3 Form 3: Name on Fascia FOR SHELL SCHEME EXHIBITORS ONLY _ Please write below your company name as you wish it to appear on your stand fascia and ensure that it matches the company name provided on Form 1, your company s catalogue entry. Please note the name should be typewritten or printed in block capitals, with a maximum of 24 characters. Exhibitors who fail to submit this form on the deadline specified will have their name as it appears in the contract. Exhibitors who make changes to the company name on fascia will be charged extra.

4 Form 4: Exhibitor Stand Contractor Exhibitors may appoint their own contractor for the purpose of stand building. However all mechanical and electrical works must be installed by the exhibition official contractors. Exhibitors are requested to ensure that their respective contractors observe all rules and regulations. Exhibitors are reminded that they would be responsible for all actions, observance of rules and regulations of their contractors during the entire exhibition from the period of build-up to tear-down of the exhibition. 5 Form 4: Exhibitor Stand Contractor Please list below name of contractor personnel who will be building your stand. Attach additional sheet if necessary. Please print in BLOCK LETTERS or type the names of your personnel. 6 Form 5: Electrical & Lighting _ Shell scheme exhibitors will get one 13 amp single phase power socket as part of their package. Any additional power supply required must be ordered using this form. Space only exhibitors are provided with a 15 amp 3 phase/ 30 amp single phase which is part of the contract. Exhibitors requiring additional power supply should complete this form. City Neon cannot guarantee service for any late orders. A 20% surcharge applies to all orders received less than 15 days before buildup and 50% for orders on site. 7 Form 6: Electric & Lighting Layout : This form must be submitted together with the electrical order form to the official stand contractors Location of your electric requirements will be installed at the discretion of the official stand fitting contractor if the electric layout plan is submitted after the deadline. Any relocation after installation will be at the expense of the exhibitor.
