16.04 B757 SERVICE CHECK (FORM 757-16.04-1) B757 SERVICE CHECK SIGNATURE RECORD (FORM 757-16.04-1) NOTE: The Sign-Off Section of the Service Check (Form 757-16.04-1) is a signature record and contains general information only. The Description Section following this form MUST be used to accomplish the Service Check. GENERAL DESCRIPTION AND OPERATION NOTE: The Sign-off Section of all Line Maintenance Checks is a signature record and contains general information only. The Detailed Instructions section MUST be used to accomplish all Line Maintenance Checks. Remove the yellow Maintenance in Work placard from F/O s seat back pocket and place in front of throttle levers. Remove placard and re-stow after check is complete. Utilize GSE as much as possible to decrease APU operation. Communicate with flight crew on arrival for any servicing or special requirements. Check aircraft Log Book for pilot reports. Ensure aircraft documents are onboard. To include Aircraft Registration (Must be typed original with signature), Standard Airworthiness Certificate (Must be original on card stock, approximately 5.5 x 4 ), ship s radio license, and Insurance Certificate. Complete B757 Daily EICAS Non-Normal Fault Message Retrieval Form (Form 757-31.01-1). Enter all discrepancies and corrective actions in Aircraft Log Book (ATI-021). Record all non-airworthy and non-safety items that are found during the check on the Non-Routine Maintenance Form (ATI-018). If any errors are identified with a Line Maintenance Check, submit a Manual/Job Card/Form Corrections Request and forward to Publications or Quality Control. Any material referenced in the text or tables of the Line Maintenance Checks may be substituted in accordance with the Consumable Material Equivalents List located in Chapter 20, B757 Maintenance Manual. This bullet item does not apply to AD related tasks. This check includes ETOPS tasks which require Techninians to adhere to GMM 3.901 guidlines when performing maintenance on ETOPS aircraft. CERTIFICATION OF AIRWORTHINESS All signoff pages of the check have been received, action taken where necessary, and initialed properly. Aircraft Logbook properly noted. Attach any ATI Non-Routine Maintenance Forms (ATI-018) and forward all Sign-Off Sheets to Aircraft Records, LIT. Service Check Completed. K FLIGHT COMPARTMENT 1 1. Ensure that the Landing Gear pins are installed prior to starting the Service Check. 2 2. Ensure Pitot Tube Covers (if installed) are removed prior to starting Service Check. 3 3. Check aircraft log book for pilot reports and replenish supply of logbooks as necessary. 4 4. Press MASTER DIM and TEST SWITCH. Check for proper operation of all flight compartment lights (including Emergency Exit Light). NOTE: Ensure each indicator has two functional lamps; if not, re-lamp as necessary. 5 5. Check hydraulic fluid quantity using EICAS status display. (ETOPS) Left System - 1.00 ±.08 Right System - 1.00 ±.08 Center System - 1.00 ±.14 Record fluid system quantities: Current level: LH SYS CTR SYS RH SYS After Servicing (If Serviced): LH SYS CTR SYS RH SYS NOTE: Make logbook entry when servicing is required. 6 6. Operationally check the (TCAS) Traffic Alert and Collision Avoidance System. 7 7. Comply with Engine, APU and Cargo fire bottle squib test at the control panel. (ETOPS) 8 8. Comply with Auto Fire / Overheat Logic Test System (AFOLTS) system check. (ETOPS) 9 9. Test Fuel Quantity Indicating System (FQIS) (AD 2010-23-03). 10 10. Operationally check the Hydraulic Motor Generator (HMG). 11 11. Check crew oxygen system pressure - minimum allowable - 1250 PSI. (ETOPS) 12 12. Check for presence, condition and security of the following flight compartment emergency equipment: crash axe, emergency locator transmitter (ELT), halon fire extinguisher, protective breathing equipment (PBE) and emergency flashlights. 16.04.2 10-04-13 TR 00-05
13 13. Check for presence, security and condition of flight crew life vests. Replace if due within 30 days or notify Maintenance Control. 14 14. Clean cockpit windows. 15 15. Check exterior lights for illumination and operation. 16 16. APU HOURMETER AND CYCLE COUNTER (ETOPS) Record APU hour-meter reading here,, and in the log book, block 22/23. Record APU cycle counter reading (if installed) here,, and in the log book, block 22/23. Once per week during maintenance visits (applicable for Saturday or Sunday), perform APU Health Check on Form 757-49.01-1 and record in Aircraft Logbook. 17 17. Read out and record in Aircraft Log Book all EICAS and EPCS Maintenance Messages. NOTE: All EICAS and EPCS Maintenance Messages must be recorded in A/C log and appropriate action taken. Strict adherence to procedures of this step must be followed concerning certain combinations of EICAS and EECM messages. NOTE: If the maintenance level message can not be erased or repaired, then it may be forwarded to Maintenance Control if operational necessity and time does not permit repair of the item. 18 18. (Prior to departures on HF required flight, otherwise N/A this step.) Perform a system test of the HF Communication System. 19 19. Check that portable oxygen bottles indicate 1800 PSI. 20 20. Check emergency life raft for condition and security. 21 21. Perform a general inspection of the lavatory for lighting, condition and operation. 22 22. Check condition and operation of Flight Deck Seats and associated hardware. CARGO COMPARTMENT 1 1. Visually check the floorboards, cargo handling / locking devices and vertical side restraints for condition/ security. 2 2. Visually check forward barrier bulkhead, door, and door tracks for condition / security. 3 3. Visually check cargo compartment liner and sealing tape for condition / security. 4 4. Check for placards missing and legibility. Replace as needed per ATI 757 MM, Chapter 11. 5 5. Visually check main cargo door operating mechanism for condition, leaks and operation. Operate door through one complete cycle. 6 6. Visually check aft bulkhead for condition / security. 7 7. Visually check cargo door exterior skin and periphery. 8 8. Visually check cargo door edges for nicks or deformations which may impede pressurization. 9 9. Check cargo door seal for cuts, abrasions and condition of depressors. 10 10. Visually check hydraulic plumbing, cylinders, valves, tubing and hydraulic panel components for leaks, chaffing, and wear. 11 11. Visually check cargo door link assemblies for wear, bending and obvious fastener wear. 12 12. Visually check lower jamb area for obvious wear, damage and distortion. 13 13. Check control and warning lights for operation to ensure all microswitches and smoke detectors are properly functioning using the CFFO Press-To-Test switch. AIRCRAFT GENERAL 1 1. With flaps at 25 or more and hydraulic system pressurized, visually check the following areas from ground level for damage, corrosion, fluid leaks, loose panels, general condition and security of attachments: Fuselage; Empennage; Left, Right and Center wing lower exterior surface; slats; vanes; spoilers; ailerons; and wheel wells. NOTE: All existing fluid leaks must be investigated and documented by proper ATA code to support the FAA approved repetitive system alert program. Clean residue from existing leaks after completion of inspection or corrective action. 2 2. Check crew oxygen discharge disc, static ports, TAT, pitot probes, angle of attack vanes for condition and security. Check positive pressure relief valves for flag indication. Check antennas for condition / security. 3 3. Check ram air inlet and exhaust doors and cabin outflow valves. 4 4. Drain fuel tank sumps. Notify Maintenance Control of significant amounts of water removed. NOTE: Allow time for water to separate from fuel after arrival or refueling, time rate 1 hour for each foot of fuel depth NOTE: If ambient temperature is less than 32 F, see AMM Chapter 12, Cold Weather Servicing. 16.04.4 10-04-13 TR 00-05
5 5. (REQUIRED BY AD 92-17-04) Check condition of brakes for leakage, carbon condition for damage or evidence of stator fractures and brake wear indicator pin extension with parking brakes set. Replace brake if brake wear indicator pin is 1/16 or less above housing sub-assembly with parking brakes set. NOTE: AD 92-17-04 does not allow dispatch of A/C with brake wear indicator pin below flush with brake parked. 6 6. Perform a visual inspection of wheel for obvious damage and broken or missing wheel tie bolts. Gaps under nuts or washers indicates a recent fractured tie bolt. Replace any wheel with a broken or missing tie bolt. 7 7. Check tires for wear, cuts, peeled tread/separation and other damage. Replace tires per one of the following: When any portion of an interior tread groove is less than 1/32 deep. When any of the fabric reinforcing ply is exposed on any tread surface. Cuts, Punctures, Sidewall damage or other conditions exceeding the AMM limits. NOTE: Interior grooves are those exclusive of the grooves in or adjacent to the shoulder areas and must have 1/32 or greater groove remaining. NOTE: Ref. AMM 32-45-04 for additional wear information, cut limits and other allowable conditions. NOTE: Tires may not be deferred or remain on carry over out of Service Check unless approved by Maintenance Control. 8 8. Check tire pressures using calibrated pressure gauge and compare to wheel mounted gauge, if installed. Cold Not Cold Nose 155 160 Main 185 200 WARNING:SERVICE TIRES USING CALIBRATED PRESSURE GAUGE. DO NOT USE AN UNREGULATED, HIGH PRESSURE NITROGEN SOURCE TO SERVICE TIRES. USE OF AN UNREGULATED, HIGH PRESSURE NITROGEN SOURCE COULD LEAD TO EXPLOSIVE TIRE FAILURE AND POSSIBLE INJURY. NOTE: AD 87-08-09 requires servicing with dry nitrogen only. 9 9. With a clean cloth, apply a film of MIL 5606 fluid and wipe the exposed, chrome surface of the Shock Strut Pistons to remove any accumulated dirt or debris. 10 10. Perform a general visual inspection of landing gear struts for condition and evidence of leakage. Refer to the applicable AMM for corrective action. 11 11. Check lower cargo compartments including cargo nets for cleanliness, condition and security. Pay particular attention to cargo compartment liners for any damage. Repair liners as necessary to prevent operational findings and delays for repair. NOTE: All tears, holes or damage penetrations through cargo compartment ceiling, sidewall, shrouds, contour panels and door liners must be repaired PRIOR TO FURTHER FLIGHT except as authorized in specific aircraft minimum equipment list (MEL). 12 12. 13 13. Visually inspect RVSM Critical Area around static ports. LEFT ENGINE 1 1. Perform a walk around check of Left engine and pylon. Check for fluid leaks and obvious damage to engine and pylon including cowling, panels, engine inlet and exhaust area. 2 2. ROLLS ROYCE ENGINES ONLY: Visually check the left engine thrust reverser C-duct forward latch access door per the instructions contained in Rolls-Royce non-mod service bulletin RB.211-78-9575. (Ref. SB RB.211-78-8930 & RB.211-78-9235). NOTE: This step is Not Applicable for 752CX (pre-sb aircraft.) 3 3. Check Left engine oil quantity (at tank) and service to max. Record quantity of oil added in Aircraft Log Book and below. Record 0 if no oil is added. (ETOPS) WARNING:WAIT FOR A MINIMUM OF FIVE MINUTES BEFORE YOU REMOVE THE OIL TANK CAP AFTER ENGINE SHUTDOWN. THE PRESSURE IN THE TANK CAN CAUSE A SPRAY OF HOT OIL WHICH CAN CAUSE SERIOUS INJURY TO PER- SONS OR DAMAGE TO EQUIPMENT. FINAL CLOSE OUT ITEMS 1 1. STATEMENT OF CHECK COMPLETION. All sign-off pages of the check have been received, action taken where necessary and initialed properly. Aircraft Logbook entry made properly. A. Install Pitot tube as required. B. Remove and stow landing gear pins. C. Send signed off Service Check Form 757-16.04-1 with completed aircraft log pages to the Records Department. NOTE: The Service Check Description section contains all the maintenance tasks necessary to accomplish each particular item called out in the sign-off pages. NOTE: FAR 121.369(c) requires ATI to be able to positively identify the individual approving work. Signatures and Initials on work documents MUST match the Signature Record. 2 2.
