11 Policies and Procedures UES Approved Contractor Requirements In order to be placed on UES Installer s list the following items are needed: Copy of AZROC License Copy of Business License Spec Sheet on products proposed for installation Completed company listing approval form Three successful UES inspections Three unsuccessful UES Inspections justifies contractor Removal
12 Policies and Procedures Paperwork Flow Residential/Small Commercial All UES applications and agreements can be found at Submit Application, Agreement, W-9 and Assignment of Payment form to UES PRIOR to beginning project UES reviews and approves project Acceptance letter is sent to customer and contractor Installation must be completed within 180 days of acceptance Apply for city/county permits Permit must be secured within 60 days of acceptance Submit city/county permits to maintain incentive reservation UES releases Check Meter Base and Labeling Components Non-Residential systems must supply their own meter base
13 Policies and Procedures Paperwork Flow Continued Complete project and receive city/county final approval Submit city/county inspection approval AND installation certification (PV ONLY) to UES to request Meter Installations and UES inspection Net meter and Production Meter is set for customer followed by the Final UES interconnection inspection System is to be left inactive until UES Net Meter is installed Incentive check is requested based upon final approved UES inspection Incentive check is processed and sent to customers 4-6 weeks after final commissioning
14 Policies and Procedures DG METER All DG Meters will be set by our meter shop once the city final inspection is complete. A dummy meter will be used until the meter installation for all testing purposes for the contractor.
15 Policies and Procedures Net Metering A billing Net Meter and DG Meter is requested once a final successful UES inspection has occurred * Once a change out has been completed the customer will be placed on a Net Rate appropriate for their type of service Customers will only be granted credit against their usage once this rate has been put in place *Customer may be charged for production if system is energized prior to the Net Meter installation
16 Policies and Procedures Net Metering True-Up will occur in October Limited to 125% of connected load
17 Policies and Procedures UES Solar PV Inspection Requirements System must meet ALL specifications of Attachment A of the RECPP Agreement Labeling, Meter and Disconnect Placement must adhere to UES Service Requirements 1.22 *10ft. rule 4 inches of clearance should be between the back of the module and the mounting surface There should be no shading obstructions during the solar window (9:00 AM- 3:00 PM) DO NOT energize systems until the Net Meter and DG Meter have been set

19 Policies and Procedures Labeling of PV Components Labeling is to the UES Contractor upon the submittal of the building permit for a project. The outside cover of the main service panel must be labeled as illustrated in Label # 1 The DG Disconnect must be labeled as illustrated in Label # 2 The Check Meter or DG Meter must be labeled as illustrated in label #3

24 New Program Up-Front Incentives Triggers If the following percentage of UES s 2012 residential or commercial incentive budget is reserved on or before the following date UES s residential or business incentives will be reduced in the following ways. 45% June 30 Incentives fall to $0.85/watt. 70% Sept. 30 Incentives fall to $0.85/watt if June 30 trigger was not activated. Incentives fall to $0.70/watt if June 30 trigger was activated. 90% Nov. 30 Incentives fall to $0.85 if no prior triggers were activated. Incentives fall to $0.70 if one prior trigger was activated. Incentives fall to $0.50 if two prior triggers were activated.

26 2012 Performance Based Incentives Once application is accepted the required documentation is due within 120 days and has 365 days total for project completion. Executed REC Agreement; Assignment of Payment form; Building and/or construction permits and/or a full set of design development or construction drawings (80% or more complete); and New Supplier Fact Sheet for party designated to receive incentive payment; W-9 from customer Interconnection Application and Agreement Standard for all DG interconnections to UNSE s grid.
