Knob hdwe. and Seat to Back Bracket Top Our Basic Inserts are designed to provide a therapeutic interface surface between the wheelchair user and the mobility base. Their primary functions are to promote improved sitting posture and provide pressure relief. Basic Inserts are made from nine ply high strength wood bases,quality foams and/or gel materials and a large variety of upholstery fabrics to meet the needs of your clients. We offer our Basic Inserts in a variety of configurations that include the seat insert, the back insert, a seat to back bracket as well as all the attaching hardware. Seats are cut full width and backs are between the backposts unless specified. Standard foams are 1 1/2" high density on the seat and 1" high density on the back. Please see the Measurement Forms for further information regarding custom options and modifications. The seat and back are connected with a seat to back bracket which allows for easy seat depth adjustments with the simplicity of traditional hook style hardware. This option is also useful for powerdrive wheelchairs with limited under seat space and an existing seat pan or upholstery. 5 Solid Seats with Hardware Our Solid Seats with hardware replace standard wheelchair upholstery. They provide a solid base to improve sitting posture and serve as a platform for the attachment of other positioning components.the usual construction is 1/2" nine ply high quality wood bases, 1 1/2" high density foam and premium covering materials. Hardware choices include fixed drop hooks, adjustable height and angle hooks or knob release hardware. Please specify between seat rail or full width mounting. 6 Solid Seats without Hardware These Seat Inserts can sit on top of existing seat upholstery. or attach with seperate hardware. A 1/2" nine plywood base and 1" of high density foam are standard. A variety of covering materials are available. Velcro bottom availableupon request. Model SSI101 has T- nuts for hardware attachment. Solid Seat Inserts SSI100- SSI101 Full Width and Drop Bases DBF100 - Base w/ Flush Hooks ( full width only) Drop Bases are constructed form 1/2" black polypropylene and have hook fastener installed to secure cushions. They are available for most single crossbrace wheelchairs up to 22" wide. For frame widths wider than 22" or for dual crossbraces, 3/4" HD plywood will be substituted and a Heavy Duty upcharge will be added. Black vinyl is standard with the Heavy Duty construction. 7 Solid Seat Modifications ATSM - Anti Thrust Seat Modification The Anti-Thrust Seat Modification helps improve sitting posture by decreasing posterior pelvic tilting of the pelvis. It also may help decrease extensor tone. BILS - Built in Lateral Thigh Supports This modification is designed to decrease excessive hip abduction and prevent the lateral aspect of the distal thigh from contacting the armrest or legrest of the wheelchair. CSM - Contoured Seat Modification The Contoured Seat Modification has built in Adductors, Abductor and an ischial recess. This modification keeps the pelvis and thighs in proper position. Contoured Seats can be done as a generic or as a custom. See Measurement Forms. ASD Asymmetrical Seat Depth The Asymmetrical Seat Depth Modification provides for differences in thigh ( femur) length and/ or helps accommodate for non-correctable pelvic rotation. Please see Measurement Form to confirm that the correct side is cut down. BIMTS - Built in Medial Thigh Support This modification helps properly position the thighs by limiting medial movement of the distal thigh. Please use Contoured Seat sketch on the Measurement Form for specifications or specify generic. SOL - Incontinent Liner - Spray On This special coating helps prevent moisture from entering the foam and wood base. The pressure relieving qualities of the foam are not affected so the cushioning remains comfortable and moisture resistant. Coating is a latex free material. GSM - Grid Seat Modification GSM - Grid Seat Modification This seat modification adds foam blocks that can be modified for pressure reliefor to accommodate boney prominences. The blocks are attached to the seat with hook and loop fastener. The standard Darlexx cover is fully removable to allow complete access for modifications. Special foams and thicknesses are available. 5

8 Solid Seat Modifications ISM - I - Seat Modification These cut out areas allow additional adjustment of lateral hip and thigh support brackets. Please use Measurement Forms. WSF - Wedge Seat Foam Standard Wedge Seats (AP - front higher) help prevent anterior pelvic tilt and can decrease extensor tone by increasing hip flexion. The Reverse Wedge ( AP - rear higher) can help properly position the pelvis and facilitate coming to standing. Lateral wedges can accommodate some pelvic obliquities. Please specify foam thickness and direction if other than standard. shown from rear with open cover This modification gives the Seating Specialist the ability to independently adjust the height of each side of the pelvis on obliquities that can be reduced. It also gives the ability to accommodate and equalize pressures on clients with fixed obliquities. It can also be used as a "pelvic well" to reduce sitting pressures under the pelvis. Another use is to place speciality foams directly under the ischial tuberosities to decrease sitting pressure and increase user comfort. This modification is supplied with several different types of polyurethane and viscoelastic foams. Please specify overall foam thickness. Two inch foam thickness is standard and the minimum thickness recommended. GELS - Gel Seat Modification This modification adds a gel insert under the pelvis to decrease sitting pressures and increase user comfort. LSS - Laterally Split Seat Modification Can be used to accommodate for Hip Flexion Range of Motion Limitations. Allows User to sit more Vertically. Works best w/ Knob Hardware. Covers can 3 seperate pieces(shown) or 1 piece. FIPS - Foam in Place Seat Modification The Foam in Place Seat Modification has built in ABS( plastic) laterals to provide support during the foaming process. 1 1/4" viscoelastic foam is included to ensure that adequate pressure relief is attained. Dual side mount zippers allow easy access to pour the foam. A Bag Pack of Soft Liquid Sunmate Foam is standard. Bowl Packs are available as a no charge option. One unit of foam is standard - additional foam available. Clinician must have FIP experience. 6

9 Solid Backs Solid Backs are fabricated from 1/2" high strength plywood, 1" high density foam and a large selection of covering materials. Several hardware options are available. (note: Some items shown with additional modifications) BFH101 - BFH102 Solid Back w/ Fixed Drop Hooks (for between the backpost mounting or for between backpost base and full width foam) BFH100 - Solid Back w/ Flush Hooks - (full width backs only) BAH100 - Solid Back w/ Adjustable Hook Hardware Solid Back Inserts are made with 1/2" high strength plywood, 1" high density foam and a large selection of covering materials. They are designed to fit in front of standard back upholstery and have loops for attachment to the backposts. (except SBI101 which requires additional hardware to install on the wheelchair frame) BKR100 - Solid Back w/ Knob Hardware This Solid Back comes complete with two pair of our premium Knob Release Brackets. This bracketry provides maximum durability and adjustability. It can be adjusted to accommodate a range of seat depths and seat to back angles. SBI13W - 13" wide x 18" high - fits most 16" wide chairs SBI15W - 15" wide x 18" high - fits most 18" wide chairs SBI100 - Custom Sizes - Please specify size SBI101 - Solid Back w/ T- Nuts - Please specify size and T- Nut pattern. 7

10 Solid Back Modifications DERF - Dynamic Extension Relief Forks Dynamic Hardware is available on Solid Backs with Knob Release Hardware and frame widths between 12 and 18 inches. Three different spring tensions are available to accommodate users from 40 to 250 pounds. This hardware provides shock absorption caused by excessive tone, spasticity or by repositioning. It extends the life of the wheelchair and Basic Insert by reducing stress on the wheelchair backposts. Not available with backs that are curved by wood. (Curved by foam and most other modifications are available) top view DERF top view IDERF Independent Dynamic Extension Relief Forks This modification is available on backs with Knob Release Hardware, frame widths up to 24 inches and can accommodate users up to 250 lbs. This modification is available only on flat backs. The IDERF modification will allow slight rotation in addition to extension. It will absorb shock causedby excessive tone, spasticity or by repositioning. It will extend the life of the wheelchair frame and Basic Insert by reducing stress on the backposts. Not Available with backs that are curved by wood. (Curved by Foam and most other back modifications are available) shown with optional Curved by Foam Back Modification IDERF CURVE CURVE - Curved Back Modification This modification uses laminated hardwood that is formed into a gentle curve. The Curved Back gives increased user contact which increases user comfort and improves lateral stability. It is available for backs up to 22 inches in actual width. Top View CBF CBF - Curved by Foam This modification uses a flat plywood base and curved foam to provide increased user comfort and improved lateral support. Can be used with all dynamic options as well as with all other Back Modifications. LRF LRF - Foam Lumbar Roll Modification The Foam Lumbar Roll helps provide lumbar support for the wheelchair user. It comes with two different profiles of lumbar rolls or you can send a sketch for custom sizes. A zippered cover provides access for adjusting the height of the support. 8

11 Solid Back Modifications I- Back / T- Back These modifications have cut out areas that allow for increased adjustment of lateral thoracic supports. It is especially useful for between the backpost backs and for users that have solid backs that are significantly wider than their chest width. IBM - I- Back Modification TBM - T- Back Modification Biangular Back Modification This back modification consists of upper (thoracic) and lower (pelvic) sections that are connected with adjustable angle hardware. This assembly is then covered with one piece of foam and one upholstered cover to provide a smooth interface surface. This modification helps promote a more upright sitting posture and increases sitting tolerance. BBM - Biangular Back Modification Rotational Biangular Back Modification This back modification has an upper and lower section that are upholstered separately. These sections are connected by a pair of our multi-axis Rock-n-Lock brackets. This allows not only conventional biangular adjustability but also the ability to have independent rotational adjustments of the upper and lower sections. RBBM - Rotational Biangular Back Modification Rear View Lumbar Roll Modification with Hdwe. This Modification allows external adjustment of both the height and the anterior/ posterior movement of the lumbar roll while the user is seated in the chair. Once the proper adjustment is attained, the secondary knobs lock the support in place. Please give range for vertical (height) adjustment desired. LRH - Lumbar Roll w/ Hardware 9

12 Solid Back Modifications GBM Grid Back Modification COBM Contoured Back Modification Grid Back Modification This back modification allows the clinician to fabricate a custom contoured back at the time of the initial fitting. The standard cover is a four way stretch material that allows maximum contouring as well as pressure relief. Standard foam grids are two inch thick HR foam. Other foam types and thicknesses are available. Contoured Back Modification This modification has built in lateral supports and a built in adjustable height lumbar support. The Contoured Back Modification helps improve sitting tolerance by increasing user comfort. The standard cover has a four way stretch interface surface and is zippered for easy access when adjustments are necessary. Foam in Place Back Modification This modification utilizes a plywood base and ABS laterals that allow the backs to be made to any height or width specification The interface surface uses a four way stretch material for maximum contouring. The cover has a zipper at the top for easy access. Comes complete with Soft Liquid Sunmate Chemicals to make a custom molded back. One unit of Soft Sunmate Bag Pack is standard. Bowl Packs are available as a no charge option. Other foams and quantities are available upon request. Clinicians must have FIP experience. F IPB Foam in Place Back M odification Low Profile Foam in Place This modification can be used to provide a custom contoured back for clients that do not require aggressive contouring or built in lateral supports. Comes complete with foam chemicals - one unit of Soft Liquid Sunmate. The Bag Pack is standard and the Bowl Pack is a no charge option. Clinicians must have FIP experience. FIPB LP - Low Profile Foam in Place SPRCO - Spinal Relief Cut Out Spinal Relief Cut Out This option has a cut out area to help decrease pressure over the spinous proccesses. Standard cut out is 3 inches wide and one inch deep. Cut out areas can also be filled with a softer foam to provide support while minimizing the pressure over the spine. LCBM Lateral Control Back Modification Lateral Control Back Modification This modification usually consists of a main center panel that has the mounting hardware and 2 hinged thoracic supports on each side. These lateral supports can be adjusted to provide optimal thoracic support. Available as standard configuration (shown on left) or as an upcut configurstion that allows for use with lateral hip supports (shown on right) 10

13 NuShape Seating is custom contoured seating for clients with significant fixed postural deformities and/or abnormal tone. Each system is carved on a 3 axis router, hand detailed and layered with a variety of interface materials to suit the pressure relief requirements of the client. This process provides a full contact surface which gives maximum stabilty and optimal pressure relief for your client. ( A cast or mold is needed to fabricate) NuShape seats are fabricated from 1/2" 9 Ply Plywood, a 4 lb. Polyethylene Foam base and layered with a variety of Pressure Relieving interface foams. The User contact surface is usually finished in a strech material - Darlexx, Dartex or Neoprene. Swingaway Lateral Trunks, Quick Release Lateral Hip / Thigh Supports and Flip Down Abductors may be added. Please see Order Form and Price List for Technical Information. 11

14 Lateral Thoracic Supports Lateral Thoracic Supports - These are shipped in pairs complete with brackets, pads, and fasteners. Please see Hardware Section for components. Standard pads have 1/2" neoprene foam medially and 1/4" foam on the lateral aspect. Overall thickness of pad is approximately 3/4". Thin pads are available as a no charge option. Standard Pad sizes are: Small - 3" high x 4" long Medium - 4" high x 5" long Large - 5" high x 6" long Custom pads up to 6" high x 8" long are available without an upcharge. Please give part number, pad sizes, user chest width, actual backrest width, and upholstery color at the time of order. Flat Pads are standard with fixed brackets and curved pads are standard with swingaway brackets See Measurement Forms for Custom Sizes. Countersunk holes are standard for pad attachment. Slotted Brackets available for most applications. LTS-F-E - Fixed Lateral Thoracic Supports- Each LTS-F - Fixed Lateral Trunk Supports - Pair Aluminum brackets combine high strength and low weight while providing good durability. Flat Pads are standard with the fixed brackets. (curved pads are a n/c option) Standard brackets have a 5" anterior/ posterior measrement and fit backs up to 3" overall thickness. Brackets are convertible to Summer/ Winter with the addition of Receiver Brackets. Flush mount will be sent as standard. See Parts Price Lists for individual components. LTS-F-E - Fixed Lateral Thoracic Support - Each LTS-SA - Swingaway Lateral Thoracic Supports - Pair LTS-SA-L - Swingaway Lateral Trunk Supports - Left Side LTS-SA-R - Swingaway Lateral Trunk Supports - Right Side These supports are a 3 part assembly consistingof a back mounting bracket, a swingaway hinge and an extension bracket that attaches to the pad. This modular configuration can provide various offsets as well as different anterior / posterior pad placements. This bracket uses a lift to release mechanism for good reliability and strength. Comes standard with flush and 1" medial offset extension brackets. LTS-SA - Lift to Release Swingaway - Pair LTS-PB-E - Push Button Swingaway Lateral Thoracic Support LTS-PB - Push Button Swingaway Lateral Thoracic Supports - Pair These supports use our premium Swingaway Bracketry which has the 3 part modular configuration described above. In addition it has an easy to release swingaway hinge that is also very strong and durable. These brackets come standard with flush and 1" medial offset extension brackets. 15 Lateral Thoracic Supports LTS- SW - Summer/ Winter Thoracic Supports - Pair LTS- SW -E - Summer/ Winter Thoracic Support - Each LTS-SW-E These brackets have a receiver bracket with a spring loaded plunger. This hardware allows the brackets and pads to be adjusted laterally to compensate for changes in clothing thickness. It also allows the supports to be quick release to facilitate lateral transfers. Should not be used with backs that are between the back post mount unless an I - Back Modification is used. LTS-SWPB - Summer/ Winter Push Button Swingaway - Pair LTS-SWPB -E - Summer/ Winter Push Button Swingaway - Each LTS- SWPB This hardware combines the benefits of our premium Swingaway hardware with the adjustability of Summer/ Winter Brackets. LTS-FCO - Clamp On Thoracic Supports - Pair LTS-FCO-E - Clamp On Thoracic Supports - Each LTS-FCO Our clamp on brackets securely attach to the back post and provide lateral support for clients without a Solid Back or who are using a back that doesn't allow mounting of conventional lateral supports. Please specify wheelchair model and size, backpost diameter ( 3/4", 7/8" or 1") and user chest width. Can be used with standard wheelchair back upholstery. Wrap around back upholstery must be modified. LTS-PBCO - Clamp On Swingaway Thoracic Supports - Pair LTS-PBCO-E - Clamp On Swingaway Thoracic Supports - Each LTS-PBCO These supports clamp on to the back posts and use our Push Button Swingaway bracketry to facilitate transfers. 16 Lateral Hip and Thigh Supports Lateral Hip and Thigh supports help position the pelvis on the seating surface by preventing unwanted lateral movement. The pads are fabricated from high quality materials and are attached to the Solid Seat with strong but lightweight aluminum brackets. Heavy duty steel brackets are available in some fixed configurations. Please specify height, length and thickness of the pads. (standard thickness is 1" overall) Flush mount brackets will be sent unless specified otherwise. Seat width, seat thickness and user hip width measurements will ensure that proper brackets will be supplied. LHS-F - Fixed Hip Supports LHS-F - Fixed Lateral Hip Supports - Pair LHS-FL - Fixed Lateral Hip Support - Left Side LHS-FR - Fixed Lateral Hip Support - Right Side These supports use a slotted bracket on the bottom to provide lateral adjustability. 17 Medial Thigh Supports Medial Thigh Supports (Hip Abductors) are designed to decrease excessive hip adduction by placing a pad between the medial aspects of the distal thighs. The pads are height and depth adjustable. Please specify pad size, seat thickness, and upholstery option. ABD-FQR- Quick Release Flip Under Medial Thigh Support. - This support flips down under the seat allowing the most leg room for transfers. Bracket is both height and depth adjustable.. ABD-DI - Drop In Medial Thigh Support This support mounts to a seat pan or the bottom of a Solid Seat. It can be mounted on chairs where under seat space is limited - Bracket has both height and depth adjustability. 15

Mounting the support pad laterally (distal) helps improve control. The pads lift and swingaway to allow transfers in and out of the wheelchair. 22 Head Supports - Pads HR-CPAD - Concave Pad The concave pad provides good posterior support while allowing users with some head control the ability to move laterally and to rotate. Available in custom sizes upon request. Standard pad sizes are: Small - 3" high x 6" wide (inside) Medium - 4" high x 7 1/2" wide Large - 4" high x 9" wide HR-LCPAD - Lateral Control Pad This pad provides good posterior and lateral support. The low profile sides do not block the users field of vision. Comes standard with a built in occipital contour. Standard Sizes are : Small - 5 1/2" high x 6" wide (inside) Medium - 6 1/4" high x 7 1/2" wide Large - 7" high x 9" wide Closed Open HR-BLPAD - Belled Headrest Pad The belled pad is designed to keep the client's head in front of the pad and to make it more difficult for the client to hook their head around behind the headrest. This pad is usually ordered wider than a standard pad. HR-ACPAD - Adjustable Concave Headrest Pad This headrest consists of three internally hinged sections that are covered by a single foam pad and cover. Inside width is adjustable from 4" to 11" wide. Overall Height is 3 1/2". A 3/16" Hex Key is included for adjustment. A stretch cover is recommended for maximum adjustabliity. HR-KFPAD-M - Komfi Fit Pad This uniquely shaped headrest pad provides exceptional posterior and lateral head support. 1.5 inches of soft viscoelastic foam allow the pad to contour to the users head. It's multi-articular hardware allows adjustments in all directions. The standard bracket can accommodate heads that are up to 5" off midline. Optional hardware can provide additional adjustability in all directions. This bracket can be used with all pad styles. HR-LBRKT - Double L- Bracket -This double L- Bracket assembly allows vertical adjustment and up to 5" of anterior/ posterior adjustment. This bracket can be used with all pad styles except molded headrests. HRM-OC Occipital Contour This modification adds a contoured occipital build up to increase contact area and provide improved user comfort. HRM-PLUSH - Plush Pad Modification.
