Any services selected except Anatomical Donation, will include a charge for our basic cremation services which accounts for our overhead. If it is necessary for you to purchase any items that you do not specifically indicate due to legal or regulatory requirements, we will provide you an explanation in writing on the statement of Funeral Goods and Services. The prices reflected in this General Price List are effective as of January 1, 2015, and are subject to change without notice. (1)

2 Direct Cremation (Without Visitation or any Ceremony) $1,125.00* Transfer of your loved one from the place of death to our facility within 50 mile radius of Rolla ($3.00 per mile after 50 miles) 24 hour per day Care of your loved one Preparation of all necessary document Cremation process using our state-of-the-art equipment Alternative Cremation Container Assistance with placement of Newspaper and/or Radio notices Return of the cremated remains in 1) Temporary Plastic Urn (included in above price) 2) An Urn purchased from our stock 3) An Urn purchased from our catalog 4) An Urn or Receptacle provided by the family Direct Cremation with Graveside Service $1,695.00* All benefits of the Direct Cremation Package as well as: Coordination, set-up and supervision for a Graveside Service at the cemetery of your choice (within 50 mile radius of Rolla. $3.00 per mile after 50 miles) Register Book (50) Memorial Folders/Prayer Cards (25) Thank You Cards Tent, grass and chairs Opening and closing of grave fee set and paid to cemetery (add $150 if done by Rolla Cremation) (2)

3 Direct Cremation with Memorial Service $1,895.00* All benefits of the Direct Cremation Package as well as: Coordination, set up and Licensed supervision of a visitation held one hour prior to Memorial Services at the venue of your choice, within 50 mile radius of Rolla ($3.00 per mile after 50 miles) Coordination and Licensed supervision of the Memorial Service Register Book (50) Memorial Folders/Prayer Cards (25) Thank You Cards Grave Side Service following Memorial Service add $ with tent, chairs and grass setup (without tent, chairs and grass set up add $150.00) Opening and Closing of grave fee set by and paid to cemetery (add $ if done by Rolla Cremation) *PLEASE NOTE* The above charges are for person whose weight does not exceed 300lbs Over 300lbs add $ Over 400lbs add $ Over 500lbs add $ lbs and above add $750 (3)

4 Anatomical Body Donation $695.00* Deceased has to be a pre-registered donor with the Medical School Transfer of your loved one from the place of death to our facility within 50 mile radius of Rolla ($3.00 per mile after 50 miles) Transportation to University of Missouri, Columbia Washington University in St. Louis St Louis University in St Louis Preparation of all necessary documents Assistance with placement of Newspaper/Radio Stations notices *PLEASE NOTE* Each Medical School has their own criteria for accepting persons for Anatomical Donation. In some cases the medical school will prohibit an Anatomical Donation due to weight issues, manner of death, etc. Rolla Cremation and Memorial has no control over this and cannot be held responsible for decisions made by the Medical Schools. 6 (5) PAYMENT OPTIONS ALL PAYMENTS ARE DUE AND PAYABLE BEFORE SERVICES ARE PREFORMED WE ARE SORRY BUT WE ARE UNABLE TO OFFER PAYMENT PLANS ON AT NEED SERVICES WE ACCEPT CASH, CHECKS, AND THE LISTED *CREDIT CARDS FOR FORMS OF PAYMENT If paying by credit card the following fees will apply *A 2.75% processing fee will be added to total if payment is made by a credit card that is present and can be swiped by us. *A 3.50% processing fee will be added to total if payment is made by a credit card that is not present and the card number is entered manually by us.
