New Firefighter Endorsement" January 1, 2011! Effective January 1, 2011, a driver may operate firefighting equipment with a non-commercial or commercial class A,B or C driver license with a new Firefighter F endorsement.! " Endorsement will no longer appear on the actual driver s license but will be in the driver s record through DMV. " Current restricted firefighter licenses will remain in valid until the expiration date of the license. " The term restricted has been replaced by endorsement. 5! New Firefighter Endorsement" January 1, 2011! Renewal License - Downgrade! " Visit your local DMV office to change the license from a restricted commercial endorsement to one of the following driver license with Firefighter Endorsement:!! Commercial A, B, or C driver license (Endorsement)!! Non-Commercial A, B, or C driver license (Endorsement) (Best choice non-commercial)!! " Upgrades to FULL commercial license with a firefighter endorsement, you will need to complete the driver license application (DL44C), provide a Medical Exam Report (DL51), pass the knowledge test for the unrestricted license.! " The drive test may be waived when applying for a driver license with the same class.! 7! New Firefighter Endorsement" January 1, 2011! How to apply for the Endorsement! " Complete CSFM Form with signature from Fire Chief prior to attending Driver Operator 1A to show proof of in-house driver training.! " Submit Medical Form to the DMV Database prior to starting the application form!! Commercial (A, B or C) DL51 (Medical Exam Report by Physician form DL546A)!! Non-Commercial (A, B or C) DL546 (Self Questionnaire) Available on-line (Valid for 2 years)!! FYI AB 1567 (1/1/13) Allows Firefighters to use DL546 again, saving unnecessary expense for physician required exam DL546A...! 8! New Firefighter Endorsement" January 1, 2011! Submit an Original Firefighter Employment and Training Verification Letter to add the F Endorsement. The letter must be on Fire Department letterhead and include the following:! " Certification of employment or registration as a volunteer Firefighter! " Certification of completion of fire equipment operator training (CSFM Driver Operator 1A)!
