If you wish to register for the Feed-in Tariff (FIT) Scheme with SSE Energy Supply Ltd as your FIT provider, please complete the following application form. Please read and complete each section of this form carefully. You will need to provide all required supporting documents with your application and agree to our Terms and Conditions which are included with this pack or available on our website. On receipt of your completed application, you will be registered on Ofgem s Central FITs Register (CFR). We will send a contract for you to sign and return which will enable you to be paid for your generation from your Eligibility Date. If you have an MCS accredited installation, your Eligibility Date will be the date we receive your completed application form and supporting documents. If any of the documents required to register your installation are incomplete or missing, this may affect your Eligibility Date. If you are applying for a Solar PV installation, in order to secure the higher FIT Generation Tariff you must provide a valid Energy Performance Certificate (EPC) of grade D or above or confirmation of exemption. Please note: If we receive your completed application form and supporting documents via post within 5 working days of your commissioning date, we will pay you from your opening generation meter reading confirmed on your MCS certificate. In order for us to accept your application you must: 1. Receive your electricity supply from SSE Energy Supply Ltd, M&S Energy or Equipower at the installation address; or 2. Not be connected to the National Grid (Off Grid); or 3. Receive your electricity supply from a supplier who is not a FIT Licensee; or 4. Receive your electricity supply from a supplier who is a Voluntary FIT Licensee. If your electricity is supplied by British Gas, Npower, Scottish Power, EDF, EON or a company who is also classed as a Mandatory FIT licensee, we will not accept your application. Please contact your Electricity supplier to register you onto the FIT Scheme. A glossary of terms has been provided for your reference and can be found at the back of this application pack. Missing documents may affect your Eligibility Date and the FIT payments you would receive under the scheme. **Your FIT Application Checklist** To register you for the FIT Scheme, we will need the following: Completed application form (Where all relevant sections are completed) Accreditation Certificate (MCS or ROO-FIT Accreditation) Proof of Purchase and must be in the name of the FIT Generator (An invoice or receipt of purchase confirming ownership of the installation and that it has been paid in full) Proof of identity (This can be a utility bill or bank statement dated within the last 3 months which connects you with the address of the installation. If you are supplied by us for your Electricity or Gas you do not need to provide this document. If you are acting as the FIT Generator on behalf of a company, please provide us with confirmation on company headed paper.) Agree to our Terms and Conditions For customers registering Solar PV installations please ensure you also provide and complete the following: An EPC (This needs to show that the property where the installation has been commissioned has achieved an EPC grade of D or above to receive the higher FIT generation tariff providing the multi-installation tariff does not apply. The EPC must be valid within the last 10 years.) Section 2: FIT Generator Details The FIT Generator is the person who owns the renewable installation and will be the person named on your Proof of Purchase. The FIT Generator will be registered on Ofgem s Central FITs Register (CFR). Please note we can only register one person as the FIT Generator. If the FIT Generator is registering on behalf of a company, please also confirm the company details in question 2.2. 2.1. Section 3: Installation Address The installation address is where your renewable installation has been commissioned. 3.1 Is the Installation address the same as the Contact address in Section 2? 5.5 Export Tariff - (This question should only be completed if your TIC is over 30kWh) The government will confirm a guaranteed tariff rate for exported electricity every year. However, you have the choice to negotiate your tariff on the open market. Please confirm if you wish us to register you for the guaranteed tariff rate for your export. IMPORTANT If you have a Solar PV Installation you MUST complete Section 7. If you have any other technology type, please proceed to Section 8. Section 7: Feed-in Tariff Solar PV Declarations All applications for accreditation of new solar PV installations (including extensions to existing installations), with an eligibility date on or after 1 April 2012, need to be accompanied by a copy of this document with the relevant section signed and dated. This will then be used by FITs Licensees/Ofgem as appropriate to determine whether or not (i) the energy efficiency requirement applies and, if so, has been met; and (ii) the multi-installation tariff rates should apply. If your application is for a new PV installation with an eligibility date on or after 1 April 2012, you must sign two of the enclosed declarations; one declaration from the energy efficiency section and one declaration from the multiinstallation section. Tick one of the boxes in relation to the energy efficiency declarations and one of the boxes in relation to the multi installation declarations. Then go on to sign the two relevant declarations. However, if your application is for an extension to an existing PV installation, you must sign one. 7.1 Energy Efficiency declaration Tick one of the following boxes in relation to the energy efficiency requirement and sign the relevant declaration overleaf: The energy efficiency requirement does apply and an Energy Performance Certificate (EPC) rating of level D or above has been achieved (complete declaration 1) The energy efficiency requirement does not apply because my installation is not attached or wired to provide electricity to a relevant building (1) (complete declaration 2) The energy efficiency requirement does apply and an EPC rating of level D or above has not been achieved (complete declaration 3) (1) relevant building means a roofed construction having walls, for which energy is used to condition the indoor climate, other than such a building for which an energy performance certificate cannot be issued; and a reference to a relevant building includes a reference to part of such a building which has been designed or altered to be used separately Page 11

Please sign only ONE from declarations 1, 2 or 3 for the Energy Efficiency Declaration Declaration 1 I certify in respect of this application for accreditation that all of the following are applicable: a. the eligible PV installation is attached to or wired to provide electricity to one or more relevant buildings(1); b. a valid energy performance certificate is enclosed in respect of the building (or one of the buildings) to which the PV installation is attached or wired to provide electricity; c. the enclosed energy performance certificate is the most recently issued energy performance certificate in respect of that building; d. the enclosed energy performance certificate certifies that the relevant building to which it relates has been assessed as being within bands A-D; e. ( e and f relevant if energy performance certificate issued in England and Wales only: if energy performance certificate issued in Scotland ignore e and f below and sign the declaration) (i) I am an owner or tenant of the relevant building to which the enclosed energy performance certificate relates; or (ii) I certify that I am not a person mentioned in (i) but such a person has co-signed this document; f. I: or (where this declaration is co-signed by another person, that person) consent to the disclosure of the enclosed energy performance certificate and / or the information contained within it to one or more FIT Licensees, the Gas and Electricity Markets Authority (Ofgem) and ministerial departments of the government and devolved administrations for retention and use by them for all purposes connected with administering, auditing, reporting on and performing statistical analysis on the Feed-in Tariff scheme for the duration of that scheme. Declaration 3 I certify that declarations 1 and 2 above do not relate to my eligible solar PV installation. An EPC level D or above is required AND has not been achieved. I understand that this means I will receive the lower FIT Generation tariff. Signed Dated If you have signed Declaration 3 you are declaring that you do not meet the requirement and you understand that you will receive the lower FIT generation tariff. 7.2 Multi-installation Declaration Tick one of the following boxes in relation to the multi-installation requirement and sign the relevant declaration overleaf: The FIT Generator (2) or nominated recipient (3) owns or will receive FIT payments from 25 or more other eligible solar PV installations (complete declaration 4) Neither the FIT generator or nominated recipient owns or will receive FIT payments from 25 or more other eligible solar PV installations (complete declaration 5) (2) FIT Generator means the Owner, identified as such in the CFR, of an Eligible Installation used or intended to be used for Small-scale Low-carbon Generation, whether or not that person is also operating or intending to operate the Eligible Installation (3) nominated recipient means a person appointed by a FIT Generator to receive FIT Payments in respect of an accredited FIT Installation owned by that FIT Generator Please sign only ONE from declarations 4 or 5 for the Multi-installation Declaration. If your application is for the accreditation of an extension to an existing PV installation in which case you do not need to sign either of these declarations. Declaration 4 I ( the FIT Generator ) (and(4) I ( the Nominated Recipient * ) certify in respect of this application for accreditation that either the FIT Generator or the Nominated Recipient (if there is one) are, or have applied to be, the FIT Generator or Nominated Recipient for 25 or more other eligible PV installations located on different Sites. A completed application should have at least an MCS/ROO-FIT/ROC ID provided and an EPC if applicable. If one of these documents is missing from the application this will affect the Eligibility Date of the installation and delay registration. Declared Net Capacity. This can be found on your MCS Certificate provided by your installer. This is the date you will be eligible for the FIT Scheme and FIT Payments Provides information on your property's energy rating. Your EPC must show an EPC rating of D or above to receive the higher FIT generation tariff providing the multi-installation tariff does not apply. Where you have extended your original installation which has already been registered onto the FIT Scheme A Government incentive scheme where FIT Generators will receive money for the electricity they have generated and exported from your renewable installations which are eligible for the scheme. Renewable installations with a generation capacity of up to 5MW (or 2kW for Micro CHP) are eligible for the FIT Scheme. The person who legally owns the renewable installation (this is the person that has paid for the installation). Supplier who is registered to facilitate the FIT Scheme. This is fitted by your installer and it measures the amount of electricity generated by your renewable installation. The rate received per unit of electricity for generated by your renewable installation This is a device connected to your renewable installation which converts the direct current generated into an alternating current.
