2 Fall of Highwall Kentucky Crushed, Broken Limestone Lafarge West, Three Rivers Quarry METAL/NONMETAL MINE FATALITY - On January 7, 2013, a 49-year old assistant plant manager with 30 years of experience was injured at a crushed stone operation. The victim was working on a lift, taking samples from a highwall, when a large rock fell (240 lb. )and struck him. He was hospitalized and died on January 19, 2013.

Identify and control all hazards. Train all persons to recognize adverse conditions and environmental factors that can decrease highwall stability and understand safe job procedures to eliminate all hazards before beginning work. Look, Listen and Evaluate pit and highwall conditions daily, especially after each rain, freeze, or thaw. Remove loose or overhanging material from the face. Correct hazardous conditions by working from a safe location. Ensure that work or travel areas and equipment are a safe distance from the toe of the highwall.
5 Machinery Nevada Lime Lhoist No. America of Arizona Apex Quarry & Plant METAL/NONMETAL MINE FATALITY - On January 21, 2013, a 54-year old mechanic with 6 years of experience was killed at a lime operation. The victim went to a kiln pre-heat deck to repair a leaking hydraulic cylinder that activates a pusher arm on the kiln. He was caught between the corner of the angle iron and the plate connecting the push rods.

Always follow the equipment manufacturer's recommended maintenance procedures when conducting repairs to machinery. Task train all persons to recognize all potential hazardous conditions and understand safe job procedures to eliminate all hazards before beginning work. Before working on or near equipment, ensure that the equipment power circuits are locked out/tagged out and that the equipment is blocked against hazardous motion. Require all persons to be positioned to prevent them from being exposed to any hazards. Monitor personnel to ensure safe work procedures, including lock out/ tag out and safe work positioning, are followed. Ensure guarding is in place to cover potential pinch points and moving parts in areas routinely accessed by personnel.
8 Explosives and Breaking Agents Missouri Crushed, Broken Limestone NEC Fred Weber Inc North Stone METAL/NONMETAL MINE FATALITY - On March 27, 2013, a 61-year old loader operator with 24 years of experience was killed at a crushed stone operation. The victim was in a front-end loader about 50 feet from the base of a highwall when a blast was initiated. Broken rock struck the frontend loader and covered it. The rock was removed from the front-end loader and the victim was recovered about 10 hours after the blast occurred.

Establish and discuss safe work procedures. Identify and control all hazards associated with the work to be performed along with the methods to properly protect persons. Task train all persons to recognize all potential hazardous conditions, to ensure all persons have left the blast area, and to understand safe job procedures for elimination of the hazards before beginning work. Maintain and use all available methods of communication, such as sirens and radios, to warn persons of an impending blast. Establish methods to ensure that all persons are out of the blast area. Before firing a blast give ample warning to allow all persons to be evacuated. Guard or barricade all access routes to the blast area to prevent the passage of persons or vehicles. Verify that the blasting procedures are effective and being followed at all times.

An excavator was being used to position a 36-inch diameter by 40-foot long section of pipe to connect it to another section of pipe. The pipe, attached to the excavator by a lifting strap, shifted and fell on the victim.

Attach taglines to loads that may require steadying or guidance while suspended. Securely block equipment against hazardous motion to ensure energy cannot be released while performing work. Never work in the fall path of objects/materials of massive weights having the potential of becoming off-balanced while in a raised position. Implement measures to ensure persons are properly positioned and protected from hazards while performing a task. Monitor personnel routinely to determine that safe work procedures are followed.
14 Falling/Sliding Material Louisiana Salt North American Salt Company - Cote Blanche Mine METAL/NONMETAL MINE FATALITY - On April 16, 2013, a 58- year old shaftman with 32 years of experience was seriously injured at an underground salt mine. The victim and two coworkers were replacing a bushing on the side of a skip hoist in the production shaft. The victim was standing on a steel beam outside the handrails of a covered work platform where the coworkers were standing, when a piece of salt fell and struck him. He was transported to a hospital where he died on April 17, 2013.

Implement measures to ensure persons are properly positioned and protected from falling material while performing shaft maintenance work. Perform shaft maintenance work from a substantial platform with adequate overhead protection. Perform maintenance work for skip hoists and other conveyances on the surface whenever possible. Monitor personnel routinely to determine that safe work procedures are followed.

Implement measures to ensure persons are properly positioned and protected from hazards while performing a task. Provide emergency stop mechanisms at the control panel(s) and at ground level where maintenance or repair work is performed. Provide appropriate controls to protect any person working near a stalled conveyor from unexpected motion. Maintain communications with all persons performing the task. Before starting belt conveyors, ensure that all persons are clear.

He was operating a 35 ton articulated haul truck down a haul road. The truck went out of control and hit a berm, propelling it in the air. The truck came to a stop with the bed overturned and the cab upright. The victim was ejected from the truck.

Do not operate mobile equipment with reported brake problems. Use other means to move the mobile equipment to a safe area for inspection and repair. Ensure that mobile equipment operators are task trained adequately in all phases of mobile equipment operation, including the mobile equipment s capabilities, operating ranges, load-limits and safety features, before operating mobile equipment. Maintain equipment steering and braking systems in good repair and adjustment. Always follow the manufacturer s service and maintenance schedules. Never rely on engine brakes and transmission retarders as substitutes for keeping brakes properly maintained. Conduct adequate pre-operational checks to ensure the service brakes will stop and hold the mobile equipment prior to operating. Operators of self-propelled mobile equipment shall maintain control of the equipment while it is in motion. Operating speeds shall be consistent with conditions of roadways, tracks, grades, clearance, visibility, curves, and traffic. Do not attempt to exit or jump from moving mobile equipment.

The victim looked into an operating crusher and a tooth, that broke free from an excavator bucket, was ejected from the crusher and struck him.

Before working on or near equipment, ensure the equipment power is off and locked out/tagged out. Ensure the equipment has been securely blocked against hazardous motion to ensure energy cannot be released while performing work. Always maintain equipment in a safe operating condition. Provide a safe means of access for persons required to maintain a cone crusher. Provide guards, shields, or other devices to protect persons from the hazard of flying or falling materials generated from the operation of screens, crushers, or conveyors. Implement measures to ensure persons are properly positioned and protected from hazards while performing a task.

The victim was driving a loaded haul truck out of a quarry when the truck traveled through a berm and over an 80-foot highwall. The victim was ejected from the truck.

Operate mobile equipment at speeds consistent with the conditions of roadways, tracks, grades, clearance, visibility, curves, and traffic. Do not exit or jump from moving mobile equipment.

The victim was attempting to remove a rock from a pug mill hopper when he was engulfed by the material in the hopper.

Install a heavy screen (grizzly) to control the size of material and prevent clogging. Task train persons to recognize all potential hazardous conditions and to understand safe job procedures for elimination of the hazards before beginning work. Before working on or near equipment, ensure that the discharge operating controls are deenergized and locked out. Wear a safety harness and lanyard, which is securely anchored and tended by another person, prior to entering bins, hoppers, tanks, or silos.

Mountain Stone - Northrup Quarry METAL/NONMETAL MINE FATALITY - On September 19, 2013, a 32-year old laborer with 14 years of experience was killed at a dimension stone operation. The victim was operating a 2½ ton truck up a steep roadway. He was hauling water tanks in the bed of the truck when the load shifted and the truck overturned, crushing him.

The victim was standing on a step ladder, pulling cable in a cable tray. The mounting bracket for the tray broke loose from the wall and the tray struck the step ladder. The victim fell 5 feet from the ladder, striking his head on the concrete floor. The victim was transported to a hospital where he died on October 19, 2013.

Ensure that the correct anchors are used and that the supporting structures are adequate when installing a cable tray. Always be aware of your surroundings and any hazards that may be present. Properly position ladders used to reach elevated areas.

The victim was operating a haul truck when it veered off the left side of a haul road and traveled through a berm. The haul truck went over an embankment and overturned in a settling pond.

40 Explosives/Breaking Agents Colorado Gold Ore Star Mine Operations, LLC - Revenue Mine METAL/NONMETAL MINE FATALITY - On November 17, 2013, a 33-year old powderman trainee with 5 weeks of experience and a 59-year old shift supervisor with 36 years of experience were killed at a silver mine. The two miners were in an area of the mine where explosives had been detonated the day before. Other miners working in the area were able to evacuate. Mine rescue teams entered the mine, found the two victims, and brought them to the surface. During the recovery operation, rescue teams detected fatal levels of carbon monoxide. Twenty miners were taken to the hospital and three were kept overnight.

Ensure all active working areas are ventilated prior to allowing miners to work in those areas. Monitor gasses as frequently as necessary to determine the adequacy of control measures. Use properly maintained and calibrated gas detection instruments with alarms for concentrations outside of safe limits that are audible and visual. Ensure all miners are trained to recognize all potential hazards and emergency procedures, including evacuation procedures. Dispose of damaged or deteriorated explosive material in a safe manner in accordance with the instructions of the manufacturer.
43 Electrical Kentucky Crushed, Broken Limestone Pine Bluff Sand & Gravel Company - Cumberland River METAL/NONMETAL MINE FATALITY - On November 18, 2013, a 33-year old contract electrician foreman with 14 years of experience was injured at a crushed stone mine. The victim was working in a 480-volt electrical enclosure, pulling cable for a new pump that was being installed, when he contacted energized conductors. He was transported to a hospital where he died on November 22, 2013.
44 Electrical Kentucky Crushed, Broken Limestone Pine Bluff Sand & Gravel Company - Cumberland River Best Practices Ensure that persons are trained on all electrical tests and safety equipment necessary to safely test and ground the circuit where work is to be performed. Positively identify the circuit on which work is to be conducted. De-energize power and ensure that the circuit is visibly open for circuits being worked on and circuits near the work area. Lock and Tag! Place YOUR lock and tag on the disconnecting device. Use properly rated Personal Protective Equipment (PPE) including Arc Flash Protection such as a hood, gloves, shirt, and pants. Ensure ALL electrical components in the enclosure are de-energized by testing for voltage using properly rated test equipment. Install warning labels on the terminal covers of bottom feed circuit breakers warning that "Bottom terminal lugs remain energized when the circuit breaker is open."

METAL/NONMETAL MINE FATALITY - On December 4, 2013, a 63-year old lead man with 16 years of experience was killed at a crushed stone mine. The victim initiated a blast and was struck by flyrock from the blast. He was standing 153 feet from the nearest blast hole and was struck by rock as large as 19 inches long by 14 inches wide by 7 inches thick.

Guard or barricade all access routes to the blasting area to prevent the passage of persons or vehicles. Before firing a blast, give ample warning to allow all persons to be evacuated. Clear and remove all persons from the blasting area unless suitable blasting shelters are provided to protect persons from flyrock. Verify that the blasting procedures are effective and being followed at all times.
47 Slip/Fall of Person Michigan - Iron Ore Tilden Mining Company LLC - Tilden Mine METAL/NONMETAL MINE FATALITY - On December 6, 2013, a 61-year old reagent handler with 39 years of experience was killed at an iron ore mine. The victim was working on top of a snow and ice covered railroad tanker car when he fell to a concrete floor approximately 12 feet below.
48 Slip/Fall of Person Michigan - Iron Ore Tilden Mining Company LLC - Tilden Mine Best Practices Establish and discuss safe work procedures. Identify and control all hazards associated with the work to be performed and the methods to properly protect persons. Task train all persons to recognize all potential hazardous conditions and ensure they understand safe job procedures for elimination of the hazards before beginning work. Remove snow and ice from work areas. Always use fall protection with a lanyard anchored securely when working where there is a danger of falling.
49 Falling/Sliding Material Georgia Construction S/G Brown Brothers Sand Company - Witched Stephen METAL/NONMETAL MINE FATALITY - On December 13, 2013, a 53-year old utility worker with 19 years of experience was killed at a sand mine. The victim was standing near the edge of a bank when it collapsed engulfing him.
50 Falling/Sliding Material Georgia Construction S/G Brown Brothers Sand Company - Witched Stephen Best Practices Establish and discuss safe work procedures before beginning work. Identify and control all hazards associated with the work to be performed and the methods to properly protect persons. Task train all persons to recognize all potential hazardous conditions that can decrease bank or slope stability and ensure they understand safe job procedures for elimination of the hazards. Evaluate all pit, highwall, slope, and bank conditions daily. Be especially vigilant for these conditions after each rain, freeze, or thaw. Slope trenches back at a stable angle or install shoring when working in and around trenches. Correct hazardous conditions by working from a safe location.
51 Other (Drowning) Kentucky - Construction S&G Hunter Sand & Gravel, LLC. - Dredge IV METAL/NONMETAL MINE FATALITY - On December 10, 2013, a 27-year old deck hand with 4 years and 8 months of experience drowned at a dredge operation. He was working on a dredge that had a barge attached to it. The victim stepped on the barge and fell into the water.

Always wear a life jacket where there is a danger of falling into the water. Remove snow and ice from work areas. Ensure safe access is provided where persons are required to work or travel. Maintain three points of contact. Install and use lifeline tie-off runs and fall protection. Provide communication devices and establish procedures requiring persons to alert coworkers when they are outside the dredge s handrails.
