2 This addendum is being issued to answer question submitted by interested parties and to revise the price schedule. All other terms conditions and specifications remain unchanged. This addendum must be returned with your bid submittal. How are the sharps stored? as an entity does not generate medical sharps. However, as a municipal entity any items discovered on our proty must be proly disposed. How often are the pickups? Pickups vary based on need at any particular time. Do you schedule multiple locations to be picked up on the same day? Pickups vary based on need at any particular time. Can waste only be incinerated / recycled? Or can the waste be sent to fuels blending or landfills for disposal? Disposal shall be conducted in accordance with local, state and federal laws. What type of chemicals are you putting in your lab pack rcra HazWaste box? Expired chemicals such as acids, bases, organics. What type of chemicals are you putting in your lab pack non haz box? Expired chemicals that don t meet RCRA Hazardous waste definition. What type of chemical is in the drum for corrosive liquids? Wastewater with ph less than 2 with heavy metals. See attached file What was the total spend for the services covered under this solicitation in each of the last two calendar s? $79,402.50, $45, YTD $48, How many lab pack services were formed in the past two s? 3 Are container pickups coordinated on an LTL basis and if yes what is a typical or average number of stops pickup? A request can be for multiple locations. Pickup must occur in accordance with the terms of the contract Page 21 item 24 of the solicitation requires 24/7 response capability with a 2 hour response to the call and a 24 hour onsite requirement. How many times has this requirement been used in the last 2 s and for what types of services? 1 time to pump out a UST sump Page 41 line item 43 and 44 medical sharps. Typically medical sharps are packaged in small poly puncture proof containers not 55 or 30 lon drums. Are the medical sharps packaged loose in the drums or are they in smaller poly containers inside of the drums? Sharps would have to be packaged in a 12 lon medical sharp container able to handle syringes and ampules. The price lines have been changed to reflect this. What locations generate the medical sharps? Page 3 of 14

The contractor would be required to package the sharps at the location where they are located. Page 40 line item 22 and 23 Corrosive liquid, what is this material? There are a wide variety of corrosive materials and the treatment methods and costs vary widely. Example: Wastewater sample that is below ph 2 and high in metals Please provide profiles for any bulk waste streams including line items 33, 30 and 27. Profiles are unavailable. Please provide constituents and concentrations of the organic acids and organic bases line items 50 through 54 on page 41 of the solicitation. As a municipal entity any items discovered on our proty must be proly disposed. Thus each pick up may require a profile. What type of acid is line item 34, 35 and 36? Please provide constituents and concentrations. Contaminated hydrochloric acid (50%), Sulfuric acid (concentrated) Nitric acid (concentrated), etc For line items priced pound such as items 36 through 42 on pages 40 and 41 of the solicitation where do we place a minimum volume or price container? This should be listed as price/pound No minimum quantities are specified or allowed,these items should be priced by the pound. Page 37 and 38 of the solicitation appear to be for supplying containers used in packaging the waste material. Is that correct? If yes are supplies to be provided at the time of waste removal? It is the contractors responsibility to provide the supplies and package the material. Page 4 of 14

5 Some of the supply items seem to be duplicates, for example what is the difference between item 5 on page 37 the metal 55 lon drum and item 31 on page 38 the empty drums, metal 55? Is one new and one a reconditioned drum and if yes which is which? s should be listed as RCRA empty used drums for disposal and placed under Category 2 for waste removal. This has been addressed in the Revised Schedule On page 43 under cleaning please define what you require for the scrubber tank item 3? Is this some type of vapor control system or is this intended to represent the portable stainless steel tank required as part of the tank cleaning? Yes This is for the cleaning of the chlorine and sulfur dioxide scrubber systems that are located at various facilities. Note: A temporary roll-off box may also be needed in these type of situations Page 23 of 95 of the solicitation under Tank Cleaning requires the provision of a temporary stainless steel tank and material placed into the tank for storage onsite. a. How long would the temporary tank be onsite? b. How would the contractor bill for the temporary tank, is that what the line item 1 under tank cleaning is for or was that line item 3 and 4 was for? c. What material historically has been placed in the temporary tank? d. How many tank cleanings were done in the last two calendar s? e. Are multiple tanks cleaned during one project or are they typically scheduled one at a time? f. What materials would be removed under the tank cleaning item d vs the fuel tank cleaning item e? a. As long as required to complete the cleaning oation, and provide a waste profile if required prior to transport; afterwards the tank is removed by the Contractor to dispose of the material. b. A price line has been added for tank cleaning which should include the temporary tank pricing c. Generally wash down water from fleet shops, and car wash facilities d. Twice/ e. Tanks are cleaned as needed through owner request. Contractor may schedule multiple cleanings as long as contract pick-up requirements are met. f. ; Fuel tank cleaning: the waste predominantly contaminated rain water from the sumps and buckets. Tank cleaning: waste may include chemical sludge, grit, hydrocarbons Ect.. On page 43 of 95, Group 3: Cleaning What are the size of the above ground tanks? Is the fuel inside the tanks gasoline or diesel? What are the size of the underground tanks? Is the fuel inside the tanks gasoline or diesel? Refer to page 60 of the bid document Page 5 of 14

7 TAB 4 ATTACHMENT D REVISED PRICING Pricing will be evaluated and will be a part of determining the overall Best- Value Bid. All pricing shall be enclosed in a separate sealed envelope, marked PRICING. Vendor must submit Pricing on Pricing Schedule provided in this best value bid. Only one (1) copy of pricing is required. The pricing will be evaluated based upon the lowest total price submitted on the Pricing Schedule. The Proposal with the lowest price will receive forty (40) points. All other proposals will be allotted a centage of the 30 points based on a comparison with the lowest priced proposal.
