Effective Date:12/17/16 Standard 15.3 Dcument Number:KUCSH0032 Rev:09 15.3.1 INTRODUCTION: 15.3.1.1 This standard establishes the minimum requirements fr safely perating vehicles and mbile equipment n r ff KUC prperty. This standard applies t all vehicles, including mbile equipment wned and / r perated by KUC r its cntractrs, that is used n r ff-site, and t the persnnel wh perate such equipment. 15.3.2 DEFINITIONS: 15.3.2.1 Class 2 Strbe Light Hazard warning strbe light certified by the Sciety f Autmtive Engineers. Cite law and safety cncern 15.3.2.2 Heavy Mbile Equipment (HME) Mbile equipment specially designed fr executing mining tasks. 15.3.2.3 Light Duty Vehicle (LDV) Any land-based vehicle with a grss vehicle weight rating less than 10,000 punds (example: van, pickup, SUV, etc.). 15.3.2.4 Medium Duty Vehicle (MDV) Any land-based vehicle with a grss vehicle weight rating greater than 10,000 punds (example: service truck, bus, crane truck, etc.). 15.3.3 REQUIREMENTS: 15.3.2.1 Cmprehensive risk analyses shall be cnducted t identify the factrs that impact vehicles and driving safety. The risk analyses will: Include all aspects f vehicles and driving and will have up-tdate actin plans in place t manage identified issues. (The plans will be reviewed peridically t imprve vehicle and driving safety) Identify the cnditinal and behaviral factrs that impact vehicles and driving safety at KUC (See Risk ID: 64). Outline the prtcl fr site driving and licensing f KUC emplyees and cntractrs (See Risk ID: 1382). Review the elements f the Ri Tint Light Duty Vehicle Guideline (See Risk ID: 1383). 15.3.2.2 All vehicles used fr wrk purpses require a risk assessment. Risk assessment dcumentatin will be kept n file at each facility and will be updated as required. A VEHICLES & DRIVING RISK ASSESSMENT FORM is attached as Exhibit 15.3.1. The assessment must: Invlve a representative frm the peratin and maintenance grups wh will use the vehicle; and Address all aspects f safe peratin including munting and dismunting, handling, driver visin, brake failure, tire blw-uts, seat belts, fire extinguishers / suppressin systems and access / egress fr peratrs, maintainers and passengers, rll-ver prtectin and ergnmics. 15.3.2.3 The peratr must cmplete a pre-peratin inspectin checklist befre the equipment is placed int peratin n each shift. The inspectin checklist will be based n the risk assessment fr that fleet r unit. When defects make cntinued peratin hazardus, the equipment shall be taken ut f service and placed in a designated area psted fr that purpse, r an alternative effective methd f marking / tagging the unit shall be used t prhibit further use until the defects are crrected. Equipment will nt be perated if the preshift inspectin identifies a serius r ptentially serius safety prblem. Defects n vehicles r mbile equipment, affecting safety that are nt crrected immediately shall be reprted t the apprpriate supervisr and recrded. Inspectin cards will remain n file fr six mnths. 15.3.2.4 All vehicles and mbile equipment must be fitted with fixed seats and safety belts fr the driver and all passengers. The driver and all passengers must wear their seat belt at all times. 15.3.2.5 All light vehicles used fr wrk purpses must cmply with all aspects f the Ri Tint Light Vehicle Guidelines unless the risk assessment specifies therwise (See Risk ID #1383). 15.3.2.6 In general, all vehicles and mbile equipment used fr wrk purpses that are capable f exceeding the lwest psted speed limit must be fitted with a functinal speedmeter. Exceptins t this requirement are made fr thse types f equipment where perating speeds are strictly cntrlled and enfrced by site perating prcedures / plicies and site licensing requirements. 15.3.2.7 All persns required t drive / perate vehicles at KUC must cmply Page 2 f 11 Kennectt Utah Cpper. Effective Date:12/17/16 Standard 15.3 Dcument Number:KUCSH0032 Rev:09 with the requirements utlined in the KUC Risk Assessment - Prtcl fr Site Driving and Licensing f Emplyees and Cntractrs (Risk ID #1382) and at a minimum, have : A valid State r Civil driving license and depending n the vehicle, a valid Department f Transprtatin Cmmercial Drivers License, and A site specific license when required t drive at the Mine r Tailings Impundment. Certain exceptins t these licensing requirements may be granted in rare circumstances at the sle discretin f KUC. 15.3.2.8 All accident / incidents that result in an ccupatinal injury r illness and / r damage t physical assets and near miss incidents shall be prmptly reprted and investigated. (See Safety & Health Standard 6.1) In the event f an ff-site accident in a cmpany wned vehicle refer t Exhibit 15.3.2 Off-site Vehicle Accidents. 15.3.2.9 Vehicles and mbile equipment shuld be kept free f excess dirt, grease and il. Cabs and beds will be kept clear f lse items such as pp cans, lunch pails, unsecured fire extinguishers, rags, etc.,. Windws are t be cleaned as ften as necessary. Peridic cleaning will be cmpleted as needed. 15.3.2.10 Prir t entering shp facilities fr repair wrk, mbile equipment is t be cleaned f debris build-up. 15.3.2.11 Beds f trucks are nt t be used as refuse cntainers. Only necessary equipment may be carried and is t be neatly stred. Tl lckers and bxes are t be rganized and free f debris. Hwever, direct hauling f debris t prper dispsal cntainer / landfill is acceptable where the lad is prperly secured and cvered. 15.3.2.12 Fundamentally Stable Parking: Mbile equipment shall nt be left unattended unless prperly secured unable t mve under its wn energy, r the influence f gravity. All peratrs will park their vehicles in a psitin that will nt endanger persnnel r equipment. Then t satisfy the requirements f fundamental stability the peratr will: 1. Place the transmissin in park r lw gear. 2. Apply the park break 3. Turn ff the engine, and 4. If the vehicle is nt psitined against a suitable berm, in a parking ditch, r ther designed immbilizing device, place a wheel chck n a tire in the directin the vehicle will rll. Determinatin will be made in the risk assessment whether a vehicle can tw r be twed. Rigging r lifting equipment will nt be used fr twing applicatins. 15.3.2.14 Speed limits and signage will be set in accrdance with generally accepted rad safety standards. Where there are established speed limits and traffic rules, they must be reviewed regularly and enfrced. Traffic rules, signals, and warning signs shall be standardized at each site. 15.3.2.15 The use f any cellular device while perating any vehicle n cmpany prperty r while n cmpany business is prhibited. DRIVING IN THE MINE 15.3.2.16 N persn may perate a vehicle inside the mine area unless he / she has a pit license r is cntinuusly escrted. Because f the cnstantly changing mine cnditins, n persn shall be licensed t drive in the mine unless they are required t d s mre than nce in a tw-week perid. Site licensed emplyees and cntractrs wh are away frm the wrkplace fr lnger than 14 cnsecutive calendar days r 10 wrking days must receive a rerientatin prir t returning t wrk and perating vehicles in the Mine. Renewal f site licenses will be based an assessment f cmpetency t drive and / r perate equipment. The frequency f assessment will be either annual, r derived frm a risk assessment fr each vehicle type. All light and medium duty vehicles perating n haul rads must have: Headlights n at all times. An amber strbe light n at all times. Strbe lights must meet r exceed Class 2 certificatin. A white lighted whip flagple with a whip flag that extends at least 12 ff the grund and is visible frm all directins. Clear identificatin n sides, rear, and rf f the vehicle with letters / numbers at least A4 in size (8.3 x 11.7 in). Page 4 f 11 Kennectt Utah Cpper. All ft traffic entering a shvel pit r appraching a manned shvel, dzer, frnt-end lader r blasthle drill must make psitive (tw way) cntact with the peratr f that equipment prir t entering the wrking area f the equipment. - Psitive (tw-way) cntact must be made at least 50 meters (160 feet) away frm the equipment r befre passing under a cable bridge, whichever is greater. Psitive cntact des nt have t be made when: - A haul truck is entering an area t be laded. - A haul truck is passing near a dzer in the dump area. - An area is physically demarcated by a berm r ther barrier s as t allw passing. - A vehicle is passing (rather than appraching ) a shvel dzer, frnt-end lader r blasthle drill in transit t anther destinatin, prvided that pass is nt made within the immediate wrking radius f the equipment being passed. Overtaking (passing) in the pit area is allwed subject t the exercise f safe and prudent driving practice. N vehicle will vertake a haul truck r water truck n the blind side withut first making psitive cntact with the driver. Vehicles may nt pass at any time within a rad intersectin, at the crest f a hill, r under any circumstance where visin is restricted. DRIVING ON THE TAILINGS IMPOUNDMENT 15.3.2.17 N persn may perate a vehicle n the tailings impundment unless he / she has a tailings impundment license r is cntinuusly escrted. Renewal f site licenses will be based an assessment f cmpetency t drive and / r perate equipment. The frequency f assessment will be derived frm a risk assessment fr each vehicle type, but will nt exceed fur years. (See MSHA Petitin fr Mdificatin Dcket N. M-86-20-M) Each access pint frm the uter radway area t the impundment radway shall have psted warnings that the Page 5 f 11 Kennectt Utah Cpper. The impundment radway shall nt be used as a thrughfare and traffic shall be limited t a ne-way directin. The impundment radway speed shall nt be mre than 15 miles per hur, depending n weather, rad, and visibility cnditins. Speed limit signs shall be psted at apprpriate entrance lcatins t the impundment radway. Reflective delineatrs shall be installed at n mre than 500-ft intervals alng the impundment radway and shall be visible, under nrmal weather cnditins, frm equipment using the radway. 15.3.3 TIRE AND RIM SAFETY 15.3.3.1 Tire and rim safety requirements applies t tires and rims f size 24 inch and greater. It is supprted by the tire and rim safety wrk cycle 15.3.3.2 Operatins must establish a Tire management plan that is apprved by the senir manager at the peratin and reviewed every 12 mnths. 15.3.3.3 There must be demnstrated jb cmpetencies and safe wrk prcedures fr all tire maintenance, servicing activities and tire fire emergency respnse. The cmpetencies must specify the frequency fr re-certificatin, which must be n less than every three years. 15.3.3.4 N persn may apprach a vehicle within 24 hurs f; lightning strike, cntact with high vltage electricity, tire fire. An exclusin zne f 1000 feet f a tire fire must be established and nly be accessed by emergency service persnnel that are shielded while fighting the fire. 15.3.3.5 Restricted wrk znes (RWZ) and exclusins znes must be established fr the tire installatin, remval and handling prcesses. 15.3.3.5 All tire and rim handling equipment must have fall back preventin in place prir t anyne entering the RWZ. 15.3.3.7 Tires with split rims must be deflated t zer and ther tires t a nminal pressure n greater than 5psi prir t remval f any retaining devices. In a dual assembly bth tires must be deflated. 15.3.3.8 Tire inflatin is subject t the fllwing requirements: Remte inflatin must be used fr all tire inflatin Page 6 f 11 Kennectt Utah Cpper. Bth tires in a dual assembly must be dismunted and inspected. 15.3.3.9 N welding, cutting r applicatin f heat surces t a rim r wheel must be dne while the rim r wheel is fitted with a tire - whether inflated r deflated. 15.3.3.10 A peridic testing and/r inspectin regime must be in place fr tires, rims/wheels and assemblies. 15.3.3.11 All tires, rims/wheels must be made unserviceable when deemed unfit fr service r befre being sent ff site fr dispsal. 15.3.3.12 A tracking system must be in place t track the lifecycle f tires, rims/wheels. 15.3.4 RESPONSIBILITIES 15.3.4.1 Operatrs are respnsible fr cnducting and dcumenting the apprpriate pre-peratinal inspectin prir t perating the vehicle. Operatrs are respnsible fr the rutine cleanliness f cabs, platfrms and windws f vehicles. 15.3.4.2 It is the respnsibility f the driver t ensure that all passengers wear their seat belts, at all times when the vehicle is in peratin. 15.3.4.3 Each KUC facility shall have a system in place t ensure that the renewal f site licenses will be based n an assessment f cmpetency t drive and r perate the equipment. The frequency f assessment will be either annual, r derived frm a risk assessment fr each vehicle type. 15.3.5 TRAINING 15.3.5.1 N persn may drive a vehicle unless he / she is trained, cmpetent, tested and licensed t perate that vehicle. The training must address hazards assessed fr that vehicle r fleet, and the task(s) fr which it is t be used. 15.3.4.2 N persn may perate a vehicle inside the mine area unless he/she has cmpleted the pit driver training r is cntinuusly escrted. In thse cases where the driver is unable t attend t these matters, the respnsibility rests with any ther KUC / Ri Tint emplyee present at the accident. Cntact plice. After an Accident The driver r any ther KUCC emplyee invlved in an accident in a cmpany vehicle must infrm their manager and the Ri Tint Risk Management Office as sn as pssible after the accident and cmplete a Ri Tint incident reprt frm (See Exhibit 15.3.3). All vehicle accidents must be investigated in accrdance with Standard 6.1 Accident / Incident Investigatin. If there is damage t the KUC vehicle, and the vehicle is safe t perate, return it t the facilities and cntact Kennectt Operatinal Services (KOS) wh will arrange fr repairs. If the vehicle is nt safe t perate, if pssible, call KOS t arrange fr twing. If the Plice arrange the twing, get the apprpriate infrmatin and cntact KOS fr fllw-up. All persnnel shuld be aware f the trauma that can be experienced by peple invlved in accidents r when rendering assistance. This trauma may surface sme time after the accident.
