2 Backup Hand DRILLING EQUIPMENT ROTARY TOOLS 0 PPE Requirements - Hard Hat, Safety Glasses, Work Gloves, Steel Toe Shoes, Back Support, fall protection. During Pre-Job safety meeting, review JSA s pertaining to and STARTING EQUIPMENT as per job specific. All personnel sign off on meeting and current emergency contact numbers are readily available. Inspect Equipment as Directed: Pick Up Slings, Shackles, Tag Lines, Sheaves, Back Up Alarms, Fall Protection, Hand Tools, Man Lifts, Fork Lifts, anti-fall equipment, hoisting equipment. Make sure that all open holes are covered / flagged. READY TO RIG UP Assist in running lines: Fuel, Air, Electric, Steam, Hydraulic, Water. Ensure that all lines and connections are visually inspected prior to hook up. Check to make sure that all breakers are off prior to plugging in electric lines (all power sources are off) 3 Assist in connecting chains, belts and install guards to drawworks and power supply. Ensure all bolts, fasteners are in place and secure. READY TO POWER UP RIG 4 Prior to running cold start (if applicable). Make sure that all fire extinguishers are in place and in working condition. Inspect Equipment as directed (refer to maintenance manual): Check oil, check fuel, belt positioning, make sure guards are in place, cable connections, battery, battery box/cover, crank, and air lines. Watch for pinch points. Make sure that battery is covered. Use proper hearing protection. Use extreme caution when switching belts. 5 Assist in starting the cold start (if applicable). Use electric start if possible. 6 Prior to start of generator set. Lock out tag out station in place ready for use. Make sure all ground rods are in place.

3 Backup Hand DRILLING EQUIPMENT ROTARY TOOLS Inspect Equipment as Directed (refer to maintenance manual): 7 Check oil, check fuel, check coolant & cooling system, make sure guards are in place, cable connections, grounding devices, check breakers, air filter, electrical hook-ups, belts, battery, battery box / cover, rubber mat below breaker panel, and air lines. Assist in starting generator set. Watch for pinch points. Make sure battery is covered. Be aware, there may be foreign material blowing from radiator or air starters during initial start up. Check for proper voltage and hertz. Use proper hearing protection. 8 Prior to start of air compressors. Inspect Equipment as Directed (refer to maintenance manual): Check oil, belt positioning, make sure guards are in place, air filters, air dryers, proper valve positioning, check gauges, electric lines, air tank pressure relief valves, and air lines. 9 0 Assist in starting air compressors. Check system for air leaks. Drain condensation from system. Assist in rigging up water tank and circulating system. Check plumbing to water source, centrifugal pumps, electric motors, wiring, switches, plug-ins, hoses, connections, grounding, covers, handrails, discharge lines, valves. If applicable assist in rigging up boiler/rig heating system. Check electrical wiring, pumps, switches, valves, fuel lines, fuel regulators, water lines, blow down lines, steam lines, grounding, duct work, man hole covers, pressure relief valves, chemical injecting unit, gauges, and connections. Watch for pinch points. Automatic air compressor shut off working properly. Guards in place. Use proper hearing protection. Check for proper rotation of air compressor. Ensure no leaks in water circulating system. Use fall protection 00% tie off. Watch for pinch points. Ensure no leaks in heating system. Use fall protection

4 Backup Hand DRILLING EQUIPMENT ROTARY TOOLS READY TO POWER UP DRAWWORKS Prior to power up of Drawworks (Engines Compound / Electric Motor). Inspect Equipment as directed (refer to maintenance manual): Check engines compound, check all fluid levels, chains, guards are in place, belt positioning, leaks on all lines and equipment, proper placement of all bolts and pins, drawworks tie downs, electrical power supply cords. Check traction motors, ensure connections are tight, inspect brushes, inspect electrical power cords and ends, all guards are in proper place, blower filter / hoses. Ensure lock outs are working properly, keys in place, high voltage signs are in place, all controls in the neutral / shut off position. 3 Assist in start up of Drawworks (Engines Compound / Electric Motors). Watch for pinch points, emergency shut down operational, guards in place. Use proper hearing protection. READY TO RAISE DERRICK 4 Prior to raising derrick. Raise derrick during daylight hours or with adequate lighting. Inspect Equipment as directed (refer to maintenance manual): Inspect lines, sheaves, cables, rollers, pins, safety keys, electrical lines, lights, bolts, safety cables, bridle line in sheaves properly, stand pipe and kelly hose properly secured with cables. Kelly hose secured to derrick not to standpipe. Use fall protection 00% tie off, no loose tools left in derrick, stand pipe secured, derrick climber assist line tied off, geronimo cart secure. 5 Assist in the installation of dog knot / drilling line clamp, put the proper number of wraps of drill line on the drum, and the installation of guards. Use fall protection 00% tie off. Inspect dog knot / drilling line clamp for excessive wear or improper installations. 6 Assist in the installation of drilling line on dead man / tie down anchor. Assist in installing drilling line to anchor in the proper direction with correct number of wraps and check that guide bolts are in place. Assist in installing clamp plates, nuts and bolts. Assist in installing weight indicator lines to load cell on dead man. Assist in installing secondary safety clamp device directly behind dead man. Inspect brass insert for wear and ensure that it is installed in the proper direction. Secondary safety clamp device as close to dead man as possible. 3

5 Backup Hand DRILLING EQUIPMENT ROTARY TOOLS While drilling line is tightened up ensure that all lines are in proper position. Re-inspect derrick components. Assist in securing derrick snubbing line, make sure all personnel are in proper positions. As derrick is raised, assist in saddling / snubbing in derrick to pinning position with tension on snub line. Inspect derrick pins as directed (drive pins towards each other) and install safety keepers. Assist in installing the remaining floor plates and handrails to secure floor area. If applicable, install rotary guard and chain assembly. Assist in installing the derrick anti fall equipment and ladder. Assist in unbridling and securing bridle lines according to rig design. Make sure that shackles are secured with safety devices. READY TO RIG UP FLOOR Assist in setting in remaining air or hydraulic tuggers / hoist and secure. Tie in air or hydraulic power supply and spool on line as directed. If applicable assist in rigging up wind walls / winterizing. Check tarps/steel wind walls, straps, hooks, pin pockets, safety pins, braces and frame work as directed. Check for tools and other loose objects in derrick. No personnel should be under suspended load (derrick). Observe all lines and hoses in derrick while derrick is being raised (so that they don t get fouled). Use fall protection 00% tie off, tie off line on all tools used in derrick. When working overhead, ensure that there are no personnel below the work area. Use fall protection 00% tie off. Watch for pinch points. Ensure all floor openings are covered. Use fall protection 00% tie off. Watch for pinch points. When working overhead ensure that there are no personnel below the work area. Inspect line, brake, oil, hoses, motor, and valves (inline isolation valve at hoist). Use fall protection 00% tie off. Watch for pinch points. Use tag lines. 4

Inspect Equipment as directed (refer to maintenance manual): Proper pick up slings, choker chains, tag lines and shackles/cleaves. Look for excessive wear on all equipment, be sure that shackles / cleaves are secured with safety keepers. Tongs (pins, jaws, hangers, snub lines, dies, keepers, torque load cell, shackles / cleaves, break out line, jerk chain, handles). Slips (dies, keepers, pins, handles, hinge pins). Dog collar clamp (dies, keepers, pins, handles, hinge pins). Bails (secured to hook and elevator). Elevators (latch, springs, safety latch, hinge pins, elevator ears, check for proper sized bolts). Pipe Spinners (rollers / chains, air or hydraulic supply lines, hydraulic power unit, oil levels, snub lines, handles, safety cables, hanging spring, shackles / cleaves, lubricator). 7 Assist in picking up swivel, kelly, upper kelly valve, and hook up kelly hose according to rig design and JSA. Watch for pinch points, check kelly hose for wear. Use 00% tie off. Ensure proper installation of safety line. Inspect Equipment as directed (refer to maintenance manual): Bushings, rollers, thread connections, oil level, kelly hose secured, grease swivel packing, bales on swivel, proper lifting eyes / nubbings, and check kelly for rounding wear. 8 If applicable assist in installing air or hydraulic kelly spinner according to rig design and JSA. Inspect Equipment as directed (refer to maintenance manual): Thread connections, oil level, hose connections, fly wheel ring, turnbuckles, shackles / cleaves, spinner Ensure that shackles / cleaves are secured with keepers and that air-hydraulic lines are secured. 5

7 Backup Hand DRILLING EQUIPMENT ROTARY TOOLS 9 Assist in rigging up the remainder of the floor. 3 Wireline unit, mud bucket, floor safety valves with proper wrenches, proper wrench for kelly cock, stand pipe, handling subs, instrumentation gauges, lights, safety cables, bit breaker, related BHA equipment, floor safety mat, drilling line spooler / line guide. Inspect Equipment as directed (refer to maintenance manual): Electrical wiring, oil levels, brakes, control valves, counter, sheaves, handles, line wear, hanging cables, rubber seals, guards, safety valves operational, full opening lifting eye / nubbing, O-rings / gaskets, stand pipe valve working properly, safety cables on high pressure hoses, connection threads and faces / seals, hoses, wiring, air leaks, oil leaks, gauges functional, lenses and bulbs, safety cables on lights, bearing, bolts, rollers, shackles / cleaves. READY TO RIG UP MUD SYSTEM Ensure wireline unit is properly connected to floor. Use fall protection 00% tie off. Watch for pinch points. Ensure that all breakers are in the off position before plugging in any electrical equipment, lock out tag out. Assist in documenting the dimensions of BHA equipment, and handling subs Assist in ensuring anti fall protection devices and BOP scaffolding are in place and that conductor is installed. Assist in installing the flowline to the conductor and secure properly. Fill up line hooked up with proper safety cables. Assist in the installation of mud box / buckets return line to shaker tanks. Install / Ensure that a valve is in the bottom of the conductor and plug installed as directed. If conductor is not pre-set, review JSA on setting conductor to well design. Assist as directed in completing rig up of mud pumps and mud tanks. Check for leaks while circulating conductor and complete mud system as directed. Use fall protection 00% tie off. Watch for pinch points. Use tag lines or proper tail in device. Use fall protection 00% tie off. Watch for pinch points. Use tag lines or proper tail in device. 6

8 Backup Hand DRILLING EQUIPMENT ROTARY TOOLS Assist with the pressure testing of mud lines to lower kelly valve. After test is complete assist in bleeding off mud lines and opening the kelly valve. After rig up is complete assist with pre-spud inspection. Attend pre-spud safety meeting prior to drill out. END OF PROCEDURE Keep all personnel clear of high pressure lines while testing. 7

10 Backup Hand TOP DRIVE 0 PPE Requirements Hard Hat, Safety Glasses, Work Gloves, Steel Toe Shoes, Back Support, fall protection. During Pre-Job safety meeting, review JSA s pertaining to TOP DRIVE with all third party personnel at the start of the day, one person will be designated to give signal directions for movement of equipment (usually the Truck Supervisor with hand held radio), Rig Manager and Drillers responsibility to direct and organize crew throughout rig up. All personnel sign off on meeting and current emergency contact numbers are readily available. Make sure all rig up personnel have adequate radio communications (truck & crane operators). Inspect Equipment as directed: Load Lines, Air Tuggers / Hydraulic Tuggers (and lines), Pick Up Slings, Tail Chains, S-Hooks, C- Hooks, Boomers / Load Binders, Shackles, Tag Lines, Sheaves, Top Drive raising assembly, Communication Equipment, Back Up Alarms, Fall Protection, Hand Tools, Fork Lifts, anti-fall equipment, check all top drive skids for loose debris, ensure that shipping pins (on top drive transport skid) are secure quantity four. READY TO SPOT TOP DRIVE SCR HOUSE (SCR MAY BE SPOTTED DURING RIG UP WITH TRUCKS AND CRANES) Assist in spotting Top Drive SCR House (Top Drive support unit) according to grasshopper design and rig design. Make sure that all open holes are covered/flagged. All third party equipment has been visually inspected. Rig up during daylight hours or with adequate lighting. If needed have Top Drive technician on location. On initial rig up or if required have Top Drive technician on location. Watch for pinch points, check loading hitches, use tag lines, watch for overhead loads, use fall protection 00% tie off. 3 Prior to powering up Top Drive SCR unit (Top Drive support unit). Inspect Equipment as directed (refer to maintenance manual): If applicable with additional power pack (Check oil, check fuel, make sure guards are in place, check coolant & cooling system, air filter, belts) cable connections are tight, grounding devices, check breakers, electrical, battery, hook-ups, belts, rubber coverings on plug-ins where required, rubber mat below breaker panel, air lines, grasshopper, hinges, electric cords, hydraulic unit, and hoses. Use 00% tie off, avoid pinch points. 4 Ready to set grasshopper. Assist in positioning of grasshopper to rig floor using proper lifting device. Use tag lines, avoid pinch points, use 00% tie off..

11 Backup Hand TOP DRIVE READY TO RIG UP TOP DRIVE (ASSUMING TOP DRIVE COMPONETS ARE ON LOCATION, DERRICK IS UP AND FLOOR IS READY FOR TOP DRIVE INSTALLATION) For initial rig up have Top Drive technician on location. 5 If applicable assist in installing torque reaction beam (Top Drive technician should be on location for initial installation). Keep floor clear of personnel. Use 00% tie off. Stay clear of overhead / suspended loads. Avoid pinch points 6 Assist in clearing the front end of substructure of all rig components (v-door / beaver slide and cat walk). Use slings for lifting devices. Make sure hand rails, gates are closed, and fall protection is in place for rigging up Top Drive. When working around open V- door section of sub use 00% tie off. 7 Assist in bringing Top Drive lifting assembly on to rig floor and assist in installing into block. Avoid pinch points. 8 If applicable, assist in installing blower inlet hood. 9 Assist in positioning of truck with Top Drive load in front of substructure. Use flagger to position truck. Make sure that truck is centered with well and that load is square on truck. 0 Assist in extending service support / remote blower to proper position using ratchet (swing out). Watch for pinch points. Assist in lifting top section of Top Drive torque guide to floor using proper lifting equipment specific to Rig JSA. (Crane, Air Tugger, Blocks). Stay clear of all lifting lines, use 00% tie off when working with V-Door / Beaver slide door open. Assist in securing top of torque guide while hooking up raising assembly components. Watch for pinch points. Assist in removing runner frame stop pins. Watch for pinch points. 3 While torque guide is unfolding from truck watch all cables and service lines for snags. While torque guide is raising to floor monitor torque guide to a safe distance from substructure while raising. Use flagger to position truck. Stay clear from trucks path. Use handheld radios. Keep floor clear of personnel. Stay clear of overhead/suspended loads.

12 Backup Hand TOP DRIVE Assist in placing local blower in working position and bolt into place. Assist in disconnecting truck winch line from the torque guide when the guide is vertical and in the mast. Assist in lowering truck winch line with air/hydraulic hoist / tugger to ground. Close the V-door gate and secure. Assist in removing the live roller assembly. Assist in installing rig components (v-door / beaver slide and cat walk). While the blocks are lowered watch for any snags. Assist in attaching the air / hydraulic hoist / tugger to lower frame as directed by derrickman. Assist in attaching second air / hydraulic hoist / tugger to lifting eye on yoke. Assist (If qualified) in removing bale pins from yoke (before complete removal of pin ensure that pin is secured with tie off), and assist in lowering to the floor with air / hydraulic hoist / tugger. Assist in installing and pinning upper connecting links to block adapter. Avoid pinch points. Avoid pinch points. Watch for wicker / bur / frayed line on winch. Avoid pinch points. Properly secure with air / hydraulic hoist / tugger while removing and watch for load shifting. Keep floor clear of personnel. Use 00% tie off. Stay clear of overhead / suspended loads. Assist in installing of electric and hydraulic lines from grasshopper to torque guide. Assist in hook up of power and communication cables to driller console. After hydraulic power unit is turned on check for leaks. Assist in removing pins from cylinders, and assist in attaching tensioning cables. Assist in clamping torque guide skid to torque reaction beam as per drillers instructions. Assist in aligning Top Drive to wellbore center as per drillers instructions. Assist with maintenance check list on the Top Drive. Check back up wrench dies. Keep floor clear of personnel. Use 00% tie off. Stay clear of overhead / suspended loads Keep floor clear of personnel. READY TO RIG UP (ASSUMING RIG COMPONETS ARE ON LOCATION) WHEN UNLOADING OR POSITIONING ANY HEAVEY OR CRITICAL LOADS (SCR-GENERATOR HOUSE, ENGINES, PUMPS, DRAWWORKS) USE ADDITIONAL LIFTING SUPPORT. Assist in setting center line and flag for setting in rig. If required role out protective liner prior to setting matting boards. Assist in setting matting boards to rig specifications. Assist in covering cellar. Assist in spotting substructure on matting boards, assemble, and center over hole. Ensure that all pins and safety keepers are in place. Drive all pins in towards the wellbore. Make sure that all open holes are covered/flagged. All third party equipment has been visually inspected. Rig up during daylight hours or with adequate lighting. Use slings for lifting devices. Never shake loads off of trucks. Use a crane if on location, use short tag lines, keep hands and feet clear while setting mats, replace hole covers if removed. Remove any loose materials from top of matting boards. Keep hands and feet clear, use tag lines, be aware of overhead loads, and use fall protection 00% tie off. Look for rocks, dirt, and loose items on substructure..

17 Backup Hand TRUCKS & CRANES Inspect Equipment as Directed: Bent beams, cracked weld, loose pin pockets, safety chains on pins, safety cables on lights, electric cords, dog house outriggers, mud lines, water lines, air lines, steam lines. 7 Assist in placing required BOP equipment between substructure and secure. Ensure equipment is 00% secure before releasing hoisting equipment. Avoid pinch points. 8 Continue to completely assemble substructure and level. Ensure that all pins and safety keepers are in place. Keep hands and feet clear, use tag lines, be aware of overhead loads, and use fall protection 00% tie off. 9 Assist in installing required floor plates, hand rails, racking board, beaver slide / v-door, catwalk, pipe racks / pipe tubs, stairs, blocks on block stand or catwalk, cover all open holes on rig floor, install required ground rods. If applicable, install air or hydraulic tugger / hoist. Avoid pinch points, use fall protection 00% tie off, make sure that the top of all steps are secured. 0 Assist in placing drawworks and power supply (engines compound, electric motors) in proper position and secure. Inspect loading hitches. Keep hands and feet clear, use tag lines, be aware of overhead loads, use fall protection 00% tie off, be aware of pinch points. Inspect equipment as Directed: Tie downs (turnbuckles, pins, bolts, cables, shackles, cleaves), roller chains, sprockets, guards in place, safety keepers, water lines, oil lines, air lines, loading hitches. Look for rocks, dirt, tools or loose items in skids. THE FOLLOWING SEQUENCE IS A GENERAL GUIDLINE AND WILL VARY DEPENDING ON RIG DESIGN

18 Backup Hand TRUCKS & CRANES Assist in assembling the derrick, ensuring that all pins and keepers are in the proper position. Drive all pins according to rig design. Extend A-Legs to assemble spreader bar with proper pins and safety keepers. Use fall protection 00% tie off, watch for overhead loads, watch for pinch points, and use tag lines. Use a ladder or man lift to pin A-Leg spreader bar (never stand under or between raised A-legs). Inspect equipment as Directed: Crown sheaves, bridle lines, cable sheaves & cables, safety cables on sheaves, safety cable on lights, antifall devices, electrical wires, standpipe clamps, all bolt nuts pins and keepers, climbing ladder, safety lines on tong counter weight bucket, girts (bent), crown saver boards and covers, crown light, cracked welds, lube and grease all sheaves. Look for rocks, dirt, tools or loose items in derrick beams. Use fall protection 00% tie off. 3 Assist in stringing all lines, cables, ropes, electrical lights, and secure in proper position. Make up stand pipe. Use fall protection 00% tie off. Check O-rings in hammer unions. When derrick is in the proper position, pin to derrick shoes, and place on derrick stand as directed (place derrick stand under upright bracing). Watch lines, stay clear of overhead loads, make sure the derrick stand is on level ground, watch for pinch points, have a designated person giving directions. Never place derrick stand directly under crown itself. 3 When derrick board is in proper position pin to derrick. Drive all pins from inside to outside, install all pins and keepers as directed. Assist in rigging up wind walls / winterizing around board to derrick if applicable. Avoid pinch points, use fall protection 00% tie off, look for loose items in / on board. Use tag lines. Check tarps / steel wind walls, straps, hooks, pin pockets, safety pins, sala blocks, braces and frame work. Inspect equipment as Directed: Proper securing devices on fingers, footing flaps, diving board, winches, hand rails, fall protection, pins and safety keepers properly installed, geronimo cart, all equipment properly secured. Make sure proper pin and keeper are used in the geronimo cart. Inspect geronimo cart brake equipment and rollers. 3

19 Backup Hand TRUCKS & CRANES 4 When A-Legs are in proper position assist in pinning in place, be sure that the bridle / bull / raising line stays in sheaves properly, use the cranes spreader beam with two short chokers to raise the A-Legs. Raise derrick during daylight hours or with adequate lighting. Inspect derrick pins (drive pins towards each other) and install safety keepers. Assist in installing the remaining floor plates and handrails to secure floor area. If applicable, install rotary guard and chain assembly. Ensure proper installation of derrick anti fall equipment and ladder. Unbridle and secure bridle lines according to rig design. Make sure that shackles are secured with safety devices. Prepare derrick board for handling tubulars (lower fingers, diving board, rig up anti-fall devices, and mechanical pull back device). Ensure anti fall protection / devices and BOP scaffolding are in place and that conductor is installed. Assist in installing the flowline to the conductor and secure properly. Fill up line hooked up with proper safety cables. Assist in the installation of mud box / buckets return line to shaker tanks. Install / Ensure that a valve is in the bottom of the conductor and plug installed. If conductor is not pre-set, review JSA on setting conductor to well design. Ensure that any equipment / tools on tanks that are not in use are properly stored. Ensure that all breakers are in the off position before plugging in any electrical equipment, lock out tag out. READY TO RIG UP TOP DRIVE (ASSUMING TOP DRIVE COMPONETS ARE ON LOCATION, DERRICK IS UP AND FLOOR IS READY FOR TOP DRIVE INSTALLATION) If applicable assist in installing torque reaction beam (Top Drive technician should be on location for initial installation). Ensure that derrick board area is clear to receive Top Drive. Assist in hoisting Top Drive lifting assembly to rig floor and installing into blocks. If applicable, assist in installing the blower inlet hood. Assist in extending the service support / remote blower to proper position using ratchet (swing out). Assist in lifting top section of Top Drive torque guide to floor using proper lifting equipment specific to Rig JSA (Crane, Air Tugger, Blocks).
