Item# 2 Ensure that the Geronimo Line is properly installed at all times. Item# 3 Ensure all high pressure lines are hobbled correctly with whip checks or chain hose hobbles as necessary to prevent separation. Hobbles and whip checks must be installed taunt. Item # 4 Spoil pit fencing and signage needs to be installed if necessary. Item# 5 All trailer steps need to be installed properly and secured from movement. Item # 6 Employees exposed to chemicals from mud mixing operations are required to wear protective goggles and a full face shield. Item# 12 Oxygen and fuel gas cylinders need to be stored 20 or more feet apart when not hooked up for use. Oxygen/acetylene cylinders are NOT to be stored on location. Item # 13 Regulators are not to be missing or damaged on compressed gas cylinders. Item# 14 Damaged flexible wiring must be repaired or replaced immediately by a HWD certified electrician. Item # 16 Ensure that proper stair rails have been installed for all sets of stairs with 4 or more risers. Item# 17 Employees are required to wear all necessary flame resistant clothing. Item# 18 Ensure all fire extinguishers are mounted, accessible, unobstructed, charged, and inspected weekly. Item # 19 Pipe racks must be evenly chocked and free of damage. Item# 20 Bench grinders must be properly guarded. Tool rests are not to exceed 1/8" distance from the grinding wheel and tongue guards are not to exceed 1/4". Item # 21 Ensure that there is not grease or oil accumulating on compressed gas cylinders. Item# 22 All walking working surfaces over 4 foot high must be properly guarded and all floor holes are covered. All handrails must include a top rail, mid rail, and toe board. Item# 29 Window openings in the side of the generator house must not allow an employee to fall through to 4 or more feet to a lower level or ground. Item#30 All hinges on platform gates must be in working condition and free of damage. Item# 31 Make sure that gates on platforms, mud tanks, etc. are closed to prevent employees from being exposed to a fall hazard.
