COMPANY REQUIREMENTS New Entrant Safety Audit - Required for During your first 18 months your company will be monitored, within 6-12 months or sooner, you will receive a safety audit. A Safety Audit is a review of a motor carrier s records designed to verify that a carrier has in place compliance with the following: Relevant Federal Motor Carrier Safety Regulations Hazardous Materials Regulations (if required) And up to date related record-keeping requirements such as driver qualification files, hours of service log books, vehicle inspection reports, etc... Company Drug & Alcohol Test Program FREE - Required for Regulation CFR 382.115 Your company is responsible for creating a Drug & Alcohol Workplace Policy, agreements for all employees, educational material & administrator training. Available on dotchecklist.com with purchase of consortium Supervisor Training FREE - Required for Regulation CFR 382.115 The FMCSA requires Reasonable Suspicion Training for Supervisors as a measure to help keep the roads safe. Supervisor Training consists of 1 of hour each of DOT drug and DOT alcohol training (2 hours total). Available on dotchecklist.com with purchase of consortium UCR (Unified Carrier Registration) $81 - Required for UCR Act 49 All interstate carriers - regardless if they are private property or for-hire, must file for UCR. You can do this by going to www.ucr.in.gov The UCR Program requires individuals and companies that operate commercial motor vehicles in interstate or international commerce to register their business with a participating state and pay an annual fee based on the size of their fleet. BOC-3 - Designation of Agents $25 - Required for Regulation CFR 366 A BOC-3 is a filing that designates legal agents upon which legal process may be served to you.
DRIVER REQUIREMENTS DOT Physical (by a licensed Physician registered with FMCSA) $45 - Required for Regulation CFR 391.41 Before operating behind a commercial vehicle all drivers must complete a physical by a licensed physician registered with the FMCSA. Available on dotchecklist.com Pre-employment Drug Test $35 - Required for Regulation 382.301 No employer shall allow a driver, who the employer intends to hire or use, to perform safety-sensitive functions (like driving) unless the employer has received a controlled substances test result from the MRO or C/TPA indicating a verified negative test result for that driver. You are not allowed to use any driver (including sole-owner operators) before a driver passes a pre-employment drug test. VEHICLE REQUIREMENTS Display USDOT# and Company on Vehicle $32 - Required for Regulation CFR 390.21 All drivers with a DOT# must place a visible DOT# and Company name on both sides of the vehicle they are operating. IEP's additionally are required to place there VIN# (the last 8 letters of the VIN) on the equipment used. Available on dotchecklist.com IFTA (International Fuel Tax) license $varies by state - Required for IFTA IFTA is a cooperative agreement among the Canadian provinces and most American states to make it easier for interjurisdictional carriers to report and pay taxes on the motor fuels they use. The agreement allows registered interjurisdictional carriers to get one licence, issued by their base jurisdiction, to report and pay motor fuel taxes to a single jurisdiction. If you are going on a single trip, you can obtain a single IFTA permit, or apply for an IFTA sticker at your local DMV. Emergency Equipment $varies by store Required for Regulation CFR 393.95 Each truck, truck tractor, and bus (except those towed in driveaway-towaway operations) must be equipped as follows: 3 bidirectional emergency reflective triangles or at least 6 fusees or 3 liquid-burning flares (restrictions apply) and 1 Fire Extinguisher. Motor Carriers - fire extinguisher must have an Underwriters' Laboratories rating of 5 B:C or more; or Two fire extinguishers, each of which has an Underwriters' Laboratories rating of 4 B:C or more. Transport hazardous materials (that requires placarding) - fire extinguisher must have an Underwriters' Laboratories rating of 10 B:C or more. Vehicle Inspection Report (Completion of each day's work) FREE - Required for Regulation CFR 396.11 A driver must prepare a "Driver Vehicle Inspection Report" at the completion of each day s work and shall submit those reports to the motor carrier upon his/her return to the home terminal. Must implement a vehicle inspection schedule for all vehicles, including a record of pre-checks before operating behind a vehicle.
