SUMMARY This annual report is in response to requirements under amended Environmental Compliance Approval # 8081-9Z4H48 dated October 20, 2015 for the Greenway Wastewater Treatment Centre. a) A summary and interpretation of all monitoring data and a comparison to the effluent limits outlined in Condition 7, including an overview of the success and adequacy of the Works; The monitoring data is included in the tables and graphs of the report. There were no monthly criteria exceedences in 2016. Phosphorous levels in 2016 averaged 0.45 mg/l. The annual average flow of 117 ML/day was within the limit of 152.175 ML/day. The average daily plant peak flow was 151 ML/D in 2016 and the instantaneous peak flow was 262 ML/D. The ph of the effluent ranged from 7.2 to 7.9 which are within the required range of 6.0 to 9.5. The monthly average dissolved oxygen levels were greater than the minimum required in 2016. The Greenway Wastewater Treatment Plant Expansion project is in the construction phase. The contract to construct the new and upgraded facilities has been awarded to North American Construction (1993) Ltd. and construction started in December 2015. The entire construction project is expected to last 2-3 years. The goals of the Pollution Prevention and Control Plan (PPCP) are to outline the nature, cause and extent of pollution problems, evaluate alternatives and propose remedial measures for reducing and mitigating pollution from sewer overflows, and then recommend an implementation program for achieving those goals. The City of London contracted engineering consulting firm CH2M HILL Canada Limited in 2012 to develop the 3 phased plan through a detailed analysis of the impacts of rainfall events on our existing sewer systems and the Thames River. Phase 2 of the project, which consisted of 2 stages of detailed hydraulic modelling of the City s sanitary sewer systems and subsequent evaluation and prioritization of overflows, is nearly complete. Phase 3 of the project, which will include proposing remedial measures and an implementation program for them, is scheduled to be completed by the end of 2017. The existing and proposed changes in the works are sufficient to meet the requirements of the Environmental Compliance Approval. b) A description of any operating problems encountered and corrective action taken; Treatment of sewage after a rain event/snowmelt can be handled for an initial event due to the quality of bacteria normally in sewage plants. If there are several rain events, then settling in clarifiers does not occur as well because of the increase in filamentous bacteria (they don t settle as well and some pass out of the final clarifiers increasing suspended solid losses). The Greenway Expansion includes new clarifiers for section #3 which have a better design than the existing section 3 final clarifiers. The use of alum instead of ferric was started on July 28, 2016 to evaluate whether the UV disinfection system operation would improve. The effectiveness of the disinfection system Page 2 of 96

improved in 2016 compared to results in August and September 2015. There was one E. Coli result in September that increased the geometric mean above the objective. A summary of all maintenance carried out on any major structure, equipment, apparatus, mechanism or thing forming part of the Works; A Work Order list for the Greenway Wastewater Treatment Centre is attached. c) A summary of any effluent quality assurance or control measures undertaken in the reporting period; A Plant and effluent monitoring program was conducted and the data is in tables of this report. Samples were obtained as required in the Environmental Compliance Approval 2016. The ph samples were done on grab samples once a week and the composite samples the rest of the time. The grab ph samples date and value are listed in the report in a separate table. One grab sample was missed during the week of July 17 th, 2016. The UV channel analysis includes bypasses of the screen room and aeration section due to high flows during rainfall/snow melt events. Analyses are based on 24 hr. samples for the Greenway Wastewater Treatment Centre with the exceptions of E. Coli, temperature and Dissolved oxygen which are grab samples. d) A summary of the calibration and maintenance carried out on all effluent monitoring equipment; Calibration information is in the report. e) A description of efforts made and results achieved in meeting the effluent Objectives of Condition 6; The plant met the monthly average concentration and loading objectives for BOD, suspended solids and Un-ionized Ammonia 100% of the time in 2016. The minimum Dissolved Oxygen monthly average concentration objective was achieved 100% of the time in 2016. Monthly Geometric means for E. Coli in Greenway's effluent in 2016 disinfection period were achieved 83% of the time (five out of six months). There was one E. Coli sample in September that brought the geometric mean over the objective in 2016. The monthly average phosphorous objective of 0.6 mg/l was met 83% of the time in 2016. September and October 2016 had monthly average phosphorous levels of 0.64 mg/l. The chemical for phosphorous reduction was changed on July 28, 2016 to alum to determine if this would help stop fouling on the UV Lamps. f) A tabulation of the volume of sludge generated in the reporting period, an outline of anticipated volumes to be generated in the next reporting period and a summary of the locations to where sludge was disposed; Sludge wasted in 2016 from the aeration sections was 808,283 cubic metres with an average solids of 0.65%. The primary sludge generated in 2016 had a volume of 231,515 cubic metres with average solids of 3.1%. The Greenway activated sludge is thickened in dissolved air Page 3 of 96

flotation units or Rotating Drum thickeners. Sludge from the other City of London Wastewater Treatment Plants is hauled to Greenway. The incinerator was down for replacement of the preheater and reheater and to refurbish the incinerator dome between April 12 and June 9, 2016. The Bioset system processed 3,299 tonnes that were hauled to the W12A landfill for disposal. In 2017 we anticipate generating 800,000 cubic metres of aeration sludge and 230,000 cubic metres of primary sludge at the Greenway PCC. In 2016, the centrifuge cake was incinerated or processed in the Bioset process and hauled to the W12A landfill. In 2017 we plan to incinerate or process the sludge in the Bioset system and haul to the W12A Landfill, if the incinerator is not operating. The volume of sludge is included in tables in the report. g) A summary of any complaints received during the reporting period and any steps taken to address the complaints; There were six odour complaints received at Greenway in 2016. I. On September 1, 2016 section three aeration pass 1 was taken out of service due to construction. The sludge in the bottom of the tanks was being washed down to enable the improvements to the air system. II. The September 12, 2016 odour complaint did not specify a time and the wind direction was from the North in the early morning for two hours and the rest of the day the wind direction was from the East, northeast and southeast. The plant had the south west holding tank out of service for maintenance. Construction of the plant is continuing. III. The September 14, 2016 odour complaint around 4 pm had the wind from the north. The South Holding Tank was being cleaned out for maintenance that day. Construction of the plant was continuing. IV. The September 24, 2016 odour complaint around 12:22 pm had the wind from the north. Construction of the plant is continuing and all staff and contractors were advised to keep the doors to the sludge dewatering building closed. The section 3 aeration pass 1 drain lines were closed as the drain lines may also be a source of odours. Section 3 pass 1 is having the aerators upgraded as part of the expansion. V. The October 24, 2016 odour complaint around 10:30 am had the wind from the north east. Maintenance was being done on a centrifuge and ports to the foul air blower were short circuiting into the building. The building had vents open and the ports on the centrifuge were plugged to stop the release of gasses. VI. The November 7, 2016 odour complaint around 3:30 pm had the wind from the south. The sludge dewatering building was open due to maintenance on the centrifuge. The door and windows were closed. h) A summary of all By-pass, spill or abnormal discharge events. There were 15 by-pass events in 2016 at the Greenway Pollution Control Centre. Rainfall for 2016 was 929 mm compared to 964 mm yearly average for rainfall from 1986 to 2016. There were 13 secondary by-pass events (received primary treatment) and there were 12 raw sewage bypasses. There were two bypass events that had only raw bypasses. There was three secondary bypasses that did not include a raw bypass. The total bypasses receiving primary Page 4 of 96

treatment was 140,470 M 3. The total raw sewage bypassed was 3,235 M 3. The raw by-pass was 0.008% of the volume receiving full treatment and the primary treated by-pass was 0.33% of the volume receiving full treatment. The bypassed primary sewage and raw sewage mixes with the treated flow prior to discharge to the Thames River and is included in the effluent quality monitoring data for the plant. The bypass data is in the tables of this report. An opaque discharge was noted coming from the Greenway Incinerator stack on January 8, 2016. The incinerator was shut down to investigate the cause of the opaque discharge (it takes 3 days for the incinerator to cool down sufficiently prior to being able to enter the system). Seven Tubes in the reheater heat exchanger needed to be repaired and two tubes in the preheater heat exchanger needed repair. The ash was short circuiting the scrubber and some ash buildup was found in the ducting. The ash in the ducting was cleaned out. The City replaced the reheater and preheater heat exchangers in 2016. Foam was discharged in the effluent in 2016 on January 14 and 26, March 15, July 29, August 5 and November 11. Soaps can come from industries, institutions and home owners. The City is has required a company to submit a plan to deal with surfactants from their facility so that their effluent does not foam. The plant had an issue with UV disinfection last year and it was decided to use alum for phosphorous removal instead of ferric chloride to see if this would improve effectiveness of disinfection. One Ferric iron tank was taken out of service and cleaned to prevent reactions between the alum and ferric chloride. On July 18, 2016. The tank was filled with alum. The Chief Operator received a low level alarm for Tank #2 and operators went to look for the cause since the tank had only been filled. The tank drain line and containment area drain valve had been left open and the alum had drained into the section 3 channel. Section 3 has five primary tanks and tanks #4 and 5 were not in service due to low flows. The primary tanks #1, 2 and 3 were isolated to contain the alum and tanks #4 and 5 were opened and filled. Samples were taken from section 3 final tanks and analyzed for dissolved aluminum, ammonia, BOD5, ph, total phosphorous and total suspended solids. The aluminum standard is based on clay free samples and sewage can contain clay from combined sewers or seepage into sewers and so dissolved aluminum was analyzed. Dissolved aluminum levels averaged 44 ug/l from the section 3 finals based on 18 samples taken from 9 pm on April 18 to 2:00 pm July 19, 2016. This was lower than the surface water objective of 75 ug/l. Jersey barriers around active geotubes were set up in 2015 and a berm was built along the adjacent ditch to contain ash on property. On November 27, 2016, a geotube ruptured along the seam and ash escaped out of the bag. The ash ports to the geotube were closed. A berm and curbing were built along the ditch to contain ash water on property and no ash left the property due to the rupture. The ash outside of the concrete area was removed and the ash in and around the ruptured geotube was removed. (j) a copy of all Notice of Modifications submitted to the Water Supervisor as a result of Schedule B, Section 1, with a status report on the implementation of each modification; No Notices of modification were submitted in 2016. (l) any other information the Water Supervisor requires from time to time. ASH: In August 2012 the geotube system was started to dewater the ash and the ash basins were taken out of service. Geotubes containers are constructed of a specially- woven polypropylene material. The geotube treatment process consists of filling the geotube container with a slurry of ash. Polymer is introduced into the media to promote flocculation which allows the solids to bind and separate water more efficiently. The geotube container then dewaters the clear effluent commonly known as filtrate to section #3 aeration. Once the bag is filled to its recommended storage capacity the dewatered contents are left to dry further and then the ash can be removed. The ash from the geotubes was hauled to W12A (4,135 tonnes) in 2016. In February 376 tonnes, in April 937 tonnes, in June 800 tonnes, in July 87 tonnes, in September 914 tonnes, in November 939 tonnes and in December 82 tonnes. Page 6 of 96
