1 HOW TO READ A 39 MONTH UNCERTIFIED MOTOR VEHICLE RECORD This document is intended to explain how to read a 39 Month Uncertified Driver License Motor Vehicle Record (MVR). The table below is a sample Motor Vehicle Record with section numbers and a short description listed on the left. The detailed description of each section is included on the following pages. 2 DETAILED SECTION DESCRIPTIONS OF A 39 MONTH UNCERTIFIED MOTOR VEHICLE RECORD Section 1: This section displays customer information, including the customer s name, address and current license information. Driving restrictions will display when applicable (i.e. corrective lens). * Section 2: - This section displays endorsements when applicable (i.e. motorcycle). Section 3: - This section lists the history of additional licenses, identification licenses, and/or permits if applicable.* Section 4: - This section lists previous license information (if applicable).* Section 5: - This section lists all pending or open status actions when applicable (i.e. mandatory insurance requirements, court ordered suspensions for noncompliance, MVD, court, etc.). A table of the codes that may appear on your MVR is included below. The table lists the status code number, a short label description, and a full description for each status code that may appear on your MVR. *In section 1, 3 and 4 the TID field indicates if a customer s credential is voluntary travel compliant (Y) or non-compliant (N). Status Code Status Code Label Status Code Definitions 02 Suspension Driver license and driver s privilege to drive a motor vehicle on the public highways and streets of this state are temporarily withdrawn during the period of the suspension and until application for reinstatement is made. 03 Potential ID Theft Possible ID Theft as a result of Tri West issue. 04 Implied Consent Suspension 05 Prohibit All Identification Card Services Driver license and privilege are temporarily withdrawn during the period of suspension and until application for reinstatement is made. Prohibited from obtaining an Identification card. 06 Revocation Driver license and driver s privilege to drive a motor vehicle on the public highways and streets of this state are terminated and shall not be renewed or restored, except that an application for a new license may be presented and acted on by the department. 09 NRVC Suspension - Failure to comply Non-Resident Violator Compact Driver license and driver s privilege to drive a motor vehicle are temporarily withdrawn based on an order of suspension for failing to appear on a civil violation in another state. 11 Assigned to TSS Assigned to and required to complete Traffic Survival School. 12 Pending action There is a pending corrective action against the driver license and driving privilege. 13 Mandatory Insurance Suspension (D/L only) 15 Suspension - MVD Action 16 Suspension - Court Action withdrawn. Refer driver to MVD to clear suspension. withdrawn for failure to pay on a civil traffic violation and until reinstatement fee is paid to MVD. withdrawn for failure to pay on a civil or criminal traffic violation and until proof of court satisfaction provided.
3 20 Admin Per Se Suspension withdrawn during the suspension period and until reinstatement requirements are met. The suspension is an administrative action and based on an affidavit received by law enforcement, which indicated an alcohol concentration of.08 or above. 23 Prohibited from Obtaining a Driver License/Permit 25 $50 Reinstatement Fee Due - Admin Per Se day Restricted Permit for AP/DUI Suspension Not able to obtain a driver license or permit. A reinstatement fee of $50 is due for an Admin Per Se Suspension action taken against the driver license or driving privileges. Permit has been restricted for 60 days as a result of an Admin Per Se or DUI Suspension. 34 License Canceled by MVD The annulment or termination of a driver license because of an error or defect or because the licensee is no longer entitled to the license. An application for a new license may be submitted for review by the department. 37 CDL Disqualification Driver s privilege to drive a commercial motor vehicle has been temporarily withdrawn for the time period indicated. Privileges can be reinstated after the disqualification period has ended and application for reinstatement is made; except in the case of a lifetime disqualification. 51 Identification Card Suspension 60 (CP) APS Suspension - Commercial Privileges Only 61 Ignition Interlock Device Identification card has been temporarily withdrawn during the period of the suspension and until application for reinstatement is made. Admin Per Se Suspension (Commercial Privilege only) Driver s privilege to drive a commercial motor vehicle has been temporarily withdrawn and until reinstatement requirements are met. Driver is required to have the Certified Ignition Interlock Device in any and all vehicles he/she drives; except if the vehicle is being used for commercial purposes and the driver is in possession of an employer notification form. 72 ID card SSN Cancellation Identification Card has been cancelled because Social Security Administration did not validate SSN. 82 Special Ignition Interlock Suspension/Revocation Restriction 89 Medical Suspension/Revocation - CDL Privileges Only (Temporary) 90 Medical Susp/Rev - CDL Privileges Only (Permanent) Driver is able to operate a motor vehicle during the suspension/revocation period but is required to have the Certified Ignition Interlock Device in any and all vehicles he/she drives; except if the driver is in an employer s vehicle and in possession of an employer notification form. Driver s privileges to drive a commercial motor vehicle have been suspended or revoked due to a medical condition. Suspension or revocation will remain in effect until cause is removed. Driver s privileges to drive a commercial motor vehicle have been suspended or revoked for failure to submit a DOT physical or additional requirements regarding the DOT physical and will remain in effect until cause is removed. 95 Cancellation for NSF Driver license, driving privileges or identification card have been cancelled as a result of non-sufficient funds for payment submitted.
4 Section 6: - This section identifies corrective actions and suspensions taken against your driving record. The action date, type of action, status of license and notifications status are all listed. They are as follows: Violation Actions ( A, , etc.) Violation Actions are reported to MVD by a court, law enforcement or an out-of-state jurisdiction. 1 st line: Activity Date (ACT DATE) which is the date of the traffic citation Violation code (VIOL CODE , DI, CS, FR ACTION) & Title of Statute 2 nd line: Type of disposition (status of the violation) 3 rd line: Name of county (LOC), Fine amount (FINE), Court code or Name (CRT) and speeds (LSP/ASP), if applicable 4 th line: Court docket number and vehicle plate number (PLT#), State registered in and vehicle make, if applicable Driver Improvement Actions (DI ACTION) DI ACTION is a suspension, revocation, cancellation or disqualification taken against your driver license or driving privileges. 1 st line: Activity Date (ACT DATE) which is the date a Driver Improvement Action begins (Effective from date), Violation code (VIOL CODE , DI, CS, FR ACTION) & Type of Driver Improvement Action Title 2 nd line: Internal MVD docket number, effective to date of action and if action is completed 3 rd line: Notification status (mailed, served, not applicable) & Date of service or mailing Financial Responsibility Action (FR ACTION) FR ACTION is a judgment and court ordered suspension for no proof of insurance. 1 st line: Activity Date (ACT DATE), Violation code (VIOL CODE , DI, CS, FR ACTION) & License suspended, vehicle suspended or license and/or registration (s) suspended & MVD internal docket number Continued on the next page.
5 Civil Sanction Action (CS ACTION) CS ACTION is a traffic complaint (civil sanction) suspension that has not been satisfied. If court not satisfied and MVD not satisfied 1 st line: Activity Date (ACT DATE) which is the date of the traffic complaint (civil sanction), Violation code (VIOL CODE , DI, CS, FR ACTION) & License suspended and citation number 2 nd line: Notification status which is always mailed (Date mailed) If court satisfied, but MVD not satisfied 1 st line: Activity Date (ACT DATE) which is the date of the traffic complaint (civil sanction), Violation code (VIOL CODE , DI, CS, FR ACTION) & Suspension satisfied, court satisfied date and citation number 2 nd line: Notification status which is always mailed (Date mailed) Traffic Survival School (TSS ACTION) TSS ACTION is an assignment to Traffic Survival School. 1 st line: Activity Date (ACT DATE), Violation code (VIOL CODE , DI, CS, FR ACTION) & TSS completed, permit number of authorized school.
