Michael Tommy Welna (Report No. 6577) Applicant for a Taxicab Driver's Licence (Application No. B645945) Michael Tommy Welna s licence will be issued, effective immediately, subject to the following conditions: within 30 days of the Tribunal hearing, failing which the licence will be cancelled; (2) Mr. Welna must provide to Municipal Licensing and Standards proof of for a period of four (4) years to commence on ; (4) Every six (6) months for the next four (4) yearly renewals of the licence, Mr. Welna must provide to Municipal Licensing and Standards, at his own expense, an original updated abstract of this driving record, and original updated abstract of his criminal record to be submitted yearly.
