4 DIVISION I GENERAL PROVISIONS 1.0 Purpose These guidelines establish the procedures and standards implemented by the Hazardous Materials Unified Program Agency (HMUPA) for the permanent and temporary closure of underground storage tank (UST) systems in the City and County of San Francisco. 1.1 Regulatory Authority The Hazardous Materials Unified Program Agency (HMUPA) is the lead agency responsible for providing regulatory oversight for the construction, operation, repair and removal of the USTs in the City and County of San Francisco. The HMUPA performs all UST regulatory oversight activities in accordance with California Health and Safety Code, Chapter 6.7 (HSC); Title 23 of the California Code of Regulations (T23 CCR); and the San Francisco Health Code, Article 21 (SFHC). 1.2 Scope These guidelines apply to: (1) All persons who own and/or operate USTs in the City and County of San Francisco. (2) All person conducting UST closure activities in the City and County of San Francisco. 1.3 Definitions and Terms (b) (c) Closure In Place means the closure of a UST in accordance with Division IV, paragraph 4.3 of these guidelines and the UST is left in its original location. Closure means any activity relating to either the temporary or permanent decommissioning of a UST. Combustible Gas Meter means a meter that has been properly calibrated and is capable of, when equipped with an appropriate dilution fitting or dual pump, accurately displaying the lower explosive limit (LEL) and oxygen level in an UST. The combustible gas meter must be capable of accurately displaying the LEL in an oxygen deficient environment

5 (d) (e) (f) (g) (h) (i) (j) (k) (l) (m) (n) (o) (p) (q) (r) (s) (t) (u) Emergency Inspection means an inspection resulting from the unauthorized removal of an UST or an inspection resulting from a leaking and/or abandoned UST. Facility means any one, or combination of, USTs used by a single business entity at a single location or site. Groundwater means subsurface water. HMUPA means the Hazardous Materials Unified Program Agency Inert Atmosphere means the interior atmosphere of a UST that is below 10% of the LEL, as determined by a combustible gas meter. Inert Material any solid or slurry material approved by the implementing agency for filling UST. Inspector means an inspector of the HMUPA. Intrinsically Safe means a legal and accepted industry practice or piece of equipment which has been certified as safe for the intended application and does not increase the risk of fire or explosion or endanger public health or the environment. Licensed Contractor means a contractor licensed by the California Contractor State Licensing Board. Lower Explosive Limit or LEL means the lowest concentration of a flammable gas or vapor (percent by volume in air) in which an explosion can occur upon ignition in a confined area. LOP means the Local Oversight Program within the San Francisco Department of Public Health Environmental Health Section or its successor program. Non-Routine Inspection means inspection scheduled outside of normal business hours. Normal Business Hours means the hours between 8:00 am and 5:00 pm, Monday through Friday excluding legal holidays. Notice of Completion means the notice issued by the implementing agency indicating that the closure of the UST in compliance with all applicable federal, state and local laws and regulations. ppb or ug/l means parts per billion. ppm or mg/l means parts per million. Removal means the closure of a UST and/or its associated piping where the UST and/or piping is removed from the ground and disposed or recycled. Rinsate means any byproduct resulting from the cleaning or washing of the interior surface of an UST and associated piping

6 (v) (w) (x) (y) (z) (aa) (bb) Routine Inspection means an inspection scheduled and conducted during normal business hours. Scrap Metal means metal generated as a result of any activities affecting a UST system that is not considered hazardous waste and may be recycled. Sludge means any solid, semi-solid or liquid waste. Spill means an uncontrolled release of a hazardous material. Temporary Tank Closure means the closure of a UST where the UST is closed in accordance to Section 2671 of Title 23 of the California Code of Regulations. Temporary Closure Permit means a permit issued for temporary tank closure. Unauthorized Movement means the deliberate movement of the UST within its tank bed (original position) without authorization from the implementing agency or San Francisco Fire Department. Unauthorized movement includes, without limitation: (1) pushing the UST in a sideways direction with equipment such as a backhoe or other prying device; (2) lifting the UST from its original position with equipment such as a backhoe, crane, or other hoisting device; (3) removing soil from around the UST in such manner that causes the UST to roll or move without external force other than gravity or buoyancy of the tank in water; (4) rocking or jiggling the UST to loosen surrounding soil; (5) changing the shape of the UST by means such as pushing or caving in the top or side of the UST, gouging out a section of the UST or cutting of the UST.; (6) tilting the UST from its original position by pushing or lifting one end, except where the UST is used to hold heating oil and the UST is tilted no more than a 10 degree angle. (cc) (dd) (ee) UST means underground storage tank. For the purposes of these guidelines, UST shall include home heating oil tank. UST Closure Report means a report submitted to the implementing agency by the owner/operator or their authorized agent following the closure of a UST. The UST closure report shall include: (1) a description of the site location, (2) information on the site background, (3) a site map, (4) copies of all applicable permits, (5) a chronology of the tank removal activities, (6) laboratory analytical results, (7) all disposal documentation, and (8) a summary of the site restoration. Underground Storage Tank System means a UST, connected piping, ancillary equipment, and containment system, if any

7 1.4 Contractor Requirements Any work to permanently or temporarily close an USTs costing $300 or more shall be performed by a contractor licensed by the California Contractors State Licensing Board holding a license in one of the following classifications and subject to the following restrictions: (1) General Engineering Contractor (A) - General engineering contractors may work on any UST for any purpose whatsoever at any location (2) Plumbing Contractor (C-36) - Plumbing contractors may work on any underground storage tank that provides a service to a building. This includes storage tanks for service stations. Any other type of underground storage tank may only be worked on by a General Engineering Contractor (A) (3) Limited Specialty Contractor (C-61/D-40) - Service station equipment contractors may work on fuel USTs at service stations or any other site where storage capacity of each tank does not exceed 20,000 gallons and In addition to possessing one of the above licenses, a contractor performing UST closure work must also possess a Hazardous Substance Removal Certification issued by the California Contractors State Licensing Board. DIVISION II HMUPA APPROVAL AND OTHER AGENCY PERMITS 2.0 Approval Requirements and Fees Before cleaning, modifying, cutting, dismantling, closing or removing a UST system, the owner/operator or his or her agent shall obtain approval for UST work from the HMUPA by filing a completed UST Closure Application. A completed UST Closure Application shall include the following: (1) UST Closure Application (2) UST Facility Form (3) UST Form (4) Site Map indicating: - 6 -

8 (A) the actual or proposed location of the UST relative to a fixed structure; (B) the actual or proposed location of the UST fill pipe; (C) the actual or proposed orientation of the UST; (D) North directional arrow; (E) street and cross street (5) Applicable application fee as specified in Article 21 of the San Francisco Health Code; (6) Except as provided, an approval for UST work shall be issued within ten (10) working days after the submission of a complete UST Closure Application. The approval shall be valid for ninety (90) days from the date of approval. Upon the expiration of the approval, all authorized activities set forth in the approval shall cease until the owner/operator or his or her agent obtains additional approval from the HMUPA. (7) Except as otherwise specified, all outstanding fees are the responsibility of the owner/operator and are due and payable upon receipt of invoice. Failure to pay any outstanding fees may result in the withholding of a Notice of Completion or a permit to operate until all outstanding invoices have been paid. (8) Failure to comply with these guidelines may result in the revocation of the approval for UST work and assessment of penalties as provided by law. 2.1 Monitoring Well Permit Owner/operator or his or her agent shall obtain a Soil Boring and/or Monitoring Well Installation Permit from the San Francisco Department of Public Health before conducting soil borings or installing monitoring wells. 2.2 Other Agency Permits and Notification The issuance of an approval for UST work shall not be deemed to excuse the Owner/Operator or his or her agent from the requirements of other laws. The validity of any approval issued by the HMUPA is conditioned on the Owner/Operator or his or her agent obtaining all applicable permits for the intended scope of work including, without limitations, permits from the Department of Public Works, San Francisco Fire Department and Department of City Planning (if applicable)

9 DIVISION III SCHEDULING OF INSPECTIONS AND AGENCY NOTIFICATIONS 3.0 Scheduling an Inspection Scheduling of inspections is subject to inspector availability and shall be scheduled as follows: (1) Routine inspections shall be requested at least three (3) working days in advance of the desired time for the inspection. (2) Non-Routine inspection shall be requested at least ten (10) working days in advance of the desired time for the inspection. (3) Emergency inspection shall be scheduled immediately upon the discovery of an emergency. The owner/operator shall be liable for any extraordinary costs incurred. 3.1 LOP Notification The Owner/Operator shall notify the LOP of any UST closure, when the closure activity is part of an ongoing unauthorized release investigation or remediation. Such notification shall be at least 48 hours before any closure activity. DIVISION IV UST CLOSURE ACTIVITIES 4.0 Onsite Cleaning of USTs The following procedures shall be followed for the onsite cleaning of a UST: (b) The San Francisco Fire Department shall approve in advance, any onsite cleaning of a UST that involves cutting the UST. Work on USTs that held flammable liquids shall be performed under the supervision of persons who understand the fire and explosion potential involved. Potential hazards associated with the closure of a UST that held flammable liquids shall be addressed in the Site Health and Safety Plan

10 (c) (d) (e) (f) For USTs that might be under pressure, the pressure in the UST shall be reduced to atmospheric by venting the UST at a safe location, before cleaning work is started. Cleaning operations shall be conducted in compliance with all applicable regulatory requirements. Including but not limited to: the San Francisco Fire Department, California Air Resources Board and Cal-OSHA A vacuum truck or an alternate intrinsically safe evacuation method shall be used to remove sludge, loose scale, debris and rinsate. All evacuation methods must be disclosed on the UST Closure Application and are subject to HMUPA approval All hazardous waste (sludge, loose scale, debris, residue, rinsate, etc.) generated during the UST cleaning and closure process shall be managed in accordance with all applicable hazardous waste regulations. 4.1 Management Procedures for Classifying the UST and Associated Piping as Nonhazardous All UST systems formerly used to store a hazardous material shall be handled in accordance with applicable hazardous waste disposal requirements unless all of the following conditions are met: (b) (c) The UST piping and appurtenances are free of product, sludge, rinsate and debris to the extent no material can be poured or drained from them when held in any orientation. All interior surfaces of the UST shall be visually free of all product, sludge, rinsate and debris to the extent that no material can be poured, drained, or scrapped from it when held in any orientation. Access for visual inspection of the UST shall be provided through existing manholes or through holes cut into the UST wall to allow for visual inspection of the entire UST interior, without the need to physically enter the UST. (1) If access for visual inspection is not provided, an intrinsically safe visual recording device shall be lowered into the UST to provide visual inspection of the UST s interior surfaces. (d) If the HMUPA agency determines that it is not feasible to visually inspect the UST, a sample shall be collected from the final rinsate and analyzed by a State Certified laboratory for the constituents previously contained in the UST. The UST may be treated as a solid waste if: (1) the analytical results from the final rinsate is less than 10 ppb for benzene, and less than 100 ppm of TRPH, TPH-D, or TPH-G, whichever is applicable; and - 9 -

11 (2) the concentration of flammable vapors in the UST interior is less than 5% of the LEL while the oxygen concentration is the same as that of the ambient air at approximately 20.8% when measured at the top, center and bottom of the UST. 4.2 Preparation and Removal of USTs Except as otherwise provided, all USTs used to store hazardous material shall be prepared for closure as follows: (1) The UST, associated piping and appurtenances shall be drained and shall be free of product, sludge, rinsate and debris to the extent that no material can be poured or drained from them when held in any orientation (e.g., tilted, inverted, etc.) All residual liquids, solids, or sludge shall be removed and handled as hazardous wastes or recyclable materials in accordance with Chapter 6.5 of Division 20 of the California Health and Safety Code. (2) If the UST is cut to facilitate cleaning, vapor removal and/or inspection, access shall be provided as follows: (A) Through manholes which either currently exists in the UST or one newly installed in the UST wall, or (B) Through holes cut into the UST wall so as to allow for visual inspection of the entire UST interior, without the need to physically enter the UST. (3) If the UST was used to store a flammable or combustible material, a minimum of 30 pounds of dry ice per 1,000 gallons of UST volume shall be used to inert the interior atmosphere of the UST. (A) The interior atmosphere of the UST shall be measured at the top, center and bottom of the UST. The UST shall be inerted to a level designated by the San Francisco Fire Department or to a level that is lower than 10% of the LEL for the hazardous material previously contained therein as measured by a combustible gas meter. (4) The area containing the UST shall be adequately excavated to allow for ease of removal of the UST. Excavated material shall be stockpiled in a specific and secured area of the site until a hazardous waste determination has been performed. Such material may be stockpiled and stored in a dumpster within a block radius of the site. (5) The interior atmosphere of the UST shall be adequately inerted and the UST shall be ready to remove at the time of the scheduled inspection. The UST shall not have undergone unauthorized movement

12 (6) The UST shall be removed from the excavation trench by adequately securing it to an excavator, crane or other lifting device capable of bearing the UST s weight. (A) The UST shall be lifted from the excavation in a manner that ensures that the UST will not be dragged, pushed, or otherwise move in a manner that may generate friction or sparks or damage underground utility lines. (7) Once removed, the UST shall be placed on the ground in a safe place and secured/blocked to allow for inspection of all external surfaces of the UST, and product piping, as well as the soil conditions in the excavation. Results of such inspection shall be documented in writing and with photographs where appropriate. (8) The UST shall be loaded onto the hauler s truck and properly secured in the presence of an Inspector. (A) If the tank meets the definition of a hazardous waste set forth in Title 22 of the California Code of Regulations, then it must be transported under a Hazardous Waste Manifest. The Hazardous Waste Manifest must be available for review at the time of the inspection. (B)Prior to transporting, all openings in the UST shall be plugged except for a 1/8-inch vent for pressure relief. The pressure relief opening shall be oriented at the top when the UST is loaded for transportation. C)The UST shall be secured either by straps or other means to prevent the UST from rolling off the truck. (D)The UST may not extend off the bed or trailer of the truck by more than a third of its length and width. If any part of the load protrudes 4 feet or more from the rear of the trailer bed, the truck shall be rigged with a red flag. (b) All piping associated with the UST shall be removed unless removal might damage structures or other pipes that are being used or are contained in a common trench, in which case the piping to be closed shall be emptied of all contents, flushed, and capped

13 4.3 UST Closure in Place Before initiating a closure in place of a UST, the owner/operator or his or her agent shall: (1) Obtain an Abandonment in Place Permit from the Department of Public Works if the UST is located under a public right of way; (2) Demonstrate to the satisfaction of the implementing agency that no unauthorized release has occurred and the site has not been contaminated from the UST system intended for closure in place. If petroleum hydrocarbon contamination exist at the site, the UST may not be closed in place without prior approval from the LOP. (A) The verification shall be based on soil and or groundwater samples (if applicable) collected beneath the bottom level of the UST. The location and number of samples collected shall be pre-approved by the HMUPA. (B) The collection of all required samples shall be witnessed by a HMUPA inspector and shall be collected in accordance with the protocol provided in the Sampling section of these guidelines. (b) Prior to receiving final approval to fill the UST, the applicant must submit the following documents to the HMUPA: (1) A soil and or ground water sampling map and boring log indicating the location and depth of soil and or ground water samples collected in or around the UST pit. (2) A copy of the Boring Permit obtained from the DPH, Environmental Health Management - Monitoring Well Program. (3) A copy of the Abandonment in Place Permit or letter of intent to issue said permit from the Department of Public Works. (4) A copy of all laboratory report(s) containing soil and groundwater analysis of any samples collected to verify the condition of the soils and or groundwater in the vicinity of the UST. (A) All laboratory reports of soil and ground water sample analysis shall be submitted on laboratory letterhead. (B) All laboratory reports must include a Chain of Custody document and QA/QC report. (c) A UST system used to store hazardous materials shall be emptied and cleaned in accordance with the protocol specified in the Management Procedures for Onsite Cleaning of UST section of these guidelines such that the UST system is no longer considered as hazardous waste

14 (d) (e) (f) All associated piping shall either be (1) removed in accordance with the specifications outlined in paragraph (b) of the Preparation and Removal Requirements section of these guidelines or (2) adequately flushed, and plugged or capped. Upon receiving the final approval of the implementing agency, the UST shall be filled by gravity feed unless otherwise approved by the HMUPA, with inert solid material in the presence of an inspector. The fill material shall occupy the entire volume of the UST or as much of the UST volume as is deemed practical by the implementing agency. Any person seeking to remove a UST that was previously closed in place shall notify and obtain approval from the HMUPA before performing such removal. 4.4 UST Temporary Closure Persons choosing to temporarily close a UST shall comply with all of the following requirements to complete and maintain temporary closure of the tank: (1) All residual liquid, solid, or sludge shall be removed from the UST and handled in accordance with the applicable provisions of Chapters 6.5 and 6.7 of Division 20 of the California Health and Safety Code. (2) The UST may be filled with a nonhazardous liquid. At the end of the temporary closure period, the liquid shall be tested for the presence of petroleum hydrocarbons and/or other contaminants. The test results submitted to the HMUPA prior to removing the liquid from the UST. (3) Except for required venting, all UST fill and access locations and piping shall be sealed using caps or concrete plugs. (4) Power service shall be disconnected from all pumps associated with the use of the UST unless the power services some other equipment which is not being closed, such as an impressed-current cathodic protection system. (b) (c) The UST monitoring required in accordance with the permit to operate shall remain in effect unless such requirements are modified by the temporary closure permit. The owner/operator shall inspect the UST, at least once every three months, to verify that the temporary closure requirements set forth in the temporary closure permit are met. A written record of the UST inspections shall be maintained by the owner/operator. The inspections shall include but are not limited to the following: (1) A visual inspection of all locked caps and concrete plugs. (2) If locking caps are used, at least one shall be removed to determine if any liquids or other substances have been added to the underground storage tank or if there has been a change in the quantity and/or the condition of liquid added pursuant to subsection (2) of this section

15 (d) Upon the expiration of the temporary closure period of the UST, the owner/operator shall: (1) obtain a permit to operate in accordance with Article 21 of the San Francisco Health Code or (2) obtain an extension to the valid dates of the temporary closure period for the UST, or (3) obtain an approval to permanently close the UST. (e) (f) A person seeking to permanently close a UST that is temporarily closed shall obtain prior approval from the HMUPA by submitting a UST Closure Application. The application fee required for permanent closure approval shall be waived if the tank is permanently closed prior to the expiration period of the temporary closure permit. All new and existing UST systems that have been temporarily closed shall comply with (1) repair and recordkeeping requirements, (2) release reporting and investigation requirements, and (3) release response and corrective action requirements specified in Article 21 of the San Francisco Health Code; Title 23 of the California Code of Regulations; and Chapter 6.7 of Division 20 of the California Health and Safety Code. DIVISION V SAMPLING 5.0 General (b) Soil and/or water samples shall be collected at the direction of and witnessed by an Inspector either during or immediately following the UST closure activities. Persons collecting samples or analyzing samples shall follow soil/ground water sampling and analytical procedures detailed in the Leaking Underground Fuel Tank (LUFT) Manual and the Tri-Regional Board Staff Recommendations for Preliminary Evaluation and Investigation of Underground Tank Sites. 5.1 Sample Collection for UST Removals (b) The Inspector shall approve the sampling method, sampling locations and number of samples required. Soil sample collection for USTs that contained flammable or unknown materials shall be conducted as follows:

16 (1) Undisturbed soil samples shall be obtained in native soil a minimum of two feet beneath the bottom of the former UST location whenever possible. Undisturbed soil samples shall be collected and transported in a manner that prevents the lost of volatile organic compounds. (2) Soil samples shall be collected by driving a tube type sampler or augering an auger type sampler directly into native soil at the desired location in the UST pit. (3) If soil samples can not be taken from the tank pit due to personal safety concerns then the soil samples may be taken from soil removed from the pit with a backhoe or similar device. Soil samples from the backhoe bucket shall be collected by driving a clean brass sampling tube into the soil contained in the backhoe bucket with a rubber mallet or other suitable device (4) Soil samples shall be collected in clean brass tubes at least three inches long. The sampling tubes shall be completely filled and immediately covered on both ends with parafilm, Teflon, aluminum foil, or similar material designed to prevent loss of volatile compounds from the sample. The sampling tubes shall be capped and reinforced with duct tape or an appropriate substitute. (c) Soil sample collection for removed USTs that contained only combustible or nonflammable materials such as heating oil, diesel fuel, or halogenated solvents shall be conducted as follows: (1) Soil samples shall be obtained from native soil a minimum of two feet beneath the bottom of the former UST whenever possible. (2) Soil samples shall be collected by driving a tube type sampler, by augering an auger type sampler, by using of a clamshell shovel, or by using of a trowel directly into native soil at the desired location in the pit or from soil contained in a backhoe bucket. (3) Soil samples shall be collected in clean brass tubes at least three inches long. The sampling tubes shall be completely filled and immediately covered on both ends with parafilm, Teflon, aluminum foil, or similar material designed to prevent loss of volatile compounds from the sample. The sampling tube shall be capped and reinforced with duct tape or an appropriate substitute. (d) If water is not present in the tank pit, the location and number of samples collected shall be as follows: (1) For USTs with a capacity less than 1,000 gallons, a minimum of one soil sample per tank shall be collected beneath the fill or pump end of the former tank

17 (2) For USTs with a capacity between 1,000 gallons and 10,000 gallons, a minimum of two samples, one beneath each end of the former tank, shall be collected. (3) For USTs with capacity greater than 10,000 gallons, a minimum of three samples, one beneath each end and the midpoint of the former tank, shall be collected. (4) For a UST cluster, a minimum of four samples, at the locations designated by the Inspector. (5) One pipeline sample shall be collected for every 20 linear feet of piping present at a depth inches beneath the piping or as directed by the Inspector. (6) If obviously stained or contaminated areas are detected in locations other than the specified locations, additional soil samples shall be collected from the stained or contaminated areas at the direction of the Inspector. (e) If water is found in the UST pit, soil and water samples shall be collected as follows: (1) The location and number of samples collected shall be as follows: (A) For USTs with a capacity less than 10,000 gallons, a minimum of two soil samples from the side walls next to UST ends at the soil/ground water interface and one water sample. (B) For USTs with a capacity greater than 10,000 gallons or a UST cluster, four soil samples, one from each side wall of the UST excavation, at the soil/ground water interface and one water sample. (2) Whenever possible, water samples shall be collected in a device designed to reduce the loss of volatile components such as a bailer with a sampling port. Immediately after collection, the water shall be transferred into clean liter size glass bottles and volatile organic analysis (VOA) vials. The transfer of the water sample to the sample containers shall occur with as little agitation as possible. The sampling containers shall be completely filled and topped off. A Teflon septum shall be used to seal the vial. The sealing of all sample containers shall be reinforced with duct tape or an appropriate substitute. (A) Prior to collecting a water sample from the UST pit, the existing water in the UST pit shall be completely purged and a sample collected from the resulting recharge. (B) The water sample shall be collected from the surface of the UST pit water as well as approximately inches below the water surface (if that depth is obtainable)

18 (f) Stockpiled soil intended for reuse in the UST pit shall be sampled as follows: (1) Stockpile soil samples shall be obtained by driving a clean brass sampling tube at least 3 inches long into the stockpile soil. Six inches of soil from the surface of the sampling area shall be removed immediately before sampling. The sampling tubes shall be completely filled and immediately covered on both ends with parafilm, Teflon or aluminum foil, and capped and reinforced with duct tape or an appropriate substitute. (2) A minimum of one, 4 point composite sample shall be collected for every 20 cubic yards of stockpiled soil. At the discretion of the Inspector, the sample may be composited at the laboratory or at the time of collection. (g) Stockpiled soil intended for disposal offsite shall be sampled in accordance with the requirements of the intended disposal facility. 5.2 Sample Collection for UST Closure in Place (b) UST closure in place requires the drilling of site-specific boreholes as directed by the HMUPA. Boreholes typically extend to a depth of two feet below the bottom of the UST and are located near the UST. Soil samples can be collected during the drilling of the borehole at intervals specified by the HMUPA. (1) Soil samples shall be collected in clean brass tubes at least three inches long. The sampling tubes shall be completely filled and immediately covered on both ends with parafilm, Teflon, aluminum foil, or similar material designed to prevent loss of volatile compounds from the sample. The sampling tubes shall be capped and reinforced with duct tape or an appropriate substitute. (c) If groundwater is encountered, groundwater samples shall be collected after the borehole is finished at depths and times specified by the HMUPA. (1) Whenever possible, water samples shall be collected in a device designed to reduce the loss of volatile components such as a bailer with a sampling port. Immediately after collection, the water shall be transferred into clean liter size glass bottles and volatile organic analysis (VOA) vials. The transfer of the water sample to the sample containers shall occur with as little agitation as possible. The sampling containers shall be completely filled and topped off. A Teflon septum shall be used to seal the vial. The sealing of all sample containers shall be reinforced with duct tape or an appropriate substitute. 19 5.3 Sample Processing Immediately following collection, all samples shall be processed as follows: (b) (c) (d) Samples shall be labeled or marked with a sample ID #, location, depth, collection date, collection time and collector s initials. Samples shall be sealed with an appropriate sealing tape. Samples shall be recorded on a formal analysis request/chain of custody. Samples shall be placed in a cooler, on ice (or equivalent) at approximately 4 degrees centigrade for holding and transportation. 5.4 Sample Analysis (b) The Inspector at the site shall determine the analyses to be requested. At a minimum, the analyses requested shall follow the guidelines outlined in Table 1: UST Soil and Groundwater Sample Analysis Chart. Soil and water samples shall be analyzed by a state certified laboratory using methods that provide quantitative results and are capable of meeting the following Practical Quantitation Reporting Limits: SOIL (PPM) WATER (PPB) TPH G TPH D BTEX O & G ,000.0 (1) Lower detection limits shall be verified by the laboratory. (2) If the Practical Quantitation Reporting Limits are not achievable, an explanation of the problem shall be included in the laboratory analytical report. (c) Appropriate standards for the material formerly stored in the UST shall be used for all sample analysis

20 (d) (e) Signed laboratory data sheets shall be submitted which include the laboratory s assessment of the condition of the samples on receipt including temperature, suitable container type, air bubbles present/absent in VOA bottles, and proper preservation. The data sheets shall also include the dates the samples were collected, submitted, prepared for analysis, and analyzed. At the direction of the HMUPA, the testing laboratory shall identify peaks that do not conform to the standards, including any unknown compounds that are expressed due to elution (extraction by use of a solvent)

23 DIVISION VI UST CLOSURE REPORT AND NOTICE OF COMPLETION (NOC) 6.0 UST Closure Report Requirements Within sixty (60) days of the permanent closure, either by removal or closure in place, of a UST, the owner, operator or authorized agent shall submit a report summarizing the UST closure activities. The report shall include but is not necessarily limited to the following: Site Background (1) Site address and location. (2) Description of the premises: industrial, commercial or residential including number of units. (b) UST Information (1) UST history that includes past use of UST system and summary of monitoring history. (2) Description of UST: volume, dimensions, material, age and contents. (3) Summary of any previous investigative activities and findings. (c) Site Map (1) Plot plan indicating location of former UST system and sample collections. (2) Stockpile location. (d) Copies of all applicable permits from (1) the HMUPA (2) San Francisco Department of Public Works/Bureau of Street-Use and Mapping (if applicable) (3) San Francisco Fire Department. (4) Bay Area Air Quality Management District. (e) Chronology of Closure Activities (1) Pre and post closure procedures

24 (2) Description of condition of UST system (rust, pits, holes etc. present). (3) Description of UST pit soil condition and site lithology. (4) Photographs of UST and any other pictures, if applicable. (5) Description of UST vault construction and condition, if applicable. (6) Depth at which groundwater was encountered and condition of groundwater (sheen, free product etc. present), if applicable. (7) Detail description of any over excavation activities, if applicable. (f) Laboratory Analytical Results (1) Chain of Custody for all samples analyzed. Chain of Custody must indicate the condition of samples upon receipt by laboratory. (2) Soil/Ground Water Sampling Data Form. (3) Certification of Analysis and Quality Control (QC)/Quality Assurance (QA) signed by Laboratory Director. (g) Disposal Documentation (1) Uniform Hazardous Waste Manifest for all hazardous waste disposals. (2) Bill of Lading/Soil Disposal Tags for all non-hazardous waste soil disposals. (3) UST Certificate of Destruction or Receipt for disposal as scrap metal. (h) Site Restoration (1) Date of site restored. (2) Procedures and materials used. (i) UST Closure/Modification Inspection Record (SFDPH Job Card). 6.1 Issuance of Notice of Completion (NOC) The HMUPA shall issue a Notice of Completion (NOC) for all sites for which a satisfactory UST Closure Report has been submitted and no evidence of a leak from the removed UST of materials formerly stored therein provided that there are no outstanding violations, enforcement orders, fines, penalties or fees assessed against the owner/operator. The HMUPA shall refer to the LOP for additional investigation and remediation all sites for which evidence suggests the contamination of the subsurface of the site
