1 1- Intrductin ASA Spnsred ACI Shtcrete Nzzleman Certificatin Plicy fr Hsts and Participants This dcument describes the respnsibilities f and relatinship between ASA, ACI, the Hst, the Educatin/certificatin cmmittee, and the candidates. This plicy supersedes all previus actin f the ASA Bard f Directin with respect t ASA administratin f certificatin and educatin sessins, and is effective August 22, Every ASA Spnsred certificatin sessin is rganized thrugh the participatin f a Hst. The Hst may make the certificatin sessin available publically r privately fr a restricted audience. The hst shall have n relatinship with the sessin Examiner that is prhibited in this plicy (see sectin 2.7 Cnflict f Interest). As decided by the ASA Educatin cmmittee, ASA will spnsr certificatin activities nly if the candidates have been thrugh an ASA apprved Educatin prgram. An Educatin prgram may immediately precede the certificatin sessin, if desired. 2- Definitins 2.1 ASA Natinal Spnsring Grup: The entity identified by ACI as their agent in administering the ACI Shtcrete Nzzleman Certificatin prgram accrding t established ACI plicy. 2.2 Hst: The Hst is the rganizatin, grup r individual in charge f planning and crdinating the facility fr the certificatin sessin. The Hst culd be a cnsultant, cntractr, assciatin, unin, etc 2.3 Organizer: The single persn identified by the Hst as the crdinating cntact fr ACI, ASA, the Educatr and the Examiner. 2.4 Examiner: Individual apprved by ACI as an Examiner, Appinted t ASA s rster f ACI apprved Examiners by ASA, and appinted by ASA t act as and fulfill all Examiner duties f a sessin, including the Educatin sessin. 2.5 Participant: An individual pursuing Nzzleman certificatin 2.6 Cnflict f Interest: The relatinship between the Examiner, Hst, and Participants f a certificatin sessin must meet all the fllwing requirements: The Examiner must nt be currently emplyed by the Hst rganizatin r any f its parent r subsidiary rganizatins. The Examiner must nt be currently emplyed by the same rganizatin as any f the participants. The Examiner must nt be currently in a business relatinship with the Hst r any f its parent r subsidiary rganizatins. The Examiner must nt be persnally related t the Hst and/r Participants. ASA reserves the ultimate determinatin in assessing if a cnflict f interest exists. 2.7 ASA Educatin Cmmittee: The bdy directly respnsible fr versight f all ASA Certificatin and Educatin activities. This cmmittee is directly respnsible fr versight and management f ASA s rster f ACI apprved Examiners and ensuring that they cnfrm t all ASA & ACI plicies related t the Nzzleman Certificatin prgram. 2.8 ACI Shtcrete Nzzleman Certificatin Prgram Plicies: The ACI Shtcrete Nzzleman prgram plicies were created and are maintained by ACI Cmmittee C660: Shtcrete Nzzleman

2 Certificatin. The prgram plicies are the ultimate authrity in cnduct and peratin f the ACI Shtcrete Nzzleman Certificatin prgram. A cpy f the plicies can be fund at ASA Call Fr Examiners: The frmal prcess by which ASA selects a qualified examiner fr a certificatin and/r educatin sessin. Bard apprval f a Hst s request fr a specific Examiner fr a certificatin sessin is a rare ccurrence and nly granted fr very unique circumstances. Petitins fr a specific Examiner t cnduct a Certificatin sessin must be submitted in writing fr cnsideratin at a regularly scheduled ASA Bard f Directin meeting. It is als nted that a Hst can pick a current Examiner frm ASA s rster t cnduct the Educatin sessin fr an additinal $500 sessin set up fee. In this case the Examiner fr the Certificatin sessin is still selected at randm by ASA with the Hst having the ability t refuse an Examiner selected if the Hst feels that there is an existing cnflict f interest Examiner / Hst Relatinship Disclsure Frm: A frm that disclses any relatinship between the Examiner and the Hst. Cmpletin and signing f the frm is required when an Examiner respnds t an ASA Call fr Examiners. 3 Respnsibilities 3.1 Hst Respnsibilities: The Hst is in charge f naming the Organizer wh will serve as the Hst s agent / representative thrughut the certificatin prcess. The Hst is respnsible fr all safety aspects related t the certificatin prcess. The Hst is respnsible fr payment f the fees and expenses related t the Certificatin sessin. All fees and expenses will be billed thru ASA at the established standard fees. The Hst must prvide a safe wrking area fr the shtcreting, requisite cmpleted certificatin frms, materials, and equipment fr certificatin. The Hst is als respnsible fr taking the required cres (under Examiner supervisin) frm each Participant s test panels The Hst must prvide a current certificate f insurance t ASA staff prir t the certificatin sessin. The Hst s insurance must include general liability insurance with a minimum f 1 millin dllars cverage per ccurrence The Hst must prvide t ASA staff prir t the sessin, a certificate f Wrkman Cmp insurance fr all participants in the sessin The Hst must have cmpleted and submitted the Hst Requirements Checklist (Appendix B). A cmplete listing f Hst respnsibilities can be fund n the Hst Requirements Checklist (Appendix B) 3.2 Organizer Respnsibilities: The Organizer must crdinate the actins f everyne invlved, including the Participants and ASA. The Organizer is respnsible fr the rganizatin f the certificatin sessin and the executin f all Hst respnsibilities. 3.3 Participant Respnsibilities: The participant is respnsible fr cmpleting and signing their prerequisite 500 hurs f nzzleman experience n an ASA Wrk Experience frm. Participants are als required t be available fr a brief cnversatin by phne with the Examiner as part f the ASA Wrk Experience evaluatin. The cmpleted frm(s) shuld be submitted t the Hst s Organizer fr final review and signature. Falsificatin f wrk experience dcumentatin will cnstitute remval frm the certificatin sessin and pssible future certificatin sessins. 3.4 ASA respnsibilities: As a Natinal Spnsring Grup, ASA is respnsible fr all certificatin fees

3 charged by ACI, and fr billing Hsts fr all ASA incurred charges assciated with the certificatin sessins. ASA will crdinate all sessin details between the Hst s Organizer, the sessin Examiner, and ACI. ASA is slely respnsible fr chsing the Examiner fr an ASA Spnsred certificatin sessin. ASA will handle all financial transactins related t the certificatin sessin. An verview f the prcess fr administering an ASA Spnsred certificatin sessin can be fund in Appendix A. 3.5 Examiner Respnsibilities ASA Hst Relatinship Disclsure Frm: The Examiner is required t cmplete and sign the ASA Hst Relatinship Disclsure Frm upn answering the ASA Call fr Examiners. The Examiner is respnsible fr ntifying ASA if at any pint they realize that their relatinship with the Hst and/r Participants creates a Cnflict f Interest as defined in Sectin ACI Shtcrete Nzzleman Certificatin Prgram Plicies: The Examiner s respnsibilities are described in the ACI Shtcrete Nzzleman prgram plicies, which is created and maintained by ACI C660: these include marking the perfrmance exam, grading the cres and supervising the written exam Verificatin f Participant s Wrk Experience: The Examiner will receive cpies f each participant s wrk experience dcumentatin frm ASA staff. An Examiner s evaluatin f a Participant s Wrk Experience cnsists f tw separate steps. Step #1: The Examiner must review the infrmatin submitted n the Participants Wrk Experience Frm fr cmpleteness and feasibility. Step #2: The Examiner must call and speak directly with the Participant prir t apprving / signing the Participant s Wrk Experience Frm. The cmplete cntent and length f the Examiner s interview with the Participant is left t the Examiner s judgment and discretin in deciding if, in their pinin, the Participant pssess the required minimum amunt f wrk experience. Every Participant interview must hwever include the fllwing: Discussin f any items n the Participant s Wrk Experience Frm that seems incmplete r questinable. Discussin n the Participant s nzzleman experience including hw lng they have been a nzzleman, the type f wrk they have been invlved in, and the type f equipment they have used. Once the Examiner is reasnably sure f the accuracy f the infrmatin n each Participant s Wrk Experience submissin and that a minimum f 500 hurs as a nzzleman has been demnstrated, the Examiner must sign and date the participant s Wrk Experience frm and return a signed cpy t ASA staff. If at any pint during the Certificatin sessin, the Examiner judges that a Participant des nt have 500 hurs experience, he shuld ntify the Hst and further questin the Participant s qualificatin. If the dubt remains, the Examiner shuld cntact ASA staff and have the Participant s Wrk Experience changed t nt sufficient Examiner Arrival t Certificatin Sessin: When the Examiner arrives n site t begin the sessin, they shuld cnfirm that all required facilities, equipment & materials are present and cnfrm t plicy. If any f these items are missing r incmplete, the Examiner shuld ntify the Hst immediately. The Examiner must als check pht Identificatin f each Participant and cnfirm that each is n the Sessin Participant list received frm ASA staff.

4 3.5.5 ASA Requirements n Perfrmance Exams: Include pictures f each Participant (face sht), the back f each test panel, and the Participant s cres. The mix design shuld als be dcumented Perfrmance Exams fr Sessins With Mre than 14 Test Panels: Fr sessins with mre than 14 test panels, ASA s default psitin / qute t the Hst will be t add an additinal sessin day(s) s that nt mre than 14 test panels will be sht in a single day. If a sessin Examiner wuld like t cnsider emplying a Supplemental Examiner in rder t eliminate the extra sessin day, the Examiner shuld cntact ASA staff. Staff will wrk with the Hst t see if they are willing t pursue this avenue and negtiate any changes in csts Return f Sessin Materials t ASA: The Examiner must mail all sessin materials t ASA fr final review via a traceable methd f shipping. ASA will then frward the cmpleted examinatin dcuments t ACI fr prcessing Examiner Cmpensatin: The sessin Examiner must ensure that he will accept cmpensatin fr time and expenses related t the certificatin sessin nly frm ASA. The Examiner will under n circumstances directly invice the Hst nr accept payment frm the Hst. 4 - Review f ASA spnsred sessins: ASA reserves the right t cnduct a review f any ASA Spnsred certificatin sessin t ensure cnfrmance t bth the ACI and ASA certificatin plicies, and t ensure an acceptable level f quality. If the ASA Bard f directin decides t initiate a review, the Bard will appint an ad-hc task grup fr each review cnsisting f at least 3 current Bard members and a staff member t versee/cnduct the review. The Bard will ensure that the members f the task grup have n cnflict f interest with the hst and participants as determined by the Bard. 4.1 Reasns fr a Sessin Review: Cmplaints & Cncerns frm ASA Members and/r Industry Persnnel abut a Specific ASA Certificatin Sessin: Cmplaints and cncerns regarding the cnduct r quality f an ASA spnsred sessin shuld be submitted directly t ASA staff. ASA staff will cnduct an initial review t cnfirm that issues are based n specific items in the ACI Prgram plicies and/r the ASA Certificatin Prgram plicies. Shuld staff determine that the cmplaint/cncern is based n specific plicies; the issue will be brught t the ASA Bard f Directin. The Bard will decide if a review shuld be cmpleted Observatins by ACI and/r ASA Staff: Shuld staff bserve ptential plicy vilatins, it may cnsider presenting the situatin t the Bard fr ptential review ASA Bard Decisin: The Bard reserves the right t review any ASA spnsred certificatin sessin fr any reasn it deems satisfactry including randm quality reviews. 4.2 Cmpnents f a Sessin Review: An ASA Certificatin Sessin review can cnsist f ne r mre f the fllwing review cmpnents:

5 4.2.1 Wrk Experience: Review the sessin s verified wrk experience. This can include a review f hw the verificatin was cmpleted Returned Perfrmance Materials: Review f materials returned and required frmat f perfrmance exam materials Interview with Examiner: Interviews will be cnducted in the full task grup, nt by ne individual. The interview can include the fllwing: : Review f the verall sessin schedule & prcess : Check if the Examiner has a prper knwledge and understanding f the shtcrete field the hst wrks in : Interview with the Hst and/r Participants

7 General Appendix B Hst Requirements Checklist Cmmunicate Certificatin sessin needs/parameters t ASA Staff. Review & apprve Initial Certificatin Sessin Qute sent frm ASA staff. Review & apprve Final Certificatin Sessin Invice sent frm ASA staff. If an Educatin sessin is planned, distribute the study materials t the Participants prir t the sessin. (Recmmended distributin at least 7 days prir t the certificatin sessin). Cmmunicate t ASA staff if yu are willing t pen the sessin t the public (final apprval t participate f all applicants is the right f the Hst) Ensure cmpletin and return t ASA staff f Hst Evaluatin and Participant Evaluatin frms. Wrk Experience Initiate cmpletin f each Participant s Wrk Experience Frms, verify accuracy and sign each Participant s frms, and return all cmpleted Wrk Experience dcumentatin directly t ASA Staff. Ensure that each Participant is available by phne t speak directly with the sessin Examiner as part f the verificatin f their wrk experience. If an interpreter is required, it is the respnsibility f the Hst t arrange fr such services. Sessin Initiatin and Payment The sessin will be initiated nce all required dcumentatin and payment are received by ASA staff. Cmpleted Sessin Initiatin frm Signed Hst Requirements Checklist Full payment (check r credit card) f the inviced amunt including the $2,500 depsit (fr cmpanies that are nt current crprate members), plus the rush sessin fee, if applicable. Please nte: payment including depsit will be prcessed upn receipt. A refund f the sessin depsit ($ minus any unfreseen csts/charges) will be issued nce the cmpleted sessin materials are received by ASA Staff. Cmpleted Wrk Experience Frms fr each Participant Cpy f pht ID fr each Participant Insurance dcumentatin See & f the ASA Hst / Participant Plicy Written Exam If the Hst elects t have the Examiner administer the written exam, a suitable classrm area must be secured. Hst must cmmunicate any ral r language requirements when cmpleting the Sessin Initiatin frm. The Hst must secure a suitable classrm area fr the educatin sessin fr review f the study materials. Perfrmance Exam Crdinate and secure a safe wrking area Secure and make available all equipment: gun/pump, cmpressr, hse, nzzle, blw pipe, etc (minimum Air requirement fr wet mix is 250cfm withut a blw pipe, 375cfm r greater with a blw pipe. Dry rtary guns minimum air requirement is 600cfm. Determine mix and have sufficient shtcrete material available. Minimum material temp f 50 F in cld weather, Max temp f 94 F in ht weather. Fabricatin f Perfrmance Test Panels, reinfrcement and scafflding accrding t ACI Plicy. ACI allws fiberglass rebar t be used in place f steel (easier n the cre bits). Erect scafflding fr the verhead test panels and secure the test panels Prvide frklift r any ther type f lifting equipment fr the verhead panels Prvide weather prtectin fr the test panels - heat if the temperatures are 45 F r lwer, ht weather prtectin fr 90 F r higher. Ensure water set up with recmmend minimum pressure fr the dry prcess f 100 psi r greater (Nrmally 15 psi greater than the perating air pressure t the dry gun). Prvide ht water fr either prcess in cld weather (50 F r belw) Prvide burlap r plastic fr curing f the test panels Prvide persnal prtectin equipment fr each applicant Arrange fr site clean-up, equipment clean-up Prvide all equipment infrmatin and mix design t participants prir t sessin.

8 Cring Arrange fr / crdinate the cring as required in the ACI Shtcrete Nzzleman Certificatin Prgram Plicies. Prvide cring equipment with 4 cre bits (each panel takes min t cre) Prvide manpwer t mve and cre the test panels Cancellatin Plicy ASA plicy allws the Hst t reschedule r cancel a sessin up t tw (2) full days prir t the scheduled sessin start date. If the sessin is rescheduled a fee f $500 US will be charged t the Hst. If the sessin is simply cancelled a fee f $1000 US will be charged t the Hst. By signing this frm, the Hst is initiating with ASA a Certificatin sessin. The Hst understands their requirements as stated n this frm and in the ASA Hst/Participant Plicy, and agrees t take respnsibility fr all items. Shuld a requirement nt be fulfilled r sufficient, the Hst accepts respnsibility and csts fr any related delay. By signing this frm, the Hst recgnizes and accepts the Examiner f Recrd s authrity and judgment n all items and issues related t the sessin. By signing this frm, the Hst accepts respnsibility fr all csts and delays resulting frm all acts f Gd that are ut f bth the Hst s and ASA s cntrl. This includes thse that result in delay r cancelatin f the Examiner f Recrd s attendance. ASA will wrk with the Hst t minimize related delays and cst, but ultimate respnsibility lies with the Hst. All materials and full payment must be submitted n later than ne week befre sessin start.
