This document is a re-issue of Clinical/Research Bulletin No. 15 Dementia Projections for the Counties, Regional Municipalities and Districts of Ontario (January 2005), using the prevalence data obtained from the Canadian Study of Health and Aging (1994). The population data is from the latest available census (2006) as released by the Ontario Ministry of Finance ( Ontario Population Projections, ") providing values for each year between and. 2. Like Bulletin No.15, the format of this document is designed to show data for each region or county of the province, providing in each case, the population aged 65 and over (left side of page), and dementia case projections (right side of page), for each year between and. At the bottom of each page are two graphs. Dementia Projections -" is a stacked bar graph showing the number of cases of dementia projected each year broken up into 5-year age groups, and Percentage Increase Over " is a simple bar graph (with two bars for each year) showing the percentage increase over base levels for both the population aged 65 and over (black bars) and the projected dementia cases (red bars), for each year subsequent to. New to this bulletin, is the grouping of counties into census divisions according to the practice of Ontario (Eastern, South Western, North Eastern, North Western, Central, and the Greater Toronto Area). 3. The Canadian Study of Health and Aging (1994) prevalence rates are slightly higher than those of other studies, such as Preston (1986); but whether they capture the full extent of dementia is not clear. However, dementia is being diagnosed earlier than in the past, and therefore even these higher values may be somewhat conservative. In any case, these figures should be considered conservative estimates when planning health care services for dementia. 4. It should also be noted that these rates are for dementia in general and not Alzheimer's Disease in particular. Since Alzheimer's Disease accounts, by some estimates, for 75 to 90 percent of dementia in the elderly, by itself or in combination with other forms of dementia, a rough estimate of its prevalence can also be made. However, since all forms of dementia require a great deal of care and attention, it is reasonable not to confine oneself to estimates of Alzheimer's Disease alone, especially for the purpose of planning health services. 5. The year is of particular interest since it is the year that the first of the baby boomers reach 65. After that and until about 2040, figures will continue to rise substantially. 6. Since age is the most significant and easily identifiable risk factor for dementia, these projections are simply the product of observed dementia rates (prevalence) and population values; no other factors are used in their calculation. Unless significant changes to the projected population trends occur, only very substantial advances in preventative treatment could alter these projections. The population projections are for both males and females, and while the observed incidence of dementia seems to be higher in females than males, the survival rate is also higher in females and this may account for most if not all the difference. The population figures take survival rates into account.

3 It should also be noted that when projecting population estimates one must consider a number of factors such as birthrate, mortality, immigration, etc. Since estimates of these factors may vary, population projections, especially in the more distant future could therefore fall in a large range. The population projections used by the Ministry of Finance are in the middle of the estimated range. 7. There are two major consequences of these data for the Province of Ontario. First, the elderly portion of the population (i.e. those aged 65 and over) will continue to increase greatly in the near future, by 19% (over levels) in, over 41% by, and 125% by. This will lead to a great increase in all age related medical disorders, and an increased need for other social and recreational services for the healthy elderly. Second, the prevalence of dementia is also increasing rapidly. By the number of cases will increase by nearly 18% over levels. By the increase will be nearly 37%, and by that increase will be nearly 156%. That means that we will be caring for over 214,000 cases in compared to about 181,000 at present (), and by there will be over 466,000 cases in the province. These increases in cases of dementia (see graphs Percentage Increase Over, red bars) are caused by the fact that not only is the number of elderly rising (black bars), but the oldest age groups (i.e ", and ) are growing even more rapidly, and they have the highest rates of dementia (e.g. over 50% in the age group). These problems will be especially difficult for those municipalities across the province that have grown very rapidly in recent years, such as the Greater Toronto Area (especially the 905 area ). These areas grew largely due to the influx of baby boomers moving to new suburbs. This has produced a number of large and relatively young communities which have relatively few elderly people living in them. As these communities age in the coming years, they will experience exceptionally large increases in both the elderly population and cases of dementia. For example, while some rural counties such as Timiskaming will only show a dementia case increase of 101% by, regions such as York will experience almost a 280% increase. These huge increases will put significant strains on their local health care facilities, especially if effective planning is not started immediately. Preston, G.
