Abu Dhabi Department of Transport Application Form Non Objection Certificate (NOC) for a Commercial Licence to Operate Goods Vehicles with an Unladen Weight of over 2.5 tonnes Does this Application Apply to Me? You must complete this application if you wish to be granted a NOC by Abu Dhabi Department of Transport for operating goods vehicles with an unladen weight of more than 2.5 tonnes. How Do You Fill in this Application? You should ensure that you are familiar with the Department of Transport s requirements before completing this application. Further information is available in the Application Guidance document (Ref no. DOT-ST-FRT-G-14-014-EN). You must complete this application in full and provide all the required supporting documents. If the Department does not receive the required information, your application will be rejected or significantly delayed. What happens once the Application has been completed and submitted? The Department of Transport will review the application and will notify you that you have either met the requirements or that you need to provide more information. Section 2: Your Vehicles 2a) Please list the number of vehicles above 2.5 tonnes unladen weight that will be operated. Indicate the current number of vehicles in your goods vehicle fleet. You also need to indicate the maximum number of additional goods vehicles that you would like to pre-authorise for use over the next 12 months. It s primary role is to transport goods (Maximum of 6 axles) Any heavy goods vehicle, not included in the Rigid or Articulated Category and is used for the transport of goods (Maximum of 6 axles) Abnormal load vehicle > 2.5t A heavy goods vehicle used to carry loads outside typical weights and dimensions

2b) Please provide evidence that you have an operating centre at which your goods vehicles are kept. This operating centre must have adequate, authorised off road parking to accommodate the maximum number of vehicles stated in 2a when they are not in operation. Evidence must be provided for all locations used to park goods vehicles over 2.5 tonnes unladen weight. Evidence can include: Land plans with identified parking areas And / Or Photographic evidence of parking area as well as address/gps coordinates of the site With (1 or more of these) Written permission to park your declared vehicles on the respective plot Accompanying Lease contract Land ownership documents If you cannot meet this requirement now, you must commit to doing so within the next twelve months upon renewal of the commercial licence. 2c) Please tick to confirm that you have read, understood and will comply with the requirements set out in Section 2 of this application. Section 3: Maintaining Your Vehicles 3a) Please provide a description of your Truck Safety Inspection and Maintenance Management System for the goods vehicles you wish to be authorised under this licence. Section 5: Documents and Record Keeping 5a) Records of all goods vehicle inspections and maintenance together with driver management must be kept for a minimum of 3 years and made available for inspection by the Department upon request. These include but are not limited to: Completed daily truck safety inspection forms or equivalent Completed periodic safety inspection forms Records of ALL maintenance and repairs to vehicles Copy of Drivers driving licences and employment Visas Traffic fines and violations received by drivers Details of accidents in which the driver has been involved Further guidance is available in the: Application Guidance (Ref no. Section 6: Declaration Now take the time to carefully read all the undertakings below. Failure to comply with these undertakings may result in the goods licence being suspended, restricted or revoked. I confirm that all relevant Federal and Local laws and regulations in respect of the operation and management of vehicles authorised under this licence are observed. ii. iii. iv. I confirm that goods vehicles authorised under this licence will not be loaded in excess of their design weight. I confirm that the number of goods vehicles stored at each operating centre will not exceed the maximum number authorised and recorded on the licence or capacity of the site. I confirm that all goods vehicles operated under this licence will be kept parked at the location(s) stated in 2b when not in operation. v. I will grant DoT access to relevant company records upon receipt of a written request within 7 days or immediately upon request of an inspector designated and authorised by the Department of Transport. vi. vii. I will grant DoT access to company premises and vehicles for inspection upon receipt of a written request within 7 days or immediately upon request of an inspector designated and authorised by the Department of Transport. I will notify the DoT of any current or pending judicial court judgements against myself, the company, business partner(s), company director(s), nominated transport manager(s) named in this application, or employees or agents of the applicant for this licence. If any judicial court judgements are received after receiving the commercial licence, these will be communicated to the Department of Transport. viii. I will operate no more than the total number of goods vehicles stated in 2a (current + maximum additional). ix. I will ensure that the Department of Transport is notified within 28 days of any other changes, for example a change to the personnel identified in the application; number of goods vehicles; company address or status (e.g. if placed in liquidation or receivership), that might affect the issue of this NOC. 6a) Please tick to confirm that you have read, understood and will comply with the undertakings set out in Section 6 of this application.
