The are structured as follows: Minimum Requirements LORA Procurement Requirements Additional Requirements by Type Minimum Document Requirements Project/Site Specific Requirements Required for all forklifts and telehandlers irrespective of the owner (LORA, Hire Company, Supply Chain Partner) Required for all forklifts and telehandlers procured by LORA Required for all plant of that type (i.e. forklift or telehandler) Documentation required for all forklifts and telehandlers Project/site specific requirements for forklifts and telehandlers as determined by the project risk assessment FEATURE REQUIREMENTS MINIMUM REQUIREMENTS FORKLIFTS AND TELEHANDLERS FALLING OBJECT PROTECTION (FOPS) FITTED AND CERTIFIED FIRE EXTINGUISHER & BRACKET FLASHING AMBER BEACON (360 DEGREES) REVERSE / TRAVEL / SLEWING ALARM (BEEP OR SQUAWK AS PER CLIENT REQUIREMENTS) As required by relevant legislation in the jurisdiction of operation (refer Work Health & Safety Regulation 2011; Clause 214, 215, & 217 and WA Occupational Safety & Health Regulation 4.44 & 4.45) FOPS Compliance label must be permanently fitted to structure/cabin, easy to read and contain following information as minimum: Name and address of manufacturer or guard constructor Identification number of guard (if any) Machine make, model(s), or serial number(s) the guard is designed to fit Reference to the relevant standard the guard conforms to (i.e. ISO 3449), plus the performance level (i.e. Level 1) Any other information deemed relevant by the manufacturer. A minimum 4.5kg dry chemical (ABE) for all locations, within inspection/test requirements and charged. The extinguisher/s should be: Fitted in a secure, easily accessible location Fitted with legible instructions and coloured correctly (i.e. red with white band) Fitted with a pressure gauge and hose to assist accessing awkward areas Maintained in accordance with AS 1851 Minimum 1kg dry chemical (class ABE) shall be located in the cabin and clearly identified. Note - This is a minimum requirement. Specific requirements for extinguisher type and/or capacity may be defined by certain work activities or sites At least one amber beacon is fitted, with easily accessible on/off toggle switch to operate. The beacon shall be mounted as near as possible to the top of the equipment and be clearly visible in normal daylight up to a distance of 200 metres (and closing) in all directions. Fitted to central, unobstructed area of equipment. The alarm s noise level range shall be 87 to 112 db(a) measured at a distance of 1 metre from the alarm. The alarm shall active each time equipment movement is present (dependent on requirement and feature of alarm fitted i.e. reversing alarm) and not be fitted with an on/off feature. LOAD CHART DISPLAYED WITHIN CABIN WARNING SIGNAGE AFFIXED IN CABIN (FOR APPLICABLE EQUIPMENT) LOCKABLE BATTERY ISOLATION SWITCH C/W IDENTIFICATION DECAL EMERGENCY STOP/S (WITH DECAL) EASILY AND SAFELY ACCESSIBLE FROM OUTSIDE PLANT GUARDING (ENGINE, ROTARY PARTS, HEAT SHIELDS, SOUND CLADDING) SAFETY SIGNAGE/DECALS IN PLACE FOR SWL, ACCESS/PINCH POINTS, WARNINGS, EMERGENCY, ETC. A durable Rated Capacity (Load) Chart shall be provided that is accessible from the normal operating position. They shall be supplied by the OEM, relevant to the make/model of plant being operated and comply with AS 1418.19-2007, Section 3.3.2 Rated Capacity Chart. They shall include the following information: Rated Capacity working zone diagram Maximum permitted slopes when lifting (stationary) Maximum permitted slopes when travelling (pick and carry) Designated pick and carry position (on tyres) 600mm load centres on Forks on TH less than 10 tonne 600mm, 900mm, 1200mm load centres on forks on TH 10 tonne or greater Maximum in-service wind speed (in m/sec) Ground conditions applicable to rated capacity charts Tyres pressure / size Maximum travel speed NOTE - All attachment combinations must be approved for use on the TH and the appropriate load charts provided. Ensure overhead power line, underground cables and equipment minimum clearance height warning signage is affixed in cabin; clearly visible to operator. A means of immobilising the equipment by isolating the battery so the equipment cannot be started and/or operated by an unauthorised person. The isolation switch must be lockable, resistant to weather and clearly identified (i.e. isolator decal). Required Colour: Battery Isolator Red As per National Standard for Plant NOHSC: 1010 (1994), clause 14, sub section (a), (b) and (c) emergency stop devices must: (a) Be prominent, clearly and durably marked and immediately accessible to each operator of the plant; and (b) Have handles, bars or push buttons which are coloured red. (c) Not be able to be affected by electrical or electronic circuit malfunction. (d) Are of stop and Lock-off type: so that the plant cannot be restarted after an emergency unless the stop control has been reset. Location: Emergency stop devices shall be positioned for easy access and for nonhazardous operation by the operator. Guarding is in place, mitigating operator exposure to moving and/or hot parts and conform to National Standard for Plant NOHSC: 1010 (1994), clause 7, 8, 9 & 10 and, in addition, Select Plant Hire E-G-8-0533i Equipment Safe Guarding Standard. Safe Working Load limits (SWL), warning decals near crush/pinch points, emergency exit, etc. This section shall also comply with AS 1418.19-2007, specifically Section 3.3 Marking and including but not limited to sub sections: 3.3.3.1 Marking of lifting and tie-down points on the telehandler Locations for lifting and tie-down points on the telehandler shall be clearly indicated. 3.3.3.2 Filling points Filling points for fuel and hydraulic oil shall be clearly indicated on the telehandler using the symbols of AS 2359.5. 3.3.4 Representation of information Where possible, information shall be represented by symbols. Where the information is represented in words, it shall be written in plain English. Numerical representation shall be in metric units. HANDRAILS ON UPPER DECKS WHERE PERSONNEL ACCESS IS REQUIRED SAFE ACCESS ONTO MACHINES (THREE POINTS OF CONTACT) SEATBELT FITTED UHF RADIO AND AERIAL FITTED AND OPERATIONAL WHEEL CHOCKS WITH SECURE CARRYING MOUNTS (FOR RUBBER TYRED EQUIPMENT) LOAD MEASURING SCALES OR LOAD MONITORING SYSTEM ALL ATTACHMENTS APPROVED FOR USE BY OEM OR COMPETENT PERSON SLIDE ON JIB ATTACHEMENTS Suitably designed hand rails (edge protection) shall be fitted to prevent falls from height occurring when utilising upper decks. They shall be a minimum of 1m high, with gaps of less than 450mm between each mid rail, toe boards installed (minimum 100mm wide), manufactured from metal. Suitably designed access with hand rails shall be fitted to prevent trips and falls from height occurring when accessing plant and equipment. They shall be manufactured from metal. Shall be operational and meet Australian Standards. A sign should be prominently displayed in every cabin, warning the operator to wear the seatbelt provided. An operational UHF radio shall be fitted in the operator cab and be easily accessible. A compatible aerial shall be fitted securely to the equipment. Rubber and compatible with equipment size and requirements. Shall be secured to equipment with bracket to prevent unintended movement when not in use. Calibrated load scales or load monitoring system fitted, OEM or aftermarket design and compatible with the particular make and model of plant being operated. All attachments used with the telehandler shall be compatible with the machine. Only attachments approved by the manufacture or a competent person shall be used. (Definition of a competent person: Acquired through training, qualification, experience or a combination of these). E.g. Plant Assessor Telehandlers: Jib Attachments used for suspended loads are to provided by the Original Equipment Manufacturer (OEM) Attachment s with securing device e.g. Chain and Hook. This must be able to be positively secured e.g. with chain with serviceable self-locking hook. ADDITIONAL LORA PROCUREMENT REQUIREMENTS FORKLIFTS AND TELEHANDLERS ASSET IDENTIFICATION STICKERS (SELECT OWNED ASSETS ONLY) Asset Decals: Black lettering on a white background, non-reflective adhesive backed decal Suggested Size: 200 x 100mm Installation: Evenly applied to LH and RH side of asset ADDITIONAL REQUIREMENTS FOR FORKLIFTS AS2359.6 Powered Industrial Trucks, Section 6.3.1.2 Removable Attachments Removable attachments shall be marked legibly and indelibly (e.g. weather-proofed, profiled letters) with at least the following details: a) name and address of the attachment manufacturer or his authorized representative; b) model or type; c) serial number and year of manufacture; RATED CAPACITY INFORMATION PLATE ON ALL ATTACHMENTS d) mass of attachment, which may vary from the stated figure by up to 5 % or 200 kg, whichever is the lower of the two; e) distance of the centre of gravity of the attachment from its mounting face on the truck; f) rated capacity; g) in the case of hydraulically or pneumatically operated attachments, the maximum operating pressure recommended by the attachment manufacturer; h) load centre, if applicable; i) lost load centre distance; j) the instruction The capacity of the truck and attachment combination shall be complied with. ADDITIONAL REQUIREMENTS FOR TELEHANDLERS ROLL OVER PROTECTION (ROPS) FITTED AND CERTIFIED (PLACARD WITH SERIAL NUMBER) RATED CAPACITY INFORMATION PLATE ON ALL ATTACHMENTS DESIGN COMPLIANCE & INFORMATION PLATE TO AS1418.19 FITTED As required by relevant legislation in the jurisdiction of operation (refer Work Health & Safety Regulation 2011; Clause 214, 215, & 217 and WA Occupational Safety & Health Regulation 4.44 & 4.45) ROPS Compliance label must be permanently fitted to structure, legible and contain following information: Name and address of manufacturer Identification number/serial no. of guard (if any) Year of construction (i.e. year manufacturing process completed) Maximum vehicle mass for which ROPS meets performance requirements of the standard Vehicle make, model, or VIN the ROPS is intended Reference to the relevant standard to which the guard conforms Any other information deemed relevant by the manufacturer AS1418.19 Telehandlers, Section 3.3.1.3 Removable Attachments Removable attachments shall be marked legibly and indelibly with the following minimum particulars: (a) Name and the attachment manufacturer or authorized importer. (b) Model or type. (c) Serial number and year of manufacture. (d) Mass of attachment, which may vary from the figure shown by up to ±5%. (e) Distance of the centre of gravity of the attachment from its mounting face on the telehandler. (f) Rated capacity. (g) In the case of hydraulically or pneumatically operated attachments, the maximum operating pressure recommended by the attachment manufacturer. (h) The warning: THE RATED CAPACITY OF THIS ATTACHMENT MAY BE REDUCED REFER TO THE RATED CAPACITY CHART FOR THE TELEHANDLER/ATTACHMENT COMBINATION IN THE CABIN Section 3.3.1 Information plates (3.3.1.1 Telehandlers) Telehandlers shall be marked legibly and indelibly with at least the following: (a) Name and address of the manufacturer or authorised importer of the telehandler. (b) Designation of series or type. (c) Serial number and year of manufacture. (d) Unladen mass of the telehandler in working order without removable attachments and battery for battery-powered telehandlers, but with fork tynes or fixed attachments. The mass may vary from the figure shown by ±5%. (e) Maximum capacity. (f) Rated capacity at the maximum lift height with the load centre distance. (g) Rated capacities at other lift heights and load centre distances, where applicable. (h) Rated capacity with each removable attachment at the lift heights and load centre distances authorized by the manufacturer. (i) On battery-powered telehandlers, the authorised maximum and minimum masses of the battery and the voltage value. (j) Details of the attachments that the telehandler has been designed to use (e.g., work platform, jib, bucket). (k) Information covering travelling on gradients. (l) Compliance with this Standard. The rated capacities shall be easily readable by the operator in the normal operating position. Forklift and Telehandler Minimum Page 4 of 6

EXTERNAL BOOM ANGLE INDICATOR The Telehandler shall be fitted with an OEM designed and approved external gravity pendulum or bubble level inclinometer which indicates the angle of the boom. It must be clearly visible from the normal operating position and comply with AS1418.19, specifically 2.11.2.4 Boom Angle Indicator. The Telehandler shall be fitted with a lateral and longitudinal slope indicator to indicate to the operator when the TH is within the permitted lateral and longitudinal slopes. LATERAL & LONGITUDINAL SLOPE INDICATOR LOAD MANAGEMENT SYSTEM (LMS) TELESCOPIC BOOM LENGTH INDICATORS LONGITUDINAL STABILITY INDICATOR AND LIMITER OVERLOAD OVERRIDE MECHANISM (60 SECOND RESET) ENCLOSED, SEALED, AIR CONDITIONED CABIN (WHERE AVAILABLE FOR THAT MACHINE SIZE AND TYPE; OTHERWISE RISK ASSESSED) They must be visible from the normal operating position. The zero (level) and maximum permissible (side to side or front to rear) slopes shall be indelibly marked on the indicator. This may be incorporated into the electronic displays of load management systems. The indicator shall comply with AS1418.19, specifically 2.11.2.5 Lateral Slope Indicator. OEM approved and installed (or approved dealer) Load Management System such as the Manitou 3B6 system. The system must be compatible with the machine make and model requiring fitment. The operating length of the telescopic boom shall be indicated at regular intervals by either boom markings (alpha or numeric) or by another device. The indicator(s) must be visible from the normal operating position. The indicators shall comply with AS1418.19, specifically 2.11.2.8 Telescopic Boom Length Indicators. The TH must be equipped with a limiting device or system which restricts the operator from carrying out further aggravating motions to prevent overload (e.g. boom extension, boom lowering/lifting) whilst also warning the operator both visually and audibly. This device or system shall comply with AS1418.19, specifically 2.11.2.1 Longitudinal Stability Indicator and detailed Table 2.11.1 Application of Limiting and Indicating Devices. A mechanism shall be fitted that requires constant pressure from the operator to override the limiting device or system and which automatically reactivates after no more than 60 seconds. OEM designed and fitted cabin, with air conditioning suitable for conditions. If equipment isn t fitted with an air conditioned cabin, a risk assessment should be completed. DOCUMENTATION MINIMUM REQUIREMENTS (paper or electronic must be available at the time of mobilisation) ROAD REGISTRATION OPERATORS MANUAL PLANT RISK ASSESSMENT DAILY PRE-START BOOK SERVICE HISTORY PLANT LOGBOOK Registration paperwork and decal (if applicable to specific item of plant) OEM operators manual available and in good condition. Specific to make and model of equipment. Specific to category of equipment. Previous 3 months of preventative maintenance and repairs. Placed in operator cabin. PROJECT / SITE / CLIENT SPECIFIC REQUIREMENTS Items that may be required as determined by the project/site and/or client HIGH MOUNTED TAIL LIGHTS WHEEL NUT INDICATORS ADR compliant high mount lights with brake and turn signal features (reversing optional). Shall be mounted to the highest point of the vehicle (i.e. light bar or top of load guard), facing rearwards for the purpose of alerting other road users to vehicle movement. Fitted to wheel nuts to help identify any loose nuts, preventing the possibility of wheel detachment. Indicators should be of a suitable design and yellow in colour. VHF RADIO AND AERIAL FITTED AND OPERATIONAL MINE FLAG (MINIMUM HEIGHT FROM GROUND OF 3M) SPILL KIT TOOL BOX REVERSING CAMERA CALL SIGNS (HIGH VIS OR STANDARD) AUTOMATIC GREASING SYSTEM FIRST AID KIT REFLECTIVE TAPE LOCKABLE STARTER ISOLATION SWITCH C/W IDENTIFICATION DECAL WEED AND SEED An operational VHF radio shall be fitted in the operator cab and be easily accessible. A compatible aerial shall be fitted securely to the equipment. Mine flags help improve plant and equipment visibility when operating on work sites.
