Serial number label on top of head. Post Anderson connectors located in oval slot on bottom of left side post arm. Post knob and pin. Remove (1) screw from upper timer housing & slide timer housing 1"-2" up post to check timer connectors. Remove (4) screws and (2) head end caps to check lamp connectors. See Chart 14 Power Inlet Module on back of base, see Fig. 1 Serial number label on bottom of post & back side of base. Caster adapter (outrigger style) located on right rear of base See FORM310 for old style casters. Brake handle to lock head position: Clockwise = Lock Counter-Clockwise = Unlock Remove (4) screws and top cover of head to check terminal block wiring (see Chart 12) and change F2 fuse (see Chart 15). Remove (1) screw from bottom timer housing & rotate timer housing out and down to check timer fuse. Lift Pivot Latch & rotate post 90 clockwise to place post in storage mode Power Inlet Module on back of base, see Fig. 1 Serial number label on bottom of post & back side of base. Remove (4) screws and (2) head end caps to check lamp connectors. DUSA Pharmaceuticals, Inc. Symptom: Fans Running, Timer Decrementing, Lamps NOT Operating (All 4170 Systems) Remove back cover of Head with a Phillips Screwdriver (#2). Locate F2 fuse on bottom left hand side of PCB (printed circuit board) when standing behind BLU-U. Is F2 Fuse blown? Change the F2 fuse, see Chart 15 for instructions. (Customer: See MAN-1212 in fuse kit for Proper Fuse & Instructions) Install back cover of head. (May leave screws off until unit tested) Reconnect power and turn BLU-U on. Is the BLU-U functioning (lamps turn on)? Tip: If head lights flickered then went out after replacing F2 fuse; the fuse may have blown again. Ship more fuses to customer, if required, and replace fuse again prior to replacing the head. Troubleshooting Complete Reconnect head screws F2 Fuse (Fig. 1 below) Remove back cover of head. Locate Green Terminal Blocks (6 pin & 7 Pin) on bottom left hand side of PCB (printed circuit board) when standing behind BLU-U. Check that wires are secure & tighten screws if loose. Install back cover of head. (May leave screws off until unit tested) Reconnect power and turn BLU-U on. Is the BLU-U functioning (lamps turn on)? DUSA Pharmaceuticals, Inc. Symptom: Timer Decrementing, Lamps & Fans NOT Operating (1K 6K Units) Locate Anderson connectors in post. (In oval slot on bottom of post yoke) Remove Anderson connectors from slot. Are Anderson connectors securely connected? (See Figure 1: May need to remove black tape) Post Anderson Wiring Chart 6K Units 1K & 3K Units Grn/Yel Black Green Green Black #1 Red Black #1 Black #1 Black #2 Orange Black #2 Black #2 Black #3 Brown Black #3 Black #3 Reconnect Anderson connectors. (See wiring table) Reconnect power and turn BLU-U on. Is the BLU-U functioning (lamps & fans turn on)? (see Fig. 1 on Chart 1) Remove screws from head cover and remove head cover. Locate Green Terminal Blocks (6 pin & 7 Pin) on bottom left hand side of PCB (printed circuit board) when standing behind BLU-U. Check that wiring is secure & tighten screws if loose. Ensure terminal block is securely seated on PCB (push down on terminal block) Locate F2 fuse on bottom left hand side of PCB when standing behind BLU-U. Change the F2 fuse, see Chart 15 for instructions. (Customer: See MAN-1212 in fuse kit for proper fuse & instructions) Install back cover of head. (May leave screws off until unit tested) Reconnect power and turn BLU-U on. Troubleshooting Complete Troubleshooting Complete Reconnect head screws Is the BLU-U functioning (lamps & fans turn on)?, Unit is a 6K Figure 1 Post Anderson Connectors HEAD needs repair. HEAD., Unit is A 1K 3K Perform this step ONLY if troubleshooting with a field technician. Tools required: multimeter Check for 120 Volt power coming from post. Measure power from post Andersons connected to Black #2 and Black #3 wires. If no power, POST needs repair, send replacement POST. If there is power, HEAD needs repair, send replacement HEAD. Perform this step ONLY if troubleshooting with a field technician. Tools required: multimeter Check for 120 Volt power coming from post. Measure power from post Andersons connected to Black #2 and Black #3 wires. If no power, POST needs repair, send replacement POST. If power is 120 V reconnect Anderson connectors using wiring chart. Check for 120 V power on Pin 2 and Pin 3 of circular metal connector on end of post cable. If there is no power, the ADAPTER CABLE is faulty, send replacement ADAPTER CABLE. If there is power, the HEAD needs repair, send replacement HEAD. Chart 2

DUSA Pharmaceuticals, Inc. Symptom: Timer Decrementing, Lamps & Fans NOT Operating (10K Units) Turn timer key off. Disconnect circular metal connector from head. Turn timer key on and press Start/Stop button. Check power on Pin 2 and Pin 3 of circular metal connector on end of post cable. Is voltage 120 V? Disconnect the head from the stand arm, carefully remove the head and set the head aside. Remove (2) screws securing yoke to post. Disassemble yoke from post (feed head cable into yoke to aid disassembly). Check that 6 pin connector is securely fastened. Reconnect 6 pin connector if required. Reassemble yoke to post (gently pull head cable out of yoke to aid in assembly). Replace (2) screws securing yoke to post. Replace head on yoke & reconnect circular head connector. Reconnect power and turn BLU-U on. Is the BLU-U functioning (lamps & fans turn on)? Turn Timer off with key. (see Fig. 1 on Chart 1) Remove screws from head cover and remove head cover. Locate Green Terminal Blocks (6 pin & 7 Pin) on bottom left hand side of PCB (printed circuit board) when standing behind BLU-U. Check that wiring is secure & tighten screws if loose. Ensure terminal block is securely seated on PCB (push down on terminal block) Locate F2 fuse on bottom left hand side of PCB (printed circuit board) when standing behind BLU-U. Change the F2 fuse, see Chart 15 for instructions. (Customer: See MAN-1212 in fuse kit for Proper Fuse & Instructions) Install back cover of head. (May leave screws off until unit tested) Reconnect power and turn BLU-U on. Is the BLU-U functioning (lamps & fans turn on)? POST needs repair. POST. HEAD needs repair. HEAD. DUSA Pharmaceuticals, Inc. Symptom: Power (All 4170 Systems) If possible, check the wall outlet for 120VAC (either with voltmeter or other equipment such as a desk lamp). Did outlet work? problem found with BLU-U. Possible problems with facility power. Check the following: o Is the BLU-U plugged into the wall outlet? If no, plug it in o Is the Power Inlet Module Switch turned on? If no, turn it on to I o Is the Key Switch turned on? If no, turn key on... Is the timer Illuminated? If yes...push Start/Stop button Is the BLU-U functioning (lamps & fans turn on, timer decrementing)? Remove fuse holder from Power Inlet Module and check fuses (flat bladed screw driver may be needed to remove). Troubleshooting Complete Are the fuses good? Replace both of the fuses and verify proper orientation of the fuse holder for 115V or 230V. (see Fig. 1) Turn Power on at base and turn key on. If Service Technician on site Continue Troubleshooting at top of Chart 4 Part B for 1K-6K; & Chart 4 part C for 10K. To reverse fuse module: Open cover, Remove red block, Rotate 180, Replace red block. Figure 1 (All 4170 Systems) Chart 4 Part A

DUSA Pharmaceuticals, Inc. Power: Continued from Chart 4 Part A; 1K 6K Units Only Perform this step ONLY if troubleshooting with a Field Technician. Disconnect the head from the stand arm, carefully remove the head and set the head aside. Place the unit on it s side with the yoke/arm facing up and remove the dust cover from the base. Inspect resistor connectors for signs of arcing and give slight crimp if loose. Is Base/Post Anderson Connector connected? Connect the Base/Post Anderson Connector. Plug in the unit, Turn on base power inlet module Turn on the timer key. Is timer illuminated? Disconnect the Anderson Connector. Turn the power back on and measure AC voltage at Base connector side (Black wire to Black wire) Does voltage measure approx. 120VAC? Connect head to post. Press Start/Stop button on timer to start cycle. Troubleshooting Complete (reassemble unit) BASE needs repair. BASE. Is the BLU-U functioning (lamps & fans turn on)? Go to Chart 1 to check and replace F2 fuse in head Reconnect Anderson Connectors. Remove top screw from timer housing and open carefully. Replace the Timer fuse (250V 313, 1-¼ Amp) Is Timer fuse blown? (Fuse located on back of timer) Check for loose connectors inside the Timer. Reconnect power and test unit. Reconnect the timer housing. Reconnect power and test unit. Did it fix the problem? POST needs repair. POST. Did it fix the problem? Troubleshooting Complete (reassemble unit) POST needs repair. POST. Chart 4 Part B

DUSA Pharmaceuticals, Inc. Power: Continued from Chart 4 Part A; 10K Units Only Remove screw from bottom of timer housing. Rotate timer housing out and then down to remove timer housing from post. Perform this step ONLY if troubleshooting with a Field Technician. Is Timer fuse blown? (Fuse located on back of timer) Replace the Timer fuse (250V 313, 1-¼ Amp) Check for loose connectors inside the Timer. Reconnect the 6 pin electrical connector & timer housing. Reconnect power and test unit. Open timer housing. Check for loose connections inside timer. Disconnect 6 pin electrical connector. Measure voltage at Pin 1 and 2 of 6 Pin connector located inside post. Is voltage 120 V? TIMER needs repair. TIMER. Disconnect the head from the yoke, carefully remove the head and set the head aside. Loosen (1) screw at bottom of post. Disassemble post from base. Check that 6 pin connector is securely fastened. Reconnect 6 pin connector if required. Reassemble post to base. Tighten (1) screw at bottom of post. Replace head on yoke & reconnect circular head connector. Reconnect power and turn BLU-U on. Did it fix the problem? Is timer illuminated? Is the timer illuminated? Troubleshooting Complete (reassemble unit) Are fans running and lamps NOT illuminated? Replace F2 fuse in head; check wiring & terminal block in head. Go to Chart 1. BASE needs repair. BASE. Check power on Pin 2 and Pin 3 of circular metal connector on end of post cable. Is voltage 120 V? Check wiring & terminal block in head. Go to Chart 1. Troubleshooting Complete Disconnect the head from the stand arm, carefully remove the head and set the head aside. Remove (2) screws securing yoke to post. Disassemble yoke from post (feed head cable into yoke to aid disassembly). Check that 6 pin connector is securely fastened. Reconnect 6 pin connector if required. Reassemble yoke to post (gently pull head cable out of yoke to aid in assembly). Replace (2) screws securing yoke to post. Replace head on yoke & reconnect circular head connector. Reconnect power and turn BLU-U on. Is the BLU-U functioning (lamps & fans turn on)? POST needs repair. POST. Troubleshooting Complete Chart 4 Part C

Symptom: Timer 1: F001/F101 (3x F001, Post Andersons) Error Code Displayed on Timer F002/F202 (3x F002, Power Inlet Module Orientation) Error Code Displayed on Timer -OR- F002/F202 Error Fuse Module Check Power Inlet Module on Base. Verify proper orientation of the fuse holder for 115V (or 230V). Is power inlet module oriented with 115V (or 230V) visible? Unplug power cord from base. Remove Fuse Block from power inlet module and orient correctly (See Fig. 1 on Chart 6, may require the use of a flat bladed screw driver) F001/F101 Error Post Andersons Are Anderson Connectors in post yoke properly connected? (See Fig. 2 on Chart 6, may need to remove black tape) Unplug power cord from base. Reconnect Anderson Connectors (see wiring table above). Post Anderson Wiring Chart 6K Units Grn/Yel Black Black #1 Red Black #2 Orange Black #3 Brown 1K & 3K Units Green Black #1 Black #2 Black #3 Green Black #1 Black #2 Black #3 Plug unit back into wall outlet. Turn on power switch on Base. Press Start/Stop button on Timer. Does timer display 0:00? Set timer for 16:40 Press Start/Stop button Four digit Patient Code is displayed on timer. Find the displayed code in Chart 17-18 under Displayed on Timer column (or in SOP2-030 under Patient Code Number column). Is the BLU-U functioning (lamps & fans turn on, timer decrementing)?. Repeat up to 3 times. If code does not clear, check Post Anderson connectors. Find code to clear timer on Chart 17-18 in Code to Clear Timer column (or in SOP2-030 in 75 Treatments column). Hold UP or DOWN button on Timer until password is displayed. Press Start/Stop button. Troubleshooting Complete Does timer display 0:00? Is the BLU-U functioning (lamps & fans turn on, timer decrementing)? Set timer for 16:40 Press Start/Stop button Turn over to technical support for further troubleshooting with field technician. Check power through post cables per Chart 2. Chart 5

Verify proper orientation of the fuse holder for 115V (or 230V). (6K Units Only) Check Anderson Connectors in Post Yoke Are Anderson connectors properly connected? (See Fig. 2, may need to remove black tape) Post Anderson Wiring Chart Grn/Yel Black #1 Black #2 Black #3 6K Units Black Red Orange Brown Is power inlet module oriented with 115V (or 230V) visible? Unplug power cord from base. Remove Fuse Block from power inlet module and orient correctly (See Fig. 1, may require the use of a flat bladed screw driver) Unplug power cord from base. Using the pliers squeeze the v-notch back to the normal size of the pin. Replace pin and post knob. Do not over tighten the knob. Field Technician Support Did it fix the problem? Disconnect the head from the stand-post arm and carefully remove the head and set the head aside. Place the stand-post on it s side with the yoke/arm facing up and remove the dust cover from the base. Troubleshooting Complete Ensure service loop is not hung up on any items located in the base. DUSA Pharmaceuticals, Inc. Replacing F2 Fuse in Head (All 4170 Systems) Tool Required: Phillips Head Screwdriver (#2) Remove (4) screws from head cover (see Figure 1). Remove head cover and set aside. Locate Circuit Board and F2 fuse near green terminal block (see Figure 2). Remove the existing F2 fuse. Install new F2 fuse into same location taking care not to touch any other component on the board. Re-install head cover (may leave screws out for testing). Plug BLU-U into wall outlet. Turn Power Inlet Module on. Turn timer key on. Press Start/Stop button on timer. Confirm unit powers up, and lights illuminate. See tip if lamps do not light. Reinstall head cover screws. Tip: If head lights flickered then went out after replacing F2 fuse; the fuse may have blown again. Ship more fuses to customer, if required, and replace fuse again prior to replacing the head. DUSA Pharmaceuticals, Inc. Frequently Asked Questions Wavelength of BLU-U? 417 nm ± 5 nm (nm = nanometers) Power output of lamps? The BLU-U lamp power output is 9-11 mw/cm 2. The BLU-U delivers 10 Joules/cm 2 during the approved treatment time of 16:40. Customer can reference MAN-1211 Operating Manual, page 12, Procedure for treating acne, number 3. Can lamps be replaced in the field?, lamps cannot be replaced in the field. This is because the lamps are calibrated as a set in the head and lamp replacement requires recalibration of the head. Can the BLU-U be used with one or more lamps not lit?, the BLU-U should not be used when lamps are out. Check lamp connectors (see Chart 14) and terminal block in head (see Chart 12). If problem is not resolved, a replacement head is recommended. BLU-U longevity -or- number of cycles? 6K units run to 1600 cycles, 1K & 3K units run to 500, 1050 or 1600 cycles. The status indicator light will be on steady or intermittent and the lamps will be lit. See Chart 10 for instructions to check number of hours on Timer 1. Does the BLU-U require Preventive Maintenance? The BLU-U does not require scheduled preventive maintenance. The BLU-U is calibrated to 9-11 mw/cm 2 at the factory and the calibration is monitored by the electronics in the head. If a units power output is above or below the 9-11 mw/cm 2 specification the Status Indicator will illuminate and DUSA should be contacted at this time. What parts are serviceable on the BLU-U? The customer can change parts such as fuses and knobs in the field. Other components which can be changed in the field, such as the power inlet module or caster upgrade, require a MERA technician. Field replaceable parts can be found on FORM281. Why do the lamps change brightness, or pulse, during power up or operation? The lamp pulsing is normal as the electronics in the head regulate the lamp power output. If there is a lamp power output issue or electronics problem the system status indicator lamp will flash (see operating manual for error conditions) and the customer should contact DUSA customer service to troubleshoot and resolve. What safety materials or information is there for the BLU-U? Safety information can be found in MAN-1211 Operating Manual, page 4. What is the difference between patient goggles and physician goggles? Patient goggles are small and fit just over the eyes. Clinician goggles are larger. Chart 16
