1 This report is based on the Department s Letters of Intent and does not reflect modifications to recommended discipline due to Grievances, Skelly Hearings, Arbitration Hearings, Civil Service Commission Hearings, and/or Court Litigation. The discipline outlined below is based on a number of factors, including severity of action, employee s performance history, progressive discipline, and similar discipline for similar offenses. The masculine pronoun ( him, his ) includes the feminine pronoun. 1 Deputy 1 Day Handling his firearm in a carless manner resulting in a negligent discharge. 1 Senior 2 Day Failing his annual proficiency examination and failing this retest. Criminalist 1 Deputy 2 Day Intentionally antagonizing a person whom he came in contact with. 1 Deputy 2 Day While off-duty driving his personal vehicle in a reckless manner. 1 Lieutenant 2 Day Failing to conduct an inquiry and failing to take corrective measures when information was brought to his attention that an employee was intoxicated while on-duty. 3 Day Failing to conduct proper inmate safety checks. 1 Deputy 3 Day Failing to properly prepare for his court testimony and failing to acknowledge the details of an arrest report, which created inconsistencies between his testimony and another deputy s written report. 1 Deputy 3 Day Allowing an inmate to exit his cell while un-handcuffed and entering a cell to contact an uncooperative inmate. 1 Senior Criminalist 3 Day Handling an evidence submission in an improper and dangerous manner, which resulted in spillage that exposed multiple employees to the substance and forced an evacuation and closing of the facility. 2 Deputies 4 Day Failing to take appropriate action in investigating a domestic violence call for service. 1 Deputy 4 Day Failing to notify the dispatcher of an inmate transport, failing to handle calls for service while waiting to transport an inmate, failing to transport an inmate in a Page 1 of 5
2 timely manner, driving his patrol car in at an unsafe speed, and delaying in handling a call for service. 1 Deputy 4 Day Providing inconsistent testimony with his partner during a judicial hearing, failing to include important chronological detail in his arrest report, and delaying in writing a police report. 1 Deputy 5 Day Sending inappropriate messages to a co-worker containing profane language and sexual content. 1 Deputy 5 Day Failing to report witnessed force to a supervisor, failing to correctly identify the actions he took during the use of force incident, and leaving the facility prior to completing a witnessed use of force incident. 5 Day Failing to report witnessed force to a supervisor. 2 Deputies 5 Day Using a Mobile Digital Computer for non-business related matters. 5 Day Failing to take appropriate action within a reasonable time frame to ensure that proper medical attention was sought for an inmate who claimed an injury to his head. 1 Deputy 5 Day Using derogatory language/profanity while speaking with a co-worker. 1 Deputy 5 Day Being arrested for domestic violence and being named as a suspect in a suspected child abuse report for having a physical altercation with his spouse in front of his child. 1 Deputy 5 Day Failing to notify his immediate supervisor after using force and escorting an inmate to another location for a transfer without notifying a supervisor and/or obtaining a medical clearance. 1 Deputy 5 Day Being arrested for domestic violence, which resulted in the issuance of an emergency protective order and a subsequent temporary protective order. 1 Deputy 7 Day Failing to take appropriate action in investigating a domestic violence call for service. 1 Deputy 7 Day Becoming involved in a domestic dispute, and failing to remain at home when he called in sick. 3 1 Deputy 7 Day Attempting to submit an altered timesheet without following protocol to adjust or correct entries. 1 Deputy 7 Day Failing to initiate an emergency broadcast of an escaped juvenile, failing to inform his supervisor of the escape, and failing to immediately implement escape procedures. 2 Deputies 7 Day Failing to take appropriate action within a reasonable time frame to ensure that an inmate received proper medical attention. 1 Deputy 7 Day Witnessing a use of force and not assuming the role of a supervising deputy to direct and/or oversee other post-force procedures, as well as failing to communicate any concern to a sergeant. 1 Deputy 8 Day While off-duty, failing to act appropriately during a road rage incident, failing to follow the proper procedures after identifying himself as a peace officer, and failing to notify a Watch Commander regarding an off-duty incident. Failing to safeguard property belonging to a co-worker. Initiating an unauthorized pursuit resulting in a traffic collision, and not wearing his safety belt. 10 Day Providing his personal Department computer password to two Department members, allowing these persons to gain enhanced access to the Scheduling Management System, and for working in excess of the permissible number of hours within a 24 hour period. While off-duty, sending multiple derogatory and/or threatening text messages, resulting in being a named suspect in a police report. While off-duty, driving his personal vehicle in a reckless manner, and being disrespectful and unprofessional toward a supervisor. Being involved in two separate domestic violence incidents, and pointing his gun at his girlfriend following one incident. Sending inappropriate messages to a co-worker and using profane language and sexual content during a telephone call to the co-worker. Page 3 of 5

Failing to properly identify the need for and/or initiate the security contingency plan and or make proper notification when he was made aware of an escape of a juvenile from a courthouse. Driving in a reckless manner, being arrested for driving under the influence of alcohol, providing a blood alcohol content of.18% and.19%. concentration of.18%, and being in possession of a firearm while intoxicated. concentration of.17%, identifying himself as a peace officer during the investigation, and being in possession of his firearm while intoxicated. Being arrested for domestic violence, and inappropriately displaying a firearm during the domestic violence incident. content of.15%, and being uncooperative and unprofessional during the arrest. Failing to handle a call for service by documenting that he responded to a burglary alarm call when he had not, documenting the location was secure, and failing to communicate with the station and/or assisting unit that he was unable to respond to the alarm call because he was assisting with a driving under the influence investigation. Retrieving his off-duty firearm and manipulating the cylinder release without any reasonable cause or reason and negligently discharging his off-duty firearm while under the influence of alcohol.
