BOX 189, GARNER, IA 50438-0189 TEL: 641-923-3711 IMT PART NUMBER 99900686 REVISED 05-05-93 REL 06-11-98 09-03-98 07-06-00 11-05-03 10-16-06 Iowa Mold Tooling Co., Inc. is an Oshkosh Truck Corporation Company.

INTRODUCTION The IMT Crane and Vehicle Log is designed as a convenient and precise method of recording inspections, tests, maintenance, repairs, revisions and modifications to your IMT crane. Use this log to keep a complete and accurate machine history and as an aid in maintaining the vehicle and crane in safe operating condition. It should be considered as a standard part of the crane, and as such, transferred with the crane to any new owner of the crane. It should also be available to any local, state or federal inspectors responsible for insuring compliance with codes applicable to your equipment. This log provides entry charts for daily, weekly and monthly inspections. It also provides charts for the recording of accidental crane damage, and repairs made due to that damage, and a chart for lubrication and maintenance performed on a scheduled or unscheduled basis. Inspect your new crane, vehicle and crane equipment before it is put into operation. Inspect to the frequency indicated on the daily, weekly and monthly checklists. Specific inspection information is contained in the IMT Operator s Crane Safety Manual (IMT part number 99900313) and the manuals applicable to your specific model of crane. Consult those manuals for information concerning specific inspection descriptions, torque values and maintenance procedures. On the front cover of this log are areas to insert calendar year, crane model designation, crane serial number, carrier vehicle model, VIN and the name of the person ultimately responsible for the information entered. Enter this information for concise reference to your equipment and keep this log in a safe place with the equipment it is applicable to. INSTRUCTIONS HOW TO USE THE CHECKLISTS: Mark each area with either an A, W, or R. A -(Acceptable) Means the area is acceptable for operation. (Preventive maintenance here, will extend the life of the equipment and reduce cost of overall maintenance.) W -(Worn) Means the area is acceptable for operation, but should be inspected carefully before the next use of the unit. (Perform any indicated maintenance and/or repair prior to taking a unit out of service due to inspection requiring mandatory repair. Doing preventive maintenance will extend the life of the equipment and reduce costs of maintenance and premature failure of other areas of the vehicle and/or crane.) R -(Repair) Means the area is in need of repair. If such a condition exists, the unit must be IMMEDIATELY REMOVED FROM SERVICE until the condition is repaired. The repair must be noted in the repair area of the log, dated, and signed by the mechanic performing the repair. WARNING Failure to follow the recommendations in the checklists and manuals pertaining to the vehicle and crane may result in DEATH or SERIOUS INJURY to the operator and/or others in proximity to this equipment. WARNING No welding or modification to the crane is permitted, unless expressly stated in writing by Iowa Mold Tooling Co., Inc. All welding is to be performed by a welder experienced and familiar with AWS D14.3 welding standard. NOTE Written, dated and signed inspection reports and records shall be made monthly on critical items (all monthly inspection areas) such as brakes, crane hooks, wire rope, hydraulic cylinders and hydraulic relief valves. Records shall be kept where readily accessible.ansi/asme B30.5A NOTICE TO THE OWNER / USER If your crane is involved in a property damage accident, contact your IMT distributor immediately and provide them with the details of the accident and the serial number of the crane. If an accident involves personal injury, immediately notify your distributor and IMT Technical Support at the telephone number listed on the front cover. WARNING Be certain to read and understand the IMT Operator s Crane Safety Manual before operation or maintenance is performed on your crane.
