2 Introduction 2 Product Safety Reporting This document describes the reporting methodology and key contacts for any perceived unsafe conditions which may result from the Maintenance, including special processing i.e. plasma Spay, plating etc. and Repair or Overhaul of Rolls-Royce products. In accordance with the internal Rolls-Royce procedures managing Product Safety, this guidance applies to all suppliers to Rolls-Royce of Maintenance, special processing and Repair & Overhaul (MRO) services 2 of 7
3 Product Safety Reporting Background 3 To ensure that Rolls-Royce meets its regulatory authority requirements, SABRe Addendum 001 was released in November 2013 to ensure that: For suppliers undertaking any maintenance, repair or overhaul activities, any potential unsafe conditions must be communicated immediately to the Rolls-Royce Purchasing contact. The requirement for reporting unsafe conditions identified through the maintenance, repair and overhaul activities applies equally to those suppliers that have Part-145 approval in their own right, and to those that operate under the extension of the Rolls-Royce EASA Part-145 approval. As with all SABRe requirements, it is also the responsibility of suppliers to flow this requirement down to any sub-tier suppliers that they may use 3 of 7
4 Unsafe Conditions 4 This guidance pack applies to any perceived unsafe conditions under the following: 1. Discovered conditions not caused by the repair process, but outside of the Engine Manual, Maintenance Manual, inspection limits, etc. 2. Caused any discrepancy in the repair process which may result in not meeting Engine Manual, Maintenance Manual, inspection limits., or affects the component/unit functionality. 4 of 7
5 Product Safety Reporting Categories 5 To ensure that reporting is consistent, the following details the types of product safety occurrences which must be reported into Rolls-Royce: Failure, cracking, burning, distortion or excessive heat discolouration of any part. Corrosion, wear or damage resulting in rejection to Engine Manual limits and which is outside of existing experience. Failure or excessive blockages of fuel spray nozzles or any cooling holes. Damage to electrical harnesses, connectors and engine electronic control unit mountings. Conditions which do not match mandatory requirements, such as installed life limited parts beyond their declared lives, or inconsistency between engine hardware and engine records. Findings as a result of compliance with an Airworthiness Directive or Non-Modification Service Bulletin. 5 of 7
6 Product Safety Reporting Categories (Continued) 6 To ensure that reporting is consistent, the following details the types of product safety occurrences which must be reported into Rolls-Royce: Failure to implement mandated modifications. Any other condition found during maintenance, build or test affecting component/unit function. Non-compliance or significant errors in compliance with required maintenance procedures. Misleading, incorrect or insufficient maintenance data or procedures that could lead to maintenance errors. The discovery of unapproved parts. Evidence of any previous mis-assembly. 6 of 7
7 Key Contacts 7 Any unsafe conditions relating to Engine, Module, Component or Unit repair should be reported to Rolls-Royce through the local Field Service / Technical Support Office. For any unsafe conditions relating to component/unit repair on a purchase order from the Rolls-Royce Global Repair Services business, all contact should be via the Maintenance Action Reporting System (MARS) grsmars@rolls-royce.com 7 of 7
