Safety Alert INCIDENT DESCRIPTION Date : 17th September 2013 Subject : Unsafe positioning of Trolley Jack Location : ALES Mussafah Heavy Equipment Division. Issue: 003/2013 WHAT HAPPENED While Mechanic Helper was looking underneath the equipment to ensure that the hydraulic trolley jack was Correctly positioned, he placed his Right fingers between the hydraulic trolley jack handle and the body of the Jack. The jack handle was initially in the horizontal position and moved on its own from the horizontal to the Vertical position, which trapped his middle finger resulting in a fracture of his 2nd phalanx. LESSONS LEARNED * Employees must be vigilant during pre/post use checks of the equipment. * Ensure there are no body parts between trolley jack body & Jack Handle * Jack Handle should be keep in upward position during placement RECOMMENDATIONS Ensure the jack is in sound condition and good working order prior to use. Take action for immediate Repair or replacement of damaged equipment. Locate the jack in adequate, well lit working area, with assistance where required Place Jack under vehicle manufacturer s recommended lifting points. Keep area clean & tidy and free from unrelated materials. Use Jack on level and solid ground, preferably concrete. Ensure all non essential persons keep a safe distance whilst the Jack is in use. Trolley Jack - Toolbox Talk PREVENTION OF PERSONAL INJURY On Tuesday September 2013, an incident occurred resulting in one employee fracturing his finger Probable Root Cause: lack of training on safe use of Jack LESSONS LEARNT: During placing of Hydraulic Trolley jack, the following measures shall be strictly put in place in order to avoid any near misses & injuries. These steps are to be followed in order to meet the Safety requirements of the Mechanical workshop. Ensure the jack in sound condition and good working order. Take action for immediate repair or replacement of damaged parts. Never place the handle of Hydraulic trolley Jack in Horizontal level except in some conditions (when jack is underneath of the equipment) Place wedges under wheels of vehicle (but ensure wheels of Jack can move freely ). Ensure the vehicle handbrake is engaged (or in PARKED mode), and switch the engine off. Ensure all non essential persons keep a safe distance whilst the Jack is in use. Place Jack under vehicle manufacturer s recommended lifting points.
