MSPEC-81002 Elevating Inspection Requirements Rev 2 1.0 PURPOSE This document applies to the elevating work platform and related components of the mobile piece of equipment only. It does not cover all other service inspection required to maintain the mobile carrier and additional attachments. The purpose is to ensure that the maintenance plans and task lists created in SAP include all the minimum inspection requirements to ensure the safe operation of the elevating platform. The requirements listed in this specification are derived from the mining regulation and applicable standards, these are minimum requirements. There may be additional requirements specified by the equipment manufacturer that shall be considered when creating maintenance plans. This specification does not address the details of some of the periodic NDE requirements which are covered in the engineering standards listed as reference documentation. This specification is divided in 6 sections: Section 5.0 Forklifts (used with manbaskets) Section 6.0 Self-Propelled Elevating work platforms (scissor lifts, bolters, etc.) Section 7.0 Boom supported Elevating work platforms (manlift, Anfo loaders, etc.) Section 8.0 Vehicle Mounted Aerial Device (surveyor jeep, boom truck, etc.) 2.0 SCOPE This specification applies to all OWNED and RENTAL equipment designed in accordance with the referenced CSA standards listed previously. This specification pertains to all Mines and Surface plants within Vale operations. 2.1. EXCEPTIONS None 3.0 REFERENCES The following documents were used in the development of this document or have instructions and procedures applicable to it. They shall be used in their most recent revision. FORKLIFT / TRACTOR 5.1.1. FORKLIFT DAILY INSPECTION: A. Check mast rails, rollers, and chains for loose, damaged or missing parts, including hydraulic fittings/adaptors. B. Check all mast welds for cracks, especially at the mounting brackets C. Check the roller pathways for metal flakes, dirt or other obstructions that may interfere with the safe operation of the mast. D. Check the mast chains for missing or broken links, pins or cotter pins. In addition, look closely for seized parts E. Extend the carriage a few inches off the ground and make sure the chains are under equal tension. F. Extend the mast to its fullest height to make sure the mast rails and carriage extend freely without binding. Ensure proper multi stage mast sequencing. G. Make sure the internal hoses (if equipped) travel evenly in the hose guides. Adjust the hose ends if required. Tighten the fittings making sure they do not twist. H. Inspect the whole carriage for damage or excessive dirt I. If the forklift has hook forks, check for missing or damaged fork lock pins J. Inspect the forks for surface cracks, especially at the heel and welds attaching all mounting hardware to the fork shank. K. Check the forks for abnormal wear or deformation. L. Check fork shafts for missing or damaged collars and bolts M. Check for any irregularities, damage or wear is detected on the forks. N. Ensure all cylinder pins and snap rings are tight and secure O. Check cylinder rods for scratches, groove, or evidence of leaking seals P. Check all hydraulic fittings/adapters and hoses for damaged, loose or missing parts Q. Ensure all the load locks are in good operating condition R. Ensure there are no hydraulic leaks Page 4 of 23

FORKLIFT 250 HR/6MO FORKLIFT INSPECTION: This inspection shall be carried out by a competent personnel and include all items listed on the daily inspection. A. Each pair of load rollers on the uprights and carriage should be shimmed for a total side to side clearance no greater than 1/16 in. (1.5mm), occurring at the tightest point throughout the travel of the member. Pry between the upright and load rollers so that the opposite load roller is tight against the upright, and measure the clearance for the pair of rollers. B. Check the chains for wear and stretch C. Ensure the following parts/components are in good operating condition and free of defects: i. Hydraulic pipes are not bent or crushed. ii. The mast sliding sections are not contaminated with debris, shotcrete, muck. iii. The mast sections are not bent from impact iv. The electrical holding valve circuit is in good operating condition v. The bearings, rollers, plungers are not defective, seized or missing. vi. The hydraulic level is not too low vii. The hydraulic cylinders, hoses and valves are not leaking viii. The chain is properly adjusted ix. The main hydraulic fuse/cushions and/or check valves are functioning x. The push button on the control lever must be functional xi. The control lever and cables must be fully functional in each direction xii. The mast should be free of lowering and rising without biding. D. Additional other parts associated with the mast such as cylinder clamps, clips, safety lock washers, angle indicator, light, appropriate mast capacity decal, bolts, trunnion mount and forks should also be present and in proper working order. E. Forks must be inspected visually and with the aid of a fork caliper for the following: i. Check for straightness of the blade and shank ii. Check for excessive fork angle iii. Check tip alignment iv. Check fork positioning lock for damage v. Difference in height of fork tip vi. Positioning lock (if equipped) vii. Side to side play of hinged forks ix. Wear: fork blade and shank, fork hooks x. legibility of marking xi. Surface cracks (visually) F. Annually, the forks shall be inspected by magnetic particles to check for surface cracks by certified CGSB Level 2 inspector. G. While the mast is extended, inspect and lubricate the full length of the chains with SAE 40wt. Oil or Bowman Heavy load Red grease. Page 5 of 23

MSPEC-81002 Elevating Inspection Requirements Rev 2 Note: if cracks are found during inspection, contact the OEM or the plant Engineer to obtain repair/replacement procedures. 5.1.3. FORKLIFT MAST NDE INSPECTION: A. Magnetic particles of the mast welds as per ASTM E709 is required 3 years after the date of manufacture and every 2 years thereafter. The entire mast is to be disassembled and sandblasted to allow for this inspection. The inspectors must be certified to CGSB Level 2 magnetic particles. Note: if cracks are found during inspection, contact the OEM or the plant Engineer to obtain repair/replacement procedures. 5.2. MANBASKET 5.2.1. DAILY INSPECTION A. Check all welds for cracks or breaks, especially at the mounting brackets, entrance chains B. Check to ensure the anchor points are in good condition and tightened C. Check to ensure all the decals and the angle indicator are present, legible and in good condition D. After installing the manbasket onto the mast or loader, it should be cycle tested without personnel in the manbasket before using. 5.2.2. ANNUAL INSPECTION The following checks are to be made by a qualified HDET A. Check all welds for cracks or breaks, especially at the mounting brackets, entrance chains B. Check to ensure the anchor points are in good condition and tightened C. Check to ensure all the decals and the angle indicator are present, legible and in good condition D. After installing the manbasket onto the mast or loader, it should be cycle tested without personnel in the manbasket before using. 5.2.3. NDE INSPECTION The manbasket shall be completely cleaned by sandblasting and inspected visually and by magnetic particles by a CGSB certified MT level 2 inspector at a frequency of 3years from new and every 2 years thereafter. MSPEC-81002 Elevating Inspection Requirements Rev 2 6.0 SELF-PROPELLED ELEVATING WORK PLATFORMS (CSA B354.2) The following applies to self-propelled integral chassis elevated work platforms that have a platform that cannot be positioned completely beyond the base (scissor type) and that are used to position personnel, along with their necessary tools and materials, at work locations. Self-propelled elevating work platforms (aerial platforms) are power operated with primary functions, including drive, controlled from the platform. The maintenance requirements for these work platforms depend on their service rating. SCISSOR TYPE ELEVATING PLATFORM 6.1.1. SCISSOR TYPE ELEVATING PLATFORM DAILY INSPECTION All components that have a direct bearing on the safe operation of the aerial platform and whose status can change from day to day with use shall be visually inspected on a daily basis by the operator. In addition, these components, where applicable, and where practicable and possible, shall be observed during operation for defects that could affect the safe operation of the aerial platform. Items such as, but not necessarily restricted to, the following shall be regularly inspected for defects and shall be observed during operation and between regular inspections for defects: (a) tires and wheels for cuts, obvious loose or missing lug nuts, broken tie rods, and appearance of proper tire inflation; (b) all fluids, such as fuel, engine oil, coolants for proper level, and battery fluid, and hydraulic hoses for leaks or loose connections; (c) all quick disconnect hoses for proper connection; Page 7 of 23

MSPEC-81002 Elevating Inspection Requirements Rev 2 (d) structural components for obvious damage and broken parts, and cracks in welds, including scissor arms, outrigger box, arms, and pads; (e) ladder or steps for damage and debris. The ladder shall also be checked to ensure that it is firmly secured to the platform; (f) beacon and warning lights for missing and defective lenses or caps; (g) operating and emergency controls for proper function; (h) placards and warning signs for cleanliness, readability of control markings, rated capacity, and operating manual(s); (i) platform assembly for loose and missing parts, missing or loose lock pins and bolts, and placement of footswitch. In addition, the platform floor shall be checked for structural damage, holes or cracked welds, dirt, grease or oil that can create a hazard; (j) access gate for ease of movement, missing parts, latched and locking capabilities; (k) fall protection system; (l) safety devices for malfunction; (m) scissors for smooth up and down movement; (n) brakes for stopping capabilities; (o) horn for proper function; (p) outriggers and stabilizers; and (q) other items as specified by the manufacturer. Appendix A. 6.1.2. SCISSOR TYPE ELEVATING PLATFORM 250H/MONTHLY INSPECTION This inspection shall be performed after 250 h of use or after a period recommended by the manufacturer, whichever comes first. The interval between inspections may vary depending on the aerial platform's activity, severity of use, and the working environment. Periodic inspections shall be performed by a qualified person. This inspection shall include the requirements of the daily inspection and, in addition, items such as, but not limited to, the following: Service A: A. loose bolts, nuts, and pins; B. hydraulic hoses for cracks, leaks, and blistering, and evidence of excessive abrasion on all flexible and rigid hoses; C. hydraulic cylinders for drifting caused by fluid leak across the holding valve or the piston, rod seal leakage, scored and nicked cylinder rods, dented barrel, and unusual noises or vibration; D. all control mechanisms for wear and response time; E. interlocks, slope warning system, and limit switches; and F. other items as specified by the manufacturer. Service B: A. loose bolts, nuts, and pins; B. hydraulic hoses for cracks, leaks, and blistering, and evidence of excessive abrasion on all flexible and rigid hoses; C. hydraulic cylinders for drifting caused by fluid leak across the holding valve or the piston, rod seal leakage, scored and nicked cylinder rods, dented barrel, and unusual noises or vibration; D. all control mechanisms for wear and response time; E. interlocks, slope warning system, and limit switches; and F. other items as specified by the manufacturer. Appendix A. Service C (250H or annually whichever comes first): A. loose bolts, nuts, and pins; B. hydraulic oil filter for cracks and leaks, metal chips or pieces on the filter that may denote failure in the pumps, motors, or cylinder, evidence of rubber particle on the filter element that may indicate deterioration of hose, O-ring, or other rubber components; C. fuel filter for function; D. fan belts for adjustment and excessive wear; E. hydraulic hoses for cracks, leaks, and blistering, and evidence of excessive abrasion on all flexible and rigid hoses; F. hydraulic pumps and motors for cracks or leaks, leaks at joints, seal leaks, loss of operating speed, excessive heating of fluid, and loss of pressure; G. hydraulic cylinders for drifting caused by fluid leak across the piston, rod seal leakage, scored and nicked cylinder rods, dented barrel, unusual noises, or vibration; H. all control mechanisms for wear and response time; and I. other items as specified by the manufacturer. SCISSOR TYPE ELEVATING PLATFORM 750H/ANNUAL INSPECTION This inspection shall be performed annually or after 750 h of use, whichever comes first. A complete inspection of the aerial platform shall be performed by a qualified person. The inspection shall include the requirements of the daily and 250 h inspection and, in addition, shall include, but not be limited to, all critical and suspect areas and all accessible structural elements and welds, such as the following: Service A: A. outrigger and attachments B. platform rotating mechanism; C. steering knuckles and mechanism; D. fall protection anchor points; E. boom sections, pins, cylinder rod ends, and leveling devices; F. switches for wiring and all electrical connections; and G. other items as specified by the manufacturer. H. NDE critical pins (SPEC-83034) Appendix A. Service B: A. outrigger and attachments B. platform rotating mechanism; C. steering knuckles and mechanism; D. fall protection anchor points; E. boom sections, pins, cylinder rod ends, and leveling devices; F. switches for wiring and all electrical connections; and G. other items as specified by the manufacturer. Appendix A. Service C: This inspection shall be performed annually or after 750 h of use, whichever comes first. A complete inspection of the aerial platform shall be performed by the OEM Representative. A sticker with the signature of the inspector and the next inspection date shall be affixed in a conspicuous location of the elevating work platform. Page 10 of 23

MSPEC-81002 Elevating Inspection Requirements Rev 2 7.0 SELF-PROPELLED BOOM-SUPPORTED ELEVATING WORK PLATFORMS (CSA B354.4) The following applies to self-propelled boom-supported integral chassis elevated work platforms that have a platform capable of being positioned completely beyond the base and that are used to position personnel, along with their necessary tools and materials, at work locations. Self-propelled boom-supported elevating work platforms (aerial platforms) are power-operated with primary functions, including drive control from the platform. Manlifts and Anfo loaders usually meet this definition. The maintenance requirements for these work platforms depend on their service rating. Three service rating are established: Service A: Heavy duty Anfo loaders Service B: Light duty manlifts (Genie, Skyjack, JLG, Snorkel, etc.) The frequency is as follows: Service A 5 yrs after new Structural 3yrs thereafter Service B 10 yrs after new 5yrs thereafter Pins NDE Yearly None Annual Yearly Yearly Periodic Monthly None Daily Daily Daily 7.1. BOOM TYPE ELEVATING PLATFORM 7.1.1. BOOM TYPE ELEVATING PLATFORM DAILY INSPECTION All components that have a direct bearing on the safe operation of the aerial platform and whose status can change from day to day with use shall be visually inspected on a daily basis by the operator. In addition, these components, where applicable and practicable, and where possible, shall be observed during operation for defects that could affect the safe operation of the aerial platform. Items such as, but not necessarily restricted to, the following shall be regularly inspected for defects and shall also be observed during operation and between regular inspections for defects: (a) tires and wheels for cuts, obvious loose or missing lug nuts, broken tie rods, and adequate tire pressure; (b) all fluids such as fuel, engine oil, coolants for proper level, and battery fluid; (c) hydraulic hoses for leaks or loose connections; (d) all quick disconnect hoses for proper connection; (e) structural components for obvious damage, broken parts, and cracks in welds; (f) ladder or steps for damage and debris (ladder shall be firmly secured to the platform); Page 13 of 23

MSPEC-81002 Elevating Inspection Requirements Rev 2 (g) beacon and warning lights for missing and defective lens or caps; (h) operating and emergency controls for proper function; (i) placards and warning signs for cleanliness, readability of control markings, rated capacity, and operating manual(s); (j) platform assembly for loose and missing parts, missing or loose lock pins and bolts, footswitch in place; (k) platform floor for structural damage, holes or cracked welds, dirt, and grease or oil that can create a hazard; (l) access gate for ease of movement, missing parts, and latched and locking capabilities; (m) Canopy for damage, cracks and proper operation. (n) fall protection system; (o) safety devices for malfunction; (p) proper operation of elevating, rotating, and drive functions; (q) brakes for stopping capabilities; (r) horn for proper function; (s) outriggers and stabilizers; and (t) motion alarm (audible and/or visual) (u) Level indicating device (v) other items as specified by the manufacturer. MSPEC-81002 Elevating Inspection Requirements Rev 2 7.1.2. BOOM TYPE ELEVATING PLATFORM 250H/MONTHLY INSPECTION This inspection shall be performed monthly for Service A. For Service B it shall be performed after 250h of use or annually whichever comes first. The interval between inspections may vary depending on the aerial platforms activity, severity of use, and the working environment. Periodic inspections shall be performed by a qualified person. This inspection shall include the requirements of the daily inspection and items such as, but not limited to, the following: Service A (Monthly): A. loose bolts, nuts, and pins; B. hydraulic hoses for cracks, leaks, and blistering, and evidence of excessive abrasion on all flexible and rigid hoses; C. hydraulic cylinders for drifting caused by fluid leak across the holding valve or the piston, rod seal leakage, scored and nicked cylinder rods, dented barrel, and unusual noises or vibration; D. all control mechanisms for wear and response time; E. interlocks, slope warning system, and limit switches; and F. other items as specified by the manufacturer. Appendix A. Service B (250H or annually whichever comes first): A. loose bolts, nuts, and pins; B. hydraulic oil filter for cracks and leaks, metal chips or pieces on the filter that may denote failure in the pumps, motors, or cylinder, and evidence of rubber particle on the filter element that may indicate deterioration of hose, O-ring, or other rubber components; C. fuel filter for function; D. fan belts for adjustment and excessive wear; E. hydraulic hoses for cracks, leaks, and blistering, and evidence of excessive abrasion on all flexible and rigid hoses; F. hydraulic pumps and motors for cracks or leaks, leaks at joints, seal leaks, loss of operating speed, excessive heating of fluid, and loss of pressure; G. hydraulic cylinders for drifting caused by fluid leak across the holding valve or the piston, rod seal leakage, scored and nicked cylinder rods, dented barrel, and unusual noises or vibration; H. all control mechanisms for wear and response time; I. interlocks, slope warning system, and limit switches; and J. other items as specified by the manufacturer. BOOM TYPE ELEVATING PLATFORM 750H/ANNUAL INSPECTION This inspection shall be performed annually or after 750 h of use, whichever comes first. A complete inspection of the aerial platform shall be performed by a qualified person. The inspection shall include the requirements of the daily and 250 h inspections and shall include, but not be limited to, all critical and suspect areas and all accessible structural elements and welds, such as the following: Service A: A. outrigger and attachments B. platform rotating mechanism; C. main turret rotating mechanism, including the condition and torque of the ring gear bolts; D. fall protection anchor points; E. boom sections, pins, cylinder rod ends, and leveling devices; F. switches for wiring and all electrical connections; and G. other items as specified by the manufacturer. H. NDE critical pins (SPEC-83052) Appendix A. Service B: This inspection shall be performed annually or after 750 h of use, whichever comes first. A complete inspection of the aerial platform shall be performed by the OEM Representative. A sticker with the signature of the inspector and the next inspection date shall be affixed in a conspicuous location of the elevating work platform. 7.1.4. BOOM TYPE ELEVATING PLATFORM SPECIAL TEST AND NDE Service A: A. Major - 5 years after new and 3 years thereafter (SPEC-83052) or after any actual, suspected, or potential damage is sustained during an incident that could potentially affect the structural integrity or stability of the aerial platform. MSPEC-81002 Elevating Inspection Requirements Rev 2 8.0 VEHICLE MOUNTED AERIAL DEVICE (CSA C225) This section applies to the following types of vehicle-mounted aerial devices: A. extensible boom aerial device; B. aerial ladder; C. articulating boom aerial device; D. vertical tower; and E. a combination of Items A. to D.. The vehicle can be a truck, trailer, or all-terrain vehicle. Examples of Vale units meeting this definition are the Toyota jeeps used by surveyors underground and Boom truck used by the power department to access electrical poles. 8.1. VEHICLE MOUNTED AERIAL DEVICE 8.1.1. VEHICLE MOUNTED AERIAL DEVICE DAILY INSPECTION The walk around shall include visual inspection, with testing as applicable, that includes the following items: (a) components for damage, cracks, corrosion, or excessive wear; (b) operating controls and associated mechanisms, for conditions interfering with proper operation such as (i) proper operation of interlocks; (ii) controls return to neutral when released and not sticking; and (iii) control functions and operation clearly marked; (c) visual and audible safety devices, for malfunction; (d) hydraulic or pneumatic systems, for visible deterioration or excessive leakage; (e) FRP and other insulating components, for visible damage or contamination; (f) missing or illegible operational and instructional markings; (g) electrical systems of, or related to, the aerial device, for malfunction, signs of excessive deterioration, dirt, and moisture accumulation; (h) bolts, pins, locking devices, and other fasteners that are visibly loose, deformed, or missing; (i) if the unit is provided with a winch, the load line and fitting for attaching the load, for visible damage; (j) other items for defects, as determined by the user based on recommendations by the manufacturer for a specific aerial device; and (k) vehicle functional and safety devices, as determined by the user based on recommendations by the manufacturer. A functional test including the following shall be performed: (a) setting up the aerial device for operation, including activation of outriggers; (b) cycling the boom through its complete range of motion and functions from the lower controls except where such operation would create a hazard; (c) checking functionality of emergency controls; and (d) vehicle functions, as determined by the user based on recommendations by the manufacturer. VEHICLE MOUNTED AERIAL DEVICE MONTHLY INSPECTION An inspection of the mobile unit shall be performed by qualified individual, addressing all of the items in the daily in addition to the following: (a) Structural members, for deformation, cracks, or corrosion; (d) Hydraulic system, for proper oil level; (e) Hydraulic and pneumatic lines, for evidence of leakage, abnormal deformation, or excessive abrasion; (f) Compressors, pumps, motors, and generators, for loose fasteners, leaks, unusual noises or vibrations, loss of operating speed, and excessive heating; (g) Hydraulic and pneumatic valves, for malfunction and visible cracks in the external valve housing, leaks, and sticking spools; (h) Any vacuum prevention systems, for visible signs of malfunction and for verification of function (e.g., systems); (i) Hydraulic and pneumatic cylinders and holding valves, for malfunction and visible damage; (l) Performance test of all boom movements; (n) Welds, as specified by the manufacturer; (o) Legibility and proper identification of operational and instructional markings; and (p) Grease all fittings on boom assembly (q) Check unit jacks working properly (r) Check manbasket door and latch Any suspected items shall be carefully examined or tested and a determination made by a qualified person as to whether they constitute a safety hazard. All unsafe items shall be replaced or repaired before use. 8.1.3. VEHICLE MOUNTED AERIAL DEVICE ANNUAL INSPECTION An inspection of the mobile unit shall be performed by qualified individual, addressing all of the items in the daily and monthly in addition to the following: (a) parts (e.g., pins, bearings, shafts, gears, rollers, locking devices, chains, chain sprockets, wire and synthetic ropes, and sheaves) for wear, cracks, or distortion; (b) hydraulic and pneumatic relief-valve settings; (c) hydraulic and pneumatic filters, for cleanliness and the presence of foreign material in the system, indicating other component deterioration; (d) electrical systems and components, for deterioration or wear, including those not readily visible on a frequent inspection; (e) condition and tightness of bolts and other fasteners, to ensure they are in accordance with the recommendations of the manufacturer; (f) welds, as specified by the manufacturer.
