PRODUCT USE DESCRIPTION / IMPORTANT SAFETY INSTRUCTIONS PRODUCT USE DESCRIPTION The PG and Ultrasound Tables are used to support the patient during examinations and procedures. It is intended to be operated by a healthcare professional in a medical environment. No special training is required but a review of the following Safety Instructions is important for the safety of the operator and patient. The healthcare professional should read and understand this entire manual before use with a patient. There are no known contraindications to the use of this equipment. SYMBOL IDENTIFICATION This symbol, when used in this manual and on product labels, represents a caution warning. Be sure to read and comply with all precautions and warnings. This symbol, when used in this manual and on product labels, warns against an electrical shock hazard. Be sure to observe and comply with all warnings. This symbol, when used in this manual or on product labels, indicates a Protective Earth (Ground) Terminal. This symbol, when used in this manual or on product labels, indicates that the product should be protected from moisture. The humidity specifications for Transport & Storage are listed in the environmental conditions section of this manual. IMPORTANT SAFETY INSTRUCTIONS CAUTION READ AND SAVE THESE INSTRUCTIONS The Fowler (Backrest) and Leg Rest sections are not designed to support the entire weight of the patient. Do not sit on the Fowler or Leg rest sections. The table is not to be used as a gurney or to transport patient from room to room. When the footboards are in the down position, it is possible for them to hit the ground in low height and high tilt conditions. Operate the table with the footboard up if possible. If not, then check ground clearance to prevent damage to the footboard. Medical Electrical Equipment needs special precautions regarding EMC and needs to be installed and put into service according to the EMC information provided in this manual. 1

IMPORTANT SAFETY INSTRUCTIONS The use of accessories, transducers, and cables other than those specified by the manufacturer, may result in increased emissions or decreased immunity of the table. The table should not be used adjacent to or stacked with other equipment and that if adjacent or stacked use is necessary, the table should be observed to verify normal operation in the configuration in which it will be used. The table is designed to be a stand alone table used with hand held ultrasound probes. This table must not be modified or incorporated into any other equipment. As with any moving mechanism there are potential pinch points around and underneath the table top. It is the responsibility of the operator of this equipment to insure that bystanders are not in the area below or around this equipment during operation. Proper operation of this equipment is very important for the safety of the operator, patient, and any other individuals in the area of this equipment. Directions for use of this equipment are described in this manual. The operator should read these sections carefully. Weight Limit: (patient and accessories) 550 lbs./250 kg. Do not exceed. Be certain that the table is completely lowered without any tilt being present prior to discharging an ambulatory patient. The patient may lose balance and fall. This table is not suitable for use in the presence of flammable anesthetic mixture with air or oxygen or nitrous oxide. Do not put excessive weight on the extended end (beyond the main top) of the cardio panel. Maximum capacity is 150 lbs. / 68 kg. When lowering the table or using the Trendelenburg functions, make sure there is nothing underneath the table top that can impede motion (like stools, cabinets, accessory parts, cleaners, etc.). Use this furnishing only for its intended use as described in these instructions. Do not use attachments not recommended by the manufacturer. Close supervision is necessary when this furnishing is used by, or near children, invalids, or disabled persons. All motor actuators & lifting columns have a duty cycle of approximately 10%, meaning 2 minutes of operation within a 20 minute period. Exceeding this duty cycle can damage this equipment. WARNING To reduce the risk of burns, fire, electric shock or injury to persons: 1. Unplug this furnishing from the electrical outlet before cleaning. 2. Unplug from outlet before adding or removing parts. 3. Never operate this furnishing if it has a damaged cord or plug, if it is not working properly, if it has been dropped or damaged, or dropped into water. Contact Oakworks Customer Service before use. 4. Keep the cord away from heated surfaces. 5. Never drop or insert any object into any opening. 6. Do not use outdoors. 7. Do not operate where aerosol (spray) products are being used or where oxygen is being administered. DANGER For procedures where accidental table motion is a safety concern, the handset control must be locked out. Risk of electric shock - Connect this furnishing to a properly grounded outlet only. See Grounding Instructions in this manual. Electrical Shock Hazard. The power supply/control module is located under the table seat. No user serviceable parts are inside the control box. Refer servicing to qualified personnel. Unplug wall plug prior to contact with any cables connected to the power supply. 2

INSTALLATION All tables come completely assembled and ready to use. Plug the cord into a functioning outlet that is rated for the table. (see Grounding below) Arrange the power cord and control cords so that they will not create a tripping hazard and where the controls are located to your liking and are conveniently accessible. Be sure access to plug is not blocked for disconnecting the table from power. GROUNDING DANGER Risk of Electric Shock - Connect this furnishing to a properly grounded outlet only. This product must be grounded. If it should malfunction or break down, grounding provides a path of least resistance for electrical current to reduce the risk of electric shock. This product is equipped with a cord having an equipment-grounding conductor and a grounding pin. The pin must be plugged into an appropriate outlet that is properly installed and grounded in accordance with all local codes and ordinances. See U.S. sample below. DIRECTIONS FOR USE SIDE RAIL USE TO LOWER SIDE RAILS Push side rail in lightly towards the center of the table, then push locking knob down towards the table base. Fold side rails down and under the table. TO RAISE SIDE RAILS Grasp side rail and rotate to the raised position. When fully up, the side rail will lock into position. Check that the rail is locked by applying light outward pressure. RETRACTABLE FOOT RESTS (STIRRUP) USE CAUTION Pinch Point - Keep fingers away from pinch point. 1. Fully lower the manual adjustable foot rest to allow access to the stirrup. 2. Grasp Stirrup handle & pull out firmly while lifting slightly. Pull fully out. 3. Fold out to fully open position. To store stirrups, lift slightly, rotate towards the middle of the table and fold the foot rest down. Then lift slightly and push into the storage position. 4. To rotate stirrup to the desired angle, lift slightly and move to the desired position. There are 3 detent positions. Stop lifting and move slightly left or right for the nearest detent position. 5. To reduce stirrup length, lift the handle slightly and push into desired position. Stirrup will automatically lock into nearest detent position. DETACHABLE ARM BOARD USE CAUTION Do not put excessive weight or pressure on the Arm Board. This product can be used on any of Oakworks ultrasound tables with a T-Rail Adapter and can support a variety of positioning configurations. 20

DIRECTIONS FOR USE ADJUSTABLE ANGLE BACK SUPPORT CAUTION Do not sit on the Adjustable Angle Back Support. (Max Capacity 150 lbs. (68 kg.)) TO RAISE BACK SUPPORT Lift panel to desired angle. Panel will automatically lock into nearest set point. Panel can be raised to a full 90º position. TO LOWER BACK SUPPORT Press support bar against plastic plate and hold until back support is lowered to desired position. CENTRAL BASE LOCKING SYSTEM LOCKING THE SYSTEM Press down on locking arm with your foot to lock table to the floor. UN-LOCKING THE SYSTEM Lift up on locking arm with your foot to unlock table from the floor. LOCKING CASTERS USE All four casters should be locked at all times during use with a patient. LOCKING THE CASTERS 1. Place your shoe on the end of the casters locking tab. 2. Press firmly downward until you here a clicking noise and the caster is locked. UN-LOCKING THE CASTERS 1. Place your shoe on top of the casters locking tab. 2. Press firmly inward and downward until you here a clicking noise and the caster is unlocked. 21

DIRECTIONS FOR USE MANUAL ADJUSTABLE FOUR POSITION LEG REST USE CAUTION Do not sit on the Fowler (Backrest) or Leg Rest. (Max Capacity 150 lbs. (68 kg.)) TO RAISE THE LEG REST Raise the leg rest by pulling it up from the lower center end of the table to the proper position for the procedure. Use a slow or medium speed. If you raise the leg rest too fast, the height locking mechanism will not engage. The leg rest will lock into the closest height position. TO LOWER THE LEG REST Raise the leg rest above horizontal to unlock the latch. Lower the leg rest to the fully down position. For intermediate positions, follow directions TO RAISE THE LEG REST. PROCEDURE TRAY USE CAUTION Make sure the Leg Rest is fully lowered before using the procedure tray. Do not sit or push on the Procedure Tray when extended from the seat. Maximum Weight rating of 15 lbs. (6.8 kg.) Slide out the Procedure Tray by pulling on the center slot of the tray holder until the tray stops. After a procedure is performed, remove the Procedure Tray with both hands at the side notches from the tray holder. Discard fluids, clean and place back into tray holder. CLEANING AND DISINFECTING INSTRUCTIONS If fluid spill under the seat cushion, follow these instructions: 1. Remove the seat pad by pulling up from the edge. The pad is held on by Velcro. 2. Pull out the 2 pins on both sides of the metal plate near the end of the Procedure tray holder. Slide the tray holder and Procedure Tray out from the seat section. 3. Unscrew the 4 black handles at the corners of the metal plate. Slide the metal plate off from the seat section and disinfect all surfaces. 22

DIRECTIONS FOR USE SLIDING CARDIO PANEL USE CAUTION The Sliding Cardio Panel allows you to adjust the opening of the size you need, up to 15 (38 cm.). PRIOR TO MOVING THE CARDIO PANEL Make sure patient weight is not on the panel. Make sure all parts of the patient s anatomy are clear of the sliding cardio panel when closing. Do not sit or put excessive weight on the sliding cardio panel. (Max Capacity 150 lbs. (68 kg.)) MANUAL CARDIO PANEL 1. Grasp the Cardio Panel. 2. Lift up slightly. 3. Slide the panel to the desired opening width. ELECTRIC CARDIO PANEL 1. Use the hand control or optional foot control to open the cardio panel to the desired opening width. INTEGRATED FOOT BOARD USE CAUTION Mount & dismount the table in the flat position. Do not use the foot board as a step to mount the table. When the foot boards are in the down position, it is possible for them to hit the ground in low height or trendelenburg conditions. Operate the table with the foot board up if possible. If not, then check ground clearance to prevent damage to the foot boards. Make sure the foot boards are securely positioned before the patient places weight on them. DIRECTIONS FOR USE TO MOVE THE FOOT BOARD TO THE 4 (10 cm.) POSITION Grasp the rod at 90º bend and push into the table to unlock. With your second hand, grasp the foot board & rotate 180º to the down position. Grasp the rod at 90º bend and pull to the 4 (10 cm.) position. With your second hand, grasp the foot board and rotate 180º to the up position Grasp the rod at 90º bend and pull away from the table to lock in position. Check locking by moving footboard left and right. VASCULAR PATIENT RESTRAINING STRAP USE CAUTION The Vascular Patient Restraining Strap must be used during all procedures to prevent from falling off the table. VASCULAR FLAT TOP TABLES Install the strap under the rail to the top. VASCULAR WITH FOWLER TABLES Install the strap under the Fowler section & above all rails. PAPER ROLL HOLDER USE To load the paper, pull down on the 2 paper holder support arms located on the under side of the Fowler section and rotate to the 90º position. Insert the rod through the paper roll and place the rod into the holes in the support arms. Pull paper and thread through the paper cutter across the upholstered top to keep in place and to cut used paper after a procedure. Note: The paper cutter strap is attached with hook and loop fastener. This allows strap tension adjustment if necessary. 24

DIRECTIONS FOR USE TABLE EXTENDER USE CAUTION The Table Extender lengthens the table allowing for access to the patient s neck for exams such as carotid artery and thyroid. Do not put excessive weight or pressure on the table extender. NOTE: You will need an accessory mounting plate in order to use the table extender. 1. Locate the Accessory Mounting Plate. 2. Insert the Table Extender tubes into the Accessory Mounting Plate. QUICKLOCK FACE REST * U.S. Patent #5,177,823 CAUTION Do not put excessive weight or pressure on the Quicklock Face Rest. Do not extend the dowels of the Quicklock Face Rest 3 (7.6 cm.) from the Fowler (Backrest) section. Double articulating action provides nearly limitless positioning options with the QuickLock Face Rest. The adjustable pad takes pressure off sensitive facial areas while providing superb support. The QuickLock Face Rest can be used in both prone and supine positioning and folds out of the way when not in use. Adjustments are made easily by loosening the cam locks, adjusting for height and angle and then locking the cam locks into the desired position. QuickLock Face Rest NOTE: You will need an accessory mounting plate in order to use the QuickLock face rest. Important Safety Note: Maximum Distance from edge of Table = 3 (7.6 cm.) Insert dowels into Accessory Mounting Plate Loosen Cam Locks & adjust face rest into place. Tighten the Cam Locks. The Face Rest can be used for either prone or supine positioning. 25

DIRECTIONS FOR USE PATIENT LIFT COMPATIBLE BASE This option allows the use of patient lifts. The high clearance base permits patient lifts to slide easily under the center of the table for stable transfers. This option must be ordered with table. Includes larger, 5 diameter Heavy Duty locking Casters Central Base Locking System is not available with this option. T-RAIL ADAPTER USE Clamp & pole not included. The T-Rail Adapter can be attached at designated locations on the table, allowing the use of any t-rail accessory. Attach or remove using the knobs shown. NOTE: on 27 wide tables, the paper roll holder support arm must be folded down to allow installation of the T-Rail adapter next to the paper roll holder. After installation, the paper roll holder support arm can be raised to the normal position. HAND CONTROL SAFETY LOCK OUT On specialty table models, there is a hand control lock out located next to the side rail. 26

CLEANING & DISINFECTION RECOMMENDED CLEANERS/DISINFECTANTS DANGER To reduce the risk of electric shock: - Always unplug this furnishing from the electrical outlet before cleaning. The following cleaners were tested by OAKWORKS and found to be acceptably compatible with all materials used in construction of our tables: 1. Formula 409 2. Fantastik 3. Green Windex The following disinfectants were tested by OAKWORKS and found to be acceptably compatible with all materials used in construction of our tables: 1. Parker Labs: Protex TM (EPA reg# 6836-152-82613) 2. Healthlink E-Z Kill (EPA reg# 59894-10-67454) 3. MADA Medical: Madacide-FD (EPA reg# 1130-15-11703) The following disinfectants were tested by OAKWORKS and found to be acceptably compatible with most materials, with some cautions as indicated: 1. 10% Sodium Hypochlorite (bleach) solution in water (EPA reg# 5813-100) (possible mild corrosion of plated metals) 2. Metrex Cavicide One (EPA reg# 46781-12) (possible slight deterioration of upholstery and wood lacquer) OAKWORKS recommends a prepackaged wipe for cleaners/disinfectants to ensure best distribution of disinfectant for the required kill time, without leaving excess residue and/or overexposing components therefore minimizing the potential for damage to materials. Please read and follow disinfectants manufacturers directions for cleaning and disinfection. The following disinfectants were tested by OAKWORKS and found to be incompatible with some materials and are therefore NOT recommended for use: 1. Clorox Broad Spectrum Quaternary (EPA reg# 67619-20)(deterioration of upholstery, wood lacquer, plated metals) 2. Clorox Hydrogen Peroxide Disinfectant (EPA reg# 67619-24)(corrosion of metals, deterioration of upholstery and wood lacquer, leaves a hard-to-remove residue) 3. Virox Accel TB (EPA reg# 74559-1)(corrosion of metals, deterioration of upholstery and wood lacquer, leaves a hard-to-remove residue) 4. Clorox Healthcare Bleach Germicidal Cleaner (EPA reg# 56392-7)(deteriorates upholstery, wood lacquer, mild corrosion of metals, dissolves foam in pads) OAKWORKS does NOT recommend the use of cleaners/disinfectants containing Hydrogen Peroxide, Acetic Acid, or Phenolics. These chemicals can cause damage to the appearance and/or material integrity of various components. Also, while the recommended cleaners/disinfectants list includes products containing Quaternary Ammonium compounds ( quats ), not all products containing quats are approved for use. Some contain additional detergents and/or surfactants which can be detrimental to some materials. A note on Bleach: While a 10% sodium hypochlorite (household bleach) solution (EPA reg# 5813-100 or equivalent) can be an effective disinfectant and is dilute enough to be benign to most materials, it alone is not an effective cleaner and a separate product must be used for the initial cleaning steps of the procedure. Because of possible chemical incompatibilities between various cleaning products and bleach, utmost care must be taken by the user to avoid potential exposure to harmful or toxic by-products of the combination. Also, because bleach leaves a potentially corrosive residue as it evaporates, it must be rinsed with clean water after disinfection. It is therefore NOT recommended by OAKWORKS. Please note, one step cleaner/disinfectants using bleach and a detergent/ surfactant were found to damage materials and are NOT recommended. 27

CLEANING & DISINFECTION / INSPECTIONS / WARRANTY CLEANING PROCESS DANGER To reduce the risk of electric shock: - Always unplug this furnishing form the electrical outlet before cleaning. Follow the cleaners/disinfectant manufacturers directions for use. Please note that cleaning and disinfecting an OAKWORKS table is a two part process. First it must be cleaned of any visible soil, then it can be disinfected. The table should be cleaned as soon as possible after use. Please follow these procedures for best results: 1. Using a fresh wipe or wipes, clean any visible soil off of the table, working from the top to the bottom of the table. 2. Throw the used wipe(s) away. 3.
