2 Ashvale Light Bulb Replacement - Qualitative Assessment Review Date: Annually ( Assessments to be reviewed annually or when changes to equipment or procedures.) Safety Safe Systems of Work/Control Measures Future Actions at Use of ladders Ladder user Person securing ladder Any persons in the vicinity 1. Falls - sprains, fractures, broken bones, head injuries. 2. Falling objects 3. Person falling on to another person Due to the short duration of use ladders may be used for replacing light bulbs Ensure ladder is fit for the purpose, e.g. correct type of ladder for this task. Management must ensure ladders are maintained in good repair, e.g. rubber feet are in good condition. Employees must make a visual check of the ladder prior to undertaking any task. Any defects must be reported to the Operations Director and the Maintenance Department. Ensure the ladder is placed on an even surface. Changing light bulbs or fluorescent tubes around the frying range area and cooking equipment must be undertaken prior to the commencement of service and all pans and food stuffs must be covered and protected. Any surface upon which a ladder rests shall be stable, firm, of sufficient strength, and of suitable composition safely to support the ladder so that its rungs or steps remain horizontal, and any loading intended to be placed on it. A ladder shall be so positioned as to ensure its stability during use. A suspended ladder shall be attached in a secure manner and so that it cannot be displaced and swinging is prevented. A ladder shall be prevented from slipping during use by an anti-slip device. A ladder used for access shall be long enough to protrude sufficiently above the place of landing to which it provides access, unless other measures have been taken to ensure a firm handhold. No interlocking or extension ladder shall be used unless its sections are prevented from moving relative to each other while in use. Safety Safe Systems of Work/Control Measures Future Actions at handhold and secure support are always available to the user and the user can maintain a safe handhold when carrying a load unless, in the case of a step ladder, the maintenance of a handhold is not practicable when a load is carried; the use of a stepladder is justified only if in use for short period and carrying out a low risk activity Page 3 of 5

Falls - sprains, fractures, broken bones, head injuries. 2. Falling objects. 3. Person falling on to another person. Employees Contractors Customers 1. Physical contamination of food. 2. Cuts and piercing of skin. 3. Injury from flying glass. Do not stand on chairs - use a ladder when changing light bulbs. Use the correct type of light bulb for the fitting and the correct wattage. Switch off electricity supply. Only trained staff are permitted to change light bulbs (Maintenance Department). Untrained staff can cause damage to the light fitting. Also if they do not follow the correct procedure may cause injury to themselves and or others. Manager must report any faulty light fitments to the Maintenance Department. All accidents and near misses must be reported to the Operations Manager. This task must be undertaken prior to opening to the public. If a light bulb explodes ensure all the glass fragments are carefully swept up and disposed of in a safe manner. Food must be covered or kept in air tight containers to minimize the risk of physical contamination. Any food which potentially could have been contaminated by glass must be disposed of in a safe manner. Manager to assess the situation and act accordingly. Fluorescent tubes must be covered in shatter proof enclosures in food rooms. A A Page 4 of 5

Safety Safe Systems of Work/Control Measures Future Actions at Changing light bulbs/fluorescent tubes outside. Employee Contractor General Public 1. Falling from height 2. Falling objects 3. Vehicle colliding with ladder. 4. Pedestrian walking into ladder. Prior to commencing any work ensure ladder is in good state of repair Electrical wiring and equipment will be maintained as part of the planned preventative maintenance program. Isolate the electrical supply. Maintenance staff must wear rubber soled boots. Ensure the ladder has been secured to the building prior to commencing any work. Ladder must be placed on an even surface. Maintenance staff or contractors must undertake the changing of bulbs or fluorescent lighting - manager to report any repair or replacement requirements to the Maintenance Department. Maintenance staff or contractors must ensure the surrounding area is clear of obstructions and safe for pedestrians prior to any work commencing. If the ladder will extend off the footpath on to a public road the Maintenance Department must barrier off the area prior to commencing work. Do not take any unnecessary tools with you up the ladder which may fall causing injury to pedestrians. Take care to position ladder to prevent injury from any vehicle colliding with the ladder. Ensure ladder is positioned in a manner in which you will not cause damage to vehicles or road users. Extreme care must be taken in inclement weather conditions, e.g. rain, wind, ice and snow.
