2 1. Name of Installation: P.I.R. Terminals See also General Requirements 2. Physical address of location La Petrolifera Italo Rumena S.p.A. ( PIR ) Via Baiona, 260/279, Porto Corsini (Ravenna) Italy 3. Opening hours and loading appointments Monday - Friday from: 07:30 16:15 hours. All vehicles for loading/discharging are admitted if presented by 15:30 hrs and provided no other truck is already loading / unloading at that moment at the same loading bay. While unloading the product from marine tankers, it will not be possible to receive / redeliver product from/to tank trucks from the tanks involved in the marine tanker discharge. Driver needs to report for every load at the so called traffic office with full order details including order number, product name, quantity and destination. 4. Life Saving Rules From July 1, 2009 Shell's Life-Saving Rules have been fully enforced. Shell s Life-Saving Rules set out clear and simple do s and don ts covering activities with the highest potential safety risk. They are not new. Most people comply with them every day but there are still breaches taking place. The Life-Saving Rules help to make sure that rules are followed and people are protected. Compliance with the Life Saving Rules is mandatory, and the Life Saving Rules are applicable to all operations under Shell s operational and / or governance control. Each reported non-compliance will be investigated. And failure to comply will result in disciplinary action, up to and including PIR Terminals Site Requirements, Version 2. Page 2 of 8
3 termination of employment for Shell employees, or for employees of contractors or sub-contractors, removal from the site and disqualification from future Shell work. Supervisors are held accountable to communicate and ensure compliance. If you choose to break the rules, You choose not to work for Shell. The 12 Life-Saving Rules are: Work with a valid work permit when required Conduct gas tests when required Verify isolation before work begins and use the specified life protecting equipment Obtain authorisation before entering a confined space Obtain authorisation before overriding or disabling safety critical equipment Protect yourself against a fall when working at height Do not walk under a suspended load Do not smoke outside designated smoking areas No alcohol or drugs while working or driving While driving, do not use your phone and do not exceed speed limits Wear your seat belt Follow prescribed Journey Management Plan 5. 4 6. Special requirements of this location PROHIBITED: 1. To drain tanks in the parking outside the terminal and in the internal areas as well. Drivers caught in such operation, will be brought to the attention of the AUSL functionaries (Azienda Unità Sanitaria Locale: environmental section) and of judicial authorities. 2. To dispose of plastic bottles, cans, oil/air filters, rags, working clothes, foodstuff and all other litter outside the proper waste drums. 3. Smoking outside smoking area / rooms. 4. To enter the terminal (even in the cabin ) for unauthorised persons and/or animals. 5. To enter the terminals with vehicles not equipped with suitable flame traps or similar devices. 6. To exceed the speed limit for all vehicles on the terminal: 5 km / hr. 7. To stop / park vehicles in front of emergency exits, obstructing the fire escape and regular visibility, or next to any fire fighting equipment. 8. To leave tank truck lights on, when waiting for operations and during loading / discharge operations. 9. To use and leave switched on, electrical equipment and equipment that may cause sparks, like: VHF and Radio apparatus, mobile telephones, lights, matches, electric batteries. 10. It is forbidden to operate without using all necessary Personal Protection Equipment, listed on Tremcards, as per current law. OBLIGATIONS: 1. Drivers must wear the protective clothing as listed on Tremcards, received from PIR Dispatching Department before entering the terminals. 2. For loading operations: To make sure that the truck s tank is empty, clean and suitable for loading the ordered product. 3. For loading as well as unloading operations: Drivers arriving at Terminals 260 and 279 for the first time, to deliver to PIR, the dispatching department requires the following documents: PIR Terminals Site Requirements, Version 2. 5 - Driving licence, - ADR certificate, - Tank truck circulation card, - Complete Tank documentation, - Authorisation for third account transport, if applicable, - Destination and receiver, (in case they are different) - Changes to above-mentioned documents like renewal, validity extension etc. must be shown again to PIR Dispatching Department. 4. To verify and sign the loading instructions issued by PIR Dispatching Department. 5. To fit the appropriate ADR numbers on the tank trucks, before entering the terminals. 6. To place the tank truck correctly on the weigh bridge and stop the engine. 7. When the loading bay is occupied, the driver has to stay close to his truck while waiting for loading: leaving his keys in the truck cabin and the doors unlocked. 8. Drivers must follow road signs to reach loading bays and / or unloading points. 9. The driver will handover the Loading Instructions, issued by the PIR Dispatching Department to the PIR operators at the loading bays. 10. When permission is given by PIR operators, the driver must place his tank truck correctly under the loading bay or next to the unloading point. 11. To stop the engine and put brakes on. 12. During operations driver must wear protective clothing, specific for the handling of the product. 13. Drivers must connect the earth mechanism to their truck. 14. Drivers have to check if the tank truck bottom valves are closed. 15. Follow all the instructions of the plant/loading personnel. All preparations will be taken care of by PIR operators, who will give permission for loading and/or discharge. PIR operators will also verify that all applicable procedures are followed by the truck drivers. PIR Terminals Site Requirements, Version 2. Page 5 of 8
6 16. Drivers will start operations only after having received permission to do so from PIR operators. 17. During loading / unloading operations drivers will strictly follow instructions and signals in the loading bays. 18. Drivers loading at Terminal 279, must use a proper fall protection system. 19. Prior to loading, drivers must verify the shore tank number and product name, highlighted on the labels attached on the loading arm, are the same as indicated on the Loading Instruction they received from the PIR Dispatching Department. 20. When the correct loading and tank truck manhole are selected, the driver will lower the loading arm into the tank, trying to place it as close as possible to the bottom, in order to minimize bubbling and to reduce vapor production. 21. The driver must secure the loading arm to the manhole by using a proper chain (two chains if required) in order to keep it safely fixed and to avoid counter stroke when the pump is started or stopped. 22. When the used loading arm is equipped with a manual valve, loading must be started gradually, i.e. by opening the valve partially. 23. The driver will start the pump. 24. When the driver is certain that the product flow is regular, the valve on the loading arm must be opened completely. 25. While loading/unloading, drivers have to stay next to the tank truck and the driver must monitor the product level inside the tank. 26. To gauge loaded tanks, drivers must use only aluminium rods, wooden staffs or other anti-spark materials. 27. When the driver gauges the tank and during changes of tank compartments, the loading pump must be stopped. Every time the pump is stopped the driver must insert a proper drip collector on the loading arm. 28. When the loading is completed, the driver has to place the tank truck correctly on the weigh bridge, and stop the engine. 29. Everyone who detects a dangerous situation or any kind of malfunctioning equipment, must immediately inform PIR Terminal personnel. PIR Terminals Site Requirements, Version 2. Page 6 of 8
7 Emergency When a driver detects a dangerous situation he has to advise immediately PIR Terminal personnel, or advise immediately the supervisor of the PIR Dispatching Department, in order to put the Internal Emergency Plan into action by using an acoustic alarm). During the emergency, drivers must follow following instructions: Keep cool. If occupied in loading operations: Stop immediately all operations. Stop the loading pump. Close the valve on the loading arm. Release the chains from the loading arm and manhole. Remove the loading arm from the tank. Place on the loading arm a proper drip collector. Move the loading arm back to its original, safe position. Close all manholes. Disconnect the earth mechanism from the truck. Make sure that the truck is safe and ready to be moved. Wait next to the truck for eventual evacuation instructions. If order to evacuate is extended to the entire Terminal area, drivers must move their truck to a safe location outside the Terminal, as instructed by PIR personnel or by PIR Emergency Team members, to be recognised by their special clothing. If occupied in unloading operations: Stop immediately all operations. Drivers must assist PIR operators with disconnecting the unloading arm. Close all tank truck manholes. Disconnect the earth mechanism from the truck. Make sure that the truck is safe and ready to be started. Wait next to the truck for eventual evacuation instructions. If order to evacuate is extended to the entire Terminal area, drivers must move their truck to a safe location outside the Terminal, as instructed by PIR Terminals Site Requirements, Version 2. 8 PIR personnel or by PIR Emergency Team members, to be recognised by their special clothing. If waiting to start loading / unloading operations; Wait next to the truck for eventual evacuation instructions. If order to evacuate is extended to the entire Terminal area, drivers must move their truck to a safe location outside the Terminal, as instructed by PIR personnel or by PIR Emergency Team members, to be recognised by their special clothing. If a driver is not able to move his truck away from the emergency area, he must leave the area on foot, following the emergency exit signs to go to the external pick up point, which is located on the parking area between Terminal 260 and Terminal 279, just in front of shore tank no. 51. ONLY EMERGENCY TEAM MEMBERS ARE AUTHORISED TO ENTER THE EMERGENCY AREA. PIR Terminals Site Requirements, Version 2. Page 8 of 8
