとってもリスクの軽減に向けた中心的な戦略のひとつは 国民が危機に直面しても立ち直る備えをもつことである 健康で教育を受け 十分な所得があり 人権のすべてを享受している人々は 危機が現実に発生した時にずっと見通しが明るい 予防 備え エンパワーメント 危機管理をよりよくする ジェンダーの不平等を是正する 個人の立ち直る力を教育と健康をとおして養う 災害発生前に組織の能力を構築する セクシュアル / リプロダクティブ ヘルス / ライツの実現を可能にする 長期的 包摂的 公正な開発に向けて努力する 世界人口白書

13 人道的活動と開発の間の人為的分断を壊す今日 世界各地で起きている危機と激動をみると われわれは何事ももっとふさわしい対応をすることが求められる よりよい開発 よりよい人道的活動 よりよい危機管理 よりよい予防 備え 回復 そしてこれらすべての間のよりよき連結が必要である これらに一貫しているのは セクシュアル / リプロダクティブ ヘルス / ライツの完全な実現によってある程度達成できるジェンダーその他のあらゆる形での平等であり その達成ができれば 個人と社会全体の脆弱さははるかに少なくなり 回復力がより大きくなることに結びつく 人道的対応と開発を区別する今日の考えは間違っている 人道的活動は長期的開発の基礎を敷くことができる 妊娠 出産に関する権利を含む権利をすべての人が享受できるようにして 開発がすべての人のためになるものであれば 個人 組織 地域社会が危機を耐え抜く手助けとなる さらに回復速度を早めることもできる 公正で包括的で権利に基づく開発と それによって強化された回復力が生まれると 多くの場合 人道的介入の必要はなくなる 世界的な同意を得た 持続可能な開発のためのアジェンダ 2030 が開始し 2016 年の世界人道サミットが近づくにあたって この理解に則して行動し 女性と少女の健康と権利を中心に据えた人道的活動を改めて構想するべき時期である 人道的活動に向けた新しいビジョン 予防と備え 対応から回復 開発までの 効果的対応 セクシュアル / リプロダクティブ ヘルス / ライツ 公正で包摂的な開発 相互に関係する人道的活動の要素の中心にセクシュアル / リプロダクティブ ヘルス / ライツがある 回復力 回復 世界人口白書

26 Technical notes Data sources and definitions The statistical tables in The State of World Population 2015 include indicators that track progress toward the goals of the Programme of Action of the International Conference on Population and Development (ICPD) and the Millennium Development Goals (MDGs) in the areas of maternal health, access to education, reproductive and sexual health. In addition, these tables include a variety of demographic indicators. The statistical tables support UNFPA s focus on progress and results towards delivering a world where every pregnancy is wanted, every birth is safe, and every young person s potential is fulfilled. Different national authorities and international organizations may employ different methodologies in gathering, extrapolating or analyzing data. To facilitate the international comparability of data, UNFPA relies on the standard methodologies employed by the main sources of data. In some instances, therefore, the data in these tables differ from those generated by national authorities. Data presented in the tables are not comparable to the data in previous issues of The State of World Population due to regional classifications updates, methodological updates, and revisions of time series data. The statistical tables draw on nationally representative household surveys such as Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys (MICS), United Nations organizations estimates, and inter-agency estimates. They also include the latest population estimates and projections from World Population Prospects: The 2012 revision and Model-based Estimates and Projections of Family Planning Indicators 2014 (United Nations Department of Economic and Social Affairs, Population Division). Data are accompanied by definitions, sources, and notes. The adolescent birth rate represents the risk of childbearing among adolescent women 15 to 19 years of age. For civil registration, rates are subject to limitations which depend on the completeness of birth registration, the treatment of infants born alive but dead before registration or within the first 24 hours of life, the quality of the reported information relating to age of the mother, and the inclusion of births from previous periods. The population estimates may suffer from limitations connected to age misreporting and coverage. For survey and census data, both the numerator and denominator come from the same population. The main limitations concern age misreporting, birth omissions, misreporting the date of birth of the child, and sampling variability in the case of surveys. Sexual and reproductive health The United Nations Population Division produces a systematic and comprehensive set of annual, model- based estimates and projections for a range of family planning indicators for a 60- year time period. Indicators include contraceptive prevalence, unmet need for family planning, total demand for family planning and the percentage of demand for family planning that is satisfied among married or in- union women for the period from 1970 to A Bayesian hierarchical model combined with country- specific time trends was used to generate the estimates, projections and uncertainty assessments. The model advances prior work and accounts for differences by data source, sample population, and contraceptive methods included in measures of prevalence. 31 HUMANITARIAN GLOSSARY COMPLEX EMERGENCY A multifaceted humanitarian crisis in a country, region or society where there is a total or considerable breakdown of authority resulting from internal or external conflict and which requires a multi-sectoral, international response that goes beyond the mandate or capacity of any single agency and/or the ongoing United Nations country programme. Such emergencies have, in particular, a devastating effect on children and women, and call for a complex range of responses. DIRECT AND INDIRECT EFFECT A direct effect is a death or other outcome which occurred directly and primarily because of a catastrophic event. An indirect effect is a death or other outcome which occurred as an eventual, but not immediate, result of a single destructive act. Often these outcomes can be identified only on a population basis, where direct effects can be specified to the individual. For example, if the mortality rate rises after an event and there is no other cause identified, these additional deaths are considered the indirect effect of the event. HUMAN RIGHTS All human rights derive from the dignity and worth inherent in the human person. The concept of human rights acknowledges that every single human being is entitled to enjoy his or her human rights without distinction as to race, colour, sex, language, religion, political or other opinion, national or social origin, property, birth or other status. HUMANITARIAN ACTION Humanitarian action provides lifesaving services and facilitates the return to normalcy for people and communities affected by natural and human-made disasters. It also seeks to lessen the destructive impact of disasters and complex emergencies. HUMANITARIAN CRISIS An event or series of events that represents a critical threat to the health, safety, security or well-being of a community or other large group of people, usually over a wide area. Armed conflicts, epidemics, famine, natural disasters and other major emergencies may all involve or lead to a humanitarian crisis. HUMANITARIAN RESPONSE Material and logistical assistance to people due to needs created by conflict or disaster, and provided on the basis of an appeal by the government or international organizations. INTERNALLY DISPLACED PERSONS Individuals who have been forced or obliged to flee or to leave their homes or places of habitual residence, in particular as a result of, or in order to avoid, the effects of armed conflict, situations of generalized violence, violations of human rights or natural or human-made disasters, and who have not crossed an internationally recognized State border. NATURAL DISASTER A large-scale event where life and/ or property is destroyed. For an event to be recorded as a disaster in the internationally recognized Centre for Research on the Epidemiology of Disasters database, at least one of the following criteria must be met: Ten or more people reported killed. One hundred or more people reported affected. A declaration of a state of emergency. A call for international assistance. Natural disasters include droughts, earthquakes, epidemics, extreme heat or cold, floods, storms, tsunamis, volcanic eruptions and wildfires. PEOPLE AFFECTED BY CONFLICT OR DISASTER Individuals requiring immediate assistance during a period of emergency, i.e., requiring basic survival needs such as food, water, shelter, sanitation and immediate medical assistance as the result of the emergency. REFUGEE A person who, owing to a well-founded fear of being persecuted for reasons of race, religion, nationality, membership of a particular social group or political opinion, is outside the country of his or her nationality, and is unable to, or owing to such fear, is unwilling to avail himself or herself of the protection of that country.
