1 COMPANY INSTRUCTION Scope of this Register is to record checks and maintenances carried out to the lifting appliances in order to improve the standard of safety and to comply with Company policy minimizing any crew injury that can arise from a lack of maintenance. Test, examinations and inspections indicated in this Register are based on National, International and Company requirements. They are intended to ensure that ship s lifting appliances are initially certified by competent persons and to periodically establish that they are kept always in safe working condition, at satisfaction of a competent person (Senior Officer/Safety Officer, Company Safety Superintendent). This Register and related certificates shall be kept in order and shall available to all people authorized to check them. 1) All lifting appliances shall be certified (certificates should be enclosed into this Register, attached to the relevant card); 2) The Safety Officer is in charge for this Register; 3) All lifting appliances are to be marked with serial nbr, ship s number (ID) or colour, in order to be properly marked on the Register. Certification, details and maintenance done should be marked on this Register as well. 4) If existing equipment on board is lacking an original testing certificate, it must be entered in the Register as well with proper note. Same maintenance/check procedures as for certificated equipment will apply. All new lifting appliances supplied on board to have proper test certificate for the intended SWL. Periodical examination All lifting recorded into this Register shall be thoroughly examined by the Safety Officer every month: entries to be applied on this Register. Regular visual inspection of lifting equipment shall be carried out by responsible people before its use. Maintenance All lifting recorded into this Register shall be maintained under the responsibility of the Chief Mate and of 1 st Eng. and updated on a monthly basis. In compliance with the Manufacturer instructions, for wires greasing and brushing, maintenance has to be made when necessary, but at least every 3 months. All maintenances carried out have to be duly recorded into this Register. Disposal of lifting Lifting appliances must be disposed of when found out of order and not repairable. If they can be repaired, the equipment shall be labelled out of use and an entry to be made on the Register (until they are repaired by certified person). Wires or soft lines in use must be disposed after 5 years or before if during inspection they are found to be damaged or corroded. Manufactures maintenance instructions as reference are available on board.
