1 Project title: Alive and thrive project: Accelerating the reduction of maternal and neonatal mortality and morbidity in Munkunyu sub county Objective: Reduce maternal and neonatal mortality and morbidity by 50% in 24 months Project Summary: This project aims at averting maternal, neonatal death and disability in Munkunyu through decentralizing quality and skilled care to the rural households and communities. This project will apply mobile phone technology, mass media and social networks to reach out to pregnant women and mothers in most affected rural areas and communities with appropriate information that will generate utilization, increase accessibility and availability of quality and skilled care during pregnancy, child birth and post-natal periods. Rationale Decentralizing quality care to the rural village level will reach out to many pregnant women who can t access early antenatal care due to various reasons. This project is very innovative because pregnant women will not be looking for where to get help and medical assistance; but this time highly skilled medical professionals will be looking for pregnant women and mothers to give help. Under this project, it is anticipated that a close relationship will be created between a pregnant woman or mother and a health care professional using mobile phone technology and electronic social networks; mothers and pregnant women will be accessing care and treatment information at any time they need it; make appointments and meet with the mobile healthcare team at a designated place to receive high quality care and treatment. This idea supplements and improves the existing health infrastructure, because at least the sub county has a health centre III with a maternity wing; but it is not functional due to lack personnel, equipments and drugs. This project will work with the sub county and village healthcare teams to reach in every village in the sub county with a pregnant woman or mother to give help through making early screening, care, treatment and make referrals where appropriate using our efficient ambulatory service.

2 Project Activities In implementing this project we are going to carry out the following activities. Massive reach out to all pregnant women and mothers in all villages of most affected rural districts with basic health education information using mass media, telephone applications, and interactive social networks like Facebook and twitter. Start a mobile healthcare team outreach program to pregnant women and mothers of neonates and infants in villages and perform early screening, care and treatment of various conditions, and make referrals where appropriate. Construct and operationalize a modern women and children s medical centre in the hilly areas of the Rwenzori in Munkunyu Sub County to serve the targeted 200,000 people. Train traditional birth attendants and select members of the village health care teams in basic skills and information of detecting early dangers in pregnancy, how and where to refer patients. Equip village healthcare teams with basic mobile technology to help them link rural acute cases to the national referral centers and initiate a midwifery training support program for each rural sub county in districts with high maternal and fetal mortality rates. The interim objectives to be achieved during implementation of this project include: 1. To provide skilled attendance to mothers during pregnancy, childbirth, and the postnatal period. 2. To strengthen the capacity of Individuals, Families and Communities to improve maternal and neonatal health. 3. Create a strong mechanism for referral and transportation of mothers with complications and sick newborns to nation referral units and hospitals. In working towards achieving the above mentioned objectives, the following impediments or critical decision points may require a revision in the work plan: Socio-cultural rigidities that deter pregnant women and mothers from seeking healthcare, and feeding on nutritious diets. Little domestic support for pregnant mothers and breastfeeding mothers. Parental reject of neonates born with deformities. The timeline for achieving the above objectives is based on the use of project implementation time quarters; the project implementation period is divided into eight (8) quarters of three (3) months each.

3 The first objective shall be achieved with in the first three quarters of the project implementation period; the second objective shall be achieved in the last two quarters while the third objective will be immediately achieved in the first two quarters of the project implementation period once a strong ambulance service has been instituted. The proposed project activities shall be implemented within the designated project time line and allocated budget as shown in the table below.
