4 SPECIFICATION NOTES Note No. 1, (a and b) Superpave Asphalt Mix Design, HMA Base Course, PG 64-22, 0.0 to 0.3 million ESAL s, 25 mm mix: Mix Design, HMA Base Course, PG 64-22, 0.0 to 0.3 million ESAL s, 25 mm mix material, in compliance with Section 409, Publication 408, Pennsylvania Department of Transportation Specifications, latest edition. The mix will be virgin mix containing no RAM or RAP and will be generated at time of loading (not silo material). The F.O.B. price shall include loading on the Municipality of Monroeville's trucks at the Asphalt Plant. The delivered and dumped price shall be delivered to any street, road, or area within the Municipality of Monroeville. Note No. 5 - Premix Asphalt Stockpile Patching Material This item covers premix asphalt stockpile patching material, -"COARSE AGGREGATE TYPE", Komac, or equal. The price shall include loading on the Municipality of Monroeville's trucks at the Asphalt Plant. Note No. 6 - Premix Asphalt Stockpile Patching Material This item covers a premix asphalt stockpile patching material, "FINE AGGREGATE TYPE MWS. or equal. The price bid shall include loading on the Municipality of Monroeville's trucks at the Asphalt Plant. Check Legal Advertisement for Date and Time of Bid Opening. Bid Bond or Certified Check in the Amount of Ten (10) Percent of total bid is required. Separate Bond is required for each commodity package. All Bids held firm for sixty (60) days after bidding. All Bid Prices, once accepted by Council shall remain firm for a period of one (1) year.

6 GENERAL REQUIREMENTS OF BIDS: 1. Bid prices must be delivered prices. 2. Bid prices are to be free of any state, local or federal tax since the Municipality is a tax-exempt jurisdiction. 3. All sealed bids must be received by the Municipal Manager of the Municipality of Monroeville, Pennsylvania by the published deadline and marked PAVING MATERIALS 4. All Vendors supplying multiple Bids are required to seal each and every Bid individually, and clearly mark every envelope as to its contents. Once all of the Bids are prepared they are be placed in One Single Bid Package, with all individual Bids listed on the Outside of the Bid Package. BIDDERS MUST MEET ALL OF THE ABOVE GENERAL REQUIREMENTS.
