1 ADDENDUM #1 The Plymouth Airport Commission is committed to fulfilling the New Fuel Farm Facility project within the scope of the permit, not exceeding our funding and to satisfy the time constraint associated with the funding. The Plymouth Airport Commission reserves the right to determine the lowest total bid (low bidder) based upon the base bid or a bid including any or all alternatives, dependent upon funds available. The right is reserved to reject any or all bids, to waive any formality and any and all technicalities in bids and to accept such bids as may be deemed in the best interest of the Plymouth Airport Commission. Original Bid Item #1 - Removal of one (1) 10,000 gallon steel (80LL Mo gas) UST will be per Pricing Submitted with Original Bid Form; Original Bid Item #2 - Removal of two (2) 12,000 gallon fiberglass (100LL Avgas) will be per Pricing Submitted with Original Bid Form; Original Bid Item #3 - Relocation and Refurbishment of one (1) 12,000 gallon (Jet-A) AGT for use with (100LL Avgas) will be per Pricing Submitted with Original Bid Form; A. Alternate Bid Item #3A - The procurement and installation of a new 12,000 gallon UL 2085 rated AGT tank for use with 100LL Avgas at the New Fuel Farm Facility, and the legal disposal of the existing AGT upon transfer of the existing Jet-A Fuel within it. I. The Airport must remain fully operational during the progress of the work, to include performing fueling operations, thus the timing of the transfer of Jet-A Fuel from the existing AGT and the other work to be performed under this item must be performed in an appropriate sequence as noted in the Technical Specifications; II. All work under this Item is to comply with Local, State, and Federal regulations, as well as NFPA requirements; III. This Section specifies requirements for the removal and the legal disposal of the existing 12,000 gallon Jet- A AGT, as well as the procurement of a new 12,000 gallon UL 2085 rated AGT tank for use with 100LL Avgas at the New Fuel Farm Facility. This includes the transfer of the existing Jet-A fuel from the AGT, the removal and legal disposal of any sludge or waste, the disconnection and abandonment in place of the Secondary Containment Vessel drain, removal of the existing pumping system and delivery of same to PMA, the removal and the legal disposal of the existing 12,000 gallon Jet-A AGT, and the furnishing and delivery of a new 12,000 gallon UL 2085 rated AGT tank for use with 100LL Avgas at the New Fuel Farm Facility; IV. The Contractor will be responsible for the transfer of the remaining Jet-A Fuel in the existing AGT to the New 20,000 gallon Jet-A AGT being provided by the Owner and Installed by the Contractor at the New Fuel Facility. The New 20,000 gallon Tank is projected to be delivery on May 25, PMA is using down the fuel in the existing AGT to minimize this amount of fuel to be transferred, so for Bid purposes Contractor should assume 5,000 gallons of Jet-A Fuel is to be transferred to the New Fuel Facility. If the actual quantity of fuel transferred is larger or smaller than 5,000 gallons an adjustment to the value for this action will be negotiated and approved by Change Order. The Contractor is fully responsible for the legal transfer of Jet-A Fuel from the existing AGT (It should be noted that PMA has been using Dependable Petroleum, 1 Roberts Rd, Plymouth, MA 02360, (508) for Fuel Services); V. The transfer/disposal of the Jet-A Fuel, which is a regulated substance, shall be performed in accordance with applicable Massachusetts DEP regulations, and other Local, State, and Federal regulations;

2 VI. No excavation is required for the removal and legal disposal of the existing AGT, beyond that need to terminate and plug the Secondary Containment Vessel drain connection to the Airport s Drainage System and Oil Water Separator; VII. The existing concrete pad that the existing AGT is mounted on is to remain. The concrete sleepers on the slab will need to be removed and brought level with remainder of pad, unless PMA authorizes an expectation by Change Order not to remove said concrete sleepers. Exposed rebar, if any, will be drilled back and grouted; VIII. The secondary containment drain is to be disconnected and terminate below ground. The Pipe is to be capped, or plugged with Concrete, and abandoned in place. An Excavation to chase and fully remove the drain line all the way to the catch basin it Connects to is not required; IX. The existing pump system is to be removed with care and will remain the property of PMA; X. Contractor is responsible for the legal disposal of all residual sludge and waste from the AGT, including but not limited to any required LSP services, sampling, testing, lab reports, documentation and final disposal. The cost of such legal disposal shall be included in the Bid Price for Item #3A; XI. Contractor will Furnish and Deliver a new 12,000 gallon UL 2085 rated AGT tank for use with 100LL Avgas at the New Fuel Farm Facility; XII. The costs for the installation of this new 12,000 UL 2085 AGT tank as defined in Item 6.f. of the Technical Specifications, with the exception that the connection of the Secondary Containment Vessel drain to the delivery truck pad catch basin will no longer be required as the new 12,000 gallon UL 2085 AGT tank will be double walled and not require secondary containment and thus not require a drainage connection; XIII. The New Tank shall include all required markings, labels, and signs; appurtenances for connection of pump system and Veeder-Root monitoring equipment; and will require the submittal and coordination of shop drawings; XIV. The Tank shall be designed for aboveground storage of flammable and combustible liquids at atmospheric pressure, specifically 100LL Avgas; XV. Tank, piping, and associate fittings, attachments, and appurtenances shall meet the requirements of all current applicable Codes and Standards for. 100LL Avgas Fuel Storage in Above Ground Tanks, to include but not limited to NFPA 407, NFPA 30, the National Electric Code, the Code Federal Regulations, and Commonwealth of Massachusetts Regulations, relevant API Codes, and applicable FAA standards; XVI. The Tank shall be a 12,000 gallon double wall UL-2085 Fireguard Thermally Insulated, Steel Aboveground Storage Tank, or equal; XVII. Tank shall be of double wall construction and provide complete secondary containment of the primary storage tank s contents by an impervious steel outer wall, providing an integral steel secondary containment; XVIII. Tank shall be delivered as a complete UL-listed assembly with either two (2) factory supplied, weldedon saddles or a skid style base. The Tank is to be placed on a solid concrete foundation, as such the Anchor Bolt Holes shall be sized to allow for minor expansion and contraction of the Tank; XIX. Tank to be pitched to one end as appropriate and shall have a low point sump and the point of the intake suction or fixed draw pipe opening will at the high end; XX. Shop Drawings are to be submitted. Shop Drawings shall provide dimensioned drawings including plan views, sectional views, electrical wiring diagrams and installation coordination details, to include layout and arrangement of control panels, piping connections, accessories and other items to assure a

3 coordinated installation. Drawings shall indicate adequate clearance for operation, maintenance and replacement of operating devices. XXI. Tank to be tested for tightness in the factory, and in the field before commissioning, records of tightness testing (pressure testing) are to be provide/submitted before or upon delivery; XXII. Tank shall be supplied with emergency vents, Tank is to be located out of doors on a concrete slab at Plymouth Municipal Airport and will be vented to the atmosphere by means of closed venting devices; XXIII. Inner and Outer Tank shall be manufactured in accordance with UL-142 Standard for Steel Aboveground Tanks for Flammable and Combustible Liquids or higher standard. Entire tank shall be labeled for Underwriters Laboratories UL 2085 Standard for Insulated Secondary Containment Aboveground Tank for Flammable Liquids; XXIV. Tank shall be internally epoxy lined; XXV. Lifting lugs shall be provided at balancing points to facilitate handling, off-loading and installation; XXVI. Tank shall be painted white and all required placard, decals, warning signs, etc. shall be provided and affixed, to include but not limited to Flammable, No Smoking,.100 LL Avgas. Signs shall be posted on all sides as required. Shop Drawing submittal will be required; XXVII. One (1) 24 manway shall be provide; XXVIII. Valves, loading and unloading connections, switches and other control equipment shall be colorcoded to identify the grade and type of fuel they control as required by current Codes and Standards; XXIX. An Interstitial space visible monitor shall be provided; XXX. An external Side Mounted Ladder with Deck Platform is to be provided; XXXI. The Tank shall provide required appurtenances to allow proper grounding and bonding per Codes and Standards, and in accordance with manufacturer s requirements. Grounding Lugs shall be provided on the Tank as required; XXXII. Overfill protection shall be provided; XXXIII. A Ground level reading tank gauge with high level alarm at 90% tank capacity shall be provided XXXIV. Operations and maintenance manuals to be provided; XXXV. All components shall carry a minimum one-year warranty. Manufacture s Standard Warranty Statement shall be include with Bid; XXXVI. The Contractor is responsible for obtaining and completing all AGT Removal, Disposal, Construction and Operations Permits; Original Bid Item #4 - Removal of existing Fuel Dispenser System and associated piping and concrete slab will be per Pricing Submitted with Original Bid Form; A. Alternate Bid Item #4A - Removal of only the 80LL Mo gas Dispensing Pump, its associated Hose Real, and the associated underground piping and conduit ; I. All requirements under Item #4 in the Technical Specifications still apply as they apply to the removal of only the 80LL Mo gas Dispensing Pump, its associated Hose Real, and the associated underground piping and conduit;

4 Original Bid Item #5 - Performance of miscellaneous demolition at existing Fuel Storage Facility will be per Pricing Submitted with Original Bid Form; A. Alternate Bid Item # 5A - Limited miscellaneous demolition at existing Fuel Storage Facility ; All requirements under Item #5 in the Technical Specifications still apply as they apply to the removal of the 80LL Mo gas Tanks; associated piping, conduits, hose stands, foundations, and appurtenances; I. Item not being removed: a. The existing 4 fence (the fence must remain as facilities with the area will remain); b. Not all existing Inventory Control wiring and conduit will be removed, only that associated with the Mo gas Tanks, all other Inventory Control wiring and conduit to remain; c. Not all existing UST Vent Stack piping will be removed and legally disposed, only that associated with the Mo gas Tanks; Original Bid Item #6 - Construction of New Fuel Storage Facility will be per Pricing Submitted with Original Bid Form; A. Alternate Bid Item #6A - Limited Construction of New Fuel Facility All requirements as stated in the Technical Specifications for Original Bid Item #6 still apply unless specifically noted here; I. Items no longer included: a. Relocation of 12,000 gallon AGT for use with 100LL Avgas, Item f. on Original Bid #6 b. Furnish and install new pump and filter system for 100LL Avgas; item g. on Original Bid #6 c. Furnish and install remote Veeder-Root inventory control system equipment for one (1) tank (there will be no 100LL Avgas tank at the new facility) d. Electrical service for 100LL Avgas pump, tank, and controls e. Grounding connections to the 100LL Avgas Tank and Pump System
