Morton Thomas & Associates, Inc. (AMT) to document activities completed during the 1 st Quarter 2018. A site location map is included as Figure 1. In February 2016, Oil Control Program (OCP) Case 2016-0467-BA was opened following a confirmed release from a supply line associated with an underground storage tank (UST) located at the subject property. Apex completed a site investigation in August 2016 that identified petroleum impacted soil and groundwater in the vicinity of the former UST system, which was removed in February 2016. Based on Apex s site assessment, MDE recommended additional work that included the installation of three groundwater monitoring wells; monthly gauging of four wells identified as MW-1, MW-4, MW-5, and MW-6 as well as quarterly sampling of these four wells. April 16, 2018 1 st QMR 2018 North Point Government Center - OCP Case 2016-0467-BA Page 2 MONITORING WELL GAUGING During January, February, and March 2018, Apex completed monthly gauging of the four monitoring wells identified as MW-1, MW-4, MW-5, and MW-6. MW-5 was not gauged In February and March 2018 due damage sustained by the well, which is discussed below. Monitoring well locations are provided on Figure 2. Results of the monthly gauging completed during the 1 st Quarter 2017 are provided in Table 1. Measurable Light Non-Aqueous Phase Liquids (LNAPL) or free product was not detected in the four on-site wells gauged as part of the monthly monitoring, with the exception of MW-5 in January 2018. As a result of the shift in soils, the well construction of MW-5 had become compromised and the associated PVC piping was broken at approximately five feet below ground surface (bgs). Apex pulled the manhole lid of the Stormwater structure located approximately 10 feet away from MW-5. Apex observed soil, stone, and pieces of asphalt at the bottom of the Stormwater structure which is located approximately 7.5 feet bgs. The debris appeared to have originated from a Stormwater line running northwest towards the building and underneath the former piping run. On March 5, 2018, Apex mobilized to the site to expose and assess the damage to the Stormwater line and make repairs to MW-5. Apex excavated to approximately 6.5 feet to expose the Stormwater line and observed an approximately 3-foot section of the line was damaged. The Stormwater line was damaged during drilling and installation of MW-5. Given the location of MW- 5, the monitoring well was abandoned to allow for repairs to the Stormwater line to be completed. April 16, 2018 1 st QMR 2018 North Point Government Center - OCP Case 2016-0467-BA Page 3 Apex contracted with a Maryland licensed well drill to properly abandon the monitoring well. The well was abandoned by pulling the 4-inch PVC piping from the ground and backfilling the void with bentonite. The monitoring well abandonment report is provided in Attachment 1. On March 8, 2018, Apex met on site with representatives from Baltimore County and MDE to discuss the replacement of MW-5. Representatives on site discussed multiple locations for the replacement well within the former piping run and within areas were LNAPL was previously observed in temporary wells. MDE recommended the replacement well be installed within close proximity of the former MW-5 location. However, Apex notes that this location was outside of the former piping run, but within a few feet of MW-5. At MDE s recommendation, all parties agreed on the replacement well location. Prior to preparation of this report, the replacement well has not been installed, but is anticipated to be installed prior to the end of April. MONITORING WELL SAMPLING On March 29, 2018, Apex mobilized to the site to sample the on-site monitoring wells (MW-1, MW-4, and MW-6). MW-5 was abandoned on March 5, 2018 and a replacement well will be installed at a later date. Prior to sampling, the monitoring wells were gauged with an oil/water interface probe. Measurable LNAPL was not detected in the three monitoring wells during the 1 st Quarter 2018 sampling event. The monitoring well locations are depicted on Figure 2. Relative groundwater elevations were calculated by subtracting the depth-to-water reading from the top of casing elevation in each monitoring well. The recorded depth-to-water levels are presented in Table 1. A summary of groundwater elevations to date is provided in Table 2. An approximated groundwater contour map was constructed and is presented as Figure 3. Based on the relative groundwater elevations, groundwater flow is anticipated to be in a south-southeast direction. The groundwater samples were collected from each monitoring well using low-flow purging and sampling procedures. A bladder pump and dedicated disposable polyethylene (PE) tubing were used to purge the wells. Once groundwater parameters stabilized (+/- 10%), the groundwater samples were collected. The samples were retained in laboratory prepared glassware, labeled, and placed in a cooler with ice for preservation. Samples were then delivered to SGS Accutest Laboratories for analysis following proper chain-of-custody procedures. The samples were analyzed for volatile organic compounds (VOCs) including fuel oxygenates and naphthalene using U.S. Environmental Protection Agency (EPA) Method 8260; and total petroleum hydrocarbons for diesel range organics (TPH-DRO) using EPA Method 8015C. Apex compared the groundwater sample analytical results to the Maryland Generic Numeric Cleanup Standards for Groundwater (Groundwater Cleanup Standards) published in the MDE Interim Final Guidance dated June 2008. The laboratory analytical results are summarized on Table 3 and the laboratory data sheets are included in Attachment 2. Based on the results of the 1 st Quarter 2018 sampling, concentrations of TPH-DRO and VOCs were not detected at concentrations that exceeded the Groundwater Cleanup Standards. April 16, 2018 1 st QMR 2018 North Point Government Center - OCP Case 2016-0467-BA Page 4 SUMMARY AND ACTIVITIES TO BE COMPLETED IN THE 2 nd Quarter 2018 During the 1 st Quarter 2018, Apex gauged the four monitoring wells (MW-1, MW-4, MW-5, and MW- 6) on a monthly basis. Measurable LNAPL was observed in MW-5 at a thickness of 0.75 feet on January 10, 2018. MW-5 was not gauged in February and March due to damage sustained to the monitoring well. Measurable LNAPL was not detected in the remaining three monitoring wells during the 1 st Quarter 2018. Based on the results of the 1 st Quarter 2018 sampling, concentrations of TPH-DRO and VOCs were not detected at concentrations that exceeded the Groundwater Cleanup Standards. Apex notes that MW-5 was abandoned on March 5, 2018. Activities planned for the 2 nd Quarter 2018 include installation of a replacement monitoring well for MW-5, continued monthly gauging and quarterly sampling of the four monitoring wells during June of 2018. In addition, if LNAPL is observed in any of the onsite monitoring well, the LNAPL will be bailed from the monitoring well and stored in a 55-gallon steel drum for later disposal.
