Mr. Taylor was contacted and he stated that sludge had been removed from the system sometime in 2005, but he had no records pertaining to the removal of solid waste required in Condition #15 of the permit. This is in violation of Condition #15 of the permit. 4C. Is the treatment system being properly operated and maintained as required by permit? YesX No NIA SUMMARY OF FINDINGSICOMMENTS The canvash was observed to be in good condition. The leach field was dry, did have some weeds, but no trees were observed in the leach field. Mr. Taylor the Owner was contacted about the weeds in the leach field and stated that he would have the leach field mowed. Mr. Taylor also informed the Inspector of sludge that was removed sometime in 2005. He had no records pertaining to the removal of the solid waste required in Condition #15 of the pennit. Box 8913 Little Rock, AR 7221 9-89 13 Re: AFIN No. 68-00040 State Permit No. 4369-'WR-1 Mr. Kennedy, On August 10,2005, Gary Needler removed approximately 750 gallons of sludge from our car wash. He has a septic tank removal business. We are unaware of how or where he disposes this. Effective November 1,2006 the Car Wash will be connected to the Highland City Sewer. CITY OF HIGHLAND 1662 Highway 621412 Hardy, AR 72542 NOTICE Tb CUSTOMERS The public sewer system is nearing substantial completion and will be available for use in your area beginning the week on November 1. As other areas become available, we will notify those areas of the connection date. You must comply with the State Plumbing Code before you can be connected to the system. All existing rules and regulations of general sanitation must be followed. All drain lines must bc connected into one line, which goes into the new grinder pump station. Inside your house or business, all fixtures must have traps and vents. The sewage collection line from your house to the grinder pump must be inspected prior to starting the pumping station. You can contact any licensed plumber to make this connection or you can do it yourself on your own property. If you have any questions about materials or what you should do, you should contact the City Hall. You should contact the City Hall at the following number, when you are ready to have the lines inspected and the pump turned on: 870-856-6199. If you applied for a free connection and were determined eligible for this connection, this work will be done for you. You will not need to contact a plumber. Representatives of White River Planning and Development District will be contacting you regarding the work related to your free connection. The bids for this work are being taken and will be opened later this month. These hookups should begin in November. If your sewer service needs to be in someone else's name and/or your mailing address has changed, please contact the City Hall at the above number with the new information. The city will begin charging for sewer service with the bills that will be issued by the Highland Water Association for their January 1" billing. Highland Water Association will add a charge of $1.50 for collection. The sewer charge will appear on your regular monthly water bill. In accordance with the rate ordinance, you will be charged beginning with this billing date; whether, you have connected to the system or not. The Residential Sewer Rates are as follows (rates are based on water consumotion): $ 10.00 First 1,000 gallons (MINIMUM BILL) $ 2.50 per 1,000 gallons Next 4,000 gallons $ 1.50 per 1,000 gallons All over 5,000 gallons The Commercial Sewer Rates are as follow (rates are based on water consumption): $ 15.00 First 1,000 gallons (MINIMUM BILL) $ 2.50 per 1,000 gallons Next 4,000 gallons $ 1.50 per 1,000 gallons All over 5,000 gallons We are very happy that the sewer system is now available for you to use and look forward to serving you in the future. Box 697 Hardy, AR 72542 Re: AFIN No. 68-00040 State Permit No. 4369-WR-1 Dear Mr. Taylor: On September 7,2006, I performed a routine compliance inspection of your facility in accordance with the provisions of the Arkansas Water and Air Pollution Control Act and the regulations promulgated thereunder. This inspection revealed the following violation: As per our telephone conversation, you stated that you had removed solid waste (sludge) from your facility sometime in 2005. Condition #15 of the permit requires maintaining records pertaining to the removal of solid materials from the facility. You had no records concerning the removal of the solid waste. This is in violation of Condition #15 of the permit. The aforementioned item requires your immediate attention. Please submit a written response to this finding to the State Permits Branch of this department when the above violation has been corrected. This response should contain documentation describing how the violation was corrected. The corrective action should be completed as soon as possible and the written response is due by "October 17,2006". Violations of your State Permit are subject to enforcement actions by this Department. You should take reasonable measures to eliminate or prevent the recurrence of such violations in the future.
