4 Safety Actions Update Submitted corrective action plan (CAP) in response to the FTA Audit of March 4, 2010 to FTA Administrator on April 29, Corrective Action plan addressed the 10 Metro recommendations and five findings related to Metro and directed to TOC. Scheduled Internal Safety Audit (ISA) beginning on May 17, 2010 TOC Triennial Safety (and Security) review beginning in June 2010 In conjunction with Transit Safety Institute (TSI) offering six transit safety courses specifically for Metro (on property) scheduled for summer/fall 2010 Page 7 of 96

7 Metro Safety Performance Indicators Rail Passenger Injuries are considerably lower than 2008, and are lower than 4 th Qtr st Qtr. Rail Fatalities have trended up from 2008 to 2010 Escalator Injuries were down in 2009, but they are trending slightly upward in 2010 Fire incidents are trending up, where smoke incidents are consistent with 4 th Qtr Bus Passenger Injuries in 2010 began low, but are trending upward Bus Accidents, both Preventable and Non-Preventable, spiked in Feb. 2010, but otherwise remain consistent t with 2008 and 2009 Page 10 of 96

MetroAccess Preventable/Non-Preventable Injuries Percentage of injuries that occurred onboard a vehicle (not collision related), off a vehicle, or as a result of a collision. Bus Collision Rate = N x 1,000,000/Vehicle Miles MetroAccess Collision Rate = N x 100,000/Service Miles Service Miles = The miles that vehicles are scheduled to or actually travel while in revenue service.
