2 Versin: 4 Page: 2 f Intrductin 1.1 Purpse The purpse f this prgram is t establish the minimum requirements fr the prper selectin, care and use f prtable ladders (including step-ladders, straight/extensin ladders, and articulating ladders). Additinal sectins in the prgram will als address fixed ladders and mbile ladder stands t ensure safety under nrmal cnditins f use as defined by the Occupatinal Safety and Health Administratin s (OSHA s) Standards. 1.2 Scpe This prgram is inclusive f Emry emplyees, including healthcare, faculty, staff, and student emplyees, wh are required t use ladders in the perfrmance f nrmal wrk activities. 1.3 Definitins Articulated Ladder. A prtable ladder with ne r mre pairs f lcking hinges which allw the ladder t be set up in several cnfiguratins such as a single r extensin ladder, with r withut a stand-ff, a stepladder, a trestle ladder, scaffld r wrk table. Cage. A guard that may be referred t as a cage r basket guard which is an enclsure that is fastened t the side rails f the fixed ladder r t the structure t encircle the climbing space f the ladder fr the safety f the persn wh must climb the ladder. Extensin Ladder. A nn-self-supprting prtable ladder adjustable in length. It cnsists f tw r mre sectins traveling in guides r brackets s arranged as t permit length adjustment. Its size is designated by the sum f the lengths f the sectins measured alng the side rails. Fixed Ladder. A ladder permanently attached t a structure, building, r equipment. Ladder. An appliance usually cnsisting f tw side rails jined at regular intervals by crss-pieces called steps, rungs, r cleats, n which a persn may step in ascending r descending. Ladder Gate. A climb preventive shield r cver installed n a fixed ladder t cntrl access t tanks r ther high structures. Ladder Safety System. A device, ther than a cage r well, designed t eliminate r reduce the pssibility f falls frm ladders. A ladder safety system usually cnsists f a carrier (the track f flexible cable r rigid rail), safety sleeve (mving cmpnent which travels n the carrier), lanyard, cnnectrs, and bdy belt r harness. (Frmerly called a ladder safety device). Pitch. The included angle between the hrizntal and the ladder, measured n the ppsite side f the ladder frm the climbing side. Platfrm Ladder. A self-supprting prtable ladder that is nn-adjustable in length, with a platfrm prvided at the highest intended standing level. The tp platfrm is surrunded n three sides by a railing that is at least 20 inches higher than the platfrm

3 Versin: 4 Page: 3 f 13 surface. (Als knwn as a pdium ladder). Pdium Ladder. See Platfrm Ladder. Rungs. Ladder crss-pieces f circular r val crss-sectin n which a persn may step in ascending r descending. Single Ladder. A nn-self-supprting prtable ladder, nnadjustable in length, cnsisting f but ne sectin. Its size is designated by the verall length f the side rail. Stepladder. A self-supprting prtable ladder, nnadjustable in length, having flat steps and a hinged back. Its size is designated by the verall length f the ladder measured alng the frnt edge f the side rails. Trestle Ladder. A self-supprting prtable ladder, nnadjustable in length, cnsisting f tw sectins hinged at the tp t frm equal angles with the base. The size is designated by the length f the side rails measured alng the frnt edge. Trlley Ladder. A semi-fixed ladder, nnadjustable in length, supprted by attachments t an verhead track, the plane f the ladder being at right angles t the plane f mtin. 1.4 Respnsibilities Envirnmental Health and Safety Office (EHSO) and applicable Hspital and Clinic Safety Management As the administrative department fr the Ladder Safety Prgram, EHSO and applicable hspital and clinic safety management are respnsible fr: Develping and prviding ladder safety training t affected persnnel; Prviding technical supprt and cnsultatin t departments f affected emplyees t interpret requirements and establish safe practices; Evaluating the written Ladder Safety Prgram annually and updating it as needed; Verifying inspectins are cnducted n a subset f prtable and fixed ladders annually; and Maintaining training recrds. Directrs, Supervisrs, and Managers Directrs, supervisrs, and managers have primary respnsibility fr the management and enfrcement f the Ladder Safety Prgram in their areas. They are respnsible fr: Prviding the prper type, rated, and ANSI-cmpliant ladders fr emplyees, students, and faculty; Peridically evaluating the effectiveness f the prgram as it applies t the wrk that their affected emplyees perfrm and prviding EHSO with their cnclusins, cmpliance challenges, and recmmendatins; Cntacting EHSO fr technical supprt when questins arise regarding cmpliance and safe prcedures; Ensuring that prper safety equipment is supplied t their affected emplyees where needed; Ensuring that emplyees attend all required training;

4 Versin: 4 Page: 4 f 13 Ensuring that designated emplyees perfrm dcumented prtable ladder inspectins; and Infrming EHSO r ther apprpriate persnnel when new emplyees are assigned wh need Ladder Safety Training. Emplyees All emplyees are respnsible fr cmplying with the rules set frth by this prgram and must: Ensure that all ladders used meet OSHA regulatins and are ANSI-cmpliant; Cmply with the guidelines cvered in this dcument; Cnduct visual inspectins f ladders befre each use; Dcument ladder inspectins accrding t Sectin 7.0 f this prgram; Ntify their supervisr when they are nt physically able t use a ladder r when questins arise regarding the safe use f a ladder, such as: defective equipment, needed accessries r any ther difficulties cmplying with the requirements f the Ladder Safety Prgram. Cmplete Ladder Safety Training. Cntractrs Cntractrs wrking n campus are required t cmply with all applicable OSHA wrkplace safety regulatins and make their safety prgrams available fr review upn request by a representative f Emry. Additinally, cntractrs are respnsible fr cmplying with applicable cmpnents f this prgram, including Sectins 2.0, 3.0, 5.0, 6.0 and 7.0 f this prgram. Cntractr s ladders are subject t inspectin; Damaged r defective ladders are nt allwed at Emry and may be immediately dispsed f fllwing the requirements in Sectin 3.1 f this prgram. 1.5 Training Requirements EHSO is respnsible fr ensuring that Ladder Safety Training is prvided t all Emry University emplyees wh are required t use ladders t perfrm any wrk activities. This training will be given: Upn initial assignment; Fllwing any accident invlving a ladder; and Every three (3) years thereafter. EHSO will maintain dcumentatin f attendance which will include the emplyee s name, department, and date f training. The training will include the fllwing: The requirements f 29 CFR and applicable sectins f ; The requirements f the Ladder Safety Prgram, including ladder inspectins and prper use and maintenance f ladders. The applicable Hspital and Clinic Safety Management Grup is respnsible fr ensuring training is cnducted in healthcare. 1.6 Recrdkeeping Requirements Dcumentatin f classrm training is recrded in the Emry Learning Management System (ELMS).

5 Versin: 4 Page: 5 f General Ladder Safety Ensure all ladders at Emry are cnstructed and used in accrdance with OSHA regulatins and ANSI standards; and all cmmercially manufactured ladders have a label indicating it meets the requirements f the ANSI standard. Visually inspect prtable ladders befre use and fllwing any incident (i.e. drpped ladders, emplyee accidents invlving a ladder, etc.) Keep rungs and steps free f grease and il. Befre climbing a ladder: Clean muddy r slippery wrk shes/bts; and Replace r remve greasy r dirt-laden wrk glves. D nt carry tls r equipment in yur hands when climbing a ladder. Carry tls n a wrk belt r in a shulder-bag/back-pack, and use a hand line t raise r lwer equipment. Always face the ladder when ascending r descending, except as permitted by certain mbile ladder stands. Keep the center f yur bdy within the side rails f the ladder and maintain three pints f cntact at all times while ascending r descending a ladder. Only ne persn at a time is allwed n a single-width ladder, including prtable r fixed ladders, unless therwise indicated by the manufacturer. Never use a prtable ladder, fixed ladder, r mbile ladder stand in excess f its intended wrking lad. 3.0 Prtable Ladders 3.1 General Requirements Only use ladders fr their intended purpse and nt fr platfrms r as walk-bards. Select the crrect type f ladder by cnsidering the duty rating, ladder type and height required t safely cmplete the jb task. The five ladder types and their duty ratings are in Table 1 Ladder Types and Duty Ratings. 6 Versin: 4 Page: 6 f 13 Ensure prtable ladders meet OSHA design requirements, including: The inside width between the side rails f each prtable step ladder is at least eleven and a half (11-1/2 ) inches; The minimum width between side rails f straight ladders r any sectin f an extensin ladder is twelve (12) inches. Step and rung spacing f prtable ladders is unifrm and nt mre than twelve (12) inches apart; Rungs and steps are crrugated, knurled, dimpled, cated with skid resistant material, r therwise treated t minimize the pssibility f slipping. Remve damaged ladders frm service and tag as Dangerus, D Nt Use, r with ther similar language, until the repairs are made. Nte: Ladders that cannt be repaired must be withdrawn frm service and destryed in such a manner as t render it useless. Ensure ladders are equipped with nn-slip bases r safety feet. Place self-supprting and nn-self-supprting ladders n a stable, slip-resistant surface. Nte: D nt place ladders n bxes, barrels, r ther unstable bases r splice ladders tgether t btain additinal height. Blck pen, lck r therwise guard drways that pen twards a ladder. Use barricades r cnes and signage when ladders must be set-up in high traffic areas, such as hallways r stairwells. Never attempt t mve, shift r extend a ladder while standing r climbing n it. Set-up ladders clse t the wrk area and d nt ver-reach. T maintain balance, d nt climb higher than the secnd step frm the tp cap n a stepladder r the third rung frm the tp n a straight ladder. D nt use the rear hrizntal r bracing attached t the rear rails f stepladders r platfrm ladders fr climbing r standing. These are designed slely fr increasing stability. When accessing an upper landing surface, such as a rf, ensure the side rails f nn-self-supprting ladders extend at least three (3) feet abve the upper landing, eave r edge. Ensure that the tp f a nn-self-supprting ladder is placed with the tw rails supprted, unless it is equipped with a single supprt attachment. Such an attachment is designed t prvide greater stability. When setting up a nn-self-supprting ladder, fllw the One-furth Rule: Place the base f the ladder ne ft away frm the supprting wall, fr every fur feet in height the ladder is extended. When transprting ladders n vehicles equipped with ladder racks, ensure the ladders are prperly supprted. The supprt pints shuld be cnstructed f material that will minimize the effects f vibratin, chafing and rad shck. 3.2 Stepladders The spreader assembly is a required cmpnent f all stepladders and must be fully pened and lcked befre use. Never climb a flded stepladder. Stepladders cannt exceed twenty (20) feet in height. D nt use the tp cap f a stepladder as a step.

7 Versin: 4 Page: 7 f Platfrm Ladders Platfrm Ladders range in size frm tw (2) feet t eighteen (18) feet in length as measured frm the bttm alng the side rail t the tp f the platfrm. The tp platfrm is surrunded n three sides by a railing that is at least 20 inches higher than the platfrm surface. A flding Bucket (Pail) Shelf may als be prvided. Ensure that the platfrm ladder is n level grund befre use, such that all fur f its side rails are supprted. Never use a platfrm ladder as a single ladder r in the partially pen psitin. 3.4 Single Ladders and Extensin Ladders Single wden ladders, metal ladders, and individual sectins f metal ladders cannt exceed thirty (30) feet in length. Tw-sectin metal extensin ladders cannt exceed frty-eight (48) feet in length. Metal extensin ladders with mre than tw-sectins cannt exceed sixty (60) feet in length. Wden tw-sectin extensin ladders cannt exceed sixty (60) feet in length. When a persn is climbing an extensin ladder at heights f twenty (20) feet r higher, ensure that a secnd persn is present t steady the ladder s base r that the tp f the ladder is effectively secured t a sund anchr pint. On tw-sectin extensin ladders, ensure that the minimum verlap fr the tw sectins is in accrdance with the fllwing: Table 2 Tw-Sectin Extensin Ladders Minimum Overlap SIZE OF LADDER (FT.) OVERLAP (FT.) Place a cvering ver expsed hinges while perfrming messy wrk t prevent cntaminants frm causing damage t the mechanisms. When set up in the stepladder cnfiguratin, Articulated Ladders range in size frm three (3) feet t fifteen (15) feet. Nte: The size f an articulated ladder is determined when it is set up in the stepladder cnfiguratin. When set up as a Single r Extensin Ladder, Articulated Ladders may have a length f n mre than thirty (30) feet. 3.6 Special Ladders/Restrictins The table belw describes special ladders and the assciated restrictins.

Equal t r less than 20 ft. 3.7 Mbile Ladder Stands Ensure all mbile ladder stands meet the fllwing design requirements: The design wrking lad is calculated n the basis f ne r mre 200- pund persns tgether with 50 punds f equipment each, where the lad is applied unifrmly t a 3 1/2 inches wide area frnt t back at the center f the width span with a safety factr f fur (4). Capable f supprting at least fur (4) times their designed wrking lad; Steps are unifrmly spaced with a rise f nt less than nine (9) inches, nr mre than ten (10) inches, and a depth f nt less seven (7) inches with a minimum sixteen (16) inch width; Steps have slip resistant treads; The slpe f the steps are between fifty-five (55) degrees and sixty (60) degrees, as measured frm the hrizntal; Ladder stands with mre than five (5) steps r having sixty (60) inches vertical height t the tp step are prvided with handrails; Handrails are a minimum f twenty-nine (29) inches high, as measured vertically frm the center f the step; Ladder stand platfrms with a platfrm ver ten (10) feet have standard guardrails and tebards n expsed sides and at the ends f the platfrm; Nte: The use f remvable gates r nn-rigid members, such as chains, is allwed. Wheels r casters are prperly designed t supprt fur (4) times the design wrking lad, have psitive wheel and/r swivel lck t prevent mvement, and at least tw (2) f the fur (4) casters are f the swivel type. Use mbile ladder stands and platfrms n level surfaces. When prvided, use the handrails while ascending r descending the mbile ladder. Always face the mbile ladder while ascending r descending, except when the slpe f the steps is 50 degrees r less abve the hrizntal. Never mve a mbile ladder stand while standing n climbing n it. D nt stre materials and/r equipment n the steps r platfrm. Maintain mbile ladder stands in gd cnditin and ensure expsed surfaces d nt develp sharp edges, burrs r ther safety hazards, such as crrsin r deteriratin. Never exceed the maximum wrk-level height f a mbile ladder stand, which is calculated as fur (4) times the minimum r least base dimensin. Where the least

Never use bxes, ladders, r ther bjects n a platfrm, r sit r stand n the guard rails t increase the wrking height. 4.0 Fixed Ladders 4.1 General Requirements Knw the basic minimum design requirements fr fixed ladders: Ladders, at a minimum, must supprt a single cncentrated lad f twhundred (200) punds; The distance between rungs, cleats and steps must nt exceed twelve (12) inches and must be unifrm thrughut the length f the ladder; The clear length f rungs must be sixteen (16) inches; The rungs f an individual-rung ladder must be designed s that an emplyee s ft cannt slide ff the end; Rungs are at least ne and ne-eighth (1-1/8) inch diameter n wd ladders; The side rails f thrugh r side-step ladder extensins must extend three and a half (3-1/2) feet abve parapets and landings; The preferred pitch f fixed ladders is cnsidered t be in the range f 75 and 90 with the hrizntal; Substandard pitch is cnsidered t be in the range f 60 and 75 with the hrizntal and is nly permitted where it is necessary t meet the cnditins f installatin; Ladders having a pitch in excess f 90 with the hrizntal are prhibited. Maintain ladders in safe cnditin and perfrm fixed ladder inspectins regularly, with the interval between inspectins determined by use and expsure. When a defect is nt repairable, ensure the ladder is tagged as Out f Service and a ladder gate is installed, until the ladder is replaced. 4.2 Requirements fr Cages and Wells Ensure cages r wells are prvided n ladders f mre than twenty (20) feet t a maximum unbrken length f thirty (30) feet, except where suitable sliding fall prtectin devices are installed: Cages must extend a minimum f frty-tw (42) inches abve the tp f landing, unless ther acceptable prtectin is prvided; Cages must extend dwn the ladder t a pint nt less than seven (7) feet nr mre than eight (8) feet abve the base f the ladder, with bttm flared nt less than fur (4) inches, r prtin f cage ppsite ladder shall be carried t the base. There shuld be n prjectins inside the cage r well. Ladder safety devices may be used n twer, water tank, and chimney ladders ver twenty (20) feet in unbrken length in lieu f cage prtectin. 4.3 Requirements fr Landing Platfrms When ladders are used ver twenty (20) feet in height (except n chimneys), ensure that landing platfrms are prvided fr each thirty (30) feet f height:

10 Versin: 4 Page: 10 f 13 If n cage, well, r ladder safety device is prvided, a landing platfrm must be prvided fr each twenty (20) feet f height; Each ladder sectin must be ffset frm adjacent sectins; Where an ffset is required, a landing platfrm must be prvided; Landing platfrms cannt be less than twenty-fur (24) inches in width by thirty (30) inches in length. Where a persn has t step a distance greater than twelve (12) inches frm the centerline f the rung f a ladder t the nearest edge f structure r equipment, ensure that a landing platfrm is prvided with a minimum step-acrss distance f tw and a half (2-1/2) inches. Ensure all landing platfrms are equipped with standard railings and te bards. 4.4 Requirements fr Ladder Safety Systems (Devices) All ladder safety systems, such as thse that incrprate lifebelts, frictin brakes, and sliding attachments, must meet the design requirements f the ladders which they serve. D nt allw the cnnectin between the carrier r lifeline and the pint f attachment t the bdy belt r harness t exceed nine (9) inches in length. Ensure all ladder safety systems and related supprt systems n fixed ladders cnfrm t the fllwing: All safety devices must be able t withstand, withut failure, a drp test cnsisting f a five-hundred (500) pund weight drpping eighteen (18) inches; All safety devices must permit the wrker t ascend r descend withut cntinually having t hld, push r pull any part f the system, leaving bth hands free fr climbing; All safety devices must be activated within tw (2) feet after a fall ccurs and limit the descending velcity f an emplyee t seven (7) feet per secnd r less. Fllw all requirements fr munting ladder safety systems n fixed ladders: Muntings fr rigid carriers must be attached at each end f the carrier, with intermediate muntings spaced alng the entire length f the carrier, t prvide the necessary strength t stp wrkers falls; Muntings fr flexible carriers must be attached at each end f the carrier; Cable guides fr flexible carriers must be installed with a spacing between twenty five (25) feet and frty (40) feet alng the entire length f the carrier, t prevent wind damage t the system; Design and installatin f muntings and cable guides must nt reduce the strength f the ladder. 5.0 Safe Use f Ladders n r Arund Electrical Equipment Fllw safe wrk practices t prevent electric shck r ther injuries resulting frm either direct r indirect electrical cntact when wrk is perfrmed near r n equipment r circuits which may be energized: The specific safety-related wrk practices must be cnsistent with the nature and extent f the assciated electrical hazards; Further infrmatin may be fund in OSHA 29 CFR Never use metallic r metal type ladders arund electrical energy, cmpnents and surces.

11 Versin: 4 Page: 11 f 13 Only use fiberglass r wd ladders when wrking arund electrical energy surces. 6.0 Maintenance Requirements Only trained and qualified persns can make repairs t ladders. Ladder repairs must restre the ladder t a cnditin meeting the manufacturer s riginal design criteria befre the ladder is returned t service. Fllw ladder manufacturer instructins fr lubricatin f mechanical parts, such as metal bearings f lcks, wheels, and pulleys. Fr articulated ladders, lubricate the hinges upn receipt and then annually r mre frequently, depending upn use. Ensure all welding is perfrmed in accrdance with the Cde fr Welding in Building Cnstructin (AWSD ). Ensure metal ladders and appurtenances are painted r therwise treated t resist crrsin and rusting, when lcatin demands. Clean aluminum, fiberglass, wd and metal ladders with sap and water: Immediately dry ladders; Seal the fiberglass cmpnents with a clear r pigmented lacquer r paste wax, apprved r recmmended by the manufacturer; Prtect wd ladders with a clear shellac, varnish r wd preservative, as needed. Never paint a wd ladder. D nt stre ladders in direct sunlight, extreme temperatures r in damp envirnments: Stre nn-self-supprting ladders hrizntally and ensure wall supprts r brackets are prvided at 4 t 6 ft intervals t prevent sagging; Stre self-supprting ladders vertically in the clsed psitin. 7.0 Inspectins Ensure that ladder inspectins are cnducted, as specified in this dcument, and that a recrd f the inspectin is maintained either n a ladder inspectin tag r via an electrnic recrd. Fig 1: Ladder Inspectin Tags Ladder inspectin tags may be affixed (using an adhesive r zip ties) t the side rail f the ladder, preferably the inside face. (See Fig 1 fr an example) If an electrnic recrd is used, the ladders must be marked with a reference number that crrespnds t the electrnic recrd. Pre-numbered tags may be used fr this purpse r a reference number may be written n the side rail f the ladder. Never permanently affix (e.g. rivets, screws, etc.) a ladder inspectin tag r reference number t the ladder, unless it is apprved by the manufacturer. Mdificatins t a ladder withut the manufacturer s apprval may cmprmise the integrity such that the ladder n lnger meets specificatin. Use the Ladder Inventry and Inspectin Recrd template t generate an annual ladder inventry recrd and t create an electrnic recrd f required inspectins.

12 Versin: 4 Page: 12 f 13 Prtable ladders, including mbile ladder stands, must be inspected annually. Fixed ladder inspectins must be cnducted accrding t the fllwing schedule: Metal ladders and metal rungs imbedded in cncrete that are expsed t atmspheres where crrsin and rusting may ccur are t be inspected annually; Wd ladders that are expsed t cnditins where decay may ccur are t be inspected annually; All ther fixed ladders are t be inspected a minimum f every five (5) years. When cnducting prtable ladder inspectins, ensure that: All side rails are free f dents r bends and rungs are nt excessively dented; All step-t-side rail r rung-t-side rail cnnectins are intact and tight; All hardware cnnectins are secure and in gd cnditin; Rivets d nt shw signs f shear; The safety feet r ther auxiliary equipment (such as rpes) are kept in gd cnditin; and The stepping surfaces are free f il, grease r ther slippery substances.
