Reference: NTB04-100a November 5, 2004 VOLUNTARY RECALL CAMPAIGN SKYVIEW ROOF GLASS Date: IMPORTANT: THIS BULLETIN HAS BEEN REVISED. Figure 10 (page 7) was revised to include a rubber seal on the glass. Please use this bulletin (NTB04-100a) ONLY. Discard all previous copies of NTB04-100. CAMPAIGN I.D.# / NHTSA #: PU401 / 04V-326 APPLIED VEHICLE: 2004 Maxima (A34) APPLIED VINS: 1N4BA41E*4C800000-875558 NOTE: You MUST use Service Comm to confirm campaign eligibility. INTRODUCTION Nissan has determined that some 2004 model year Nissan Maxima vehicles may have a defect which relates to motor vehicle safety. There is a possibility that the tempered glass for the SkyView Roof may not have been cooled properly after the forming process, which could create excessive internal stress in the glass. This could eventually cause the glass to shatter into small pebble-like pieces and injure vehicle occupants. This issue does not affect Maxima vehicles equipped with the power glass sunroof. To prevent this condition from occurring, Nissan is conducting a Voluntary Safety Recall Campaign to inspect and replace, if required, the glass in the SkyView Roof. IDENTIFICATION NUMBER Nissan has assigned identification number PU401 to this campaign. This number must appear on all communications and documentation of any nature dealing with this campaign. NUMBER OF VEHICLES POTENTIALLY AFFECTED The number of vehicles potentially affected is approximately 32,100. DEALER RESPONSIBILITY It is the retailer s responsibility to check Service Comm for the campaign status on each vehicle falling within the range of this voluntary safety recall which for any reason enters the service department. This includes vehicles purchased from private parties or presented by transient (tourist) owners and vehicles in a dealer s inventory. Federal law requires that new vehicles in dealer inventory which are the subject of a safety recall must be corrected prior to sale. Failure to do so can result in civil penalties by the National Highway Traffic Safety Administration. While federal law applies only to new vehicles, Nissan strongly encourages dealers to correct any used vehicles in their inventory before they are retailed. 1/10

SERVICE PROCEDURE 1. Confirm the Skyview Roof Glass needs to be replaced as follows: For Vehicles With A Production Date Of October 2003 Or Before: Replace the Glass. Continue with step 2 of this procedure. For Vehicles With A Production Date Of November 2003 Or December 2003: Replace the Glass if the dots around the CARLEX name (located at the driver s side rear corner of the Glass) look like any one of the examples shown in Figure 1. In this case continue with step 2 of this procedure. Front AS3 DOT 467 M078 Figure 1 TP040521a 2/10

Skyview Roof Glass Windshield Removal Tool TP040514 Figure 3 IMPORTANT: Before removing the Glass, take note of the Nissan logo position on the glass. The new glass must be reinstalled with the logo in the same position (see Figure 4). Write Logo on the masking tape at the driver s side rear corner to help you remember the logo location. 4. Remove the Skyview Roof Glass. CAUTION: Use suitable eye and hand protection when performing the below procedure. 5. Destroy the original Glass as follows: a. Put the original Glass into the box that the new Glass came in. b. Completely tape up all seams on the box. c. Hit the face of the box with a hammer until the Glass shatters. d. Discard the box. OWNER S LETTER Dear Nissan owner: This notice is sent to you in accordance with the requirements of the National Traffic and Motor Vehicle Safety Act. Nissan has decided that a defect that relates to motor vehicle safety exists in some 2004 model year Nissan Maxima vehicles, equipped with the SkyView Roof. Reason for Recall The tempered glass for the SkyView Roof may not have been cooled properly after the forming process, which could create excessive internal stress in the glass. This could eventually cause the glass to shatter into small pebble-like pieces and injure vehicle occupants. This issue does not affect Maxima vehicles equipped with the power glass sunroof. What Nissan Will Do The glass in the SkyView Roof will be inspected and replaced if required. What You Should Do Contact your Nissan dealer at your earliest convenience in order to arrange an appointment to have your vehicle repaired. Please bring this notice with you when you keep your service appointment. Instructions have been sent to your Nissan dealer. Until your vehicle is repaired, you may wish to keep the shade for the SkyView roof closed. If the glass should break, this will help reduce the chance that broken glass will unexpectedly fall inside the occupant compartment. If the dealer fails, or is unable to make the necessary repairs free of charge, you may contact the National Consumer Affairs Office, Nissan North America, Inc. at P.O. Box 191, Gardena, California 90248-0191. The toll free number is 1-800-NISSAN1 (1-800-647-7261). You may also contact the Administrator of the National Highway Traffic Safety Administration, 400 Seventh Street SW, Washington, D.C. 20590 or call the toll free Safety Hotline at (888) 327-4236. If you have paid to have a SkyView Roof replaced prior to this campaign, you may be eligible for reimbursement of the related expense. Contact Nissan Consumer Affairs at the number listed above for additional information on how to obtain a reimbursement. Federal regulations require that any vehicle lesser receiving this recall notice must forward a copy of this notice to the lessee within ten days. Thank you for your cooperation. We are indeed sorry for any inconvenience this may cause you. 10/10
