It is anticipated that the building will be removed by July 15, The tanks may be removed by the successful provider upon removal of the building. Please provide a proposal in accordance with the attached plan, instructions, and specifications to the Ogle County Highway Department by June 5, 2015, 3:30 pm local time to provide the services for removal of 6 underground storage tanks at 501 W Washington Street, Oregon, IL Proposals are to be delivered to the Ogle County Highway Department, 1989 S IL Rt 2, Oregon, IL Ogle County may choose the submitted proposal most favorable to Ogle County. Ogle County will notify successful proposal submitter of acceptance of proposal by June 17, Contact the Ogle County Highway Department at the below number with questions or to schedule a site inspection prior to submittal of a proposal. The successful contractor shall provide the labor, equipment, and materials to excavate, remove, and property dispose of the described tanks and excavation and backfill the same excavation at the above address. Scope & Services - See the attached specifications for more detail on the scope of the work. - Ownership, and responsibility for, removed tanks to become that of successful proposal provider upon excavation. - All utilities have been disconnected with the exception of water and sewer. - Successful contractor will be required to provide proper liability insurance with Ogle County listed as an additional insured. - Required insurance amounts shall be in accordance with OSFM certification requirements. 3 GENERAL SPECIFICATIONS FOR REMOVAL OF UNDERGROUND STORAGE TANKS 1. GENERAL 1.1 REQUIREMENTS INCLUDE A. General Contractor shall provide: 1. Licensed supervision of underground tank removal. 2. Reports, notices and acquisition of required permits. 3. Protection of persons and property. 4. Workers who have completed 40-hour OSHA hazardous material training. 5. Work area preparation. 6. Demolition work: a. Excavation and disposal of soils sufficient to remove underground tanks, piping, and accessories. i. Tank 1 - Underground steel tank, estimated 550 gal, 6 L x 4 W x 3-4 D (Full suspect used oil). ii. Tank 2 - Underground steel tank, estimated 550 gal, 6 L x 4 W x 3-4 D (appeared empty). iii. Tank 3 - Underground steel tank, estimated 650 gal, 7 L x 4 W x 3-4 D (appeared empty). iv. Tank 4 - Underground steel tank, estimated 650 gal, 7 L x 4 W x 3-4 D (unknown). v. Tank 5 - Underground steel tank, estimated 2,400 gal, 16 L x 5 W x? D (petroleum odor). vi. Tank 6 - Underground steel tank, estimated 4,000 gal, 27 L x 5 W x? D (unknown). b. Reinforced concrete anchor slab and accessories beneath tank if present. (See Drawing) c. 4 1.2 RELATED WORK A. Specified Elsewhere: Not Applicable 1.3 WORK BY OTHERS A. Engineer will provide sampling and testing services for soils and groundwater contamination at the time of removal. Owner will contact Engineer. B. Contractor shall notify Owner seven (7) days in advance of tank removal via the following: Curtis D. Cook, PE Ogle County Engineer (office) (cellular) C. Immediately upon removal of tank and components, contractor shall assist Engineer in collecting a series of closure soil samples from the excavation floor and sidewalls. If sample locations are not reachable, contractor shall utilize excavation equipment to obtain a representable soil sample from a specified location to be utilized by the Engineer. 1.4 QUALITY ASSURANCE. Comply with applicable requirements of USEPA, IEPA, OSFM, and OSHA regulations. 1.5 REFERENCES A. Standards: Specified Standard, in effect at date of bidding documents, are hereby incorporated by reference to the same extent as if they were included herein in full. The Standards, or cited portions thereof, govern all underground storage tank removal. B. API - American Petroleum Institute. C. 5 1. OSHA - Occupational Safety and Health Administration, 200 Constitution Avenue, Washington, DC USEPA - Environmental Protection Agency, 401 Main Street, SW, Washington, DC SUBMITTALS 3. State Standard: IEPA - Illinois Environmental Protection Agency: Illinois Administrative Code 35 Part 734 (Petroleum Underground Storage Tanks) and 35 Part 742 (Tiered Approach to Corrective Action Objectives). A. Before Start of Work: Submit the following to the Engineer for review. No work shall begin until these submittals are returned with the Engineer s authorization indicating the submittal is satisfactory. 1. Notices: Submit notices required by federal, state, and local regulations together with proof of timely transmittal to agency requiring notice. 2. Permits: Obtain all building and special permits required for the underground storage tank removal work. The costs of these fees are the contractor s responsibility. 3. Licenses: Submit copies of all state and local licenses and permits necessary to carry out the work of this Contract. 4. Health and Safety Plan: Plan shall be prepared by contractor and conform to applicable federal, state, and local laws, regulations and ordinances, including 29 CFR et.seq. Contractor personnel shall conform to Health and Safety Plan throughout performance of work. A copy of the Health and Safety Plan shall be submitted to the Engineer no less than seven (7) days prior to commencement of any work activities for review. B. Manifests: Submit copies of all load tickets and manifests. Certifying destination, receipt, and disposal of soil and demolition materials. C. Copy of the issued Clean Tank Certification and final certificate of disposal. D. Results: Submit copies of all laboratory test results for soil and groundwater samples and waste disposal analysis. 1.7 NOTIFICATIONS A. Contractor shall notify Engineer no less than seven (7) days prior to commencement of any work activities. Page 3

6 B. The Contractor shall submit a Notification Form for Underground Storage Tanks and an Application for Permit for Removal of Underground Storage Tanks form to the Illinois Office of the State Fire Marshall (OSFM) a maximum of ten (10) business days of contract award to adhere to the project timeline. Prior to submittal, the Contractors shall verify with the Engineer that all form information to be submitted is accurate and correct. All associated registration and permitting fees are the responsibility of the Contractor. C. The Contractor shall submit notification to the local fire department within 10 business days prior to commencement of work. D. The Contractor shall schedule a final removal date with the OSFM to allow for an OSFM field inspector to be present. The A/E should be immediately contacted to coordinate prior specified Work by Others. E. The Contractor shall be required to file a Certification of Removal and amended Notification Form for Underground Storage Tanks with OSFM within 30 days of completion of permitted work. F. If a release is determined to have occurred, the County of Ogle (with the assistance of the Engineer) is responsible for notifying the Illinois Emergency Management Agency (IEMA) within twenty-four (24) hours of a confirmed release. G. The Engineer shall be present on site before any excavation, tank cutting, tank cleaning, or tank removal can proceed. 1.8 UNDERGROUND OBSTRUCTIONS A. Known underground piping, foundations, and other underground obstructions in vicinity of new construction are shown on Drawing. B. Protect underground facilities encountered during excavation until it is determined whether they are active or inactive. Repair, without compensation, existing active facilities shown on Drawing damaged during operations. C. Notify Engineer immediately of unexpected subsurface conditions and discontinue all work in area until Engineer provides notification to resume Work. 1.9 SEQUENCING/SCHEDULING A. Perform all work in accord with approved Construction Schedule. All work shall be completed by August 1, Page 4

7 1.10 WARRANTY. Warrant all work in accord with General Conditions for a time period of one year. 2. PRODUCTS 2.1 FILL MATERIALS A. Crushed CA-7 from bottom of excavation to within 24 of existing ground surface. B. CA-10 for final 24 of excavation (compacted) 3. EXECUTION 3.1 GENERAL REQUIREMENTS A. Remove existing construction as specified and shown. B. Perform removal in a manner which will minimize dust, noise, and other nuisance. Maintain haul routes for disposal of material clean and free of debris. C. Protect existing facilities from damage by falling debris, dust, and construction operations. D. Perform demolition operations in a manner that in no way endangers personnel, public, existing structures, utilities, roadways, or facilities not to be demolished. E. Contractor shall be responsible for damage to personnel, public, roadways, streets, structures, utilities, facilities, and equipment caused by operations and shall repair any damage at its own expense or replace items damaged beyond repair. F. Perform work in a manner which will provide the least interference and most protection to the public and existing construction. Contractor s operations are subject to approval prior to commencement of work. G. Materials to be removed will not be permitted to accumulate on site. Remove all demolition materials. H. Any portion of existing construction whether structural, or accessory, which has become unstable through removal of other parts of construction, shall be removed as soon as practicable and no such unstable part shall be left freestanding or inadequately braced against causes of collapse at the end of each day s work. 8 I. Backfill materials shall conform to requirements of this specification section. J. Do not operate vehicles or equipment on existing construction or roadways which could be damaged. K. No demolition shall be performed on piping, electrical circuits, or equipment until system has been isolated by responsible provider. Contractor shall verify isolation of system. L. Contractor shall relocate existing active miscellaneous piping, conduit, and electrical circuits and devices not detailed on Drawing. M. Replace construction removed to facilitate operations with construction of equal quality to that removed. 3.2 REMOVAL PROCEDURES A. Work shall be performed in accordance with American Petroleum Institute (API) Recommended Practice 1604 Removal and Disposal of Used Underground Petroleum Storage Tanks, 41 Illinois Administrative Code, and any referenced standards. B. Provide monitoring equipment at site as required by the Site Health and Safety Plan. Operate and maintain in accordance with manufacturer s recommendations. Monitoring equipment shall include an organic vapor detector, an explosimeter, and all other equipment required to perform removal activities in accordance with 3.2 (A). C. Clearly identify exclusion zone around vapor hazard area in accordance with Site Health and Safety Plan and use only explosive proof equipment within exclusion zone. D. Regularly monitor atmosphere in excavation area and tank using an explosimeter until tank is removed from site. Monitor tank at bottom, middle, and top levels. E. Removal of remaining product: 1. Contents shall be removed from tank and associated piping. 2. Remove all liquid from tank using explosion proof pumps. All equipment must be bonded to tank and tank must be grounded to a separate ground when purging tank with compressed air or inert gas under pressure. Page 6

9 3. Tank contents shall be properly disposed of in accordance with State and Federal regulations. 4. Transport vehicles for tank contents shall not remain on-site for more than twenty-four (24) hour period. 5. All tank contents, sludge, tank cleanings, wash water, cleaning solutions, and other removal derived wastes shall be immediately containerized and at no point shall be discharged to the surface or to onsite sewers or drains. Disposal of such items shall occur in accordance with all State and Federal regulations and are considered a Contractor expense to be included as part of the submitted bid. F. Excavation: 1. During excavation, extreme caution shall be exercised in order to maintain the integrity of the Underground Storage Tank. 2. Provide shoring and bracing where necessary to support existing construction and protect personnel during demolition in accordance with Occupational Safety and Health Agency s construction standard Additional standard or codes shall apply if more stringent. 3. Excavation shall be done in the presence of Engineer who will check material for possible contamination using a calibrated Photoionization Detector (PID). Excavated material shall be placed in a separate stockpile pending disposal. Excavated materials presumed to be contaminated shall be stockpiled separately onsite until disposal analysis can be performed. All stockpiles shall be located on heavy-duty plastic sheeting with minimum thickness of 20 mil and shall be kept covered with such plastic sheeting. Stockpiles shall be bermed in such a manner that no runoff can occur and no precipitation may infiltrate. 4. If a release is presumed, immediately notify onsite OSFM inspector and Engineer. Discontinue work until OSFM inspector and/or Engineer provide notification to resume. G. Removal of Mechanical Items: 1. Remove mechanical equipment and materials in areas shown and specified. Removal shall include, but not be limited to, fill, gauge, vent, and product delivery piping, valves, pumps, dispenser, concrete slab-ongrade pads, guard posts, below-grade concrete anchors/pads, and associated accessories. 2. Pipe trenches shall remain open for inspection by OSFM field inspector. Page 7

10 H. Removal of Electrical Items: 1. Remove electrical equipment and materials in areas shown and specified. Removal shall include, but not be limited to conduit, wiring, wiring devices, motors, motor controls, and associated controls for level and leak detectors. 2. For conduit and wiring extending beyond demolition area, remove conduit to nearest junction box or fitting, remove wire to its point of origin, and cover opening in box or fitting. Cap underground conduit at footprint of building foundation. 3. Electrical Connections: Do not remove any electrical connections until the circuit has been de-energizes by a licensed electrical contractor. The contractor shall verify that de-energization has occurred and shall comply with lockout/tagout requirements as stipulation by OSHA. I. Underground Storage Tank Removal: 1. Do not initiate any tank removal until authorization has been given by the onsite OSFM field inspector. The OSFM field inspector shall be on site before venting, cutting, cleaning or entry operations may proceed. 2. Inerting/vapor freeing shall be completed in accordance with 41 Illinois Administrative Code and API Dry ice shall not be allowed as a method of inerting tanks. 3. Regularly monitor the tank atmosphere and the excavation area with a combustible gas indicator for flammable or combustible vapor concentration until the tank is removed from both the excavation and the site in accordance with Illinois Administrative Code 41. A confined space entry permit shall be obtained prior to tank entry and MSDS sheets must be on site. Regularly monitor the tank to insure explosive conditions do not exist. 4. Before tank is removed from excavation, plug or cap all accessible tank holes. One plug shall have 1/8 inch vent hole. Tank shall be positioned with vent plug on top of tank during subsequent transport and storage. 5. Complete excavation around tank. Remove tank from excavation and place on level surface. Use wood blocks or other secure anchors to prevent movement of tank after removal, during cleaning, and prior to loading for transport from site. Page 8

11 6. Spills or drips shall be contained and cleaned up immediately to prevent contamination of soils and groundwater during removal. J. Underground Storage Tank Cleaning: 1. Any UST removed from the excavation zone shall be properly cleaned on site the day of the removal and removed from the site within 24 hours. 2. Tank cleaning procedures shall be conducted in accordance with 41 Illinois Administrative Code and API Recommended Practice Tanks larger than 2,000 gallons in capacity shall have holes or openings no less than 3 feet x 3 feet, one on each end or side, for cleaning. Tanks less than 2,000 gallons capacity shall have one entire side removed from end to end and shall be no less than 3 feet wide. 4. Contractor shall provide the minimum number of onsite fire extinguishers during cutting of tank. K. Soil Sampling: 1. Engineer will oversee tank closure and coordinate the collection of samples from excavation area upon tank removal. 2. The County may elect to apply Unit Adjustment Prices for Contractor remediation of soil and groundwater contamination. L. Backfilling Excavation: 1. Use structural fill, defined as crushed aggregate conforming to IDOT Standard CA-7 from the bottom of the excavation to 24 below existing grade. 2. Use structural fill, defined as crushed aggregate conforming to IDOT Standard CA-10 for final Place in 8" maximum lifts and compact to 95% maximum density as determined by ASTM D SALVAGE OF MATERIALS. Salvageable materials associated with the UST systems shall become property of Contractor and shall be removed from site as work progresses. Page 9

12 3.4 DISPOSAL OF MATERIALS A. Contractor shall be responsible for proper disposal of all project derived waste material. This includes, but is not limited to, excess excavated material, nonsalvageable equipment and materials, tank, any sand, liquid, fuel, or sludge contained in tank, cleaning and rinse water used to clean tank, and other removal derived wastes. Waste material shall be transported and disposed of using transporters and disposal facilities which possess all required federal, state or local licenses or permits. Submit all final disposal manifests, load tickets, and receipts, which are to be signed and certified by the final disposal destination. B. Burning of waste lumber and other materials or trash on site is not permitted. C. No material, obstructions, or debris shall be placed or allowed to accumulate within 15' of any fire hydrant. Fire hydrants shall be accessible at all times. 3.5 CLEANUP A. Maintain public streets, alleys, or other thoroughfares used in carrying out disposal free of litter or soil attributable to this operation. Equip and load trucks or other vehicles to prevent leakage, blowing off, or other escape of any portion of whatsoever is being hauled. Cost incurred by City in cleaning up such litter will be charged to Contractor and deducted from monies due or to become due it under this contract. B. Leave areas in clean condition. 4.0 Payment for Work A. Proposal shall include a Lump Sum price to be paid to successful contractor for the completion of the work. B. Selected proposal will require the contractor to execute a contract with the Owner for this work. Contractor will also be required to furnish a Payment and Performance Bond as a part of the contract. C. Payment will be made by Owner within 45 days of completion of all required work. Page 10

The following list is considered as those days for which holiday rates of wages for work performed apply: New Years Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, Christmas Day and Veterans Day in some classifications/counties. Generally, any of these holidays which fall on a Sunday is celebrated on the following Monday. This then makes work performed on that Monday payable at the appropriate overtime rate for holiday pay. Common practice in a given local may alter certain days of celebration. If in doubt, please check with IDOL. Oil and chip resealing (O&C) means the application of road oils and liquid asphalt to coat an existing road surface, followed by application of aggregate chips or gravel to coated surface, and subsequent rolling of material to seal the surface.

16 Ogle County Prevailing Wage for May Page 3 of 7 5/7/2015 EXPLANATION OF CLASSES ASBESTOS - GENERAL - removal of asbestos material/mold and hazardous materials from any place in a building, including mechanical systems where those mechanical systems are to be removed. This includes the removal of asbestos materials/mold and hazardous materials from ductwork or pipes in a building when the building is to be demolished at the time or at some close future date. ASBESTOS - MECHANICAL - removal of asbestos material from mechanical systems, such as pipes, ducts, and boilers, where the mechanical systems are to remain. CERAMIC TILE FINISHER, MARBLE FINISHER, TERRAZZO FINISHER Assisting, helping or supporting the tile, marble and terrazzo mechanic by performing their historic and traditional work assignments required to complete the proper installation of the work covered by said crafts. The term "Ceramic" is used for naming the classification only and is in no way a limitation of the product handled. Ceramic takes into consideration most hard tiles. COMMUNICATIONS TECHNICIAN Installing, manufacturing, assembling and maintaining sound and intercom, protection alarm (security), fire alarm, master antenna television, closed circuit television, low voltage control for computers and/or door monitoring, school communications systems, telephones and servicing of nurse and emergency calls, and the installation and maintenance of transmit and receive antennas, transmitters, receivers, and associated apparatus which operates in conjunction with above systems. All work associated with these system installations will be included EXCEPT the installation of protective metallic conduit in new construction projects (excluding less than ten-foot runs strictly for protection of cable) and 120 volt AC (or higher) power wiring and associated hardware.
