AGENDA ITEM NO: CC-H-4 - CITY OF CLOVIS - REPORT TO THE CITY COUNCIL TO: FROM: DATE: SUBJECT: Mayor and City Council Public Utilities Department Receive and File - Leak repairs on West Fremont On January 9th the Water section planned on replacing three valves in the 200 block of West Fremont. After the valves were replaced, the main was turned back on, but there was a leak on a flange connection that they were unable to stop. The crew shut the three valves to isolate the leak and not affect any residents. Monthly Report- January 2014 Page 1of8

The following day the crew shut down the main again and dewatered it to install a new gasket. The three valves were uncommon and German made. They were unable to find parts to repair them so they had to be replaced with a special gasket which is designed to work with misalignments or imperfections on the flange face (Full Face Flange-Tyte gasket made by US pipe). On January 5th a vehicle hit a light pole and damaged the underground plumbing at 2479 Keats. The driver was arrested for a DUI while staff was there and they had to wait for a PG&E crew to respond to de-energize the light pole before repairs could be made to the water main. Water department staff also assisted their crew with our crane to remove the pole. \ Monthly Report - January 2014 Page 3of8

SAFETY COORDINATION City Council Report On January 8, 2014, Chad Fitzgerald and Andy lsolano of the Clovis Fire Department provided annual Fire Extinguisher training for Public Utilities Department employees. This annually required training is always a good exercise and is representative of the collaborative relationship the two departments share. On January 14th & 15th, 2014, fifteen Public Utilities Department employees were either recertified or certified for the first time for their CPR/First Aid/AED skills. Again, the Clovis Fire Department provides instructors to give this essential training to our employees so that our workforce is prepared to act appropriately in an emergency. In 2013, Kimberley Libecki, Safety Officer, and Rob Rush, Utilities Manager, successfully completed Cal OSHA's trainer certification courses for Trenching and Excavation Safety for the Competent Person through the University of California, San Diego. Since their completion of the training they worked to develop a training program specific to Public U1tilities Department trenching and excavation operations. Once this training program was approved by the UC San Diego instructor, Ms. Libecki and Mr. Rush conducted their first Trenching & Excavation Safety for the Competent Person class for the Department's Water Services Crew on January 28, 2014. Cal/OSHA statistics indicated that 2 workers per month are killed in trenching and excavation related activities. The Water Services crew is performing trenching and excavation activities on a weekly basis and their knowledge of the hazards and correct methods to perform this work is critical to their safety.
