1 1. Objective To minimise the risk of injury to all Crown employees, contractors and visitors who conduct work on and use electrical plant, equipment, furniture and fittings. 2. Scope This procedure applies to the use, maintenance and inspections of all electrical plant, equipment, furniture and fittings and installations at Crown. 3. Definitions Competent Persons (Licensed Electrical persons) Electrical Work Electrical Installation Electrical Equipment EnergySafe Victoria (ESV) Live Portable Appliance Test (PAT) Residual Current Device (RCD) Prescribed Electrical Ins tallation Non Prescribed E lectrical A person that is licensed or certified as competent under legislation to perform that work, or An apprentice that is working under supervision Includes all work involving construction, repair or maintenance on electrical installations or equipment Is the electricity supply to a building, and includes the main switchboard, distribution boards, sub-switchboards and all associated fixed wiring to isolation switches or socket outlets. Is an electricity consuming device or apparatus (including the cable) that is connected to the electrical installation. Classifications include: Hand-held / Portable: appliance which is hand held while in operation or can be moved easily while connected to the supply, intended to be held in the hand during normal use and the electrical motor forms an integral part of the equipment. (Excludes battery operated equipment). Movable: an appliance that can be moved readily from one place to another by unplugging from a general purpose outlet, but that is not moved during operation. Example: computer, printer etc. Fixed / Stationary: an appliance which in normal use is fastened to a support or otherwise secured in a specific position or is of such a size or function as to be difficult or unlikely to be moved from one place to another. Example: workshop machinery, oven, refrigerator etc Energy Safe Victoria is Victoria s statutory independent electricity, gas and pipeline safety and technical regulator. Working on or near exposed electrical conductors or circuit parts that are or can become energised because electrical power to the working equipment or system has not been shutdown. A process by which electrical appliances are routinely checked for safety and undertaken by a trained person in accordance with AS/NZS 3760 In service safety inspection and testing of electrical equipment. A device which switches off electricity supply when an imbalance is detected between any current flowing in the active and neutral leads and earth leads. Work on all or part of any electrical installations if they are ordinarily operated at low voltage or a voltage exceeding low voltage (refer to Prescribed and Non Prescribed installation work C) Is work other than listed as prescribed electrical installation work. 2 Ins tallation HV LV Test Tag not include re-lamping, replacement of diffusers, lamp shades and decorative covers on GPO s and switches. High voltage: voltage exceeding 1000 volts AC or 1500 volts DC Low voltage- Exceeding 32 V a.c. or 115 V d.c. but not exceeding 1000 V a.c. or 1500 V d.c. Fitting of tag or other indicator to electrical equipment that has been tested and found to be compliant. 4. Requirements 4.1 Guidelines General Electrical Equipment Use Managers/Supervisors shall ensure that all electrical plant, equipment, furniture & fittings are only used in the manner in which they were designed. In addition to this the following shall be followed: Loose electrical cables kept off the ground and clear of thoroughfares; Double adapters are not used under any circumstances; All electrical items have a current test and tag, where required. (Refer to Testing Intervals for Electrical Equipment B); Equipment is not used outdoors that is designed for indoor use; Use non-conductive ladders, tools, equipment, clothing and footwear; All electrical hazards are reported to the respective immediate Manager/Supervisor Procuring Electrical Prescribed items and Other Electrical Equipment The Electrical Equipment Compliance Flowchart A shall be utlised by all Crown employees to determine the correct approach for acquiring prescribed items and other electrical equipment. The Purchasing Department shall be notified where an item is deemed not compliant and not approved for requisition. (Refer to Electrical Equipment Compliance Flowchart A). The Crown Electrical Supervisor is available to discuss the correct process for procuring any irregular items. (Refer to Procurement of Goods CS-P-021). 4.2 Electrical Work The Crown Electrical Supervisor shall be notified of all electrical work taking place within the Crown site prior to the electrical work commencing (including work undertaken by Crown Projects Refer to Project s Electrical Isolation Procedure: Electrical Services). All new electrical installation work irrespective of the size or scope of the job shall be brought to the attention of the Crown Electrical supervisor prior to the commencement of work (Refer to Electrical Contract Work Flowchart -WIC01). Electrical installation work conducted on the Crown site shall only be carried out by Engineering & Maintenance employees and authorized contractors that hold an A Grade licence. A certificate of electrical safety must accompany all installations and be signed by the Crown Electrical Supervisor. All contract controllers, project managers and engineering & maintenance officers shall provide a list on a weekly basis to the Crown Electrical Supervisor of any planned project works to be undertaken. All copies of certificates of electrical safety must be forwarded to the Crown Electrical Supervisor once work has been completed. The Lock out Tag Out Register must be completed where applicable. (Refer to Isolation of Plant & Equipment CS- P-037). Page 2 of 11
3 Work on electrical equipment shall only be carried out by competent persons in accordance with their licence restrictions. Any employee or contractor that has conducted work outside their licence restrictions will be considered to have committed a major safety breach and may be subject to disciplinary action. YES Reschedule Work NO Isolate & Make Safe Work on Live Electrical Equipment Restoring Supply Certificates of ESV requires the completion of certificates of electrical safety for all prescribed and non-prescribed electrical work conducted at Crown. Certificates of electrical safety for all electrical installations and alterations carried out by employees and contractors shall be coordinated by the Supervisor and forwarded to ESV. (Refer to Prescribed and Non Prescribed Installation Work C). In a non-compliance, employees and contractors must repeat the task as per ESV s instructions until compliance is obtained. 4.3 Engineering & Maintenance Officers conducting Prescribed and Non Prescribed Work Prescribed Installation Work Prior to commencing prescribed work an engineering & maintenance officer shall obtain a prescribed work electrical safety certificate from the Crown Electrical Supervisor. The engineering & maintenance officer shall only complete parts 2-12 of the required form. Once complete the form shall be forwarded to the Crown Electrical Supervisor with a copy of the work order attached. Upon completion of prescribed work the engineering & maintenance officer shall enter their person identification number and licence number within the extra text area located within the BEIMS system. (Refer to Prescribed and Non Prescribed Installation Work C) Non Prescribed Installation Work Page 3 of 11
4 Prior to commencing non prescribed work an engineering & maintenance officer shall obtain a certificate of electrical safety from their immediate manager. The form shall be completed in its entirety and the customer copy forwarded to the Crown Electrical Supervisor. A copy of the work order shall be attached to the form and forwarded to the Crown Electrical Supervisor. The Crown Electrical Supervisor shall ensure that all certificates are lodged in accordance with Crown s 90 day certificate requirement with ESV. (Refer to Prescribed and Non Prescribed Installation Work C). 4.4 Contractors conducting Prescribed and Non Prescribed Installation Work Contractors conducting Prescribed and Non Prescribed Work for Crown shall purchase and lodge any electrical safety certificates directly to ESV on Crown s behalf. Where an inspector is required to attend the Crown site to inspect any prescribed work the contractor shall ensure that this is arranged as appropriate. (Refer to Prescribed and Non Prescribed Installation Work C). 4.5 Risk Management Where work involving electrical hazards is to be carried out, the manager/supervisor/contract controller shall ensure that: Any hazards associated with performing the task are identified The risks associated with these hazards are controlled Appropriate controls introduced to minimise the risk these must include: o At least one of a developed risk assessment/safe work procedure/safe work method statement/safe Operating Procedure including the use of the appropriate signage prior to requesting a permit to work o Wearing suitable personal protective equipment o Using barriers/barricades and rubber matting to prevent access to the work area Control of Job Requirements Crown employees and contractors shall complete all work in accordance with project documents / work orders. In the event that additional work beyond the job documents is required to be carried out or the initial agreed work activity is altered, the worker in control at the site will: Record all changes on the project documents / work order Obtain endorsement from manager/supervisor/contract controller authorizing the changes Review any risk assessments, standard operating procedures (SOPs) and/or safe work method statements and implement further controls if required Update relevant documents e.g. work order documents, as-built drawings etc Audits The Crown Electrical Supervisor shall ensure audits are conducted on electrical work by employees and contractors on a regular basis. The audits shall identify if the works being undertaken are being conducted in accordance with CrownSAFE procedures. Results may be used to identify individual and group training needs. (Refer to HSWC Assurance Activity Schedule CS-P-004B). The Crown Electrical Supervisor shall ensure all audit records are kept on the Engineering & Maintenance G drive to enable easy retrieval of information. Page 4 of 11
5 4.6 Live Electrical Work Working on or near live electrical work shall not be carried out unless it is absolutely necessary and that there is no practical alternative. Examples of live work Fault finding Testing of energized electrical equipment & electrical installation work Tightening the terminals of a live circuit breaker Replacing fittings or components when they are live Connecting additional circuits to existing switchboards Live electrical work is only permitted where: There is no practical alternative to perform the work de-energised There is no practical alternative to performing the electrical work other than live A written risk assessment, standard operating procedure and/or safe work method statement is available (Refer to Risk Management CS-P-028). Testing instruments are in-test and suitable for the work The Manager/Supervisor/Contract Controller has sighted and approved the work method for the live work Appropriate PPE is used e.g. long cotton sleeves, no exposed metal components, safety glasses, gloves etc. Tools and Equipment used for live work Insulated for the voltages potentially present Inspected on a regular basis for damage Suitable for the task 4.7 Removal of unwanted/damaged electrical equipment Defective Electrical Equipment Where electrical plant, equipment, furniture or fittings are found to be defective or damaged, the relevant manager/supervisor/contract controller shall ensure the appropriate action is taken to remove any possible hazards. This may include withdrawing the item from service and placing a yellow and black out of service tag on the item or placing a red and black repair order tag on the item. (Refer to Isolation of Plant & Equipment CS-P- 037). The manager/supervisor/contract controller shall notify: Engineering and maintenance by logging a job with the helpdesk on 03) ; or MIS electrical equipment helpdesk on Personal Protective Equipment (PPE) Electrical work may require employees or contractors to wear suitable PPE that has been pre identified via the risk assessment and various business unit procedures/safe operating procedures. (Refer to Personal Protective Equipment CS-P-052). 4.9 Electrical Incident Reporting The Manager/Supervisor/Contract Controller responsible for the person who received the electric shock shall report the incident immediately to Security Communications emergency number on Any person who receives an electric shock, regardless of the voltage (if known) shall be assessed by a First Aid Secondary Responder and placed on a 3 lead ECG where agreed. (Refer to First Aid CS-P-079 & Notifiable Incidents- Victoria CS-P-06A). Page 5 of 11
6 In addition to this the Manager/Supervisor/Contract Controller shall contact the Engineering & Maintenance Shift Manager on to notify them of the incident. Consideration shall also be given to notifying the incident to the Health & Safety Workers Compensation Manager for any serious electric shock that requires hospitalization (Refer to Notifiable Incidents- Victoria CS-P-06A). All electrical incidents shall be entered into the OSCAR system and an investigation to occur in accordance with Incident Management CS-P-006. The engineering & maintenance shift manager shall ensure appropriate ESV forms are completed and forwarded to the Electrical Supervisor for review and lodgment with ESV. Three forms must be completed, these are: ESV electrical incident report Crown electrical incident report BEIMS HS100 report (generated from the work order). Copies of all three forms shall be lodged to the ESV within four working days by the Crown Electrical Supervisor. Completed forms shall be uploaded into OSCAR Testing & Tagging of Portable Electrical Equipment The manager/supervisor/contract controller of each work area shall ensure that all portable electrical equipment is tested and tagged and the date remains current. The Crown Electrical Supervisor shall ensure that all portable electrical safety equipment is tested and tagged in accordance with the requirements outlined within Australian Standard (AS) The Crown Electrical Supervisor shall ensure that an electrical equipment register is maintained to ensure that the following details are listed: Type of equipment Location (e.g. Department, area, etc) The frequency of testing required The dates of the last and next test due The records of tests and inspections shall be kept throughout the working life of the equipment. A record is also required of any faulty equipment detailing the corrective action including service and/or repair information. (Refer to Testing Intervals for Electrical Equipment B). The Crown Electrical Supervisor shall ensure the portable electrical equipment register shall be audited on an annual basis to ensure all relevant information is being recorded. (Refer to HSWC Assurance Activity Schedule CS- P-004B). Any Contractor portable electrical equipment that is tested and tagged by a Crown Seaward machine shall not be recorded within the Crown electrical equipment register. Crown Marketing and The Palms business units shall download the electrical safety equipment register located on their individual Crown Seaward machines at least once every six months and forward the information to the Crown Electrical Supervisor. 7 New Electrical Equipment Process for Testing and Tagging (excluding Crown Projects) Any new portable electrical equipment that has been purchased shall be delivered to the Warehouse Loading Dock and placed in the designated area. Engineering & Maintenance shall be contacted for PAT to be completed and records of the testing included within the portable electrical equipment register. All electrical equipment shall be PAT tested prior to delivering it to the necessary department. (Refer to Testing & Tagging of Electrical Goods CW- P-010-1) Testing and Tagging (Crown Projects Only) All new portable electrical equipment shall be unpacked and placed in their final locations prior to testing and tagging; Each item shall be registered within a Crown Seaward machine Software Database; All portable electrical equipment shall be PAT tested conducted in accordance with AS 3760; All PAT testing must be carried out with a Seaward test and tag machine with a Seaward PC compatible printer; All equipment that has failed to pass the PAT shall be placed in a separate area and an Out of Service tag placed on it. (Refer to Application of Out of Service & Repair Order Tags CS-P-057B); Refer to Crown Projects Safe Operating Procedure Manual for more information; The Seaward register shall be downloaded within the Engineering and Maintenance office after the completion of each project Electrical Testing in Restricted Areas Security communications shall be contacted on 03) to notify of any anticipated electrical equipment testing in restricted areas such as Gaming pits, change booths etc Test Tag A tag shall be placed on all electrical equipment that has successfully passed a portable appliance test. All tags must contain at a minimum: A printed tag from a Seaward machine Company Name (Crown) Test Date Next Test due A barcode and number for use by the PAT shall be attached to the lead. Generic test tags shall be placed on Contractor portable electrical equipment when tagged by a Crown Seaward machine, where required. Under no circumstances shall a Crown tag be used on contractor equipment Hired/Leased/On Loan Electrical Equipment The testing and tagging of electrical equipment remains the responsibility of the supplier at the commencement of the hire/lease/on loan period. The manager/supervisor/contract controller shall ensure that electrical equipment that does not contain an appropriate tag shall not be accepted Electrical Equipment that Fails Testing Electrical equipment that fails any test shall be regarded as non-compliant and must be: Taken out of service and an out of service tag placed on the equipment. (Refer to Isolation of Plant & Equipment CS-P-037); Page 7 of 11
8 Arrangements are made to repair the item; Sent back to the manufacturer/supplier; or Equipment is destroyed 4.11 Residual Current Devices (RCDs) RCD s are recognized as a means of providing automatic disconnection of supply in the event of a fault in the electrical equipment or installation. However RCD s should not be considered as the sole means of protection against electrical hazards. The Crown Electrical Supervisor shall ensure that all socket outlets are protected by an RCD where practicable. Portable or plug in RCD s shall be used where electrical equipment has the potential to become hazardous or there is no fixed RCD. E.g. wet areas or where there is a reasonable chance of mechanical/chemical damage Electrical Switchboards The Crown Electrical Supervisor shall ensure that all electrical switchboards are locked and marked externally with an identification number and appropriate signage. All circuits are to be clearly identified at the board and any termination point to facilitate isolation Supervision Managers, Supervisors and Contract Controller s shall ensure that any electrical works being undertaken by an engineering & maintenance officer or contractor are appropriately supervised Apprentices All apprentices shall be supervised by a competent person that shall be responsible for the activities of the electrical apprentice. Supervision levels may vary depending on what year the apprentice is and if the apprentice has carried out similar work. For further information on supervision requirements for apprentices please refer to Supervision guidelines for apprentices working on electrical installations- ESV document, October Fixed Electrical Equipment Inspection and Maintenance All fixed electrical equipment, including switchboards, shall be regularly inspected and tested and records kept on the Engineering & Maintenance BEIMS system. Any Maintenance and repairs shall only be carried out by competent persons Training and Competency: Employees The relevant Training Manager shall ensure all Crown employees are enrolled and complete the online electrical safety module Contractors All contractors conducting work on electrical plant, equipment, furniture & fittings shall complete Crown s Health & Safety Induction in accordance to the role they will be primarily be completing at Crown. (Refer to Contractor Management CS-P-022). Page 8 of 11
9 Competent Persons- PAT Testing: Licensed Electricians Managers/Supervisors may choose to nominate and certify suitable employees to undertake PAT that do not have an electrical work licence. At a minimum the employee must have successfully completed an accredited training course titled Inspection and safety testing of electrical equipment. (Refer to Health & Safety Training CS-P-003). 5. Responsibilities 6.1 Managers/Supervisors & Contract Controllers Electrical equipment is used in the manner that it was intended to be used; All electrical appliances are regularly inspected and tested and tagged; Tagged out of service equipment is not operated or used; Electrical incidents are reported through to the necessary persons; Persons conducting electrical work do so in accordance with their licence restrictions and CrownSAFE requirements; Electrical hazards, risks are identified and controlled; Notify the Crown Electrical Supervisor of any new electrical installation work prior to commencement; Ensure all certificates of are delivered to the Crown Electrical Supervisor. 6.2 Crown Electrical Supervisor Certificates of are forwarded to EnergySafe Victoria where required; Persons are available to conduct portable testing and tagging; Electrical equipment register maintained and audited annually; Audits are conducted ; Notification and investigation of serious electrical incidents to EnergySafe Victoria; Monitor legislation. 10 6.3 Engineering & Maintenance Shift Manager Report any electrical safety incident to the Crown Electrical Supervisor. 6.4 Employees Report any faulty equipment to their immediate Manager/Supervisor; Follow any safe work requirements related to using the electrical plant, equipment, furniture or fittings. 7.
