Personnel Involved Assessed By Project Operator(s) CPT Operator Notes re Operator/Off-Sider Training Allan McConnell In situ testing is a niche business with very few rigs in Australia. There are no specific training courses or certificates Qualifications/Training: Registered Professional Engineer available. IGS undertakes our own in-house training, much of this on-the-job. Off-siders may be trainee rig operators, or even well-experienced trained operators working as assistant to a newly trained operator for mentoring purposes. General Notes: This Risk Assessment was made on the date shown below. It is a part of IGS s normal operation that the rig is also inspected daily in a pre-start as a precaution against changes that may have occurred (eg equipment failures or modifications) that may impact on risk This is recorded on the Operator s Daily Record Sheet and Pre-Start Sheet. GENERAL INFORMATION ON THE RIG Rig Data & Purpose Multi-Purpose rig on skids Year Built: 2012 Mass 1.4t min adjustable to 4t Push capability (up to 150kN) Test Types: CPTu & Tee-Bar DMT & SDMT Vane Shear Also: Piezometers Standpipes Vertek piston sampling Various Sampling Types Mobility Information This rig is normally moved ( skidded ) about on site by pulling from an anchorage by its own 35kN hydraulic winch, or by towing with some other machine or vehicle. It can however be mounted to a variety of other platforms, eg a jack-up barge, flat-bed truck tray, concrete floor, etc. When used normally, ie using its own winch, travel speed is limited to a very low value, typically less than 1km/hr. The rig is delivered to site on a truck, either our own Mitsubishi flat bed crane truck or some other suitable vehicle. It is lifted off and on the truck either in one piece or broken down into component parts of less than 1.5t each. This depends on the situation and particularly the capacity of the loading crane, available slings, etc. Issued By: Allan McConnell 28/10/2012 Page 1 of 5
1 CRUSHING AND CUTTING - Can any person be crushed or cut due to: 1.1 unexpected movement of the rig rig cannot move unexpectedly skid-mounted - no tracks or wheels amber flashing beacon N/A fitted to towing vehicle rear view mirror N/A fitted to towing vehicle pedals/controls non-slip no pedals but controls are good controls have appropriate knobs reversing lights fitted N/A traversing lights both ends reversing camera fitted N/A not required rig drives both ends 1.2 lack of capacity for plant to be slowed, stopped or immobilised rig head moves at slow speed down 20mm/sec max up 50mm/sec all controls are deadman type if levers released movement stops emergency isolator fitted rig can only traverse very slowly winch system is dead-man braked if levers are released winch stops 1.3 the plant tipping or rolling rig has very low centre of gravity 90% of rig mass is below 1m height rig can only travel at low speed traverses at slow walking speed 1.4 being thrown from the plant rig can only travel at low speed no-one actually travels on the rig 1.5 coming into contact with sharp objects engine is part enclosed/ guarded no visible signs of sharp objects Issued By: Allan McConnell 28/10/2012 Page 2 of 5
2 STRIKING - Can any person be struck by moving parts due to: 2.1 working pieces being ejected push head is purpose-designed only work pieces are CPT push rods push-pull clamps restrain rods these cannot be ejected from the pusher moves slowly - 2cm/sec slow-moving purpose-designed nip point decals fitted pusher system 2.2 mobility of plant travelling reversing/travel alarm N/A fitted to tow vehicle (if used) amber flashing beacon N/A fitted to tow vehicle (if used) rear view mirror N/A reversing lights fitted N/A reversing camera fitted N/A rig can only travel at slow speed 2.3 controls unidentified controls are readily identifiable and are labelled in clear English 3 ENTANGLEMENT Can anything become entangled in moving parts 3.1 in engine area engine is part enclosed/ guarded no moving parts are exposed 3.2 during testing pusher moves slowly - 2cm/sec nip point decals fitted 4 FALLING SLIPPING Can any person fall/slip due to: 4.1 lack of proper work platform work platform work platform is ground next to rig depends on site setting 4.3 poor housekeeping no visible lubricant leakage no consumables used in operation ie no litter build-up work area is kept clean Issued By: Allan McConnell 28/10/2012 Page 3 of 5
5 ERGONOMIC Can any person be injured due to: 5.1 poor seating operator stands when working operator deems this satisfactory off-sider stands when working off-sider deems this satisfactory 5.2 constrained body effort all controls are in operator s reach 6 HIGH TEMPERATURE Can any person be burnt due to contact with hot parts: 6.1 around engine area engine is heat guarded exhaust is out of work space exhaust blows away from work area 6.2 around rig during operation engine is heat guarded exhaust is out of work space exhaust blows away from work area hydraulic hoses are wrapped hot warning decals fitted observations indicate hydraulics may become hot but not scalding 7 ELECTRICAL Can any person be shocked due to: 7.1 coming into contact with live electrical conductors main wiring on rig is 12v 7.2 lack of tag out procedure 240 mini alternator is tagged 7.3 damaged leads & switches no visible signs of damage 7.4 batteries are not protected batteries are in separate space Issued By: Allan McConnell 28/10/2012 Page 4 of 5
